In orderto develop a quantitative modelto predictlongitudinalcrack, fartherstudy willbeexpected in revising and linkingthese models.

The varicocele： diagnostic dilemmas, therapeutic challenges and future perspectives

A varicocele is defined as the abnormal dilation of the internal testicular vein and pampiniform venus plexus within the spermatic cord. If a semen analysis is not obtained from the adolescent male, in the absence of other symptoms, the main clinical indication used by many urologists to recommend repair is testicular atrophy. The varicocele may result in testicular damage in some males causing testicular atrophy with impaired sperm production and decreased Leydig cell function, while in other males the varicocele may seemingly cause no ill effects. In adult men, varicoceles are frequently present and surgically correctable, yet the measurable benefits of surgical repair are slight according to a Cochrane review. While occurring more commonly in infertile men than fertile men, only 20% of men with a documented varicocele will suffer from fertility problems. Most varicoceles found in adolescents are detected during a routine medical examination, and it is difficult to predict which adolescent presenting with a varicocele will ultimately show diminished testicular function in adolescence or adulthood. As in adults, the mainstay of treatment for varicocele in adolescents is surgical correction. However, unlike an adult varicocelectomy （the microsurgical approach is the most common）, treatment for an adolescent varicocele is more often laparoscopic. Nevertheless, the goals of treatment are the same in the adolescent and adult patients. Controversy remains as to which patients to treat, when to initiate the treatment, and what type of treatment is the best. This review will present the current understanding of the etiology, diagnosis and treatment of the adolescent varicocele.

The sanjiang plain is one of the grain production bases in northeast *** water and groundwater are the irrigation water resources in study *** enhance the water management,the relationship between surface water and groundwater was assessed by hydrogen and oxygen stable isotope method. The surface waters were sampled in the songhua River,Heilong River,Wusuli River,Xingkai Lake and wetlands. shallow and deep groundwater samples were collected from shallow and deep wells,*** values ofδD andδ18O of surface water and groundwater samples were sorted as:lake water>wetland water>spring>shallow groundwater>river water>deep *** most enriched water sample was Xingkai Lake according to the evaporation,while the most depleted was the deep groundwater at the confluence of Heilongjiang River and Wusuli *** local meteoric water line wasδD=7.3δ*** rivers were mainly discharged by shallow *** contributions of shallow groundwater to songhua River, Heilongjiang River and Wusuli River were 57%,70%and 73%,*** groundwater plays an important role in sanjiang *** surface water combined with groundwater should be used as irrigation water.

为明确随机有限断层法在俯冲带板内地震动模拟中的适用性,以2021年日本千叶M j6.1俯冲带板内地震为例,使用该方法模拟了震中范围100 km内25组KiK-net台站井上和井下记录,并分析了模拟与观测记录的频谱、持时、峰值和空间分布等地震动特征。结果表明:模拟与观测记录的5%阻尼比拟加速度反应谱(A_(ps))在0.1~10 Hz频带范围内吻合较好;基于70%能量持时模型的模拟记录在强震动段和持时上与观测记录匹配良好;井上台模拟与观测记录的地表峰值加速度(A_(pg))接近,且两者A_(pg)衰减特征基本一致;模拟和观测记录所得A_(pg)等值线相似。此外,将模拟和观测记录与现有日本俯冲带板内地震动衰减关系式(Zhao16)进行了比较,发现Zhao16的A_(pg)预测值普遍出现高估,其A_(ps)预测值在低频和高频分别出现一定程度的低估和高估,这可能与研究区域的盆地效应和软土层有关。研究结果为随机有限断层法模拟俯冲带板内地震动的适用性提供了依据,进而为探索将该方法应用于中国具有相似俯冲带构造的地区提供参考。

Water cycle and water quality in the Lake Baiyangdian watershed of the North China Plain have undergone great changes due to over-pumping of groundwater and wastewater *** this paper,hydrogeochemical data was collected to analyze the hydrochemical characteristics and geochemistry evolution of *** study area was divided into two typical *** was in the upstream plain area,where over-pumping had resulted in significant decline of groundwater level;the other one was located in the downstream area near the Fu River and Lake Baiyangdian(Lake BYD region).In addition to the natural weathering of minerals,excessive fertilizer was also a main factor of higher ion concentration in *** to studies,due to good permeability,these regions were easy to be polluted even with deep groundwater ***,upstream shallow groundwater and surface water,including lake water,domestic along with industrial wastewater were all sources of present shallow groundwater in the Lake BYD *** indicated that anthropogenic activities rather than minerals much matter to the groundwater in these ***,wastewater largely decided the groundwater quality,which suggested that the management and restoration of surface water quality was crucial to groundwater protection.

The objective of this guideline is to outline the role of magnetic resonance imaging(MRI) in clinical decision making and outcome prediction in patients with traumatic spinal cord injury(sCI).Methods A systematic review of the literature was conducted to address key questions related to the use of MRI in patients with traumatic *** review focused on longitudinal studies that controlled for baseline neurologic status.A multidisciplinary Guideline Development Group(GDG) used this information,their clinical expertise,and patient input to develop recommendations on the use of MRI for sCI *** on GRADE(Grading of Recommendation,Assessment,Development and Evaluation),a strong recommendation is worded as " we recommend," whereas a weaker recommendation is indicated by "we suggest." Results Based on the limited available evidence and the clinical expertise of the GDG,our recommendations were:(1) "We suggest that MRI be performed in adult patients with acute sCI prior to surgical intervention,when feasible,to facilitate improved clinical decision-making"(quality of evidence,very low) and(2) "We suggest that MRI should be performed in adult patients in the acute period following sCI,before or after surgical intervention,to improve prediction of neurologic outcome "(quality of evidence,low).Conclusions These guidelines should be implemented into clinical practice to improve outcomes and prognostication for patients with sCI.
