Hyperuricemia as a Predictor of Progression of Chronic Kidney Disease: A Matched Cohort Analysis

Background: The clinical significance of uric acid (UA) in patients with chronic kidney disease (CKD) has never been elucidated in detail. Objectives: The aim of this study was to elucidate the predictive value of hyperuricemia (HU) in relation to kidney disease progression in CKD patients. Methods: We reviewed data obtained from 201 CKD patients. Renal outcome was assessed by performing the Kaplan-Meier and Cox proportional hazard regression models. The renal outcomes of CKD patients with and without HU were compared by using a propensity score-matched cohort. Results: The results of a multivariate Cox regression analysis showed that HU (UA ≥ 7.1 mg/dL) (P = 0.001), diabetes mellitus (P = 0.003), and a high urine albumin-to-creatinine ratio (P = 0.0005) were significantly associated with a ≥50% eGFR decline or EsRD. The serum UA values were positively correlated with mean blood pressure (P = 0.007), visceral fat area (P = 0.0003), and maximum carotid intima-media thickness (P = 0.0003). The Kaplan-Meier analysis in the propensity score-matched cohort indicated that the renal survival rate of the group of CKD patients with HU was significantly lower than that of the group without HU (log rank, P Conclusion: The results of this study suggest that the serum UA value is a predictor of CKD progression.

We measured the spatial variability of Cl -, Na +, sO 2- 4, NO - 3 and 7Be concentrations in snow cover along the traverse route from s16 to Dome Fuji station in east Dronning Maud Land, Antarctica. The concentrations of Cl - and Na + at the coast exponentially decreased with distance from the coast up to 200 km inland. Between 200 1000 km inland, the concentrations of Na + were nearly constant. On the other hand, the concentrations of Cl - increased with distance beyond 750 km inland. Furthermore, the contribution factors between Cl - and Na + decreased and the Cl -/Na + ratio and the nssCl -/Cl - ratio increased toward the interior of the continent. These results indicate that sea salt aerosols are transported from the coastal region toward the inland region of the Antarctic continent, and that a source of Cl - other than sea salt exists in the inland plateau of Antarctica. The concentrations of sO 2- 4, NO - 3 and 7Be in snow were relatively high in the coastal region and the inland plateau region compared with those in the katabatic wind region.

Objective: To evaluate the potential of local mosquitoes to act as vectors for dengue transmission in ***: serotype 2 Th NH28/93 was used to test the dengue susceptibility profiles of Aedes flavopictus miyarai(Ae. f. miyarai), Aedes galloisi(Ae. galloisi) and Aedes albopictus(***), which were collected in Japan. We used Aedes aegypti from Thailand as a positive control. The mosquitoes were infected with the virus intrathoracically or orally. At 10 or 14 days post infection, the mosquitoes were dissected and total RNA was extracted from their abdomens, thoraxes, heads and legs. Mosquito susceptibility to dengue virus was evaluated using RT-PCR with dengue virus-specific primers. Differences in the infection and mortality rates of the different mosquito species were tested using Fisher's exact probability ***: The infection rates for dengue virus administered intrathoracically to Ae. f. miyarai,Ae. galloisi and Aedes aegypti mosquitoes were identical by RT-PCR on Day 10 post *** of the body parts we tested were RT-PCR-positive for dengue virus. For the orally administered virus, the infection rates in the different body parts of the Ae. f. miyarai mosquitoes were slightly higher than those of Ae. albopictus mosquitoes, but were similar to the control mosquitoes(P > 0.05). The mortality rates for Ae. f. miyarai and Ae. albopictus mosquitoes were similar(P = 0.19). Our data indicated that dengue virus was able to replicate and disseminate to secondary infection sites in all of the four mosquito species(Japanese and Thai).Conclusions: Ae. albopictus is a well-known candidate for dengue transmission in Japan. However, our data suggest that Ae. f. miyarai from Ishigaki Island(near Okinawa Island) and Ae. galloisi from Hokkaido(Northern Japan) should also be regarded as potential vectors for dengue transmission in these regions. Further studies on these mosquitoes should be conducted.

The use of memory metallic stents for the urinary tract in pediatric patients has not been reported. The authors report on 2 patients with urinary tract diso rders who were successfully treated with a memory metallic stent. A thermoexpand able, nickel- titanium alloy stent was placed at the urethroureteral junction o f a 4- year- old boy with ureteral stenosis associated with cloacal exstrophy for 18 months and at the urethra of a 2- year- old girl with ischuria after a repair of cloacal anomaly for 6 months. Temporary insertion of a memory metallic stent is a safe and effective alternative for organic stricture or functional o bstruction of the urinary tract in pediatric patients.

