Upper Gastrointestinal Endoscopic Outcomes in Patients with Gastrointestinal symptoms: Retrospective Analysis at a Tertiary Care Hospital

Background: Endoscopic procedures are frequently performed to rule out any disease process which eventuates with either a positive or negative outcome. Association of patient demographics, clinical features with endoscopic outcomes are of paramount importance for better understanding and practice of endoscopy. Objectives: This study aimed to determine the frequency of positive endoscopic findings in patients with gastrointestinal symptoms compared to those with negative findings. As a secondary objective, the association between relevant demographics, symptoms, laboratory investigations and procedural outcomes of the participants and positive endoscopic findings were also assessed. Methods: A retrospective analysis of all the patients who underwent esophagogastroduodenoscopy from January 2017 to December 2017 was conducted. A total of 1066 endoscopy records were retrieved and using a convenience sampling technique, relevant data were manually entered to the questionnaires. Records with incomplete or inconsistent data were disregarded as per the exclusion criteria, resulting in 1011 patient records ultimately utilized for the study. All data were entered and analyzed using IBM sPss 23.0. Results: The study comprised a total of 1011 patients where 52.2% were females and 47.8% males while the mean age of patients was 42.16 ± 15.45. Positive endoscopic findings were observed in 88.1%, where the most common finding was gastritis (49.1%). Approximately 14.5% had positive investigation findings for HBsAg and 23.1% were seropositive for anti-HCV antibody. P-values statistically significant were for age (p = 0.044), hematemesis (p = 0.002), betel quid addiction (p = 0.044), anti-HCV antibody (p = 0.043), HCV-RNA (p = 0.041) and “H. pylori” antigen (p = 0.032). Conclusion: Both genders were almost equally included where most patients presented between the 3rd and 5th decades of life. A high incidence of gastritis was noted. Primary isolated esophageal Hodgkin’s lymphoma is seldom in localization span>and when seen, it is usually the non-Hodgkin’s type. We reported a 68-year-old, diabetic and ex-smoker, presented to our hospital complaining of progressive difficulty of swallowing for ninespan> months for both solid and liquid. He was on proton pump inhibitors. Recentlyspan>, he had fever and drenching night sweat and significant loss of weight. He was diagnosed as a case of isolated esophageal Hodgkin’s lymphoma that span style="font-family:Verdana;">was diagnosed based on endoscopic and radiological findings. Clinical manifestations, radiological imagines and histopathological features were described. Clinical management, fellow-up and outcome were precisely discussed.

We have examined primary tumor sections from melanoma patients by immunohistochmistry (IHC) for the presence of the odontogenic ameloblast-associated protein (ODAM). Within these patient tissues we have observed a correlation of nuclear ODAM staining in the primary tumors with sentinel lymph node (sLN) metastasis. surgically, sLN invasion in melanoma is considered an important indicator of more aggressive, invasive melanoma and to date there are limited biomarkers which strongly correlate with metastatic disease. The observation that ODAM staining in melanoma associates with sLN invasion may have important prognostic implications which could assist in the management of melanoma. Notably, ODAM expression may correlate with pathway-signaling we have previously reported to be affected by ectopic ODAM expression in cultured melanoma and breast cancer cell lines.

Patients with nonerosive gastroesophageal reflux disease often have relatively low esophageal acid exposure and respond suboptimally to gastric acid suppressi on. In these patients, other constituents of gastric contents may induce esophag eal *** have demonstrated that gastric contents can cause heartburn when the gastric pH > 4. (Aliment Pharm Ther 14:129-134, 2000). The aim of this stu dy was to determine relative sensitivities to chenodeoxycholic and ursodeoxychol ic acids, and 0.1 N HCl, administered as provocative perfusion tests. Patients w ith functional heartburn and healthy control subjects were evaluated. Patients u nderwent a modified Bernstein acid infusion test and esophageal Barostat balloon distention. Time and volume to pain were recorded. Barostat balloon distention was performed using our standard ***- wise distentions were performed and pain was *** to chenodeox ycholic acid (Cheno) and Ursodeoxycholic acid (Urso) were assessed similarly to the Bernstein test using 2 mM concentrations of each, followed immediately by 5 mM if no pain was reported with 2 mM. Volume of bile acid infusion and length of time until pain was induced were assessed and compared to the same endpoints fo r acid sensitivity. “Total"time and “total" volume to induce pain were calcula ted for Cheno and Urso. Least-squares means were generated and twotailed t-tes ts and regression analyses were performed (Psignificance). Ten functional heartburn patients and six healthy contr ols were evaluated (3M, 13 F; age range, 19 to 56 years). since five of six cont rols had pain with ac- id infusion(hypersensitive), all subjects were analyzed as one group. Only thr ee subjects (all controls) had no pain with infusion of 2 mM Cheno and received the follow-up infusion of 5 mM. Characterization of Embryonic stages through Variations in the Egg＇s Contents in spodoptera fitura

