Evolution and Role of Intraoperative Neurophysiological Monitoring in Intramedullary Spinal Cord Surgery: A 2-Year Series from Saudi Arabia

Introduction: Despite recent advances in neuroimaging and microsurgical techniques, surgical resection of spinal cord tumours remains a challenge. However, the evolution with advances and refinement of neurophysiological equipment and methodologies, intra-operative neurophysiolo- gical monitoring (IONM) is now regarded as an essential adjunct to the surgical management of intramedullary spinal cord tumours. This study aims to report our preliminary experience with IONM and emphasise its effective role of achieving maximum tumour resection and minimising neurological injury. Methods: This is a retrospective study performed at our institution between July 2012 and August 2013. It included a cohort of 6 consecutive patients presented with intramedullary spinal cord tumours. Their mean age was 26 years (range, 4 months - 37 years), all were males, and the mean follow up was 11.6 months. Results: We combined the use of somatosensory evoked potentials (SSEPs) and motor evoked potentials (MEPs) in spinal cord surgery. SSEPs are monitored during the incision of the dorsal midline of the spinal cord and this was used in two of our patients and MEPs were used as an essential monitoring during the tumour resection. In addition, we used free-running electromyography (EMG) and muscle MEPs (mMEPs) during tumour resection. Four of our patients (two with ependymoma, one with ganglioglioma, and one with pilocytic astrocytoma) had complete tumour resection and two patients (pilocytic and diffuse astrocytoma) had IONM changes during surgery and had partial tumour resection. At 6-month follow up all our patients had made a good recovery with no new neurological sequelae. Conclusion: This small series and literature review is presented to add and improve the understanding of IONM in intramedullary spinal cord procedures and to reinforce the importance of IONM in optimising tumour resection and neurological outcome. Medulloblastoma(MB)is considered the commonest malignant brain tumor in *** treatments consisting of surgery,radiation,and chemotherapy have improved patients’***,the recurrence occurs in 30%of *** persistent mortality rates,the failure of current therapies to extend life expectancy,and the serious complications of non-targeted cytotoxic treatment indicate the need for more refined therapeutic *** MBs originating from the neurons of external granular layer line the outer surface of neocerebellum and responsible for the afferent and efferent ***,MBs have been segregated into four molecular subgroups:Wingless-activated(WNT-MB)(Group 1);Sonichedgehog-activated(SHH-MB)(Group 2);Group 3 and 4 *** molecular alterations follow specific gene mutations and disease-risk *** current treatment protocols and ongoing clinical trials against these molecular subgroups are still using common chemotherapeutic agents by which their efficacy have improved the progression-free survival but did not change the overall ***,the need to explore new therapies targeting specific receptors in MB microenvironment became *** immune microenvironment of MBs consists of distinctive cellular heterogeneities including immune cells and none-immune *** associate macrophage and tumour infiltrating lymphocyte are considered the main principal cells in tumour microenvironment,and their role are still under *** this review,we discuss the mechanism of interaction between MB cells and immune cells in the microenvironment,with an overview of the recent investigations and clinical trials.

Genes and proteins expressed at different life cycle stages in the model protist Euplotes vannus revealed by both transcriptomic and proteomic approaches

Sexual reproduction first appeared in unicellular protists and has continued to be an essential biological process in almost all *** protists, which contain both germline and somatic genomes within a single cell, have evolved a special form of sexual reproduction called conjugation that involves mitosis, meiosis, fertilization, nuclear differentiation, genome rearrangement, and the development of unique cellular structures. The molecular basis and mechanisms of conjugation vary dramatically among ciliates, and many details of the process and its regulation are still largely unknown. In order to better comprehend these processes and mechanisms from an evolutionary perspective, this study provides the first comprehensive overview of the transcriptome and proteome profiles during the entire life cycle of the newly-established marine model ciliate Euplotes vannus. Transcriptome analyses from 14 life cycle stages(three vegetative stages and 11 sexual stages) revealed over 26,000 genes that are specifically expressed at different stages, many of which are related to DNA replication,transcription, translation, mitosis, meiosis, nuclear differentiation, and/or genome rearrangement. Quantitative proteomic analyses identified 338 proteins with homologs associated with conjugation and/or somatic nuclear development in other ciliates, including dicer-like proteins, Hsp90 proteins, RNA polymerase Ⅱ and transcription elongation factors, ribosomal-associated proteins, and ubiquitin-related proteins. Four of these homologs belong to the PIWI family, each with different expression patterns identified and confirmed by RT-qPCR,which may function in small RNA-mediated genome rearrangement. Proteins involved in the nonhomologous end-joining pathway are induced early during meiosis and accumulate in the developing new somatic nucleus, where more than 80% of the germline sequences are eliminated from the somatic genome. Transfer Learning on Deep Neural Networks to Detect Pornography

