目的综述关于胎儿医学的TORCH测试的适应证和价值(弓形虫,风疹病毒,巨细胞病毒,疱疹病毒)。方法收集近10年中一个胎儿医学小组负责的所有产妇和胎儿的TORCH测试的分析报告。分析可能由先天性胎儿感染所引起的超声影像特征。研究孕妇及胎儿确诊感染后的围产结局。结果笔者对462位产妇进行了TORCH测试。也对其中的67例胎儿样本(羊水或胎儿血样)进行了检测。另外,检测了14例胎儿样本,但缺少其母亲的样本,从而使总样本数达到476例。其中,有11例产妇为巨细胞病毒感染(2.3%),10例胎儿巨细胞病毒感染,并没有发现其他病毒感染。检测的适应证包括超声发现胎儿肠管高回声、水肿、大脑脑室增宽、高回声病灶、羊水过少、羊水过多和胎儿宫内发育迟缓。其中可能与胎儿宫内感染相关的最常见表现为胎儿肠管高回声、腹水、心脏扩大症和羊水过少。没有病例与羊水过多相关,同时胎儿宫内发育迟缓和脑室增宽总是与其他特点相关。结论在英国,由于胎儿的详细检查结果就对产妇进行完整的TORCH检查通常是没有必要的。检测可仅限于巨细胞病毒,特别是因为其他感染性病原体,包括弓形体病,在英国都是罕见的。更多了解先天性感染的不同超声特点之间的相关性也很重要。

Objectives: The study is investigating the relation of the ploidy pattern and cell cycle kinetics to different types of endometrial hyperplasia to select the high-risk women who will need strict follow up surveillance. study design: An observational study of 152 patients with endometrial hyperplasia. Endometrial samples were subjected to flowcytometric study of the nuclear DNA content to determine the ploidy pattern and cell cycle kinetics. Results: The mean age of women was 46.3 ±3.6 years. 15.8%of women were nulliparae, 36.8%were diabetic and 43.6%were hypertensive. 48.7%of women were obese (BMI >30). Most of endometrial samples (88.2%) were simple endometrial hyperplasia without atypia. The cell cycle kinetics in different types of endometrial hyperplasia shows that there were significant statistical differences as regards the s-phase fraction and proliferative index (PI) between typical and atypical hyperplasia. Conclusion: The study of cell cycle kinetics by flowcytometry might help in picking up, among all women with endometrial hyperplasia, the group of patients who need further close and strict follow up by endometrial pathologic study. This is going to minimize the cost and invasiveness of surveillance of patients with various grades of endometrial hyperplasia.

Effect of ethnicity on weight loss among adolescents 1 year after bariatric surgery

AIM:To investigate whether or not bariatric surgeryweight outcomes vary by ethnicity in a large,nationally representative sample of ***:The Bariatric Outcomes Longitudinal Database was used for analysis and contains data on surgeries performed on adolescents from 2004 to 2010from 423 surgeons at 360 facilities across the United states Adolescents(n=827)between 11 and 19 years old who underwent either gastric bypass or adjustable gastric banding surgery were included in the *** measures included changes in anthropometric measurements[weight(kg)and body mass index]from baseline to 3(n=739),6(n=512),and 12(n=247)mo after ***:A year after patients underwent either gastric bypass(51%)or adjustable gastric banding(49%)surgery,mean estimated weight loss for all ethnic groups differed by a maximum of only 1.5 kg,being34.3 kg(95%CI:30.0-38.5 kg)for Hispanics,33.8 kg(95%CI:27.3-40.3 kg)for non-Hispanic blacks,and32.8 kg(95%CI:30.9-34.7 kg)for non-Hispanic *** overall pairwise group comparisons were significant,indicating that no ethnic group had better weight loss outcomes than did ***:Bariatric surgery substantially reduces the weight of severely obese adolescents at 1 year post-procedure with little variation by ethnicity and/or *** results suggest that bariatric surgery is a safe and reasonable treatment for all severely obese adolescents with the appropriate indications.
