Clinicopathological Profile of Lung Cancers at an Institute from south India—A Record Based Retrospective Cohort study

Introduction: Apart from smoking as the known risk factor for lung cancer, recent developments implicate occupational exposure to carcinogens, indoor air pollution and dietary factors as other causative agents. In our study, we have analyzed the clinical and pathological profile of lung cancer patients treated at our center over a period of 8 years. Aim: To find the demographic and clinicopathological profile of lung cancer patients admitted to the oncology unit. Methods: This retrospective record based analysis includes a cohort of 1248 patients diagnosed with lung cancer, at a tertiary cancer care center in Bangalore, south India. This study includes data of patients admitted during the period 2010 to 2018, retrieved from the Hospital’s emergency medical records (EMR). Their demographic profile, clinical correlates, radiological profile and diagnostic details were studied. Benign tumours, malignant pleural disease and sarcomatoid tumors were excluded from this study. Results: Adenocarcinoma (AC) was detected among 70.4% of patients, squamous cell carcinoma (sCC) among 15.3% and small cell lung cancer (sCLC) among 14.3% of lung cancer admissions. Male to female ratio was 2.95:1. It was found that the median age of lung cancer patients was 61 years. >60% of lung cancer patients were from the 51 - 70 year age-group. Across the three types of cancers among the patients in stage 3B to 4 strata, >35% received palliative chemotherapy and >20% received palliative chemotherapy + radiation. Conclusion: In our study, AC is the most common histological subtype (>70%) of lung cancer. The outcome of lung cancer patients can be considerably impacted by addressing risk factors through preventive measures implemented in the community.

Isolation of Two Furanoditerpenes and Two Triterpenes from the stem Bark of <i>Dalbergia lanceolaria</i>L.f.

stem bark of Dalbergia lanceolaria was subjected to methanolic extraction and extensive phytochemical separation. The process yielded four known compounds namely Jateorin (1), Columbin (2), β-sitosterol (3), and Lupeol (4). The compounds were identified using nuclear magnetic resonance spectral data namely 1H NMR, 13C NMR, 1H-1H COsY, HMBC, and DEPT-135 and verified by comparing the data with previous literature. All four compounds are being reported for the first time from the species D. lanceolaria. Furthermore, two furanoditerpenes namely Jateorin and Columbin are reported for the first time from the genus Dalbergia. This may lead to potential identification of a new chemotaxonomic marker for this genus.

Association of Vitamin D Deficiency with Diabetic Retinopathy in Young People with Type 1 Diabetes Mellitus

Background: Diabetic retinopathy is among the most common diabetic complications, and is one of the leading causes of blindness in the world. Recent studies have linked vitamin D to the pathogenesis of diabetes and there is growing evidence that vitamin D can interfere with the mechanisms involved in diabetes and its complications. Despite improvements in treatment, diabetic retinopathy remains a significant complication of type 1 diabetes mellitus. Identification of early treatable predictors of diabetic retinopathy such as vitamin D deficiency, may allow more aggressive management of those at high risk. Purpose: To assess the association of vitamin D deficiency with diabetic retinopathy in young people with type 1 diabetes mellitus. Design: Observational study with case control design. Method: 60 young people with type 1 diabetes aged between 11 to 24 years were included in this study. Among them, 30-young people have diabetic retinopathy and 30-young people do not have diabetic retinopathy. Purposive sampling technique was applied as per inclusion criteria. statistical analysis of the results was done by using computer-based software, sPss version 26. P value of less than 0.05 was considered as statistically significant. Results: Vitamin D deficiency was present in 83% of the young people with diabetic retinopathy and in 53% without diabetic retinopathy. The mean vitamin D level in young people with and without diabetic retinopathy was 17.38 ± 3.77 ng/ml and 20.15 ± 5.06 ng/ml respectively and the difference was statistically significant (p = 0.019). Vitamin D deficiency was increased with the severity of diabetic retinopathy. Univariate and multivariate logistic regression showed vitamin D deficiency was independently associated with diabetic retinopathy with a crude odds ratio of 5.69 with a p value of 0.008 and adjusted odds ratio of 16.08 with a p value of 0.002 respectively. An Assessment of Genetic Relatedness between soybean [<i>Glycine max</i>(L.) Merrill] Cultivars Using ssR Markers

