Objective: This multicenter clinical study was to assess the clinical usability of an oral moisture-checking device in detecting the dry mouth patients and evaluating the optimal measurement site. Materials and Methods: The study group comprised 250 patients with dry mouth and 241 healthy volunteer subjects at 13 medical centers. This device was used to measure the moisture degrees of the lingual mucosa and the buccal mucosa. Subjective oral dryness, objective oral dryness, and saliva flow rates were also compared between the two groups. For statistical analysis, receiver-operating characteristic analysis was performed to calculate the area under the curve (AUC). Results: The moisture degree of the lingual mucosa was significantly lower in the dry mouth group (27.2 ± 4.9) than that in the healthy group (29.5 ± 3.1, AUC = 0.653). When a lingual mucosa moisture degree of 31.0 or higher was defined as normal, less than 27.0 as dry mouth, and 27.0 to less than 31.0 as borderline zone of dry mouth, both the sensitivity and the specificity for the diagnosis of dry mouth were close to 80%. Conclusion: These results suggest that the oral moisture-checking device is a usable screening device for dry mouth.

Hemi-hepatectomy in pediatric patients using two-surgeon technique and a liver hanging maneuver

AIM： To evaluate the efficacy of the two-surgeon technique with the liver hanging maneuver （LHM） for hepatectomies in pediatric patients with hepatoblastoma. METHODS： Three pediatric patients with hepatoblastoma were enrolled in this study. Two underwent right hemi-hepatectomies and one underwent a left hemihepatectomy using the two-surgeon technique by means of saline-linked electric cautery （SLC） and the Cavitron Ultrasonic Surgical Aspirator （CUSA; Valleylab, Boulder, CO） and the LHM. RESULTS： The mean operative time during the parenchymal transections was 50 rain and the mean blood loss was 235 g. There was no bile leakage from the cut surface after surgery. No macroscopic or microscopic-positive margins were observed in the hepatic transections. CONCLUSION： The two-surgeon technique using SLC and CUSA with the LHM is applicable to even pediatric patients with hepatoblastoma.

为了提高DDT污染土壤的修复效果,研究了非离子表面活性剂Tween60及阴离子表面活性剂SDS在单一和组合两种方式对人工污染黑土中白腐真菌Phlebia lindtneri GB1027降解去除DDT的强化作用.结果表明,Tween60和SDS均能不同程度地促进土壤中DDT的生物降解,尤其在两种表面活性剂浓度为***/g干土时,土壤中DDT的降解率分别达到最高的62.9%和53.9%.相同浓度下,Tween60比SDS更有利于提高DDT污染土壤的生物修复效果,去除率提高接近10%将Tween60与SDS以不同质量比例组合处理后的DDT去除率大小为:Tween60-SD S(3:1)>Tween60-SD S(2:1)>Tween60-SD S(1:1),尤其在质量比为3:1和2:1时的DDT去除率甚至高于Tween60单独处理时的去除率,表明将两种类型的表面活性剂组合后对提高DDT的生物可利用性产生了协同效果研究还发现,菌株接种量越高,土壤中DDT的降解去除率越高,当接菌量达到***/g干土时,DDT的30d去除率达到最高的70.9%.在土壤含水率为10%-50%范围内,白腐真菌对DDT的降解去除效果随着土壤含水率的升高而增加,当土壤含水率达到50%时,土壤中有约70%的DDT被降解去除.本研究结果进一步证实了利用表面活性剂强化白腐真菌修复有机污染土壤的可行性.

Biochemically curative surgery for gastrinoma in multiple endocrine neoplasia type 1 patients

AIM： To search for the optimal surgery for gastrinoma and duodenopancreatic neuroendocrine tumors in patients with multiple endocrine neoplasia type 1. METHODS： Sixteen patients with genetically confirmed multiple endocrine neoplasia type 1 （MEN 1） and Zollinger-Ellison syndrome （ZES） underwent resection of both gastrinomas and duodenopancreatic neuroendocrine tumors （NETs） between 1991 and 2009. For localization of gastrinoma, selective arterial secretagogue injection test （SASI test） with secretin or calcium solution was performed as well as somatostatin receptor scintigraphy （SRS） and other imaging methods such as computed tomography （CT） or magnetic resonance imaging （MRI）. The modus of surgery for gastrinoma has been changed over time, searching for the optimal surgery： pancreaticoduodenectomy （PD） was first performed guided by localization with the SAST test, then local resection of duodenal gastrinomas with dissection of regional lymph nodes （LR）, and recently pancreas-preserving total duodenectomy （PPTD） has been performed for multiple duodenal gastrinomas. RESULTS： Among various types of preoperative localizing methods for gastrinoma, the SASI test was the most useful method. Imaging methods such as SRS or CT made it essentially impossible to differentiate functioning gastrinoma among various kinds of NETs. However, recent imaging methods including SRS or CT were useful for detecting both distant metastases and ectopic NETs; therefore they are indispensable for staging of NETs. Biochemical cure of gastrinoma was achieved in 14 of 16 patients （87.5%）; that is, 100% in 3 patients who underwent PD, 100% in 6 patients who underwent LR （although in 2 patients （33.3%） second LR was performed for recurrence of duodenal gastri- noma）, and 71.4% in 7 patients who underwent PPTD. Extract of Emblica officinalis enhances the growth of human keratinocytes in culture

