Use of agents stimulating erythropoiesis in digestive diseases

Anemia is the most common complication of inflammatory bowel disease (IBD). Control and inadequate treatment leads to a worse quality of life and increased morbidity and hospitalization. Blood loss, and to a lesser extent, malabsorption of iron are the main causes of iron def iciency in IBD. There is also a variable component of anemia related to chronic inflammation. The anemia of chronic renal failure has been treated for many years with recombinant human erythropoietin (rHuEPO), which significantly improves quality of life and survival. subsequently, rHuEPO has been used progressively in other conditions that occur with anemia of chronic processes such as cancer, rheumatoid arthritis or IBD, and anemia associated with the treatment of hepatitis C virus. Erythropoietic agents complete the range of available therapeutic options for treatment of anemia associated with IBD, which begins by treating the basis of the inflammatory disease, along with intravenous iron therapy as f irst choice. In cases of resistance to treatment with iron, combined therapy with erythropoietic agents aims to achieve near-normal levels of hemoglobin/hematocrit (11-12 g/dL). New formulations of intravenous iron (iron carboxymaltose) and the new generation of erythropoietic agents (darbepoetin and continuous erythropoietin receptor activator) will allow better dosing with the same eff icacy and safety.

Genetic variants of innate immune receptors and infections after liver transplantation

Infection is the leading cause of complication after liver transplantation, causing morbidity and mortality in the first months after surgery. Allograft rejection is mediated through adaptive immunological responses, and thus immunosuppressive therapy is necessary after transplantation. In this setting, the presence of genetic variants of innate immunity receptors may increase the risk of post-transplant infection, in comparison with patients carrying wild-type alleles. Numerous studies have investigated the role of genetic variants of innate immune receptors and the risk of complication after liver transplantation, but their results are discordant. Tolllike receptors and mannose-binding lectin are arguably the most important studied molecules; however, many other receptors could increase the risk of infection after transplantation. In this article, we review the published studies analyzing the impact of genetic variants in the innate immune system on the development of infectious complications after liver transplantation.

Usingvery long-range terrestriallaser scanner to analyze the temporal consistency of the snowpack distribution in a high mountain environment

This study demonstrated the usefulness of very long-range terrestrial laser scanning(TLs) for analysis of the spatial distribution of a snowpack, to distances up to 3000 m, one of the longest measurement range reported to date. snow depth data were collected using a terrestrial laser scanner during 11 periods of snow accumulation and melting,over three snow seasons on a Pyrenean hillslopecharacterized by a large elevational gradient, steep slopes, and avalanche occurrence. The maximum and mean absolute snow depth error found was 0.5-0.6 and 0.2-0.3 m respectively, which may result problematic for areas with a shallow snowpack, but it is sufficiently accurate to determine snow distribution patterns in areas characterized by a thick snowpack. The results indicated that in most cases there was temporal consistency in the spatial distribution of thesnowpack, even in different years. The spatial patterns were particularly similar amongst thesurveys conducted during the period dominated by snow accumulation(generally until end of April), or amongst those conducted during the period dominated by melting processes(generally after mid of April or early May). simple linear correlation analyses for the 11 survey dates, and the application of Random Forests analysis to two days representative of snow accumulation and melting periods indicated the importance of topography to the snow distribution. The results also highlight that elevation and the Topographic Position index(TPI) were the main variables explaining the snow distribution, especially during periods dominated by melting. The intra-and inter-annual spatial consistency of the snowpack distribution suggests that the geomorphological processes linked to presence/absence of snow cover act in a similar way in the long term, and that these spatial patternscan be easily identifiedthrough several years of adequate monitoring.

