Therapy for benign rectal anastomotic stricture after surgery for colorectal cancer: A systematic review

Aim: To evaluate various treatment methods for benign rectal anastomotic stricture (As) following surgery for colorectal cancer. Method: A systematic review of the literature was conducted, focusing on studies that reported outcomes of different treatment modalities for benign As. The PubMed, Embase, scopus, China National Knowledge Infrastructure, and Cochrane Library databases were searched from January 2000 to December 2023. The inclusion criteria were studies involving human subjects, published in English, and reporting on therapeutic outcomes for benign As. Results: A total of 19 papers identified a range of therapeutic strategies, including nonoperative anastomotic dilation, endoscopic balloon dilation (EBD), transanal minimally invasive surgery (TAMIs), self-expandable metal stents (sEMs), endoscopic incision (EI) and newer techniques such as prostate resection instrumentation. Nonoperative anastomotic dilation can serve as an initial treatment for lower As. EI demonstrated promise in cases where EBD was ineffective, providing an alternative method for managing As. TAMIs and sEMs showed higher efficacy in refractory cases, with TAMIs being particularly effective for severe fibrotic or completely closed As. The use of rigid instrumentation with an electric knife for transanal incisions demonstrated precision but lacked the flexibility needed for complex procedures. Conclusion: While traditional methods such as nonoperative anastomotic dilation and EBD remain first-line treatments for benign As, advanced techniques such as EI, TAMIs, and sEMs offer promising alternatives, particularly in refractory cases. The choice of treatment should be tailored to individual patient conditions, with consideration for the technical expertise required and the potential for complications. © 2024 Zhejiang University

Cognition on oxidation behavior of Ni-based superalloy GH4742 when exposed to water vapor

Re-Distributing Facial Features for Engagement Prediction with ModernTCN

Automatically detecting learners’ engagement levels helps to develop more effective online teaching and assessment programs, allowing teachers to provide timely feedback and make personalized adjustments based on students’ needs to enhance teaching effectiveness. Traditional approaches mainly rely on single-frame multimodal facial spatial information, neglecting temporal emotional and behavioural features, with accuracy affected by significant pose variations. Additionally, convolutional padding can erode feature maps, affecting feature extraction’s representational capacity. To address these issues, we propose a hybrid neural network architecture, the redistributing facial features and temporal convolutional network (RefEIP). This network consists of three key components: first, utilizing the spatial attention mechanism large kernel attention (LKA) to automatically capture local patches and mitigate the effects of pose variations; second, employing the feature organization and weight distribution (FOWD) module to redistribute feature weights and eliminate the impact of white features and enhancing representation in facial feature maps. Finally, we analyse the temporal changes in video frames through the modern temporal convolutional network (ModernTCN) module to detect engagement levels. We constructed a near-infrared engagement video dataset (NEVD) to better validate the efficiency of the RefEIP network. Through extensive experiments and in-depth studies, we evaluated these methods on the NEVD and the Database for Affect in situations of Elicitation (DAisEE), achieving an accuracy of 90.8% on NEVD and 61.2% on DAisEE in the four-class classification task, indicating significant advantages in addressing engagement video analysis problems. © 2024 The Authors. We examine electron kinetic effects in broadband-laser-driven back-stimulated Raman scattering(BsRs)bursts using particle-in-cell *** bursts occur during the nonlinear stage,causing reflectivity spikes and generating large numbers of hot ***-duration simulations are performed to observe burst events,and a simplified model is developed to eliminate the interference of the broadband laser’s random intensity *** the simplified model,we isolate and characterize the spectrum of electron plasma *** spectrum changes from a sideband structure to a turbulence-like structure during the burst.A significant asymmetry in the spectrum is *** asymmetry is amplified and transferred to electron phase space by high-intensity broadband laser pulses,leading to violent vortex-merging and generation of hot *** proportion of hot electrons increases from 6.76%to 14.7%during a single violent burst *** demonstrate that kinetic effects profoundly influence the BsRs evolution driven by broadband lasers.

Data-Driven Learning Control Algorithms for Unachievable Tracking Problems

For unachievable tracking problems, where the system output cannot precisely track a given reference, achieving the best possible approximation for the reference trajectory becomes the objective. This study aims to investigate solutions using the Ptype learning control scheme. Initially, we demonstrate the necessity of gradient information for achieving the best ***, we propose an input-output-driven learning gain design to handle the imprecise gradients of a class of uncertain systems. However, it is discovered that the desired performance may not be attainable when faced with incomplete *** address this issue, an extended iterative learning control scheme is introduced. In this scheme, the tracking errors are modified through output data sampling, which incorporates lowmemory footprints and offers flexibility in learning gain *** input sequence is shown to converge towards the desired input, resulting in an output that is closest to the given reference in the least square sense. Numerical simulations are provided to validate the theoretical findings.
