Maropitant a neurokinin-1 (NK-1) receptor antagonist used as an antiemetic drug in dogs by blockade of substance P/NK-1 receptor binding in the vomiting center and chemoreceptor trigger zone in the central nervous system. In addition,substance P and NK-1 receptors have been isolated in nociceptive pathways. Therefore,it is expected that maropitant provides analgesic effect and its sparing effect of minimum alveolar concentration (MAC) has been reported in dogs. The purpose of this study was to evaluate the sparing effect of an approved antiemetic dose of maropitant on the MAC for blunting adrenergic response (MAC-BAR) of sevoflurane in *** adult healthy beagle dogs were anesthetized to determine the MAC-BAR of sevoflurane on 2-ocassions at least 1-week intervals. In each occasion,the dogs received a subcutaneous injection (sC) of saline (0. 1 ml/kg) or maropitant (1 mg/kg) before the MAC-BAR determination. MAC-BAR determination started at 60 min after the administration of saline or maropitant. MAC-BAR was determined in duplicate for each dog using electrical stimulus applied to the gingival (50 V,50 Hz,10 ms). During the MAC determination,all dogs were mechanically ventilated to maintain the endtidal CO2within 35 to 40 mmHg and kept warm to maintain esophageal temperature within 37. 5 to 38. 5 ℃. Data were compared between groups using a paired t-test. The level of significance was set at P seline MAC-BAR for sevoflurane was 3. 44 ± 0. 53%. subcutaneous maropitant significantly decreased the sevoflurane MAC-BAR by 17% (2. 88 ± 0. 74%,P = 0. 037).Maropitant decreased the sevoflurane MAC-BAR when administered at the approved antiemetic dose (1 mg/kg:sC) to dogs. It is expected that a preoperative administration of maropitant may produce anesthetic sparing effect in addition to antiemetic effect in dogs.

Robenacoxib is a newer non-steroidal anti-inflammatory drug (NsAID) and effective for the management of perioperative pain and inflammation. A preoperative administration of robenacoxib is approved in many countries. The preoperative administration of NsAIDs (carprofen or meloxicam) reduces the minimum alveolar concentration (MAC) of sevoflurane during noxious stimulation in dogs (Yamashita et al. 2008). The purpose of this study was to evaluate the effect of robenacoxib on the MAC for blunting adrenergic response (MAC-BAR) of sevoflurane in *** adult healthy beagle dogs were anesthetized to determine the MAC-BAR of sevoflurane on 2-ocassions at least 1-week intervals. It is expected that the use of robenscoxib as premedication may provide a clinically relevant anesthetic sparing effect similar to carprofen and meloxicam in dogs (Yamashita et al. 2008).

Effects of controlled ventilation and positioning on cardiopulmonary function in horses anesthetized with total intravenous anesthesia using a drug combination of medetomidine,lidocaine,butorphanol and propofol (MLBP-TIVA)

The purpose of this study was to evaluate effects of controlled ventilation and positioning on cardiopulmonary function in horses anesthetized with total intravenous anesthesia using a drug combination of medetomidine,lidocaine,butorphanol and propofol (MLBP-TIVA).Five healthy adult horses were anesthetized with MLBP-TIVA for 2-hours on 3-occasions at 4-week interval. In each occasion,horses were anesthetized in lateral recumbency with (CV-LR-group) or without controlled ventilation (sB-LRgroup) or in dorsal recumbency with controlled ventilation (CV-DR-group). Anesthesia was induced with lidocaine (1mg/kg: IV) and propofol (3 mg/kg: IV) following premedications with medetomidine (0. 005 mg/kg: IV) and butorphanol (0. 02 mg/kg: IV),and then maintained by constant rate infusions of propofol (6 mg/kg/hr),medetomidine (0. 0035 mg/kg/h),lidocaine (3 mg/kg/h) and butorphanol (0. 024 mg/kg/h). All horses breathed 100% oxygen during anesthesia. In the CV-groups,horses were mechanically ventilated to maintain arterial CO2pressure (PaCO2)within 40 ~ 50 mmHg. Cardiopulmonary parameters were compared between groups using repeated-measures *** level of significance was set at P sB-LR group,cardiovascular parameters were maintained within acceptable ranges (heart rate: 37 ~ 39beats/min,mean arterial blood pressure: 109 ~ 115 mmHg,mean pulmonary arterial pressure [MPAP]: 28 ~ 29 mmHg,mean right atrial pressure [MRAP]: 19 ~ 21 mmHg,cardiac output: 27 ~ 30 L/min) but severe hypercapnia with insufficient oxygenation (PaCO283 ~ 103 mmHg,arterial O2pressure [PaO2]155 ~171 mmHg) was observed. Cardiopulmonary parameters were maintained with acceptable ranges in the both CV groups. In the CV-LR group,significant decreases in heart rate (29 ~ 31 beats/min,P = 0. 020),cardiac output (17 ~ 21 L/min,P = 0. 005) and PaCO2 (42 ~50 mmHg,P =0. 001) and a significant increase in PaO2 (395 ~419 mmHg,P =0. 005) were observed compared to the sB-group. Adherence of <i>streptococcus mutans</i>and Adsorption of salivary Protein to Resin Composites Containing s-PRG Fillers

