Methyl methacrylate (MMA) was successfully grafted onto cellulose nanofibers (CNFs) at room temperature in an emulsion system using a diethyl(1,10-phenanthroline N1,N10)zinc(II) complex (Phen-DEZ) with oxygen as the radical initiator. The effects of reaction temperature, initiator concentration, and monomer content on the grafting reaction were investigated. The molecular weight of the non-grafted PMMA, which was produced during graft polymerization, was more than 1 million, as determined by size exclusion chromatography. The PMMA-grafted CNFs were analyzed by Fourier transform infrared spectroscopy, thermogravimetric analysis, and scanning electron microscopy, which confirmed the grafting of PMMA on the nanofiber surface. The study presents a strategy for the grafting of high-molecular weight PMMA onto CNFs in an emulsion system using Phen-DEZ and O2.

Resent studies have shown that group of non-submerged spur dikes can provide a valuable habitat for fishes and macro invertebrates in addition to the function to established a more desirable river for a flood, navigation and bank erosion. The study on the effects of non-submerged spur dikes on bed deformation, water surface oscillations and mean flow characteristics by changing the spur dikes facing upstream, downstream and normal to the main flow respectively. The results showed that the spur dikes facing downstream led to the maximum scour at the tip of the dikes, the maximum one in the main channel bed. In addition, it was found that in the case with a facing downstream, resistance is largest.

Background: Intracoronary thrombus followed by a rupture of unstable vulnerable plaque is a well-known cause of acute coronary syndrome (ACS). The no reflow/slow flow phenomenon is sometimes observed during a primary percutaneous coronary intervention (PCI) against ACS. It has already been shown that long inflation using a perfusion balloon (PB) is useful to remediate a coronary perforation. Thus, we investigated the usefulness of a PB for treating ACS. Methods: This study was a retrospective, single-center, observational study. One hundred-seven patients with ACS underwent PCI from January 2015 to December 2017 in our hospital. Fifty patients were treated by PB directly (PB group) and the remaining 57 patients were treated by another conventional balloon (C group). We used the Ryuseiò balloon (Kaneka, Japan) as a PB. The clinical outcome was the incidence of the no reflow or slow flow phenomenon, the incidence of using IABP. Results: One patient in the PB group demonstrated slow flow phenomenon temporarily, and the coronary flow was quickly restored by thromboaspiration. In contrast, nine patients in the C group had occurrences of no reflow/slow flow phenomenon. Although all patients in the C group required stenting, some patients (24%) of the PB group did not require stenting. Conclusion: We found that the use of PB had a favorable effect on the treatment of ACS. Some patients completed PCI without a need for stenting.

Safety assessment of dextrin hydrogel adhesion barrier (AdSpray®) for elective laparoscopic cholecystectomy

Objective:Adhesion barriers have not traditionally been used during laparoscopic cholecystectomy(lapchole),and so there are no data confirming the safety of the dextrin hydrogel adhesion barrier(AdSpray®)for the use during *** aim of this prospective study is to investigate the safety of AdSpray®for elective ***:Between July 2019 and March 2021,we conducted a prospective study at Saiseikai Kumamoto Hospital to assess the safety of applying AdSpray®around the hepatoduodenal ligament,liver surface,and port area after extraction of the gallbladder during elective *** assessed patient morbidity,mortality,and laboratory data on postoperative day 1 and at the outpatient follow-up ***:A total of 100 patients(43 men,57 women)received AdSpray®application during elective lapchole at our *** mean patient age was 56.5±14.8 y,and the mean body mass index was 25.1±4.8 kg/*** gallbladder drainage was performed in 3(3.0%)patients as conservative treatment for acute *** mean operative time was 91.7±35.1 min,and the median blood loss was 5 mL(range,5e120 mL).Intraoperative gallbladder perforation was observed in 4.0%of patients(n=4).There was no intraoperative bile duct ***(1.0%)patient had a postoperative subhepatic fluid collection without bacterial *** mean white blood cell count and C-reactive protein level was significantly elevated on postoperative day 1 but returned to preoperative levels by the time of the follow-up *** patients were successfully discharged after surgery,and the median postoperative hospital stay was 2 d(range,2e9 d).Conclusion:Applying AdSpray®during elective lap-chole is safe,with an acceptable rate of postoperative complications.