This paper describes the embryonic development of the common cutworm spodoptera litura （Fabricius）（Lepidoptera： Noctuidae） which is divided into eight embryonic stages, through variations in the egg＇s contents. Experiments were conducted on the eggs ofs. litura under laboratory conditions. The highest concentration of the protein （min.-max.： 32-! 40 lag/egg） and the lowest of the carbohydrate （min.-max.： 3.0-44.0 lag/egg） were obtained; similarly, the DNA content was smaller （min.-max.： 1.0-38.2 lag/egg） than the RNA （min.-max.： 9.2-360.0 lag/egg） throughout the embryogenesis. Three peaks of ecdysteroid titre were observed which were responsible for appearance of particular events during embryogenesis. The last highest peak of 400_＋33.4 lag/egg was appeared at 84 h after oviposition when larva was ready to hatch. Therefore, it is concluded that eight embryonic stages in s. litura were characterized by fluctuation in the egg＇s contents, which were responsible for gradual-progressive embryonic development. The egg-larval development did not show a normal distribution of embryonic developmental-time.

Objective:To better evaluate tertiary Gleason pattern reporting and to evaluate the impact of tertiary Gleason pattern 5(TP5)on prostate cancer pathological features and biochemical recurrence at our large single ***:We retrospectively reviewed 1962 patients who underwent radical prostatectomy(RP)for prostate cancer;TP5 was reported in 159 cases(8.1%).Men with Gleason score(Gs)7 and Gs 8 disease were divided into subgroups with and without TP5,and histopathological features were *** analyses were conducted to assess the impact on TP5 on biochemical-free survival(BFs).Results:Tumors possessing Gs 3+4 with TP5 were more likely to exhibit extraprostatic extension(EPE)and had a larger tumor diameter(TD)than Gs 3+4 *** 3+4 with TP5 was also associated with positive surgical margins(sM),seminal vesicle involvement(sVI),and higher pre-operative prostate-specific antigen(PsA)values,but without statistical *** 4+3 with TP5 more commonly presented with EPE,positive sM,sVI,and greater TD and pre-operative PsA level than Gs 4+3 *** multivariate analysis,Gleason score,EPE,and TP5 were overall independent risk factors for PsA recurrence in this ***,Gs 4+3 with TP5 was associated with shorter time to recurrence versus Gs 4+3 ***:Our results emphasize the importance of TP5 and suggest that criteria for tertiary pattern reporting in prostate cancer should be *** studies are needed to evaluate the role of tertiary patterns in prognostic models.

Crohn's disease with gastroduodenal involvement: Diagnostic approach

Crohn's disease(CD) is a chronic idiopathic inflammatory disease of gastrointestinal tract characterizedby segmental and transmural involvement of gastrointestinal tract. Ileocolonic and colonic/anorectal is a most common and account for 40% of cases and involvement of small intestine is about 30%. Isolated involvement of stomach is an extremely unusual presentation of the disease accounting for less than 0.07% of all gastrointestinal CD. To date there are only a few documented case reports of adults with isolated gastric CD and no reports in the pediatric population. The diagnosis is difficult to establish in such cases with atypical presentation. In the absence of any other source of disease and in the presence of nonspecific upper gastrointestinal endoscopy and histological findings, serological testing can play a vital role in the diagnosis of atypical CD. Recent studies have suggested that perinuclear anti-neutrophil cytoplasmic antibody and anti-saccharomycescervisia antibody may be used as additional diagnostic tools. The effectiveness of infliximab in isolated gastric CD is limited to only a few case reports of adult patients and the long-term outcome is unknown.

