Application of pulse index continuous cardiac output system in elderly patients with acute myocardial infarction complicated by cardiogenic shock: A prospective randomized study

BACKGROUND Cardiogenic shock (CS) secondary to acute myocardial infarction (AMI) complicates management of the condition, and often leads to poor prognosis. Prompt and accurate monitoring of cardiovascular and accompanying hemodynamic changes is crucial in achieving adequate management of the condition. Advances in technology has availed procedures such as pulse index continuous cardiac output (PiCCO), which can offer precise monitoring of cardiovascular functions and hemodynamic parameters. In this study, PiCCO is evaluated for its potential utility in improving management and clinical outcomes among elderly patients with AMI complicated by CS. AIM To assess whether use of the PiCCO system can improve clinical outcomes in elderly patients with AMI complicated by *** Patients from emergency intensive care units (EICU) or coronary care units (CCU) were randomized to receive PiCCO monitoring or not. The APACHE II score, SOFA score, hs-TnI, NT-proBNP, PaO2/FiO2 ratio and lactate levels on day 1, 3 and 7 after treatment were compared. The infusion and urine volume at 0-24 h, 24-48 h and 48-72 h were recorded, as were the cardiac index (CI), extravascular lung water index (EVLWI), intrathoracic blood volume index (ITBVI) and global end diastolic volume index (GEDVI) at similar time intervals. RESULTS Sixty patients with AMI complicated by CS were included in the study. The PiCCO group had a significantly lower APACHE II score, SOFA score, hs-TnI and NT-proBNP levels on day 1, 3 and 7 after treatment. The infusion and urine volume during 0-24 h in the PiCCO group were significantly greater, and this group also showed significantly higher ADL scores. Furthermore, the PiCCO group spent lesser days on vasoactive agents, mechanical ventilation, and had a reduced length of stay in EICU/CCU. Additionally, the CI was significantly higher at 48 h and 72 h in the PiCCO group compared with that at 24 h, and the EVLWI, ITBVI and GEDVI were significantly decreased at 48 h and

Background Hand, foot, and mouth disease (HFMD) is a common infectious disease in childhood caused by an enterovirus (EV), and which is principally seen in children under 5 years of age. To promote diagnostic awareness and effective treat-ments, to further standardize and strengthen the clinical management and to reduce the mortality of HFMD, the guidelines for diagnosis and treatment have been developed. Methods National Health Commission of China assembled an expert committee for a revision of the guidelines. The com-mittee included 33 members who are specialized in diagnosis and treatment of HFMD. Results Early recognition of severe cases is utmost important in diagnosis and treatment of patients with HFMD. The key to diagnosis and treatment of severe cases lies in the timely and accurate recognition of stages 2 and 3 of HFMD, in order to stop progression to stage 4. Clinicians should particularly pay attention to those EV-A71 cases in children aged less than 3 years, and those with disease duration less than 3 days. The following indicators should alert the clinician of possible deterioration and impending critical disease: (1) persistent hyperthermia;(2) involvement of nervous system;(3) worsening respiratory rate and rhythm;(4) circulatory dysfunction;(5) elevated peripheral WBC count;(6) elevated blood glucose and (7) elevated blood lactic acid. For treatment, most mild cases can be treated as outpatients. Patients should be isolated to avoid cross-infection. Intense treatment modalities should be given for those severe cases. Conclusion The guidelines can provide systematic guidance on the diagnosis and management of HFMD.

The higher grain production, the more social deprivation? A case study of Henan province in traditional agricultural areas of China

This paper examines the spatial-temporal dynamics of inequality from the perspective of social deprivation in the context of continuous growth of grain production using national census data from 2000 and 2010 for Henan province,*** analytical results show an overall level of social deprivation that is high and a widening social deprivation gap at the county *** social deprivation shows great spatiotemporal heterogeneity,the higher deprived counties are mainly distributed in central and eastern Henan province,while counties with lower social deprivation are clustered in northwestern regions,formed a belt-shape spatial differential between the south(higher deprived counties clustered)and northwest(low deprived counties clustered)Henan in 2000,and changed toward a core-periphery semi annular spatial structure in 2010,the low deprived counties surrounded by high deprived *** social deprivation shows a"west low and east high"spatial pattern,similar to the grain production in Henan *** development level was low in most counties in yudong plain despite of keeping the continuous increase of grain production,meaning the higher grain output did not result in the more social development expected in Chinese traditional agricultural areas,and the regional disparity is expected to increase in the near future if development conditions are not changed,which could inspire more targeted rural governance options.