Peroxisome proliferators (POPs), such as hypolipidemic drugs or industrial phthalate ester plasticizers, are widely known as non-genotoxic hepatocarcinogens in rodents. As one of the possible mechanisms of POP-induced carcinogenesis, the 'Oxidative Stress' theory has been postulated. In this review, in order to reconsider the significance of 'Oxidative Stress' to POP-induced carcinogenesis, we focus on in vivo studies examining formation of 8-hydroxydeoxyguanosine (8-OH -dG), a marker of oxidative DNA damage with mutagenic potential, after treatment of rodents with POPs. Some studies clearly demonstrated that 8-OH-dG levels in the liver DNA were increased by POP-treatments. These findings suggest that 'Oxidative Stress' could contribute as one factor to POP-induced carcinogenesis. Furthermore, we refer to other multiple biological changes caused by POP-treatment presumably contributing to the carcinogenic mechanisms, and consider possible roles of 'Oxidative Stress' in the carcinogenesis process

Double balloon enteroscopy to retrieve an accidentally swallowed dental reamer deep in the jejunum

Accidentally swallowed foreign objects are not uncommon but difficult to manage without complications. We describe the case of a 68 year old man who accidentally a swallowed sharp-pointed dental reamer that had reached deep in his jejunum. Double balloon enteroscopic retrieval was performed with polypectomy snare but the reamer was entangled in the wire loop of the snare and penetrated the jejunal wall. After releasing the reamer by pushing and pulling the snare for approximately 30 min, the reamer was retrieved with biopsy forceps. This is the first report of double balloon enteroscopic removal of a dental reamer. Furthermore, this is a novel case with regard to decision making in situations when sharp objects are swallowed.

A 47-year-old man presented with general fatigue and dark *** laboratory data showed increased levels of hepatic *** patient was positive for hepatitis B virus(HBV)markers and negative for antihuman immunodeficiency *** HBV-DNA titer was set to 7.7 log copies/*** patient was diagnosed with acute hepatitis *** HBV infection route was *** serum levels of hepatic transaminases decreased to normal ranges without any treatment,but the HBVDNA status was maintained for at least 26 mo,indicating the presence of persistent *** isolated HBV from the acute-phase serum and determined the genome sequence.A phylogenetic analysis revealed that the isolated HBV was genotype *** this patient,the elevated peak level of HBV-DNA and the risk alleles at human genome single nucleotide polymorphisms s3077and rs9277535 in the human leukocyte antigen-DP locus were considered to be risk factors for chronic *** case suggests that there is a risk of persistent infection by HBV genotype H following acute hepatitis;further cases of HBV genotype H infection must be identified and ***,the complete determination of the HBV genotype may be essential during routine clinical care of acute hepatitis B outpatients.

Cytochrome P4502J2(CYP2J2)metabolizes arachidonic acid(AA)to cardioprotective epoxyeicosatrienoic acids(EETs).Dronedarone,an antiarrhythmic drug prescribed for treatment of atrial fibrillation(AF)induces cardiac adverse effects(AEs)with poorly understood *** previously demonstrated that dronedarone inactivates CYP2J2 potently and irreversibly,disrupts AA-EET pathway leading to cardiac mitochondrial toxicity rescuable via EET *** this study,we investigated if mitigation of CYP2J2 inhibition prevents dronedarone-induced cardiac *** first synthesized a deuterated analogue of dronedarone(termed poyendarone)and demonstrated that it neither inactivates CYP2J2,disrupts AA-EETs metabolism nor causes cardiac mitochondrial toxicity in *** patch-clamp experiments demonstrated that pharmacoelectrophysiology of dronedarone is unaffected by ***,we show that dronedarone treatment or CYP2J2 knockdown in spontaneously beating cardiomyocytes indicative of depleted CYP2J2 activity exacerbates beat-to-beat(BTB)variability reflective of proarrhythmic *** contrast,poyendarone treatment yields significantly lower BTB variability compared to dronedarone in cardiomyocytes indicative of preserved CYP2J2 ***,poyendarone and dronedarone display similar antiarrhythmic properties in the canine model of persistent AF,while poyendarone substantially reduces beat-to-beat variability of repolarization duration suggestive of diminished proarrhythmic *** findings prove that deuteration of dronedarone prevents CYP2J2 inactivation and mitigates dronedarone-induced cardiac AEs.
