Cytisus aeolicus is a narrow endemic species restricted to the Aeolian archipelago(sE Tyrrhenian sea,Italy)and it is one of the most evolutionarily isolated plants in the Mediterranean flora.Historical and literature data suggest that both metapopulations and isolated individuals of C.aeolicus are gradually shrinking.Field investigations and drone images demonstrate that the C.aeolicus metapopulation from stromboli experienced a strikingly fast increase during the last decades.As of 2019,more than 7000±3000 mature individuals occur on stromboli,i.e.14 to 20 times more than those counted during the last census,25 years ago.The diachronic analysis of aerial photos concerning last 80 years and the analysis of the growth rings of some selected plants pointed out that the surface occupied,the demographic structure and the distribution pattern of the subpopulations of stromboli has been highly fluctuating during last decades.Moreover,data issuing from field observations in permanent plots placed in a transect between two isolated mature individuals showed that,under natural conditions,the germination rate of the seedlings of C.aeolicus can be very high and their establishment rate may exceed 40%.By contrast,seedlings mortality is subject to strong annual fluctuations.Additionally,the pollen morphology of the strombolian metapopulation of this rare and isolated species is studied here for the first time.Contrary to what is stated in recent literature,the C.aeolicus metapopulation from stromboli is healthy and very dynamic,albeit frequently damaged by the volcanic activity.Regular and repeated field surveys carried out during 3 years(2017-2019)allowed improving our knowledge on the life cycle of C.aeolicus and a new extinction risk assessment of the species,according to IUCN criteria,is presented.

The albumin-bilirubin(ALBI)score to assess the risk of decompensation in patients with initially compensated cirrhosis may improve their prognostic evaluation.This letter critically evaluates the research,which utilizes the ALBI score to forecast decompensation in cirrhosis patients over a three-year period.This score was initially developed to assess liver function in hepatocellular carcinoma,its prognostic utility for non-malignant liver diseases has now been explored,recognizing decompensation as a pivotal event that significantly affects patient’s survival.some concerns regarding the methodology of this research may be raised,particularly the exclusive use of radiological diagnosis,potentially including patients without definite cirrhosis and thus skewing the decompensation risk assessment.The reported predominance of variceal bleeding as a decompensating event conflicts with established literature,that often reports ascites as the initial decompensation manifestation.The letter highlights the absence of details on esophageal varices and their management,which could introduce bias in evaluating the ALBI score's predictive power.Furthermore,the letter points out the small sample size of patients with high-risk ALBI grades,potentially compromising the score's validity in this context.We suggest prospective future research to investigate the dynamic changes in the ALBI score over time to reinforce the validity of the ALBI score as a predictor of decompensation in non-malignant liver disease.

The impact of Pinus halepensis afforestation on Mediterranean spontaneous vegetation:do soil treatment and canopy cover matter?

We investigated central Mediterranean Pinus halepensis plantations under semi-arid climate in order to evaluate the combined effect of soil treatment and afforestation practices on spontaneous plant species composition, richness and evenness, and on the trend and speed of vegetation dynamics. Phytosociological releves of three different plot typologies, i.e. （1） soil-treatment and plantation, （2） only soil-treatment, （3） no soil-treatment and no plantation, were compared by （a） multivari- ate analysis and （b） with reference to species richness and evenness. Moreover, in order to compare vegetation dynamics within the planta- tions with those ones ongoing in semi-natural garrigue communities, we compared life form and syntaxonomic spectra between phytosociological relev6s taken at 8 years of distance. DCA showed that floristic species composition and similarity are influenced by the canopy cover of Pine trees as well as by soil-treatment practices. Although species richness and evenness are not clearly related to neither soil treatment nor Pine afforestation, canopy cover clearly plays a major role： in fact, the highest Ph cover rates correspond to the lowest values of understory species richness. This is true also if only species of biogeographical/conservation interest are considered. Regarding vegetation dynamics, sites with dense Pine canopy cover evolve much slower than the adjacent garrigue communities. The same factors invoqued to explain the patterns of floristic composition and similarity （i.e. allelopathy and competition for light, water and nutrients） may also explain the lowering of diversity of therophytes and the strong decline of the cover perfomed by both therophytes and hemicryptophytes underneath the canopy of dense Ph plantations. Thus, in sites where Ph cover exceeds about 80%, thinning is recommended not only in order to accelerate succession, but also to give a natural ＇shape＇ to afforestations.

Peliosis hepatis: Personal experience and literature review

Peliosis hepatis(PH) is a disease characterized by multiple and small,blood-filled cysts within the parenchymatous organs. PH is a very rare disease,more common in adults,and when it affects the liver,it comes to the surgeon's attention only in an extremely urgent situation after the lesion's rupture with the resulting hemoperitoneum. This report describes the case of a 29-year-old woman affected by recurring abdominal pain. Computed tomography scans showed a hepatic lesion formed by multiple hypodense areas,which showed an early acquisition of the contrast during the arterial phase. Furthermore,it remained isodense with the remaining parenchyma during the late venous phase. We decided on performing a liver resection of segment Ⅶ while avoiding a biopsy for safety reasons. The histopathologic examination confirmed the diagnosis of focal PH. PH should always be considered in the differential diagnosis of hepatic lesions. Clinicians should discuss the possible causes and issues related to the differential diagnosis in addition to the appropriate therapeutic approach. The fortuitous finding of a lesion,potentially compatible with PH,requires elective surgery with diagnostic and therapeutic intents. The main aim is to prevent the risk of a sudden bleeding that,in absence of properly equipped structures,may have a fatal outcome.