15 compounds,including two new ones crepidatuols A(1)and B(2)were isolated from the stems of Dendrobium *** planar structures of these compounds were elucidated by spectroscopic methods(NMR,MS,UV,and IR)and comparison with those from literatures.10 compounds were send for enhancing activities on nerve growth factor(NGF)medicated neurite outgrowth in PC12 cells and the results indicated that crepidatuol A(1),confusarin and 3-(2-acetoxy-5-methoxy)-phenylpropanol showed enhancing activities at the concentration of 10.0μM.

Resolving multi-orbital effects on high harmonic generation from aligned N2 molecules in linearly and elliptically polarized intense laser fields

We perform an experimental study of the multi-orbital effect on the high-order harmonic generation(HHG) from aligned N_2 molecules in both linearly and elliptically polarized intense laser *** by a home-built extreme ultraviolet(xuV) flat grating spectrometer with the pump-probe method, the angular distributions of different orders of HHG are obtained, which show distinctive behaviors for harmonics in the plateau and the cut-off *** ellipticity dependence of HHG is investigated by aligning the molecular axis parallel or perpendicular to the laser *** results indicate that both the highest occupied molecular orbital(HOMO) as well as the lower one(HOMO-1) contribute to the HHG of N2 molecules, in either linearly or elliptically polarized intense laser *** study paves the way for understanding the ultrafast electron dynamics of molecules exposed to an intense laser field.

The Tsinghua-NAOC （National Astronomical Observatories of China） Telescope （hereafter, TNT） is an 80-cm Cassegrain reflecting telescope located at the Xinglong Observatory of NAOC, with the main scientific goals of monitoring various transients in the universe such as supernovae, gamma-ray bursts, novae, variable stars, and active galactic nuclei. We present a systematic test and analysis of the photometric performance of this telescope. Based on the calibration observations taken on 12 pho- tometric nights, spanning the period from 2004 to 2012, we derived an accurate trans- formation equation between the instrumental ubvri magnitudes and standard Johnson UBV and Cousins RI magnitudes. In particular, the color terms and the extinction coefficients of different passbands are well determined. With these data, we also ob- tained the limiting magnitudes and the photometric precision of TNT. It is worthwhile to point out that the sky background at the Xinglong Observatory became gradually worse over the period from 2005 to 2012 （e.g., -21.4 mag vs. ,-20.1 mag in the V band）.

Upgrade of macromolecular crystallography beamline BL17U1at SSRF

Beamline BL17 U1 at shanghai Synchrotron Radiation Facility is an energy-tunable macromolecular crystallography beamline that has been in user operation since 2009. Growing demand from the user community for a small beam and related experimental methods have motivated upgrades of the devices in the *** modes have already been developed for operation. A self-integrated diffractometer reduces the sphere of confusion of the rotatory axis to 1μm. The new diffractometer is equipped with an upgraded on-axis viewing system that can improve the resolving power. Additionally,the area detector was also upgraded to the newest generation of detectors, the EIGER X 16 M, which can collect data at 133 Hz. After these upgrades, the endstation became virtually new. This paper covers the upgrade of the endstation devices and gives the first data collection results.

