To evaluate if any association existed between the extent of hepatic necrosis in initial liver biopsies and patient ***-seven patients with fulminant liver failure, whose liver biopsy exhibited substantial necrosis, were identified and included in the study. The histological and clinical data was then analyzed in order to assess the relationship between the extent of necrosis and patient survival, with and without liver transplantation. The patients were grouped based on the etiology of hepatic necrosis. Each of the etiology groups were then further stratified according to whether or not they had received a liver transplant post-index biopsy, and whether or not the patient *** core tissue length ranged from 5 to 44 mm with an average of 23 mm. Causes of necrosis included 14 autoimmune hepatitis, 10 drug induced liver injury (DILI), 9 hepatitis virus infection, and 4 unknown origin. Among them, 11 showed submassive (26%-75% of the parenchymal volume) and 26 massive (76%-100%) necrosis. Transplant-free survival was worse in patients with a higher extent of necrosis (40%, 71.4% and 100% in groups with necrosis of 76%-100%, 51%-75% and 26%-50%, respectively). Additionally, transplant-free survival rates were 66.7%, 57.1%, and 25.0% in groups of autoimmune hepatitis, DILI, and viral hepatitis, respectively. Even after liver transplantation, the survival rate in patients as a result of viral hepatitis remained the lowest (80%, 100%, and 40% in groups of autoimmune hepatitis, DILI, and viral hepatitis, respectively).CONCLUsIONAdequate liver biopsy with more than 75% necrosis is associated with significant transplant-free mortality that is critical in predicting survival.

A new ferritic creep resistant steel has been developed by eliminating Nb and adding 1.5 mass % Re to a ferritic steel grade T/P23 with the aim of enhancing its mechanical properties at high *** ingots of both steels, new grade and AsTM T/P 23, were hot rolled at 900℃ and then submitted to a thermal treatment consisting of solubilization at 1050℃ and tempering at 700℃. Tempered bainitic microstructures obtained contain second phases reinforcing carbide particles, mainly M_6C and M_（23）C_6 at the boundaries of both, prior austenite grains and bainitic ferrite laths, as well as MC within the grains. Mechanical properties at temperatures ranging from 540 to 600℃ were studied by strain-ratechange tests in compression at strain rates between 10^（-7） and 10^（-4）s^（-1）. These tests showed high stress exponents（n ≥ 20） and activation energies（Q ≈ 400 k J/mol） for both alloys, which were associated with a dislocation movement mechanism with a strong interaction between dislocations and precipitates. On the other hand, a creep exponent of 5 was derived for the stress dependence of minimum creep rate from conventional-type creep tests at 600℃. Although this stress exponent is usually related to a dislocation climb controlled creep mechanism, remarkable microstructural degradation observed with increasing creep time makes difficult to elucidate the true deformation mechanism controlling creep.

Common variable immunodeficiency (CVID) is one of the most prevalent primary immunodeficiency disorders, characterized by an alteration in the maturation of B lymphocytes. Patients with this condition are redisposed to a higher risk of infections. despite being an immune deficiency disorder, the prevalence of autoimmune disorders is reported in more than 20% of patients. The likelihood of patients’ gastrointestinal tract being affected is relatively low, close to 6%. We present the case of a 22-year-old man with a history of CVID without medical treatment, who presented with upper gastrointestinal bleeding secondary to esophageal varices due to cirrhotic portal hypertension. Infectious and toxic causes of cirrhosis were ruled out. Histological changes compatible with autoimmune hepatitis (AIH) were documented by liver biopsy. The diagnosis of autoimmune diseases is a challenge in the presence of IDCV, we highlight the importance of establishing a timely diagnosis and an intentional search for these conditions to offer timely treatment and avoid late complications.

Characteristics of developing scour-holes at a sand-embedded 0.2 m diameter circular cylinder are presented. Non-intrusive, high-resolution topographic measurements of developing scour-holes were made with an experimental installation using a laser distance sensor (LDs) and precision step-motors. A clear-water experiment was conducted with bed shear stress equal to 95% of the critical bed shear stress for the initiation of sediment motion at the undisturbed plane sand bed with a d50 of 0.26 mm. During the running experiment of 21 hours duration, measurements were taken by the LDs in different azimuthal half-planes with θ = 0, 15, 30, 45, 60, 75, 90, 105, 120, 135, 150, 165 and 180o in order to study the spatio-temporal variation of geometric properties in developing scour-holes. After 21 hours, the equilibrium condition was approached, with a scour rate less than d50 per hour. Measurement of maximum scour depth in different azimuthal half-planes at different points in time during the experiment show that scour started at the cylinder side and surrounded the cylinder after about 20% of experiment time. slopes of developing scour-holes presented three regions with different inclinations, which were attributed to vortex action. Both maximum scour depth and maximum scour radius at different azimuthal half-planes are well correlated with the maximum scour depth at plane with θ = 0o. When providing maximum scour depth at the cylinder front, the presented correlations are useful for simple calculation of scour-hole topography. This work provides an experimental database for advanced numerical simulation for loose sediment hydraulics.

In the Mexican Intertropical Convergence Zone, particle size distributions within 500 m of cloud boundaries at altitudes of 1000, 2500, and 4200 m, were compared against size distributions at the same levels but 1500 m away from the clouds. The differences in the distributions near and far from the cloud are related to processes that may change particle properties inside the cloud. Chemical changes in the aerosols are deduced from the particles＇ refractive index, as derived from comparisons with the scattering coeflcient measured by a nephelometer. An analysis of ten cloud systems indicates that vertical transport of cloud base aerosol followed by entrainment/detrainment is the cloud processing signature most frequently observed in the comparisons （65%）. Changes in the chemical composition are observed in approximately 20% of the cases and another 20% of the cases showed removal by precipitation. About 5% of the comparisons showed clear evidence of changes by coalescence. The principal effect of these cloud-processed aerosols is observed in the increase of optical depth in the layer from 30 m to 4200 m in the near-cloud regions, in comparison with the atmosphere further from clouds.

The Impact of Hurricane strikes on short-Term Local Economic Activity: Evidence from Nightlight Images in the Dominican Republic

The Dominican Republic is highly exposed to adverse natural events that put the country at risk of losing hard-won economic, social, and environmental gains due to the impacts of disasters. This study used monthly nightlight composites in conjunction with a wind field model to econometrically estimate the impact of tropical cyclones on local economic activity in the Dominican Republic since 1992. It was found that the negative impact of storms lasts up to 15 months after a strike, with the largest effect observed after 9 months. Translating the reduction in nightlight intensity into monetary losses by relating it to quarterly gross domestic product(GDP) suggests that on average the storms reduced GDP by about UsD 1.1 billion(4.5% of GDP 2000 and 1.5% of GDP 2016).