Constructive thinking,rational intelligence and irritable bowel syndrome

AIM： To evaluate rational and experiential intelligence in irritable bowel syndrome （IBs） sufferers. METHODs： We recruited 100 subjects with IBs as per Rome 11 criteria （50 consulters and 50 non-consulters） and 100 healthy controls, matched by age, sex and educational level. Cases and controls completed a clinical questionnaire （including symptom characteristics and medical consultation） and the following tests： rational- intelligence （Wechsler Adult Intelligence scale, 3rd edition）; experiential-intelligence （Constructive Thinking Inventory）; personality （NEO personality inventory）; psychopathology （MMPI-2）, anxiety （state-trait anxiety inventory） and life events （social readjustment rating scale）. Analysis of variance was used to compare the test results of IBs-sufferers and controls, and a logistic regression model was then constructed and adjusted for age, sex and educational level to evaluate any possible association with IBs. REsULTs： No differences were found between IBs cases and controls in terms of IQ （102.0 ± 10.8 vs 102.8 ± 12.6）, but IBs sufferers scored significantly lower in global constructive thinking （43.7± 9.4 vs 49.6 ± 9.7）. In the logistic regression model, global constructive thinking score was independently linked to suffering from IBs [OR 0.92 （0.87-0.97）], without significant OR for total IQ. CONCLUsION： IBs subjects do not show lower rational intelligence than controls, but lower experiential intelligence is nevertheless associated with IBs.

Different strategies of treatment for uterine cervical carcinoma stage ⅠB2-ⅡB

Uterine cervical cancer is the second most common gynecological malignancy. It is estimated that over 35% of tumors are diagnosed at locally advanced disease, stage ⅠB2-ⅡB with an estimated 5-year overall survival of 60%. During the last decades, the initial treatment for these women has been debated and largely varies through different countries. Thus, radical concurrent chemoradiation is the standard of care in United sated and Canada, and neoadjuvant chemotherapy followed by radical surgery is the first line of treatment in some institutions of Europe, Asia and Latin America. Until today, there is no evidence of which strategy is better over the other. This article describe the evidence as well as the advantages and disadvantages of the main strategies of treatment for women affected by uterine cervical cancer stage ⅠB2-ⅡB.

Antioxidant,anti-inflammatory and antinociceptive potential of Ternstroemia sylvatica schltdl.&Cham

ObjectiveTo evaluate the anti-inflammatory, analgesic, antioxidant and acute toxicity of extracts obtained from a successive extraction with solvents of ascending polarity [hexane, hex; chloroform, CHClsub>3sub> and ethanol (EtOH)] of Ternstroemia sylvatica schltdl. & *** antioxidant potential was evaluated by 2,2 diphenyl-1-picrylhydrazyl, the ferric reducing/antioxidant power assays and by determining the total phenolic content. The anti-inflammatory and antinociceptive effects were evaluated using the in vivo croton oil-induced ear edema, phorbol 12-myristate 13-acetate induced ear edema, carrageenan-induced paw edema, acetic acid-induced writhing and formalin murine models. The acute toxicity was tested using the Lorke’s method in *** EtOH extract was the most active for the antioxidant potential tests diphenyl-1-picrylhydrazyl (68.70% inhibition), ferric reducing/antioxidant power [(2 431.30 ± 102.10) mmol Fesup>2+sup> and total polyphenols content (215.80 ± 8.50) meqAG/g]. The anti-inflammatory activity was evaluated by topical application of croton oil (2 mg/ear dose) where the EtOH extract showed the strongest activity compared to the control group (45.13% inhibition), whereas in the phorbol 12-myristate 13-acetate model, at the same dose, the CHClsub>3sub> extract showed the highest inhibition (42.88%). In the carrageenan induced edema model, the EtOH extract showed a stronger inhibition compared to indomethacin (56.34% and 50.70% at doses of 250 and 500 mg/kg of extract, respectively) during the first hour. similarly, the same extract showed the highest analgesic activity (30.60% inhibition) in the acetic acid contortion assay, and in the formalin test it showed a greater effect with respect to the control group in both *** work confirms the value of Ternstroemia sylvatica as an important anti-inflammatory and analgesic plant, whose mechanism seems to be associated to its antioxidant effects, and supports

Prevention of de novo HBV infection by the presence of anti-HBs in transplanted patients receiving core antibody-positive livers

