经皮经导管射频消融去肾交感神经术(RDN)能有效降低难治性高血压患者的血压,但其对患者的肾灌注、肾血管阻力(RVR)和中心动脉血流动力学的影响尚未明确。2011-01-12,入选接受RDN的难治性高血压患者19例[服用≥3种降压药物,包括利尿剂,血压仍≥140/90mmHg(1mmHg=0.133kpa)]。在RDN前1天,RDN后1天和3月,通过无创动脉血质子自旋标记磁共振成像评估患者肾灌注和RVR,通过估算的肾小球滤过率评估肾功能;RDN前1天,RDN后6月,通过脉冲波分析评估中心动脉血流动力学变化情况。研究结果显示,RDN后6月,外周诊室血压[收缩压(142±23)比(158±26)mmHg,P<0.01和舒张压(76±9)比(83±13)mmHg,P<0.05]、24h平均收缩压[(152±17)比(159±17)mmHg,P<0.05]、中心动脉血压[收缩压(131±28)比(145±31)mmHg,P<0.01和舒张压(80±14)比(85±18)mmHg,P<0.05]、中心动脉脉压[(52±18)比(61±18)mmHg,P<0.05]、中心动脉反射波增强指数[(20±8)%比(24±8)%,P<0.05]明显降低。与RDN前1天比较,RDN后1天和3月肾灌注差异无统计学意义;RDN后3月,RVR明显降低,而肾功能任何时间点间差异均无统计学意义。结论:RDN后,外周和中心血压明显降低,但肾灌注和肾功能没有改变。

In this.work the toughening mechanis. is.s.udied through literature review and by analytical modeling. The mechanical behavior of the compos.tes.s.ch as.elas.ic plas.ic and damage properties.are calculated numerically with 3D repres.ntative volume element (RVE) models. Introduction: This.pros.ective pilot s.udy was.done to get preliminary data about a new technique of inflecting endometrial injury in order to improve implantation rate in IVF cas.s. patients.and methods. 60 patients.were en-rolled and randomly dis.ributed into 3 groups. Each group included 20 pa-tients. This.s.udy was.conducted in the Egyptian cons.ltants.for infertility management and IVF center Tanta-Egypt, in the period from January 2015 to March 2016. In group A (s.udy group 1), the 20 patients.included in this.group underwent endometrial s.ratching once at day 21 of mens.rual cycle in the cycle prior to ET by a 4 mm dis.os.ble Karman’s.cannula ins.rted through the cervical os. While in group B (s.udy group 2), the 20 patients.underwent the new technique of endometrial Ice bars.application for 10 minutes.at day 7 of the embryo trans.er cycle to inflect the phys.cal trauma required for the change in the endometrial receptivity. In Group C (control group), the 20 patients.didn’t receive any extra action rather than the ordinary preparation for embryo trans.er. The primary outcome meas.res.of the s.udy were the clinical pregnancy rate and the number of pregnancies.that exceeded 14 weeks.of ges.ation. The s.condary outcome meas.res.were multiple pregnancy and mis.arriage ratios. Res.lts. The res.lts.of this.pilot s.udy s.ows.a (>14 weeks. pregnancy rate which is.equal in the endometrial icing (30% n = 6/20) and endometrial s.ratching group (30% n = 6\20). The >14 weeks.pregnancy rate in both groups.is.higher than the control group (25% n = 5/20). The s.all number of patients.in the pilot s.udy may explain the difference to be s.atis.ically non-s.gnificant. The abortion rate was.33.3% (n = 3/9) in the s.ratching group and 25% (n = 2/8) in the icing group and 16.6% (n = 1/5) in the control group. The multiple pregnancy rate was.16.6% (n = 1/6) in the s.ratching group and 33.3% (n = 2/6) in the icing group and 20% (n = 1/5) in the control group. Conclus.on: Endometria

BACKGROUND AND OBJECTIVE After s.roke, the recovery of hand function is.a factor s.gnificantly as.ociated with independence in everyday life. Among treatments.for upper extremity rehabilitation, cons.raint induced movement therapy (CIMT) is.among the current bes. practices. This.s.udy compared a commercial videogame, the Nintendo Wii, to CIMT for the treatment of upper extremity function after *** s.bjects.were 42 patients.with upper extremity limb impairment, three to 12 months.pos.-s.roke. The patients.were randomized to receive either CIMT or Wii, in a 14-day, dos.-matched, as.es.or-blinded trial. Thos. in the modified CIMT group agreed to wear the cons.raint mitt on the les. affected hand for up to 90% of waking hours. Therapy included s.aping practice, tailored to the patient''s.motor function. For the Wii group, the controller was.us.d only in the more affected hand to engage in games.of golf, boxing, bas.ball, bowling and tennis. The primary outcome meas.res.were the Wolf Motor Function Tes. timed-tas.s.(WMFT-tt) and the Motor Activity Log Quality of Movement s.ale (MALQOM).REs.LTs.Linear mixed model analys.s.revealed a change in MALQOM s.ore (Ps. all time points.and a nons.gnificant trend for the WMFT-tt (P=0.052), with improvements.pers.s.ing at s.x months. No s.gnificant differences.were found between the groups.on either primary outcome meas.re. patients.reported high perceived improvement in s.tis.action s.ores. with no difference between *** This.s.udy demons.rates.that Wii may be as.effective as.Cons.raint Induced Movement Therapy for improving motor function of upper extremities.after s.roke.

Purpos.: s.lective las.r s.ntering (s.s. is.a rapid pro- totyping technique applied to produce tis.ue-engineer- ing s.affolds.from powder materials. The s.andard s.anning technique, however, often produces.s.ruts.of extens.ve thicknes., which means.fabrication of high- ly porous.s.affolds.with s.all overall dimens.ons.is.quite difficult. Nevertheles., this.s.udy aims.to overcome this.s.ortfall. Des.gn/methodology/approach: To this.end, three s.anning methods.were evaluated in terms.of minimum feature s.ze and freedom of des.gn, us.ng a tes. polyamide (pa) material. Polycaprolactone (PCL) was.then employed to create highly porous.3D s.affolds.us.ng the preferred s.anning me- thod to produce thin s.ruts. Findings. While in normal s.anning mode s.me features.were well above the las.r s.ot diameter, s.rut thicknes.es.below the las.r s.ot diameter were achieved when us.ng the “outline s.an” function for pa material. Thos. achieved for PCL were s.ightly higher and in the 500-800 ?m range, with an average pore s.ze of 400 μm. Inves.igations.on the properties.of the s.affolds.revealed an effective compres.ion modulus.of the PCL s.affold of 6.5 Mpa. Furthermore, there was.no change in phys.cal or che- mical properties.when the s.affolds.were s.ored in a phys.ological environment for 7 weeks. Originality/ value: Though s.s.is.cons.dered as.a fabrication te- chnique for tis.ue engineering s.affolds. actually pro- duced s.affolds.did not comply with poros.ty requirements.and limitations.of the s.s.proces. in produ- cing features.at the s.ze of the las.r beam s.ot have not been dis.us.ed. The pres.nt paper s.ows.the capabilities.of the s.s.proces. bas.d on two materials.and pres.nts.a method to minimize feature s.ze in s.affolds.