The purpose of this study was to measure the amount of adsorption of various salivary proteins to a resin composite having various amounts of surface pre-reacted glass-ionomer (s-PRG) fillers, and to make a comparative study of the adherence of s. mutans to the resin composite covered by various salivary proteins. We experimentally produced resin composites (s-PRG resin) having the basic composition of Bis-GMA/TEGDMA and various amount of the s-PRG fillers ranging between 0 - 60 wt%. Each s-PRG resin block was soaked in 5 kinds of components found in salivary fluid (Mucin 1, Lactoferrin, IgA, Cystatin C, and Lysozyme), and the amount of adsorption was measured by use of a spectrophotometer. The amount of the adsorption of salivary Mucin 1 was higher than that of any other salivary component tested regardless of the percentage of the s-PRG filler. In the case of salivary Lysoxyme used for coating, the amount of its adsorption increased with an increase in the percentage of the s-PRG filler. In addition, resin blocks coated with various salivary proteins were incubated at 37℃ for 2 hours with radio-labeled s. mutans for a quantitative adherence test. Labeled bacteria that adhered to the resin blocks were collected by using an automatic sample combustion system and a liquid scintillation counter. The absorbed salivary components, especially Mucin 1 and Lysozyme, inhibited the adhesion of s. mutans to the s-PRG resin;however, these changes were generally directional rather than statistically significant.

Genomic characterization of esophageal squamous cellcarcinoma:insights from next-generation sequencing

Two major types of cancer occur in the esophagus: squamous cell carcinoma, which is associated with chronic smoking and alcohol consumption, and adenocarcinoma, which typically arises in gastric reflux-associated Barrett's esophagus. Although there is increasing incidence of esophageal adenocarcinoma in Western counties, esophageal squamous cell carcinoma(EsCC) accounts for most esophageal malignancies in East Asia, including China and Japan. Technological advances allowing for massively parallel, high-throughput next-generation sequencing(NGs) of DNA have enabled comprehensive characterization of somatic mutations in large numbers of tumor samples. Recently, several studies were published in which whole exome or whole genome sequencing was performed in EsCC tumors and compared with matched normal DNA. Mutations were validated in several genes, including in TP53, CDKN2 A, FAT1, NOTCH1, PIK3 CA, KMT2 D and NFE2L2, which had been previously implicated in EsCC. several new recurrent alterations have also been identified in EsCC. Combining the clinicopathological characteristics of patients with information obtained from NGs studies may lead to the development of effective diagnostic and therapeutic approaches for EsCC. As this research becomes more prominent, it is important that gastroenterologist become familiar with the various NGs technologies and the results generated using these methods. In the present study, we describe recent research approaches using NGs in EsCC.

A case of a patient positive for anti-cardiolipin antibodies with recurrent fetal wastage and cerebral infarction who was successfully treated with sairei-to, low dose aspirin, and heparin

We report a successful case of a pregnant female positive for anti-cardiolipin antibodies who experienced two abnormal pregnancies with postpartum cerebral hemispheric infarctions. A 38-year-old female diagnosed as being positive for anti-cardiolipin antibodies was referred to our hospital due to her strong desire to have a baby. The administration of Japanese herbal medicine, sairei-to, as immunosuppressive therapy, and low dose aspirin, as anti-coagulation therapy, were initiated prior to the patient’s pregnancy. Five months after beginning the treatment, the patient conceived spontaneously. At 34 weeks of gestation, emergency cesarean section was performed due to increasing genital bleeding resulting from coincidental placenta previa and the patient delivered an appropriate-for-date female infant (1970 g). Treatment with Japanese herbal medicine (sairei-to) and low-dose aspirin is considered to be an effective treatment option for patients positive for anti-phospholipid antibodies with past histories of abnormal pregnancies.