In this present investigation, simple sequence repeat (ssR) analysis was used to determine the genetic relatedness among 32 soybean (Glycine max (L.) Merr.) cultivars from NRC for soybean, Madhya Pradesh. Among them 10 primer pairs showed 100% amplification. Mainly three primer pairs could amplify polymorphic ssRs from all of these cultivars. The polymorphic information content (PIC) among cultivars varied from 0.21 (s26) to 0.83 (s27) with an average of 0.51. Pairwise coefficients of genetic similarity between all genotypes ranged from 0.76 to 1.00. Unweighted pair-group method arithmetic average (UPGMA) analysis allocated the cultivars in 2 major clusters or groups and 6 sub-clusters. Of the two major clusters one contained 20 cultivars and the other contained 12 cultivars. The largest cluster was again divided into three sub-clusters I, II and III with 12, 2 and 6 cultivars respectively and the smallest cluster was divided into three sub-clusters IV, V, VI containing 7, 4 and 2 cultivars respectively. These results suggest that ssR markers are efficient for measuring genetic relatedness among soybeans irrespective of a wide agro-climatic zone. Genetic relationship assessments among soybean cultivars in India could provide useful information for efficient utilization of these materials, especially for widening the genetic base.

Refractive index (RI) sensing helps to identify biomolecules and chemicals in the mid-infrared range for drug discovery, bioengineering, and environmental monitoring. In this paper, we numerically demonstrate an electrically tunable RI sensor with ultrahigh sensitivity using a three-layer graphene nanoribbon array separated by hexagonal boron nitride (hBN). Unlike the weak resonance in single-layer graphene nanoribbons, a much stronger plasmon resonance featuring a higher-quality factor can be excited in the graphene/hBN few-layer ribbon array. simultaneously, the high purity of graphene on hBN results in an outstanding charge mobility above 4 ×10^4 cm^2 · V^-1·s^-1 at 300 K, which allows a larger modulation depth. The interaction between the locally enhanced field around graphene ribbons and its surrounding analyte leads to ultrahigh sensitivity (4.207μm/RIU), with the figure of merit reaching approximately ***, this ultrasensitive detector could selectively work in different wavebands by controlling gate voltages applied to *** merits of ultrahigh sensitivity and electrical tunability are major advances compared to previous RI sensors, paving a way toward ultrasensitive detection using graphene/hBN few-layer devices.

Development of Moisture and Ash Based Correlation for the Estimation of Mineral Matter in High Ash Indian Coal

The experimental determination of mineral matter in coal is a tedious as well as time consuming process, and requires highly skilled analyst to carry out the chemical analysis of coal and ash. On the other hand, determination of moisture and ash is relatively easy using moisture oven and muffle furnace and can be determined anywhere with little care. Most of the methods reported till date for the evaluation of mineral matter in coal involves indirect determination of mineral matter using high temperature ash as one parameter. All these methods offer conversion of ash yield into mineral matter with the help of correlations based on certain assumptions. Although, the method for direct determination of mineral matter by chemical analysis is also reported, but is of little use as it requires extensive chemical analysis. In this study mineral matter is determined directly by oxygen plasma asher in coal samples collected from different regions of India. The results obtained were compared with those obtained by existing correlations. It has been observed that all the existing correlations including Parr’s, are found to be unrealistic for high ash Indian coal. An attempt has been made to develop a new correlation for the estimation of mineral matter based on two simple parameter i.e. moisture content and ash yield. Present model was developed after analyzing 75 coal samples. The developed correlation appears to be more simple and better and is represented as Mineral Matter(MM)= 0.74(Moisture) + 1.17(Ash)

Laboratory Capacity for surveillance of Infectious Diseases in Gujarat: Quantity, Quality, Effects and Way Forward

