Proximal and distal rectal cancers differ in curative resectability and local recurrence

AIM: To evaluate patients with proximal rectal cancer (PRC) (> 6 cm up to 12 cm) and distal rectal cancer (DRC) (0 to 6 cm from the anal verge). METHODs: Two hundred and eighteen patients (120 male, 98 female, median age 58 years, range 19-88 years) comprised 100 with PRC and 118 with DRC. The proportion of T1, T2 vs T3, T4 stage cancers was similar in both groups (PRC: T1+T2 = 29%; T3+T4 = 71% and DRC: T1+T2 = -31%; T3+T4 = 69%). All patients had cancer confined to the rectum -those with synchronous distant metastasis were excluded. surgical resection was with curative intent with or without pre-operative chemoradiation (c-RT). Follow-up was for a median of 35 mo (range: 12 to 126 mo). End points were: 30 d mortality, complications of operation, microscopic tumour-free margins, resection with a tumour-free circumferential margin (CRM) of 1 to 2 mm and > 2 mm, local recurrence, survival and the permanent stoma rate. REsULTs: Overall 30-d mortality was 6% (12): PRC 7 % and DRC 4%. Postoperative complications occurred in 14% with PRC compared with 21.5% with DRC, urinary retention was the complication most frequently reported (PRC 2% vs DRC 9%, P = 0.04). Twelve percent with PRC compared with 37% with DRC were subjected to preoperative c-RT (P = 0.03). A tumour-free CRM of 1 to 2 mm and > 2 mm was reported in 93% and 82% with PRC and 88% and 75% with DRC respectively (PRC vs DRC, P > 0.05). However, local recurrence was 5% for PRC vs 11% for DRC (P s survival was 65.6% and 60.2% for PRC vs 67% and 64.3% for DRC respectively. No patient with PRC and 23 (20%) with DRC received an abdomino-perineal resection. CONCLUsION: PRC and DRC differ in the rate of abdomino-perineal resection, post-operative urinary retention and local recurrence. survival in both groups was similar.

Predictors for success of non-operative management of adhesive small bowel obstruction

The genus scincella Mittleman,1950,within the family scincidae,is a diverse group comprising 39 described *** taxonomy of the genus is complex due to morphological similarities among species,which often obscures the true extent of biodiversity,resulting in several undescribed species requiring further *** China,particularly within the unique ecosystems of the Hengduan Mountains,the diversity of scincella is notably underestimated,with only 12 species currently *** the present study,we describe a new species of the genus scincella,scincella liangshanensis ***.,from sichuan Province,southwest *** characteristics and phylogenetic analyses revealed that the putative new species is distinct from all other known *** discovery not only expands the known diversity of scincella but also emphasizes the inadequacies in our current understanding of herpetological diversity in *** addition of this species to the scientific record highlights the necessity for ongoing biodiversity assessments to uncover hidden species and enhance our understanding of the intricate web of life in one of the world’s biodiversity hotspots.

Public knowledge,practices and perceptions on typhus fevers in southern sri Lanka

Objective:To assess public knowledge,practices and perceptions on typhus fevers in sri ***:A descriptive study was done in four selected typhus-prone areas in southern sri Lanka.A mixed-method was employed using face-to-face interviews and questionnaire-based surveys among confirmed cases of typhus and at-risk populations,***,percentages,and means were used to characterize socio-demography and evaluate disease ***:The lay terms for typhus fevers reported in the studied region were“peacock fever”,“tick fever”and“bird fever”.A total of 499 subjects participated[mean±sD,(45±16)years]in the questionnaire-based survey,and 13.6%(n=68)reported past experience of typhus fever,1.2%(n=6)identified the disease as“typhus”while 58.7%(n=293)and 11.8%(n=59)knew it as‘peacock fever’and‘tick fever’,*** etiological agent was unknown to 95.2%(n=475),but 53.5%(n=267)were aware that it was ***(57.3%,n=286),eschar(35.7%,n=178),headache(22.0%,n=267)and myalgia(19.2%,n=96)were identified as key *** disease experience was significantly associated with higher awareness of the main disease symptoms(fever:χ^(2)=15.713,Psher’s exact test,P=0.023;eschar:χ^(2)=12.049,Psease prevention *** with a past history of typhus fever had sought treatment at state hospitals(55.9%,38/68)and private sector hospitals(5.9%,4/68).Conclusions:Public awareness on preventive practices for typhus fevers was rare among the participants though vector-borne aspect was known to *** disease awareness was deficient among those without past experience of typhus *** sensitization on vector avoidance strategies is highly recommended.

