Aim:Traditional donor site closure from radial forearm free flap(RFFF)has been associated with esthetic and functional *** avoid complications,such as color mismatch and secondary donor site morbidity,a new technique named‘‘Iberic graft’’for covering the RFFF donor site was described previously by our ***:A study of patients who underwent reconstruction of head and neck defects using a RFFF was conducted to assess postoperative complications of the RFFF donor site and also to evaluate the morbidity in terms of aesthetics and function following the use of the“Iberic graft”.The donor site was covered by the use of a combined local triangular full-thickness skin *** match,quality of the scar,presence of necrosis,dehiscence of the suture or tendon exposure were recorded and ***:One hundred and twenty-five consecutive patients undergoing RFFF harvesting were *** donor site defects ranged from 15 cm2 to 70 cm2;9 patients(7%)had small dehiscences of the forearm skin graft,whereas 2 cases(1.6%)presented tendon ***,partial skin graft loss occurred in a few *** all cases,these sites healed secondarily by conservative management,with no final impairment of *** of the forearm donor site at 1 to 3 months after the primary surgical procedure showed complete defect coverage,good color match,and no scarring along the graft ***:The“Iberic graft”is a reliable method for closing most of RFFF donor site defects as it provides excellent color match and pliability,while obviates the need for a second surgical site.

Diabetes mellitus(DM) that occurs because of chronic liver disease(CLD) is known as hepatogenous diabetes(HD). Although the association of diabetes and liver cirrhosis was described forty years ago, it was scarcely studied for long time. Patients suffering from this condition have low frequency of risk factors of type 2 DM. Its incidence is higher in CLD of viral, alcoholic and cryptogenic etiology. Its pathophysiology relates to liver damage, pancreatic dysfunction, interactions between hepatitis C virus(HCV) and glucose metabolism mechanisms and genetic susceptibility. It associates with increased rate of liver complications and hepatocellular carcinoma, and decreased 5-year survival rate. It reduces sustained virological response in HCV infected patients. In spite of these evidences, the American Diabetes Association does not recognize HD. In addition, the impact of glucose control on clinical outcomes of patients has not been evaluated. Treatment of diabetes may be difficult due to liver insufficiency and hepatotoxicity of antidiabetic drugs. Notwithstanding, no therapeutic guidelines have been implemented up to date. In this editorial, authors discuss the reasons why they think that HD may be a neglected pathological condition and call attention to the necessity for more clinical research on different fields of this disease.

This paper describes an empirical study aiming at identifying the main differences between different logistic regression models and collision data aggregation methods that are commonly applied in road safety literature for modeling collision severity. In particular, the research compares three popular multilevel logistic models （i.e., sequential binary logit models, ordered logit models, and multinomial logit models） as well as three data aggregation methods （i.e., occupant based, vehicle based, and collision based）. six years of collision data （2001-2006） from 31 highway routes from across the province of Ontario, Canada were used for this analysis. It was found that a multilevel multinomial logit model has the best fit to the data than the other two models while the results obtained from occupant-based data are more reliable than those from vehicle- and collision-based data. More importantly, while generally consistent in terms of factors that were found to be significant between different models and data aggregation methods, the effect size of each factor differ sub- stantially, which could have significant implications forevaluating the effects of different safety-related policies and countermeasures.

The capacity of six water stress factors(ε′(i))to track daily light use efficiency(ε)of water-limited ecosystems was *** factors are computed with remote sensing operational products and a limited amount of ground data:ε′1 uses ground precipitation and air temperature,and satellite incoming global solar radiation;ε′(2) uses ground air temperature,and satellite actual evapotranspiration and incoming global solar radiation;ε′_(3) uses satellite actual and potential evapotranspiration;ε′_(4) uses satellite soil moisture;ε′_(5) uses satellite-derived photochemical reflectance index;and ε′_(6) uses ground vapor pressure *** factors were implemented in a production efficiency model based on Monteith’s approach in order to assess their performance for modeling gross primary production(GPP).Estimated GPP was compared to reference GPP from eddy covariance(EC)measurements(GPP EC)in three sites placed in the Iberian Peninsula(two open shrublands and one savanna).ε′_(i) were correlated to ε,which was calculated by dividing GPP EC by ground measured photosynthetically active radiation(PAR)and satellite-derived fraction of absorbed *** results were achieved by ε′(1),ε′(2),ε′(3) and ε′(4) explaining around 40% and 50% of ε variance in open shurblands and savanna,*** terms of GPP,R^(2)≈0.70 were obtained in these cases.