soy protein isolate and egg white protein were added to cassava-banana gluten-free pasta and the effects on the nutritional quality,digestibility properties,protein digestibility corrected amino acid(PDCAA),and sensory acceptance of the pasta was observed.Banana-cassava composite flour(75:25)was blended with soy protein isolate or egg white protein at the following rates:0,5,10,and 15 g/100 g flour.Cooked pasta samples were analysed for total phenolic content(TPC),antioxidant activity,amino acid profiles,protein content,starch digestibility,protein digestibility and protein digestibility corrected amino acid score(PDCAAs).Addition of both proteins decreased starch digestibility,increased protein digestibility,improved the balance of the amino acid profile,and PDCAAs whereas only soy protein isolate enhanced the TPC and antioxidant capacity of the banana-cassava pasta.An egg white protein-fortified banana-cassava pasta had better customer acceptance and purchase intent than soy protein isolate inclusion.

背景：早期机械性血管重建是降低合并心源性休克的急性心肌梗死患者死亡率的一种治疗措施。从1999年起美国心脏病学会（ACC）和美国心脏协会（AHA）制定的指南即将其作为年龄〈75岁患者的Ⅰ类推荐。但是，人们对实践工作中指南的执行情况知之甚少。目的：评价合并心源性休克的急性心肌梗死患者实施早期血管重建的趋势和死亡率，以便了解国家指南是否对血管重建率产生影响。设计、地点及病例：前瞻性观察研究，共入选293633例sT段抬高的心肌梗死患者（25311例[8．6％]合并心源性休克，其中7536[29％]入院时已有心源性休克）。研究对象来自1995年1月至2004年5月的国家心肌梗死注册登记（National Registry of Myocardial Infarction，NRMI），共有775家具备血管重建能力（定义为能实施心脏导管、经皮冠状动脉介入术[percutaneous caronary intervention，PCI]和心脏直视手术）的美国医院参加。主要观测指标：治疗模式和住院死亡率。结果：在有血管重建能力的医院，直接PCI率从27．4％上升到54．7％（P〈0．001），这与sT段抬高急性心肌梗死中使用PCI比率的变化平行。急性冠状动脉旁路移植手术率变化不大（从2．1％到3．2％）。倾向校正后多变量分析显示，直接PCI与住院期间死亡率的下降有关（OR，0．46；95％可信区间，0．40-0．53）。指南公布后所有年龄段患者血管重建率无明显变化。心源性休克院内总死亡率从1995年的60．3％下降到2004年的47．9％（P〈0．001）。结论：在有血管重建能力的医院，心源性休克患者接受PCI可改善生存情况。ACC葙AHA指南对血管重建率未产生明显影响，提示应加大指南的宣传和执行力度。

Context: Early mechanical revascularization in patients with acute myocardial infarction(AMI) complicated by cardiogenic shock is a therapeutic strategy that reduces mortality. It has been a class I recommendation in guidelines from the American College of Cardiology and the American Heart Association since 1999 for patients younger than 75 years. However, little is known about implementation of these guidelines in practice. Objectives: To assess trends in early revascularization and mortality for patients with cardiogenic shock complicating AMI and to determine whether the national guidelines affect revascularization rates. Design, setting, and Patients: Prospective, observational study of 293 633 patients with sT-elevation myocardial infarction(25 311 8.6% had cardiogenic shock; 7356 29% had cardiogenic shock at hospital presentation) enrolled in the National Registry of Myocardial Infarction(NRMI) from January 1995 to May 2004 at 775 Us hospitals with revascularization capability(defined as the capability to perform cardiac catheterization, percutaneous coronary intervention PCI , and open-heart surgery). Main Outcome Measures: Management patterns and in-hospital mortality rates. Results: There was an increase in primary PCI rates from 27.4% to 54.4% (Pspitals with revascularization capability that paralleled the change in PCI for sT-elevation myocardial infarction. There was no significant change in rates of immediate coronary artery bypass graft surgery(from 2.1% to 3.2% ). Propensity-adjusted multivariable analyses demonstrated that primary PCI was associated with a decreased odds of death during hospitalization(odds ratio, 0.46; 95% confidence interval, 0.40-0.53). There were no differences in the rates of change in revascularization rates based on the date when the guidelines were released regardless of patient age. Physical properties, chemical composition, minerals content, amino acids profile and anti-nutritional factors span>span>i.e.span> phytic acid, tannins and trypsin inhibitor of lima bean seeds (span>Phaseolus lunatusspan>) which newly cultivated in Egypt were investigated. Addition to study the effect of common processing methods (soaking, cooking, roasting, and dehulling) on the chemical composition, minerals content and anti-nutritional factors of lima bean seeds in order to be used in biscuit preparation. Results showed that protein content was significantly (span>Pspan> ≤ 0.05) increased in dehulled Lima bean (27.06%) compared to raw lima bean (26.02%). All processing methods significantly (span>Pspan>span style="font-family:'Minion Pro Capt';"> ≤ 0.05) increased soluble Dietary Fiber (sDF), however soaked seeds contained the highest significant Total Dietary Fiber (TDF) (30.18%) and Insoluble Dietary Fiber (IDF) (22.15%). Raw lima bean was superior in calcium, iron, magnesium, sodium, potassium, copper, and zinc compared with all processed samples.
