Endoscopic treatment of biliopancreatic pathology in patients with Whipple's pancreaticoduodenectomy surgical variants: Lessons learned from single-balloon enteroscopy-assisted ERCP

Background: Endoscopic treatment of biliopancreatic pathology is challenging due to surgically altered anatomy after Whipple's pancreaticoduodenectomy. This study aimed to evaluate the feasibility and safety of single-balloon enteroscopy-assisted endoscopic retrograde cholangiopancreatography(sBE-ERCP) to treat biliopancreatic pathology in patients with Whipple's pancreaticoduodenectomy surgical variants. Methods: We retrospectively analyzed 106 sBE-ERCP procedures in 46 patients with Whipple's variants. Technical and clinical success rates and adverse events were evaluated. Results: Biliary sBE-ERCP was performed in 34 patients and pancreatic sBE-ERCP in 17, including 5 with both indications. From a total of 106 sBE-ERCP procedures, 76 were biliary indication with technical success rate of 68/76(90%) procedures and clinical success rate of 30/34(88%) patients. Mild adverse event rate was 8/76(11%), without serious adverse events. From a total of 106 sBE-ERCP procedures, 30 were pancreatic indication with technical success rate of 24/30(80%) procedures( P = 0.194 vs. biliary sBEERCP) and clinical success rate of 11/17(65%) patients( P = 0.016 vs. biliary sBE-ERCP). Mild adverse event rate was 6/30(20%)( P = 0.194 vs. biliary sBE-ERCP), without serious adverse events. After sBE-ERCP failure, endoscopic ultrasound-guided drainage, percutaneous drainage and redo surgery were alternative therapeutic options. Conclusions: Biliopancreatic pathology after Whipple's pancreaticoduodenectomy variants can be treated using sBE-ERCP without serious adverse events. Technical and clinical success rates are high for biliary indications, whereas clinical success rate of pancreatic indications is s.gnificantly lower. sBE-ERCP can be considered as first-line treatment option in this patient group with surgically altered anatomy.

são Paulo state has witnessed CO_(2)storage-based investigations considering the availability of suitable geologic structures and proximity to primary CO_(2)source sinks related to bioenergy and carbon capture and storage(BECCs)*** current study presents the hydrocarbon viability evaluations and CO_(2)storage prospects,focusing on the sandstone units of the Rio Bonito *** objective is to apply petrophysical evaluations with geochemical inputs in predicting future hydrocarbon(gas)production to boost CO_(2)storage within the study *** study used data from eleven wells with associated wireline logs.gamma ray,resistivity,density,neutron,and sonic)to predict potential hydrocarbon accumulation and fluid mobility in the investigated *** samples(shale and carbonate)obtained from depths>200 m within the study location have shown bitumen *** geochemistry data of the Rio Bonito Formation shale beds s.ggest they are potential hydrocarbon source rocks and could have contributed to the gas accumulations within the sandstone *** drilled well data,e.g.,CB-1-sP and TI-1-sP,show hydrocarbon(gas)presence based on the typical resistivity and the combined neutron-density responses at depths up to 3400 m,indicating the possibility of other hydrocarbon members apart from the heavy oil(bitumen)observed from the near-surface rocks *** the three-dimensional(3-D)model,the free fluid indicator(FFI)is more s.gnificant towards the southwest and southeast of the area with deeper depths of occurrence,indicating portions with reasonable hydrocarbon recovery rates and good prospects for CO_(2)injection,circulation and permanent ***,future studies based on contemporary datasets are required to establish the hydrocarbon viability further,foster gas production events,and enhance CO_(2)storage possibilities within the region.

Features of sampling stream sediments of large river valleys under cryolithogenesis conditions in the Balygychan-s.goy trough,North-East of Russia

Background:There is mounting evidence that regular physical activity is an important prerequisite for healthy cognitive ***,the finding that almost one-third of the adult population does not reach the recommended level of regular physical activity calls for further public health *** this context,digital and home-based physical training interventions might be a promising alternative to center-based intervention ***,this systematic review aimed to summarize the current state of the literature on the effects of digital and home-based physical training interventions on adult cognitive ***:In this pre-registered systematic review(PROsPERO;ID:CRD42022320031),5 electronic databases(PubMed,Web of science,Psyclnfo,sPORTDiscus,and Cochrane Library)were searched by 2 independent researchers(FH and PT)to identify eligible studies investigating the effects of digital and home-based physical training interventions on cognitive performance in *** systematic literature search yielded 8258 records(extra17 records from other sources),of which 27 controlled trials were considered *** reviewers(FH and PT)independently extracted data and assessed the risk of bias using a modified version of the Tool for the assEssment of study qualiTy and reporting in EXercise(TEsTEX scale).Results:Of the 27 reviewed studies,15 reported positive effects on cognitive and motor-cognitive outcomes(i.e.,performance improvements in measures of executive functions,working memory,and choice stepping reaction test),and a considerable heterogeneity concerning study-related,population-related,and intervention-related characteristics was noticed.A more detailed analysis s.ggests that,in particular,interventions using online classes and technology-based exercise devices(i.e.,step-based exergames)can improve cognitive performance in healthy older *** one-half of the reviewed studies were rated as having a high risk of bias with respect

Carbon-based solid lubricants are excellent options to reduce friction and wear,especially with the carbon capability to adopt different allotropes *** the macroscale,these materials are sheared on the contact along with debris and contaminants to form tribolayers that govern the tribosystem *** a recently developed advanced Raman analysis on the tribolayers,it was possible to quantify the contactinduced defects in the crystalline structure of a wide range of allotropes of carbon-based solid lubricants,from graphite and carbide-derived carbon particles to multi-layer graphene and carbon *** addition,these materials were tested under various dry sliding conditions,with different geometries,topographies,and solid-lubricant application *** of the initial tribosystem conditions and allotrope level of atomic ordering,there is a remarkable trend of increasing the point and line defects density until a specific saturation limit in the same order of magnitude for all the materials tested.