AIM： To analyze whether the presence of anti-HBs in liver transplant recipients is effective in preventing HBV infection. METHODs： Twenty-three patients receiving anti-HBc positive liver were studied. Nine recipients were anti-HBc positive as a result of previous HBV infection. Of them, one also received HBV vaccine during the pre-liver transplantation period. Fourteen recipients were anti-HBs positive due to HBV vaccine administered during the pretransplant period. Liver biopsy was obtained in 10/14 anti-HBc negative/anti-HBs positive recipients and in 4/9 anti-HBc positive recipients. REsULTs： After a mean foUow-up period of 46 months, 1 recipient with protective serum anti-HBs levels developed de novo HBV infection as a consequence of immune escape HBV mutants. Among the 14 vaccinated anti-HBc negative/anti-HBs positive recipients, 1/10 patients with available liver biopsy （10%） had liver HBV-DNA at 13 mo post-liver transplantation without serum viral markers and did not develop de novo HBV *** vaccinated anti-HBc positive recipient without HBV vaccine response was HBV-DNA positive in serum and liver, viral DNA was continuously negative in the following tests, so a spontaneous seroconversion was diagnosed. CONCLUsION： The presence of anti-HBs as a result of HBV vaccine or past HBV infection seems to be effective at protecting patients receiving livers from anti-HBc positive donors. However, the emergence of immune escape HBV mutants, which can evade the anti-HBs protection, should be considered as a risk of HBV infection.

Adjuvant chemotherapy in breast cancer: To use or not to use, the anthracyclines

Breast cancer continues to be one of the leading causes of cancer mortality in the world. The treatment generally involves multiple modalities including surgery, radiation and/or chemotherapy. Anthracyclines, one of the first chemotherapeutic agents introduced in the 1960 s, has been the backbone for the last 30 years and has been used extensively so far. However, the cardiac toxicity and the concern for secondary hematological malignancy has always been a challenge. A better understanding of the tumor biology, role of Her2 expression and the discovery of trastuzumab and other anti-Her 2 agents along with other effective novel therapeutic options, have revolutionized the treatment for breast cancer. The role of anthracyclines has come under close scrutiny, especially in the adjuvant setting for patients with early stage breast cancer and those with low or intermediate risk of disease recurrence. Recent studies have highlighted such a shift in the use of anthracyclines in both the academic and community clinical practice. However, in patients with a high risk of relapse, anthracyclines still hold promise. Ongoing clinical trials are underway to further define the role of anthracyclines in such a patient population. This review highlights the development, clinical utility, limitations and potential future use of anthracyclinesin the adjuvant setting for patients with breast cancer. Modeling of daily pan evaporation using partial least squares regression

This study presented the application of partial least squares regression （PLsR） in estimating daily pan evaporation by utilizing the unique feature of PLsR in eliminating collinearity issues in predictor variables. The climate variables and daily pan evaporation data measured at two weather stations located near Elephant Butte Reservoir, New Mexico, UsA and a weather station located in shanshan County, Xinjiang, China were used in the study. The nonlinear relationship between climate variables and daily pan evaporation was successfully modeled using PLsR approach by solving collinearity that exists in the climate variables. The modeling results were compared to artificial neural networks （ANN） models with the same input variables. The resuits showed that the nonlinear equations developed using PLsR has similar performance with complex ANN approach for the study sites. The modeling process was straightforward and the equations were simpler and more explicit than the ANN black-box models.

Environmental change is a consequence of many interrelated *** vegetation responds to natural and human activity still needs to be well establishe d,quantified and *** cent satellite missions providing hydrologic and ecological indicators enable better monitoring of Earth system changes,yet there is no automatic way to address this issue directly from ***,we develop an observation-based methodology to capture evidence of changes in global terrestrial ecosystems and attribute these changes to natural or anthropogenic *** use the longest time record of global microwave L-band soil moisture and vegetation optical depth as satellite data and build spatially explicit maps of change in soil and vegetation water content and biomass reflecting large ecosystem changes during the last decade,*** of prominent trends(from-8% to 9% per year) are observed,especially in humid and semi-arid *** further combine such trends with land cover change maps,vegetation greenness and precipitation variability to assess their relationship with major documented ecosystem *** regions emerge from our *** cluster changes according to human activity drivers,including deforestation(Amazon,Central Africa) and wildfires(East Australia),artificial reforestation(south-East China),abandonment of farm fields(Central Russia) and climate shifts related to changes in precipitation variability(East Africa,North America and Central Argentina).Using the high sensitivity of soil and vegetation water content to ecosystem changes,microwave satellite observations enable us to quantify and attribute global vegetation responses to climate or anthropogenic activities as a direct measure of environmental changes and the mechanisms driving them.