While the internet has a lot of positive impact on society,there are negative *** to everyone through online platforms,pornography is,inducing psychological and health related issues among people of all *** a difficult task,detecting pornography can be the important step in determining the porn and adult content in a *** this paper,an architecture is proposed which yielded high scores for both training and *** dataset was produced from 190 videos,yielding more than 19 h of *** main sources for the content were from YouTube,movies,torrent,and websites that hosts both pornographic and non-pornographic *** videos were from different ethnicities and skin color which ensures the models can detect any kind of video.A VGG16,Inception V3 and Resnet 50 models were initially trained to detect these pornographic images but failed to achieve a high testing accuracy with accuracies of 0.49,0.49 and 0.78 ***,utilizing transfer learning,a convolutional neural network was designed and yielded an accuracy of 0.98.

Clustered Hyperbolic Categories

We introduce a class of categories, called clustered hyperbolic categories, which are constructed from a categorized version of preseeds called categorical preseeds using categorical mutations that are a ＂functorial＂ edition of preseed mutations. Every Weyl preseed p gives rise to a categorical preseed P which generates a clustered hyperbolic category; this is formed by copies of categories each one of which is equivalent to the category of representations of the Weyl cluster algebra H（p）. A ＂categorical realization＂ of Weyl cluster algebra is provided in the sense of defining a map Fp from any clustered hyperbolic category induced from p to the Weyl cluster algebra H（p）, where the image of Fp generates H（p）.

New design simulation for a high-altitude dual-balloon system to extend lifetime and improve floating performance

This paper presents a comprehensive simulation for a high-altitude dual-balloon system which consists of zero-and super-pressure balloons. Preliminary calculations were established to describe the basic concept of the new integrated system. A mathematical model was investigated to simulate the ascending and floating processes which considered the atmospheric conditions and thermodynamic variations. Based on the simulation, results showed that the floating altitude stability between daytime and nighttime was improved. This integrated system supports higher floating altitude levels than those of ordinary balloons and extends the lifetime of floating balloon systems. Moreover, results demonstrated that the integrated system was lighter than ordinary balloons, saving useful weight for effective payload. Furthermore, exploiting the advantages of both kinds of balloons while avoiding the difficulties was a significant goal in the current design to promote the floating performance of high-altitude balloons technology.

Joint slip investigation based on finite element modelling verified by experimental results on wind turbine lattice towers

Slippage corresponds to the relative displacement of a bolted joint subjected to shear loads since the construction clearance between the bolt shank and the bolthole at assembly can cause joint slip. Deflections of towers with joint slippage effects is up to 1.9 times greater than the displacements obtained by linear analytical methods. In this study, 8 different types of joints are modelled and studied in the finite element program, and the results are verified by the experimental results which have been done in the laboratory. Moreover, several types of joints have been modelled and studied and load-deformation curves have also been presented. Finally, joint slip data for different types of angles, bolt diameter and bolt arrangements are generated. Thereupon, damping ratios （ζ） for different types of connections are reported. The study can be useful to help in designing of wind turbine towers with a higher level of accuracy and safety.

Analysis of brain disorder in the neuroimaging of Magnetic Resonance Imaging(MRI),Positron Emission Tomography(PET),and Computed Tomography(CT)needs to understand the functionalities of the brain and it has been performed using *** learning algorithms have also been applied in genomics data *** brain disorder diseases of Alzheimer,Schizophrenia,and Parkinson are analyzed in this *** main issue in the traditional algorithm is the improper detection of disorders in the neuroimaging *** paper presents a deep learning algorithm for the classification of brain disorder using Deep Belief Network(DBN)and securely storing the image using a Deoxyribonucleic Acid(DNA)Sequence-based Joint Photographic Experts Group(JPEG)Zig Zag Encryption Algorithm(DBNJZZ).In this work,DBNJZZ implements an efficient and effective prediction model for disorders using the open-access datasets of Alzheimer’s Disease Neuroimaging Initiative(Adni),the Center for Biomedical Research Excellence(Cobre),the Open Access Series of Imaging Studies(Oasis),the Function Biomedical Informatics Research Network(Fbirn),a Parkinson’s dataset of 55 patients and 23 subjects with Parkinson’s syndromes(Ntua),and the Parkinson’s Progression Markers Initiative(Ppmi).This algorithm is implemented and tested using performance metric measures of accuracy,Root Mean Square Error(RMSE),Mean Absolute Error(MAE),and Mean Absolute Percentage Error(MAPE).DBNJZZ gives better performance with an accuracy of 99.21%and also surpasses previous methods on other measures.