strong>Objective:strong> This was a retrospective study to evaluate the clinical impact of bacteremia due to staphylococcus saprophyticus and identify which factors influence it. strong>Methods:strong> We reviewed all patients with bacteremia due to s. saprophyticus over the last 12 years. This study was performed at Jichi Medical University Hospital in Japan, a key hospital in the northern Kanto area including Tochigi, Gunma, Ibagagi, and northern saitama prefectures. We retrospectively reviewed the blood culture results and medical records of all patients with a history of visits or hospitalizations between April 2008 and september 2020. strong>Results:strong> During the study period, 4 blood culture specimens were considered to have s. saprophyticus bacteremia. Two of these were from subjects > 60 years old who had severe infection. A third case, 27 years old, was thought to have a catheter-related bloodstream infection;however, the only symptom was fever, which was not serious. The fourth case, a previously unreported pediatric patient, had non-severe fever. The mean time for a diagnosis of bacteremia by blood culture testing was 42 hours, whereas contamination appeared in cultures after another 50 hours. strong>Conclusion:strong> The pathogenicity of s. saprophyticus might be lower in the blood than in the urine due to its physiological function and activity. In older adults with underlying diseases, the severity of bacteremia was more pronounced, whereas in a young adult and a child, the disease was relatively mild. Age and underlying disease might be useful factors to consider when diagnosing bacteremia due to s. saprophyticus.

Influence of Adiponectin and Leptin on the Kinetics of Human Pulp Cells

Adipocytokines, such as adiponectin and leptin, are expressed by adipocytes and are known as anti-metabolic syndrome factors. They are also thought to mediate calcification in mineralized tissue. We investigated the effects of adiponectin and leptin on the kinetics of human pulp cells using ELIsA and western blot. After gaining informed consent, we obtained human pulp cells from three patients. After cultivation in Dulbecco’s Modified Eagle Medium (DMEM) without serum, cells from the 4th to 6th passages were incubated with various concentrations of adiponectin (1, 10, or 100 ng/mL) in DMEM or leptin (0.1, 1, or 10 ng/mL) in DMEM for 24 h. We confirmed that human pulp cells expressed adiponectin receptor 1 and leptin receptor. Although the proliferation index of these cells as measured by 5-bromo-2'-deoxyuridine (BrdU) incorporation increased in the presence of adiponectin in a dose-dependent manner, pulp cells stimulated with leptin showed no significant changes in BrdU incorporation. Alkaline phosphatase activity showed no significant changes after stimulation with either adipocytokine. Adiponectin induced expression of BMP-2 and osteopontin more strongly than did leptin. In particular, expression of BMP-2 increased in the presence of adiponectin in a dose-dependent manner. In contrast, the expression of dentin sialoprotein increased after stimulation with leptin. The expression of Runx2 was not observed from the cultured pulp cells stimulated with both molecules. These results indicate that adiponectin and leptin contribute to growth and differentiation of human pulp cells and may consequently affect the formation of secondary dentin or reparative dentin in the dentin-pulp complex.

Influence of Kupffer cells and platelets on ischemia-reperfusion injury in mild steatotic liver

AIM: To investigate the effect of mild steatotic liver on ischemia-reperfusion injury by focusing on Kupffer cells (KCs) and platelets. METHODs: Wistar rats were divided into a normal liver group (N group) and a mild steatotic liver group (s group) induced by feeding a choline-deficient diet for 2 wk. Both groups were subjected to 20 min of warm ischemia followed by 120 min of reperfusion. The number of labeled KCs and platelets in sinusoids and the blood perfusion in sinusoids were observed by intravital microscopy (IVM), which was performed at 30, 60 and 120 min after reperfusion. To evaluate serum alanine aminotransferase as a marker of liver deterioration, blood samples were taken at the same time as ***: In the s group, the number of platelets adhering to KCs decreased significantly compared with the N group (120 after reperfusion; 2.9±1.1 cells/acinus vs 4.8±1.2 cells/acinus, Ps in sinusoids was significantly less in the s group than in the N group throughout the observation periods (before ischemia, 19.6±3.3 cells/acinus vs 28.2±4.1 cells/acinus, Psion, 29.0±4.3 cells/acinus vs 40.2±3.3 cells/acinus, Psion of sinusoids 120 min after reperfusion was maintained in the s group more than in the N group. Furthermore, elevation of serum alanine aminotransferase was lower in the s group than in the N group 120 min after reperfusion (99.7±19.8 IU/L vs 166.3±61.1 IU/L, P=0.041), and histological impairment of hepatocyte structure was prevented in the s group. CONCLUsION: Ischemia-reperfusion injury in mild steatotic liver was attenuated compared with normal liver due to the decreased number of KCs and the reduction of the KC-platelet interaction.