Purpose: Obstructive apnea is sometimes seen in patients with Beckwith- Wied emann syndrome. The cause of apnea is not limited to macroglossia, and the surgi cal indication for obstructive apnea has not yet been established. The authors p erformed polysomnography for the assessment of apnea. Method: Overnight polysomn ograms were obtained in 2 patients who developed obstructive apnea after 1- sta ge repair for omphalocele. Case 1: Apnea index (AI), de fined as apneic events per hour, indicated 17.3, and spO2 below 95% occupie d 80% of the total sleep time. Computed tomography and magnetic resonance imag ing indicated obstruction of the airway between macroglossia and the hypopharynx . Central tongue resection and the division of the frenulum linguae for associat ed ankyloglossia were performed 97 days after birth. One month after surgery, ap neic events disappeared and spO2 below 95% occupied only 1% of the total sle ep time. Case 2: Obstructive AI indicated 28.1. Division of the frenulum linguae and anterior glossopexy were performed 55 days after birth. Postoperative polys omnogram indicated a marked reduction of AI. Conclusions: These results indicate d that polysomnography was useful for evaluating obstructive apnea and that adva ncement of the tongue by division of the frenulum linguae may be recommended for the treatment of obstructive apnea in patients with Beckwith- Wiedemann syndro me.

Background: FertiQoL, which measures the Quality of Life (QOL) of reproductive partners, has been translated and used in 45 languages in the world. The reliability and validity of the original English version of FertiQoL have been confirmed. However, there is still no report on the reliability and validity of the Japanese version by a large-scale survey. This study aimed to examine the reliability and validity of the Japanese version of the FertiQoL scale for measuring the QOL of patients with reproductive problems. Methods: An unsigned self-filled questionnaire survey was conducted in patients undergoing infertility treatment at seven facilities in the Kanto area in Japan using the 34 items of the Japanese version of the FertiQoL scale. The study design was quantitative cross-sectional descriptive research. The investigation period was from April 2013 to April 2018. The contents of the investigation were attributes, FertiQoL scale, and distress scale. To determine the construct validity, principal component analysis, confirmatory factor analysis, and correlation analysis for each subscale were performed using sPss statistics Ver. 23.0 and AMOs Ver. 23.0. The study was performed after obtaining approval from the Research Ethics Review Committee of the responsible institutions to which the researchers belong as an ethical consideration. Results: The participants included 1201 patients undergoing infertility treatment and who provided valid responses. The Cronbach’s α was 0.92, and confirmatory factor analysis identified six domains with 34 items that showed the following values: goodness of fit index = 0.877, adjusted goodness of fit index = 0.855, comparative fit index = 0.893, and root mean square error of approximation = 0.059. The correlation coefficient was 0.669 (p Conclusions: The Japanese version of FertiQoL showed adequate reliability and validity for assessing the QOL of patients with reproductive problems in Japan.

本文报道了￣（８５）Ｓｒ、￣（１３４）Ｃｓ和￣（６０）Ｃｏ在采自本院野外试验场，从地表到地下２ｍ深黄土与地下水间分配系数的测定结果，以及核素浓度、ｐＨ和温度对分配系数的影响。黄土采集的深度范围分别为：地表－０．５ｍ，０．５－１ｍ，１－１．５ｍ，１．５－２ｍ，共采集１６个样品；地下水取自试验场的井水。实验采用静态法。实验结果表明，１６个样品测定的￣（８５）Ｓｒ、￣（１３４）Ｃｓ和￣（６０）Ｃｏ的分配系数的均值分别为８２、７．４×１０￣３和５．４×１０￣３ｍＬ／ｇ；在不同深度的黄土中测得的分配系数基本一致。ｐＨ值的变化对￣（１３４）Ｃｓ和￣（８５）Ｓｒ的分配系数有较大影响，对￣（６０）Ｃｏ则影响不大。

To examine whether bacterial endotoxin is detectable in menstrual effluent and to analyze a possible association between endotoxin levels and a pregnancy rate after IVFET. Prospective observational study. University hospital. Thirtyeight infertile women undergoing endotoxin assay and IVFET. Endotoxin was assayed by the limulus amoebocyte lysate test. Levels of bacterial endotoxin and a pregnancy rate. In 38 samples of menstrual effluent taken from 38 women, bacterial endotoxin was detected with a range of 7.1 to >1,000 pg/mL in 37 samples and was not detected in 1 sample. After IVFET, pregnancy occurred in 9 of the 38 women. The mean (±.sD) endotoxin level in these 9 pregnant women was 71.3 ±.52.5 pg/mL and was significantly lower compared with >236.2 ±333.6 pg/mL in the 29 nonpregnant women. All pregnancies occurred in 28 women with an endotoxin level of ≤200 pg/mL, whereas no pregnancies occurred in 10 women with an endotoxin level of >200 pg/mL, producing the significantly higher pregnancy rate in the former group than in the latter. Bacterial endotoxin was detectable in menstrual effluent from infertile women. The pregnancy rate after IVFET was significantly higher in women with an endotoxin level of ≤200 pg/mL than in women with an endotoxin level of >200.0 pg/mL.