The genus scincella Mittleman,1950 of the family scincidae currently includes 38 species."To date,however,taxonomic assessment remains ***,phylogenetic analyses based on DNA sequences of four mitochondrial genes supported a putative new species from sichuan Province,southwest China,as an independent *** genetic distance of 16s rRNA between the new species and closest congener was 8%,and the population was morphologically distinguishable from all other known *** herein describe the scincella population as anew species based on both phylogeny and comparative *** new species can be distinguished from its congeners by a combination of the following morphological characters:(1)body slender,mediumsized,snout-vent length 35.0-62.1 mm;(2)infralabials seven,rarely eight;(3)supraciliaries 5-7;(4)tympanum deeply sunk without lobules;(5)midbody scale-row counts 27-30;(6)dorsal scales smooth and enlarged,paravertebral scale-row counts 60-75,ventral scalerow counts 46-59,gulars 25-30;(7)upper edge of lateral longitudinal stripes relatively straight with six rows of dorsal scales in middle;(8)number of enlarged,undivided lamellae beneath fingerⅣ9-11,number of enlarged,undivided lamellae beneath toeⅣ13-16;(9)ventral side of tail densely ornamented with dark brown or black spots;and(10)grayish-brown discontinuous regular dorsal stripes 5-7,distinct black dorsolateral stripes,starting from posterior corner of eye and continuing to lateral side of tail.A diagnostic key to all scincella members from China is also provided The new species is currently only known from Wenchuan and Lixian counties,sichuan Province,China,and brings the number of scincella species in China to *** study emphasizes the incompleteness of knowledge on herpetodiversity in China.

Microbiology of gallbladder bile in uncomplicated symptomatic cholelithiasis

BACKGROUND:Few studies have assessed microflora and their antibiotic sensitivity in normal bile and lithogenic bile with different types of gallstones. METHODs:We performed a case control study of 70 bile samples(35 cholesterol and 35 pigment stones from 51 females and 19 males,aged 21-72 years with a median age of 37 years)from patients who underwent laparoscopic cholecystectomy for uncomplicated cholelithiasis,and 20 controls(14 females and 6 males,aged 33-70 years with a median age of 38 years)who underwent laparotomy and had no gallbladder stone shown by ultrasound *** bile samples were aerobically cultured to assess microflora and their antibiotic *** procedures were undertaken under sterile conditions. REsULTs:Thirty-eight(54%)of the 70 patients with gallstones had bacterial *** isolates(26%)were from cholesterol stone-containing bile and 29 isolates (82%)from pigment stone-containing bile(P=0.01,t test). Twenty-eight of these 38(74%)bile samples were shown positive only after enrichment in brain heart infusion medium(BHI)(P=0.02,t test).The overall bacterial isolates from bile samples revealed *** predominantly,followed by ***,Enterococcus spp.,Klebsiella *** s. *** were no bacterial isolates in the bile of controls after either direct inoculation or enrichment in ***:Bacterial isolates were found in pigment stone-containing ***-lithogenic bile revealed no bacteria,showing an association between gallstone formation and the presence of bacteria in *** sensitivity patterns of isolated organisms were similar irrespective of the type of stone.

Dynamic Nexus between Air-Transportation and Economic Growth: A systematic Literature Review

Today’s economic growth is not promising without air transport, up-to-date airports infrastructure, efficient and safe airline services and international air transport networks. According to WB [1], 40% of the world-wide tourists travel by air transport. The main aim of this study is to analyze the literatures of air transport and economic growth study. A systematic literature review (sLR) method was used in order to examine related articles from ABDC list journals which are ranked on A*, A or B, published in period 1992-2018. This study gives a summary of the trends and research themes which have been identified. Key scholars, as well as their approaches and locations and citations are also mapped. The findings of this study show that there is a growing interest in researching and publishing on Air transportation and economic growth. This study fills a deficiency in specialized literature concerning air transportation in developing countries and provides a foundation for future research in the field of air transportation.

solitary mastocytoma: A rare presentation on the buccal mucosa

Mastocytoma usually presents as a solitary lesion whereas mastocytosis is a rare skin condition which typically manifests with or without accompanied systemic symptoms. Although a few cases of solitary mastocytoma have been documented on mucosa (vulva), none has been reported on the oral mucosa in the English literature. The present case is an example of an exceptional rare presentation of solitary mastocytoma of the oral mucosa.
