AIM:To evaluate whether an endoscopy position detecting unit(UPD-3) can improve cecal intubation rates, cecal intubation times and visual analog scale(VAS) pain scores, regardless of the colonoscopist's level of ***:A total of 260 patients(170 men and 90women)who underwent a colonoscopy were divided into the UPD-3-guided group or the conventional group(no UPD-3 guidance).Colonoscopies were performed by experts(experience of more than 1000colonoscopies)or trainees(experience of less than 100colonoscopies).Cecal intubation rates,cecal intubation times,insertion methods(straight insertion:shortening the colonic fold through the bending technique;roping insertion:right turn shortening technique)and patient discomfort were *** discomfort during the endoscope insertion was scored by the VAS that was divided into 6 degrees of ***:The cecum intubation rates,cecal intubation times,number of cecal intubations that were performed in<15 min and insertion methods were not significantly different between the conventional group and the UPD-3-guided *** number of patients who experienced pain during the insertion was markedly less in the UPD-3-guided group than in the conventional *** and multivariate analysis showed that the following factors were associated with lower VAS pain scores during endoscope insertion:insertion method(straight insertion)and UPD-3guidance in the trainee *** the experts group,univariate analysis showed that only the insertion method(straight insertion)was associated with lower VAS pain ***:Although UPD-3 guidance did not shorten intubation times,it resulted in less patient painduring endoscope insertion compared with conventional endoscopy for the procedures performed by trainees.

Gastric carcinoma is one of the malignancies that are most frequently associated with esophageal *** describe herein our device for advanced esophageal cancer associated with early gastric cancer in the antrum.A 57-year-old man presenting with dysphagia and upper abdominal pain was admitted to our *** examinations revealed locally advanced squamous cell carcinoma (SCC) of the middle thoracic esophagus (T3N0M0 Stage ⅡA) and mucosal signetring cell carcinoma of the gastric antrum (T1N0M0 Stage ⅠA).Although the gastric tumor appeared to be an intramucosal carcinoma,its margin was obscure,so endoscopic en-bloc resection was considered *** chose surgical resection of the gastric tumor as well as the esophageal SCC after neoadjuvant chemotherapy with 5-fluorouracil and cisplatin for advanced esophageal *** transthoracic esophagectomy with three-field lymph node dissection,the gastric carcinoma was removed by gastric antrectomy,which preserved the right gastroepiploic vessels,and a pedunculated short gastric tube was used as the esophageal ***-eight months after the surgery,the patient is well with no evidence of cancer *** it minimizes surgical stress and organ sacrifice,gastric tube interposition is a potentially useful technique for esophageal cancer associated with localized early gastric cancer.

Development of highly-efficient photovoltaic (PV) modules and expanding its application fields are significant for the further development of PV technologies and realization of innovative green energy infrastructure based on PV. Especially, development of solar-powered vehicles as a new application is highly desired and very important for this end. This paper presents the impact of PV cell/module conversion efficiency on reduction in CO2 emission and increase in driving range of the electric based vehicles. Because imines could be used as convenient starting materials in various fields, the development of an easy synthetic method of imine was strongly desired. In response to this demand, we thought that it would be an effective synthesis method if an aldehyde and an amine could be reacted to give an imine in good yield under solvent- and catalyst-free conditions. In fact, we tried the reaction of benzaldehyde with various amines under solvent- and catalyst-free conditions followed by removal of water that was produced in the reaction system by a vacuum pump, and desired imines could be obtained in good yields. Observation of this reaction using a nuclear magnetic resonance spectrometer revealed that the reaction rate was extremely fast at the initial stage but slowed over time. However, the reaction of benzaldehyde with aniline differed greatly, and the reaction rate dramatically improved in 47 - 48 minutes after the start of the reaction. At this time, we found that the reaction system underwent a phase transition from the liquid phase to the solid phase.

A 56-year-old male visited our hospital for evaluation of an occipital *** computed tomography showed hypervascular enhancement with osteolytic change in the skull and a huge enhanced mass in the *** resonance imaging showed bone metastasis in the thoracic *** for hepatitis B surface antigen and hepatitis B core antibody were positive and his liver condition was Child-Pugh grade *** diagnosis was hepatocellular carcinoma(HCC) with skull and vertebrae metastases on chronic hepatitis *** was treated with radiation therapy for bone metastases and transcatheter arterial chemoembolization for *** he developed acute respiratory failure because of aspiration pneumonia,congestion and oedema with haemorrhage of the lungs and *** showed HCC with multiple bone *** liver tumor was categorized as well-differentiated HCC,Edmondson classification Ⅰ,trabecular type and pseudoglandular *** the liver mild infiltration of lymphocytes was seen in Glisson's capsules which were signif icantly enlarged with well preserved limiting *** necrosis was not *** fibrosis was *** 8 cm × 7 cm × 3 cm metastatic lesion had formed in the left occipitotemporal part of the cranial *** lesion was osteolytic and showed invasion into the dura *** the subdural cavity nor the brain showed involvement from the metastatic ***,skull metastasis from HCC is very rare and it affects the patient's prognosis and the quality of ***,it is very important to make an early diagnosis and carry out proper management of skull metastasis from HCC.

Use of water jet instruments in gastrointestinal endoscopy

In recent years, water jet instruments have been used in the field of gastrointestinal endoscopy, mainly in two clinical situations: Investigation and treatment under endoscopic view. Injecting water jet into the gastrointestinal lumen is helpful for maintaining a clear endoscopic view, washing away blood or mucous in the lumen or on the surface of the tip of the endoscope. This contributes to reducing time and discomfort of examination. Water jet technology is an alternative method for dissecting soft tissue; this method does not harm the small vessels or cause mechanical or thermal damage. However, its use in clinical settings has been limited to the transmucosal injection of water into the submucosal layer that elevates the mucosa to prepare for endoscopic mucosal resection or endoscopic submucosal dissection, instead of tissue dissection, which may occur because of the continuous water jet. A preclinical study has been conducted using a pulsed water jet system as an alternative method for submucosal dissection by reducing intraoperative water consumption and maintenance of dissection capability. This review introduces recent studies pertaining to using a water jet in gastrointestinal endoscopy and discusses future prospects.

随着经济水平的提高，生活方式的西化，含有大量精制糖的西点，成为年轻人能量摄入的重要组成部分。据报道，与全麦食品相比，摄入大量含精制糖的食品可引起低血糖。短暂的低血糖可引起明显的脑功能紊乱和认知障碍，丧失驾驶能力。然而，是否短时间内摄入大量糖分引起胰岛素分泌增加，从而导致低血糖尚未得到证实。蔗糖和麦芽糖均为双糖，经常被用为食品中的调味料或添加剂。本实验研究比较短时间内重复大量摄入蔗糖或麦芽糖对胰岛素分泌的影响。
