Endoscopic treatment of biliopancreatic pathology in patients with Whipple's pancreaticoduodenectomy surgical variants: Lessons learned from single-balloon enteroscopy-assisted ERCP

Background: Endoscopic treatment of biliopancreatic pathology is challenging due to surgically altered anatomy after Whipple's pancreaticoduodenectomy. This study aimed to evaluate the feasibility and safety of single-balloon enteroscopy-assisted endoscopic retrograde cholangiopancreatography(sBE-ERCP) to treat biliopancreatic pathology in patients with Whipple's pancreaticoduodenectomy surgical variants. methods: We retrospectively analyzed 106 sBE-ERCP procedures in 46 patients with Whipple's variants. Technical and clinical success rates and adverse events were evaluated. Results: Biliary sBE-ERCP was performed in 34 patients and pancreatic sBE-ERCP in 17, including 5 with both indications. From a total of 106 sBE-ERCP procedures, 76 were biliary indication with technical success rate of 68/76(90%) procedures and clinical success rate of 30/34(88%) patients. mild adverse event rate was 8/76(11%), without serious adverse events. From a total of 106 sBE-ERCP procedures, 30 were pancreatic indication with technical success rate of 24/30(80%) procedures( P = 0.194 vs. biliary sBEERCP) and clinical success rate of 11/17(65%) patients( P = 0.016 vs. biliary sBE-ERCP). mild adverse event rate was 6/30(20%)( P = 0.194 vs. biliary sBE-ERCP), without serious adverse events. After sBE-ERCP failure, endoscopic ultrasound-guided drainage, percutaneous drainage and redo surgery were alternative therapeutic options. Conclusions: Biliopancreatic pathology after Whipple's pancreaticoduodenectomy variants can be treated using sBE-ERCP without serious adverse events. Technical and clinical success rates are high for biliary indications, whereas clinical success rate of pancreatic indications is significantly lower. sBE-ERCP can be considered as first-line treatment option in this patient group with surgically altered anatomy.

Removing harvest residues from hardwood stands affects tree growth, wood density and stem wood nutrient concentration in European beech (Fagus sylvatica) and oak (Quercus spp.)

Background:Higher exportation of harvest residues from forests due to increased demand for woody biomass,has reportedly diminished soil mineral resources and may lead to degraded tree nutrition as well as ***,as nutrients become less available in the soil,the remobilization of nutrients in biomass tissues(plant internal cycling)helps sustain tree *** study aims to quantify the impact of Removing Harvest Residues and Litter(RHRL)during five years on tree growth,wood density,and stem wood nutrient concentrations in young beech and oak forest ***:Our study found that,RHRL significantly decreased tree growth ring width by 14%,and wood density by 3%,in beech trees,in near bark *** also significantly reduced nutrient concentration in near bark and near pith areas of both studied ***,Na and s were found lower by 44%,76%,and 56%,respectively,in near bark area of beech *** near bark area of oak trees,K,Ca,mg,Na,s,and Fe were lower by 20%,25%,41%,48%,41%,and 16%,respectively.K and mg concentrations decreased more strongly in near pith area compared to near bark area suggesting internal translocation of these two ***:In beech trees,wood density proved to be an important factor while quantifying the effect of removing harvest residuals on tree growth and *** nutrient loss intensified the remobilization of nutrients con-tained in older tree rings(close to the pith)towards newly formed rings(close to bark).In our study,in beech trees,K was found to be the most recycled major *** results demonstrate the potential of such analysis for providing valuable insight into the effect of RHRL in premature stands on the physiological adaptive strategies of trees and an indication of soil fertility status.