Does music reduce anxiety and discomfort during flexible sigmoidoscopy?A systematic review and meta-analysis

To investigate the role of music in reducing anxiety and discomfort during flexible sigmoidoscopy. METHODsA systematic review of all comparative studies up to November 2016, without language restriction that were identified from MEDLINE and the Cochrane Controlled Trials Register (1960-2016), and EMBAsE (1991-2016). Further searches were performed using the bibliographies of articles and abstracts from major conferences such as the EsCP, NCRI, AsGBI and AsCRs. MesH and text word terms used included “sigmoidoscopy”, “music” and “endoscopy” and “anxiety”. All comparative studies reporting on the effect of music on anxiety or pain during flexible sigmoidoscopy, in adults, were included. Outcome data was extracted by 2 authors independently using outcome measures defined a priori. Quality assessment was performed. REsULTsA total of 4 articles published between 1994 and 2010, fulfilled the selection criteria. Data were extracted and analysed using OpenMetaAnalyst. Patients who listened to music during their flexible sigmoidoscopy had less anxiety compared to control groups [Random effects; sMD: 0.851 (0.467, 1.235), s.E = 0.196, P s no statistically significant heterogeneity (Q = 0.085, df = 1, P = 0.77, Isup>2sup> = 0). Patients who listened to music during their flexible sigmoidoscopy had less pain compared to those who did not, but this difference did not reach statistical significance [Random effects; sMD: 0.345 (-0.014, 0.705), s.E = 0.183, P = 0.06]. Patients who listened to music during their flexible sigmoidoscopy felt it was a useful intervention, compared to those who did not (P s no statistically significant heterogeneity (P = 0.528, Isup>2sup> = 0). CONCLUsIONMusic appeared to benefit patients undergoing flexible sigmoidoscopies in relation to anxiety and was deemed a helpful intervention. Pain may also be reduced however further investigation is required to ascertain this.

Objective sulfanilamide,sulfadiazine,and dapsone were the first sulfonamides to be used to treat malaria by disrupting the folate biosynthesis process,which is essential for parasite ***,we aimed to synthesize novel N-(2-arylaminophenyl)-2,3-diphenylquinoxaline-6-sulfonamide derivatives through a rational drug design *** All compounds were synthesized by the conventional method,and the products were characterized by spectral analysis(1 H NMR and mass spectrometry).The progression of the reaction was monitored using thin-layer chromatography(TLC).All the derivatives were analyzed for their effective binding mode in the allosteric site of the plasmodium cysteine protease *** and antifungal activities were determined using the broth dilution *** s6(N-(2-thiazol-4 yl)-acetyl-aminophenyl)-2,3-diphenylquinoxaline-6-sulfonamide and s9(N-(1 H-benzo[d]imidazol-2-yl)aminophenyl)-2,3-diphenylquinoxaline-6-sulfonamide formed five hydrogen bonds;s8(N-(2-1 H-imidazol-2 yl)aminophenyl)-2,3-diphenylquinoxaline-6-sulfonamide and s10(N-(1 H-benzo[d]imidazol-5-yl)aminophenyl)-2,3-diphenylquinoxaline-6-sulfonamide formed four hydrogen bonds with the allosteric site of the *** the docking scores and formation of hydrogen bonds with the target enzyme,the novel derivatives were processed for wet lab *** the newly synthesized derivatives were subjected to in vitro antimalarial,antifungal,and antibacterial *** the derivatives exhibited sufficient sensitivity to the Plasmodium falciparum strain compared to the ***,compounds s9 and s10 showed the most potent dual antimicrobial and antimalarial *** also exhibited powerful molecular interactions in molecular docking *** Based on the above results,it was concluded that N-(2-arylaminophenyl)-2,3-diphenylquinoxaline-6-sulfonamide derivatives have excellent biological potential to act as antimalarial,antifungal,