Field mold stress induced catabolism of storage reserves in soybean seed and the resulting deterioration of seed quality in the field

Excessive rainfall provides a favorable condition for field mold infection of plants, which triggers field mold(FM) stress. If FM stress occurs during the late maturation stage of soybean seed, it negatively affects seed yield and quality. To investigate the responses of soybean seed against FM stress and identify the underlying biochemical pathways involved, a greenhouse was equipped with an artificial rain producing system to allow the induction of mold growth on soybean seed. The induced quality changes and stress responses were revealed on the levels of both transcriptome and metabolome. The results showed that soybean seeds produced under FM stress conditions had an abnormal and inferior appearance, and also contained less storage reserves, such as protein and polysaccharide. Transcriptional analysis demonstrated that genes involved in amino acid metabolism, glycolysis, tricarboxylic acid, β-oxidation of fatty acids, and isoflavone biosynthesis were induced by FM stress. These results were supported by a multiple metabolic analysis which exhibited increases in the concentrations of a variety of amino acids, sugars, organic acids, and isoflavones, as well as reductions of several fatty acids. Reprogramming of these metabolic pathways mobilized and consumed stored protein, sugar and fatty acid reserves in the soybean seed in order to meet the energy and substrate demand on the defense system, but led to deterioration of seed quality. In general, FM stress induced catabolism of storage reserves and diminished the quality of soybean seed in the field. This study provides a more profound insight into seed deterioration caused by FM stress.

Background:Biomarkers based on immune context may guide prognosis prediction.T-cell inactivation,exclusion,or dysfunction could cause unfavorable tumor microenvironments,which affect immunotherapy and ***,none of the immuno-biomarkers reported to date can differentiate colorectal-cancer(CRC)***,we aimed to classify CRC patients according to the levels of T-cell activation,exclusion,and dysfunction in the tumor ***:RNAseq data of 618 CRC patients from The Cancer Genome Atlas and microarray data of 316 CRC patients from Gene Expression Omnibus were analysed using the Tumor Immune Dysfunction and Exclusion *** clustering was used to classify ***:Based on the expression signatures of myeloid-derived suppressor cells,cancer-associated fibroblasts,M2-like tumor-associated macrophages,cytotoxic T-lymphocytes,and PD-L1,all patients were clustered into four subtypes:cluster 1 had a high level of immune dysfunction,cluster 2 had a low level of immune activation,cluster 3 had intense immune exclusion,and cluster 4 had a high level of immune activation and a moderate level of both dysfunction and exclusion *** with cluster 1,the hazard ratios and 95%confidential intervals for overall survival were 0.63(0.35-1.13)for cluster 2,0.55(0.29-1.03)for cluster 3,and 0.30(0.14-0.64)for cluster 4 in multivariate Cox *** immune clustering and prognosis patterns were obtained upon validation in the GSE39582 *** subgroup analysis,immune clustering was significantly associated with overall survival among stage I/II patients,microsatellite stable/instability-low patients,and patients not treated with adjuvant ***:Our findings demonstrated that classifying CRC patients into different immune subtypes serves as a reliable prognosis predictor and may help to refine patient selection for personalized cancer immunotherapy.

Puerarin exhibits greater distribution and longer retention time in neurons than astrocytes in a co-cultured system

The phytoestrogen puerarin has been shown to protect neurons and astrocytes in the brain, and is therefore an attractive drug in the treatment of Alzheimer’s disease, Parkinson’s disease and cerebral ischemia. Whether puerarin exhibits the same biological processes in neurons and astro-cytesin vitro has rarely been reported. In this study, cortical neurons and astrocytes of newborn Sprague-Dawley rats were separated, identiifed and co-cultured in a system based on Transwell membranes. The retention time and distribution of puerarin in each cell type was detected by lfuorescence spectrophotometry and lfuorescence microscope. The concentration of puerarin in both co-cultured and separately cultured neurons was greater than that of astrocytes. Puerarin concentration reached a maximum 20 minutes after it was added. At 60 minutes after its addi-tion, a scant amount of drug was detected in astrocytes; however in both separately cultured and co-cultured neurons, the concentration of puerarin achieved a stable level of about 12.8 ng/mL. The results indicate that puerarin had a higher concentration and longer retention time in neu-rons than that observed in astrocytes.