Objective: Keratinocytes are the predominant cell type in the epidermis and play key roles in epidermal function. Thus, identification of the compounds that regulate the growth of keratinocytes is of importance. Here we searched for such compounds from the herbs used in traditional medicine ***: Human keratinocytes were cultured in the presence or absence of the herbal extracts for 2 weeks; the effect of the extracts on cell growth was determined by staining the cells with Coomassie brilliant blue. To detect the compounds that regulate the growth of keratinocytes, the herbal extracts were subjected to high-performance liquid chromatography(HPLC).Results: We found that the extract of Emblica officinalis enhanced the growth of keratinocytes in ***, we fractionated the extract of E. officinalis using HPLC and identified the fractions responsible for the enhanced growth of ***: The extract of E. officinalis enhanced the growth of human keratinocytes in culture.E. officinalis contains the compounds that would be beneficial for human skin health because enhanced growth of keratinocytes would promote wound healing.

Effect of an extract of Ganoderma lucidum in men with lower urinary tract symptoms： a double-blind, placebo-controlled randomized and dose-ranging study

Aim： To conduct a double-blind, placebo-controlled randomized and dose-ranging study to evaluate the safety and efficacy of the extract of Ganoderma lucidum （G. lucidum） in men with lower urinary tract symptoms （LUTS）. Methods： We enrolled male volunteers （〉 50 years） with an International Prostate Symptom Score （IPSS; questions 1-7）〉 5 and a prostate-specific antigen （PSA） value 〈 4 ng/mL. Volunteers were randomized into groups of placebo （n = 12）, G. lucidum of 0.6 mg （n = 12）, 6 mg （n = 12） or 60 mg （n = 14）, administered once daily. Efficacy was measured as a change from baseline in IPSS and the peak urine flow rate （Qmax）. Prostate volume and residual urine were estimated by ultrasonography, and blood tests, including PSA levels, were measured at baseline and at the end of the treatment. Results： The overall administration was well tolerated, with no major adverse effects. Statistical significances in the magnitude of changes between the experimental groups were observed at weeks 4 and 8. No changes were observed with respect to Qmax, residual urine, prostate volume or PSA levels. Conclusion： The extract of G. lucidum was well tolerated and an improvement in IPSS was observed. The recommended dose of the extract of G. lucidum is 6 mg in men with LUTS.

Randomized clinical trial of an ethanol extract of Ganoderma lucidum in men with lower urinary tract symptoms

Aim： To evaluate the safety and efficacy of an extract of Ganoderma lucidum that shows the strongest 5α-reductase inhibitory activity among the extracts of 19 edible and medicinal mushrooms by a double-blind, placebo-controlled, randomized and dose-ranging study in men with lower urinary tract symptoms （LUTS）. Methods： In this trial, we randomly assigned 88 men over the age of 49 years who had slight-to-moderate LUTS to 12 weeks of treatment with G. lucidum extract （6 mg once a day） or placebo. The primary outcome measures were changes in the International Prostate Symptom Score （IPSS） and variables of uroflowmetry. Secondary outcome measures included changes in prostate size, residual urinary volume after voiding, laboratory values and the reported adverse effects. Results： G. lucidum was effective and significantly superior to placebo for improving total IPSS with 2.1 points decreasing at the end of treatment （mean difference, -1.18 points; 95% confidence interval, -1.74 to -0.62; P 〈 0.0001）. No changes were observed with respect to quality of life scores, peak urinary flow, mean urinary flow, residual urine, prostate volume, serum prostate-specific antigen or testosterone levels. Overall treatment was well tolerated with no severe adverse effects. Conclusion： The extract of G. lucidum was well tolerated and improved IPSS scores. These results encouraged a further, large-scale evaluation of phytotherapy for a long duration using the extract of G. lucidum on men with LUTS.

Enzymatic Degradation of Organochlorine Insecticide Chlordane by Phlebia brevispora

[ Objective ] This study aimed to clarify the mechanism of enzymatic degradation of organochlorine insecticide chlordane by the white rot fungus Pldebia brevispora. [ Method ] The degradation characteristics of chlordane were determined by the crude enzyme extracted from P. brevispora strain by pure culture, ultrasonic fragmentation and centrifuge separation. [ Result] About 33.2% and 10.4% of chlordane were degraded respectively by intracellular enzyme and extracellular enzyme within 20 min. At pH 5.0 and 3℃, the crude enzyme showed the greatest degradation activity （49.9%）, with its Michaelis-Mentn＇s constant （Km ） and maximum degradation rate （ Vmax ） of 3.49 and 8.38 ttmol/min, respectively. [ Conclusion] Extracellular enzyme can degrade chlordane through dehydrogenation or dehydrochlorination. Intracellular enzyme plays a major role in chlordane degradation. It can transform chlordane to some hydroxylated metabolites through hydroxylation or substitution of chlorine atom by hydroxyl group.
