食管曲张静脉出血的治疗至今仍有争议.旧方法(内窥镜硬化治疗)的重用与新方法(血管栓塞治疗)的推荐,致使争论更为加剧.近年来,控制食管曲张静脉出血的非手术方法日益受到注意,这与急诊手术死亡率和脑病发生率高有关.门脉高压病人的食管曲张静脉为什么出血,原因尚不了解.出血与曲张静脉的大小有关,但曲张静脉的大小和广泛程度与门脉高压的水平无关.胃-食管返流并不造成出血.曲张静脉出血后的非手术治疗者,在4～6周内死亡率为42%(主要因出血致死).大约有1/3的病人在6周内再次出血.生存者如不手术,有1/3的病人6周后还会复发出血.幸存者病情的实质性改善,均有赖于早期安全而有效的治疗.近几年在控制食管曲张静脉出血的非手术治疗方面已取得一些进展.

Background Complications are frequently reported after hypospadias repair and there is a need to understand the factors that influence their *** Data from boys with hypospadias born between 2000 and 2020 were obtained from the International Disorders of sex Development(l-DsD)*** regressions,fisher's exact tests and spearman's correlation tests were performed on the data to assess associations between clinical factors and complication *** Of the 551 eligible boys,data were available on 160(29%).Within the cohort,the median(range)External Masculinization score(EMs)was 6(2,9).All pres.n.ed with one or more additional genital malformation and 61(38%)pres.n.ed with additional extragenital *** of androgen action,androgen s.n.hesis and gonadal development were diagnosed in 28(18%),22(14%)and 9(6%)boys,*** remaining 101(62%)patients were diagnosedas having non-specific 46,XY Disorders of sex ***(50%)boys had evidence of abnormal biochemistry,and gene variants were identified in 42(26%).Median age at first hypospadias surgery was 2 years(0,9),and median length of follow-up was 5 years(0,17).Postsurgical complications were noted in 102(64%)*** were no significant associations with postsurgical *** Boys with proximal hypospadias in the I-DsD Registry have high rates of additional comorbidities and a high risk of postoperative *** clinical factors were significantly associated with complication *** complication rates with no observable cause suggest the involvement of other factors which need investigation.

In this paper, the method of experimental estimation of the temperature in a penetration channel in electron beam welding is described on the basis of chemical elements concentration in the vapors above welding zone. The temperature of a vapor-gas phase in the penetration channel is determined when equating calculated and experimental concentrations of the elements.

to identify plant macrofossils,estimate peat humification and calculate hydroclimatic indices based on current bog species,with the overall aim of determining the climate conditions associated with evolution of the bog during the Medieval Climate Anomaly and the Little Ice *** proxies,together with historical and climate data,proved to be good indicators of the changes in bog surface ***:of the core led to identification of 9 different periods:two corresponding to the so-called Medieval Climate Anomaly(930 to 1345 AD,1075–665 calibrated years before pres.n.[*** BP]);four corresponding to the Little Ice Age(1345 to 1905 AD;665–105 cal yr BP);and three corresponding to the last century(1905 to 2000 AD).The findings revealed a generally dry climate that lasted until the 14th century,followed by a transition to a long period with a more humid,but characteristically very variable climate,which ended at the beginning of the 20th century and was followed by a rapid transition to more humid conditions and finally,a change to drier *** Medieval Climate Anomaly was indicated by the abundance of dry-adapted mosses(Leucobryum glaucum,Hypnum cupressiforme)and characterized by warm dry conditions and high levels of peat humification,with alternating wet *** LIA period was dated by a large abundance of sphagnum species(an indicator of wetness)and a gradual increase in the humification ***,four different climate phases were differentiated in this ***-resolution reconstruction of the evolution of the CVM bog and the multiproxy approach have together enabled a more detailed identification of climatic variations in this area,which are generally consistent with the global models,as well as better definition of the elusive climatic oscillations in the last millennium and confirmation of the importance of local modulation of global *** study provides.n.w information and a detailed chronology of climatic events that will hel

Background： Dairy cows can readily overconsume dietary energy during most of the prepartum period, often leading to higher prepartal concentrations of insulin and glucose and excessive body fat deposition. The end result of these physiologic changes is greater adipose tissue lipolysis post-partum coupled with excessive hepatic lipid accumulation and compromised health. Although transcriptional regulation of the adipose response to energy availability is well established in non-ruminants, such regulation in cow adipose tissue depots remains poorly ***： Effects of ad-libitum access to high [HIGH; 1.62 Mcal/kg of dry matter（DM）] or adequate（COn; 1.35 Mcal/kg of DM） dietary energy for 8 wk on mes.n.eric（MAT） and subcutaneous（sAT） adipose tissue transcript profiles were assessed in non-pregnant non-lactating Holstein dairy cows us.n. a 13,000-sequence annotated bovine oligonucleotide microarray. statistical analysis revealed 409 and 310 differentially expressed genes（DEG） due to tissue and diet. Bioinformatics analysis was conducted us.n. the Dynamic Impact Approach（DIA） with the KEGG pathway database. Compared with sAT, MAT had more active biological processes related to adipose tissue accumulation（adiponectin secretion） and signs of pro-inflammatory processes due to adipose tissue expansion and macrophage infiltration（generation of ceramides）. Feeding the HIGH diet led to changes in m RnA expression of genes associated with cell hypertrophy（regucalcin）, activation of adipogenesis（phospholipid phosphatase 1）,insulin signaling activation（neuraminidase 1） and angiogenesis（semaphorin 4 G, plexin B1）. Further, inflammation due to HIGH was underscored by m RnA expression changes associated with oxidative stress response（coenzyme Q3, methyltransferase）, ceramide s.n.hesis.n.acylsphingos.n. amidohydrolase 1）, and insulin signaling（interferon regulatory factor 1, phosphoinositide-3-kinase regulatory subunit 1, retinoic acid receptor alpha）.