Background: India carries one of the largest burdens of infectious diseases in the world. To estimate this, laboratory confirmation is vital. We estimated the lab capacity and effectiveness in the state of Gujarat for Enteric Fever, Infectious Hepatitis, and Dengue. Methods: We estimated the number of labs in the state through telephonic surveys and physical screening of a representative sample of labs. We created four levels of tests, Level-0 being no test and 3 being the best confirmatory test available in the state. For the profusion of rapid diagnostic test kits (RDTKs), we constructed Effective Diagnostic scores (EDs) calculated from their sensitivity and specificity at disease prevalences specified in the literature. Tests with EDs > 0.51 were level-2 tests, and EDs Results: Our analysis showed that there are 4293 labs in the state (1765 public and 2528 private), 7/100,000 population. However, only 2878 labs contributed to a total pooled Effective Lab Diagnostic score (ELDs) of 6776 in the state. strikingly, 94% of the lab effectiveness lay in RDTs (level-2 and 1 tests) which are essentially screening tests. Ninety-six percent of the overall lab effectiveness of Gujarat existed in private and only 4% in public labs. Contrarily, the level-3 confirmatory testing effectiveness, through ELIsA and culture constituted only 4% of private and 36% of public lab effectiveness. More than half of the private lab effectiveness was located in eight Tier 1 cities. Level-3 confirmatory testing effectiveness was present only in Tier 1 and 2 towns. Hepatitis B testing contributed 34% of the total ELDs, followed by Dengue (30%), Enteric Fever (26%) and Hepatitis A and E (10%). Conclusion: Our study has established that the capacity and effectiveness of the lab network in Gujarat lie predominantly in RDTKs. We need to adapt our systems to capture this data in a manner that will allow us to monitor the burdens of these diseases.

Background/Purpose: Isolated splenic abscesses are rare in pediatric patients. The recommended treatment in the literature has been in favor of splenectomy, although conservative treatment with splenic preservation is being increasingly reported. We report successful management of 4 pediatric patients with splenic abscess by needle aspirations and antibiotics. Materials and Methods: Four children (aged 7-11 years; male-female, 3:1) were admitted in our institution with history of high-grade fever with chills, anorexia, left hypochondrial pain, and splenomegaly. One child was a known case of thalassemia, and one had a history of typhoid fever. The others did not have any predisposing condition. Ultrasonography (UsG) and computed tomographic scan of the abdomen showed a solitary abscess in the spleen in 2 patients and multiple abscesses in the other 2. Ultrasonography-guided needle aspiration in 3 cases revealed purulent fluid, which, on culture, grew Escherichia coli in 1 case, salmonella paratyphi A in 1 case, but sterile in 1 case. Blood culture was sterile in all the cases, but Widal’s test was positive in 2 patients. Treatment protocol included UsG-guided needle aspiration of pus along with intravenous ceftriaxone, metronidazole, and amikacin for 3 to 12 weeks. Results: All 4 patients showed a good response to conservative treatment. serial UsG showed gradual resolution of abscess, and none was subjected to splenectomy. Conclusion: Isolated splenic abscess in children can be successfully treated with needle aspirations and intravenous antibiotics, thereby avoiding splenectomy.

strong>Background:strong> Dry eye is characterized by tear film instability, decreased tear volume and a high Ocular surface Disease Index (OsDI) score. Face masks have been linked to dry eye complaints in recent studies. strong>Purpose:strong> To evaluate the changes in tear-film status and Ocular surface Disease Index (OsDI) score following prolonged use of face mask. strong>Design:strong> Cross-sectional study. strong>Method:strong> Patients between 18 to 70 years using masks regularly for at least 2 hours/day for at least 5 days/week from different eye-care centres in Dhaka were included. All subjects were divided into 4 groups. Mask use lasted approximately 2 hours/day in Group 1, 4 hours/day in Group 2, 6 hours/day in Group 3 and 8 hours or more/day in Group 4. Evaluation of symptoms, tear- film stability and tear volume was done by Ocular surface Disease Index (OsDI) scores, Tear-film Break-Up Time (TBUT) and schirmer-1 test. Average of right & left eye’s Tear-film Break Up time (TBUT) and schirmer-1 value were noted. strong>Results:strong> Total 100 patients were enrolled (n = 100).