sporadic venous malformations are genetic conditions primarily caused by somatic gain-of-function mutation of PIK3CA or TEK,an endothelial transmembrane receptor signaling through *** malformations are associated with pain,bleedings,thrombosis,pulmonary embolism,esthetic deformities and,in severe cases,life-threatening *** authorized medical treatment exists for patients with venous ***,we created a genetic mouse model of PIK3CA-related capillary venous malformations that replicates patient *** showed that these malformations only partially signal through AKT *** compared the efficacy of different drugs,including rapamycin,a mTORC1 inhibitor,miransertib,an AKT inhibitor and alpelisib,a PI3Kαinhibitor at improving the lesions seen in the mouse *** demonstrated the effectiveness of alpelisib in preventing vascular malformations’occurrence,improving the already established ones,and prolonging *** these findings,we were authorized to treat 25 patients with alpelisib,including 7 children displaying PIK3CA(n=16)or TEK(n=9)-related capillary venous malformations resistant to usual therapies including sirolimus,debulking surgical procedures or percutaneous *** assessed the volume of vascular malformations using magnetic resonance imaging(mRI)for each *** demonstrated improvement in all 25 *** malformations previously considered intractable were reduced and clinical symptoms were *** showed a decrease of 33.4%and 27.8%in the median volume of PIK3CA and TEK malformations respectively,over 6 months on *** conclusion,this study supports PI3Kαinhibition as a promising therapeutic strategy in patients with PIK3CA or TEK-related capillary venous malformations.

The simplest picture of excitons in materials with atomic-like localization of electrons is that of Frenkel excitons,where electrons and holes stay close together,which is associated with a large binding ***,using the example of the layered oxide V_(2)O_(5),we show how localized charge-transfer excitations combine to form excitons that also have a huge binding energy but,at the same time,a large electron-hole distance,and we explain this seemingly contradictory *** anisotropy of the exciton delocalization is determined by the local anisotropy of the structure,whereas the exciton extends orthogonally to the chains formed by the crystal ***,we show that the bright exciton goes together with a dark exciton of even larger binding energy and more pronounced *** findings are obtained by combining first principles many-body perturbation theory calculations,ellipsometry experiments,and tight binding modelling,leading to very good agreement and a consistent *** explanation is general and can be extended to other materials.

Nanoscale resetting of the Th/Pb system in an isotopically-closed monazite grain: A combined atom probe and transmission electron microscopy study

Understanding the mechanisms of parent-daughter isotopic mobility at the nanoscale is key to rigorous interpretation of Ue The Pb data and associated dating. Until now, all nanoscale geochronological studies on geological samples have relied on either Transmission Electron microscope(TEm) or Atom Probe microscopy(APm) characterizations alone, thus suffering from the respective weaknesses of each technique. Here we focus on monazite crystals from a ~1 Ga, ultrahigh temperature granulite from Rogaland(Norway). This sample has recorded concordant UeP b dates(measured by LA-ICP-ms) that range over 100 my, with the three domains yielding distinct isotopic Ue Pb ages of 1034 ± 6 ma(D1; srich core), 1005 ± 7 ma(D2), and 935 ± 7 ma(D3), respectively. Combined APm and TEm characterization of these monazite crystals reveal phase separation that led to the isolation of two different radiogenic Pb(Pb*) reservoirs at the nanoscale. The s-rich core of these monazite crystals contains Cae srich clusters, 5 -10 nm in size, homogenously distributed within the monazite matrix with a mean interparticle distance of 40 -60 nm. The clusters acted as a sink for radiogenic Pb(Pb*) produced in the monazite matrix, which was reset at the nanoscale via Pb diffusion while the grain remained closed at the micro-scale. Compared to the concordant ages given by conventional micro-scale dating of the grain,the apparent nano-scale age of the monazite matrix in between clusters is about 100 myr younger, which compares remarkably well to the duration of the metamorphic event. This study highlights the capabilities of combined APm-TEm nano-structural and nano-isotopic characterizations in dating and timing of geological events, allowing the detection of processes untraceable with conventional dating methods.

The aim is to propose a dynamic model of forest growth and biomass suitable to varied ecosystems with different species,soil types,climate conditions and forest *** model is combining different approaches(growth & yield,process-based and biogeochemical cycles) to take into account carbon,water and nutrient cycles and to include several processes such as wood production,transpiration,litterfall,litter decomposition or losses of nutrients by *** a model is necessary to anticipate and adapt forest management under different environmental and management scenarii(global changes).Considering the whole forest ecosystem,the seasonality of canopy dynamics and litterfall production is involved in key processes:photosynthesis and carbon production,stand transpiration and water cycle,litter decomposition and nutrient cycling.A dynamical probabilistic model for leaves demography has been *** model is strongly constrained by environmental factors and is able to rank their *** adjustment can highlight relationships between different scales of processes involved,from cellular-scale to *** aim is to provide:i) a predicting model suitable to a large range of ecosystems,ii) hierarchical analyses of the environmental processes driving canopy dynamics.