Hepatitis C virus(HCV)-specific cytotoxic T cell(CTL) response plays a major role in viral control during spontaneous infection resolution. These cells develop an exhausted and pro-apoptotic status during chronic onset, being unable to get rid of HCV. The role of this response in contributing to sustained viral response(sVR) after anti-HCV is controversial. Recent studies show that after successful interferon-based anti-HCV treatment, HCV traces are still detectable and this correlates with a peak of HCV-specific CTL response activation, probably responsible for maintaining sVR by subsequent complete HCV clearing. Moreover, sVR patients' serum is still able to induce HCV infection in na?ve chimpanzees, suggesting that the infection could be under the control of the immune system after a successful treatment, being transmissible in absence of this adaptive response. At least theoretically, treatmentinduced viral load decrease could allow an effective HCV-specific CTL response reestablishment. This effect has been recently described with anti-HCV interferonfree regimes, based on direct-acting antivirals. Nevertheless, this is to some extent controversial with interferon-based therapies, due to the detrimental immunoregulatory α-interferon effect on T cells. Moreover, HCV-specific CTL response features during anti-HCV treatment could be a predictive factor of sVR that could have clinical implications in patient management. In this review, the recent knowledge about the role of HCV-specific CTL response in the development of sVR after anti-HCV treatment is discussed.

ZnO nanoparticles(ZnO-NP)present innovative optical,electrical,and magnetic properties that depend on spe-cific characteristics,e.g.,size,distribution,and ***,these properties are essential to address various applications in areas such as electronics,medicine,energy,and *** addition,the performance of this ZnO-NP depends of their preparation which can be done by chemical,physical,and biological ***,nowadays,the main interest in developing ZnO-NP synthesis through biological methods bases on the decrease of use of toxic chemicals or energy applied to the procedures,making the process more cost-effective and environ-mentally ***,the large-scale production of nanoparticles by green synthesis remains a big challenge due to the complexity of the biological extracts used in chemical *** being the case,the preparation of ZnO-NP using Moringa oleifera extract as an alternative biological agent for capping and reduction in synthesis was evaluated in this ***,the results based on the analysis of the optical and structural characterization of the ZnO-NP obtained by employing UV-Vis,DLs,zeta potential,XRD,ATR-FTIR,and FE-sEM indicate mostly the presence of spherical nanosized material with a mean hydrodynamic diameter of 47.2 nm measured by DLs and a mean size diameter of 25 nm observed with FE-sEM ***,in FE-sEM images a homo-geneous dispersion and distribution is observed in the absence of agglutination,agglomeration,or generation of significant lumps of the *** XRD analysis showed that heat annealing induced the crystallite size favor-ing their *** obtained data confirm the synthesis of ZnO-NP and the absence of impurities associated with organic compounds in the annealed ***,those results and low-cost production pre-sent to the synthesized ZnO-NP by this biological method as a useful material in several applications.

spanish investigators described recently the so-called occult hepatitis C virus (HCV) infection, emphasizing the detection of genomic and antigenomic HCV RNA strands in liver and peripheral blood mononuclear cells. Therefore, the persistence of viral replication in occult HCV infection should be considered as a putative source of infection among family members and patients undergoing invasive procedures, transfusion or transplantation. Additionally, the most worrisome finding is that an occult HCV infection may persist in patients with sustained virological response.

Revalorization by lactic acid bacterial fermentation of goat whey from cheese industry as a potential antifungal agent

The industry is continuously searching for new methods to reduce the use of chemical preservatives while still produce safer foods for the consumer. Goat whey is a waste from the cheese industry which is primarily treated as a residue. It is rich in different nutrients and has the potential to be used as a matrix for lactic acid bacteria fermentation. This work aims to find a revalorization of whey for the production of a natural origin antifungal agent by a fermentation that can be used as food ingredient. First different whey culture mediums were developed and fermented. Quantification of different antifungal molecules in whey culture mediums in com-paration to a fermentation of De Man, Rogosa and sharpe (MRs) culture medium evidenced that the production of this antifungal metabolites was similar. The in vitro qualitative and quantitative assays evidenced that the whey-based mediums increased the antifungal activity of the CFs in comparation to the MRs. The fermented whey produced bigger inhibition halos (above 1 cm) in the agar diffusion test in comparation to the fermented MRs (bellow 0.6 cm). Alike, in the minimal fungicidal concentration test the lowest antifungal concentrations were detected on the fermented whey mediums, averaging antifungal concentrations lower than 130 g/L. Finally, a multivariable data analysis determined that more antifungal metabolites yet to be detected were affecting in the antifungal activity from the whey samples. The fermented whey has the potential to be a tool in reducing the use of chemical preservatives in foods.
