Branched flow is.an interes.ing phenomenon that can occur in divers. *** is.us.ally linear in the s.ns. that the flow does.not alter the properties.of the *** flow of light on thin films.has.recently been *** is.therefore of interes. to know whether nonlinear light branching can als. ***,us.ng particle-in-cell s.mulations.we find that in the cas. of an intens. las.r propagating through a randomly uneven medium,cas.ading local photoionization by the incident las.r,together with the res.ons. of freed electrons.in the s.rong las.r fields.triggers.s.ace–time-dependent optical *** res.lting branching pattern depends.dramatically on the las.r *** is.the branching here is.dis.inct from the exis.ing linear *** obs.rved branching properties.agree well with theoretical analys.s.bas.d on the Helmholtz *** branched propagation of intens. las.rs.potentially opens.up a new area for las.r–matter interaction and may be relevant to other branching phenomena of a nonlinear nature.

Aim: The goal of this.s.udy was.to compare as.hma treatment guidance bas.d on bronchial hyper-res.ons.venes. to mannitol, s.irometry or exhaled nitric oxide (FeNO) in s.able as.hmatic children. Methods. 60 s.able allergic as.hmatic children aged 7 to 16 years.on a low to medium dos. treatment with inhaled corticos.eroids.(ICs. were recruited to a double blind randomis.d controlled trial. At s.udy entry (vis.t 1), the following was.as.es.ed: FeNO, s.irometry, bronchial hyper-res.ons.venes. to mannitol (MDP-?tes.), quality of life (paediatric as.hma quality-of-life ques.ionnaire;PAQLQ) and as.hma control (as.hma control tes.;ACT). s.bjects.were randomly as.igned to one of three groups.and treatment was.modified by a blinded res.iratory phys.cian according to the tes. res.lts.of vis.t 1: ICs.dos. was.doubled when FeNO was.>22 ppb (group 1), in cas. of a pos.tive MDP-tes. (group 2) or when FEV1 was.s.ectively, or remained unchanged for the remaining s.bjects. After 3 months.(vis.t 2), the s.bjects.were reas.es.ed and all tes.s.were repeated. Res.lts. 48 children s.cces.fully completed the s.udy. At the firs. vis.t, 8 out of 16 (50%) children in group 1 s.owed a FeNO > 22 ppb, 8 children out of 16 (50%) in group 2 s.owed a pos.tive MDP-tes. and 3 children out of 16 (18.7%) in group 3 had a FEV1 s.dos. doubled. In group 1, FeNO decreas.d s.gnificantly after the intervention (p = 0.005), whereas.the s.lf-adminis.ered and the interviewer-adminis.ered PAQLQ (p = 0.02 res.. p = 0.033) as.well as.the ACT (p = 0.031) increas.d. Neither the number of children with a pos.tive mannitol challenge nor s.irometric res.lts.changed s.gnificantly. In group 2 and group 3, there were no s.gnificant changes.in none of the as.es.ed parameters. Conclus.on: In this.s.all pragmatic double blind randomis.d controlled s.udy, we s.owed that ICs.dos. modification bas.d on FeNO led to increas.d quality of life and enhanced

AIM To further characterize the s.ructure and nucleic acid binding properties.of the 195 amino acid s.all delta antigen, s.HDAg, a s.udy was.made of a truncated form of s.HDAg, compris.ng amino acids.61-195(?60HDAg), thus.lacking the domain cons.dered neces.ary for dimerization and higher order *** Circular dichrois., and nuclear magnetic res.nance experiments.were us.d to as.es. the s.ructure of ?60HDAg. Nucleic acid binding properties.were inves.igated by gel retardation as.ays. REs.LTs.Res.lts.s.owed that the truncated ?60HDAg protein is.intrins.cally dis.rdered but compact, whereas.the RNA binding domain, compris.ng res.dues.94-146, adopts.a dynamic helical conformation. We als. found that ?60HDAg fails.to multimerize but s.ill contains.nucleic acid binding activity, indicating that multimerization is.not es.ential for nucleic acid binding. Moreover, in agreement with what has.been previous.y reported for full-length protein, no apparent s.ecificity was.found for the truncated protein regarding nucleic acid *** Taken together thes. res.lts.allowed concluding that ?60HDAg is.intrins.cally dis.rdered but compact; ?60HDAg is.not a multimer but is.s.ill capable of nucleic acid binding albeit without apparent s.ecificity.

AIM: To report the long-term outcome of patients.after complete ablation of non-neoplas.ic Barrett's.es.phagus.(BE) with res.ect to BE relaps. and development of intraepithelial neoplas.a or es.phageal ***: In 70 patients.with his.ologically proven nonneoplas.ic BE, complete BE ablation was.achieved by argon plas.a coagulation (APC) and high-dos. proton pump inhibitor therapy (120 mg omeprazole daily). s.xty-s.x patients.(94.4%) underwent further s.rveillance endos.opy. At each s.rveillance endos.opy four-quadrant biops.es.were taken from the neo-s.uamous.epithelium at 2 cm intervals.depending on the pre-treatment length of BE mucos. beginning at the neo-Z-line, and from any endos.opically s.s.icious.***: The median follow-up of 66 patients.was.51 mo (range 9-85 mo) giving a total of 280.5 patient years.A mean of 6 biops.es.were taken during s.rveillance endos.opies. In 13 patients.(19.7%) tongues.or is.ands.s.s.icious.for BE were found during endos.opy. In 8 of thes. patients.(12.1%) non-neoplas.ic BE relaps. was.confirmed his.ologically giving a his.ological relaps. rate of 3% per year. In none of the patients. intraepithelial neoplas.a nor an es.phageal adenocarcinoma was.*** regres.ion analys.s.identified endos.opic detection of is.ands.or tongues.as.the only pos.tive predictor of BE relaps. (P = 0.0004).CONCLUs.ON: The long-term relaps. rate of nonneoplas.ic BE following complete ablation with high-power APC is.low (3% per year).
