AIM： To evaluate the effect of combination treatment with the interferon （IFN） and angiotensin-converting enzyme inhibitor （ACE-Ⅰ） on several fibrotic indices in patients with refractory chronic hepatitis C （CHC）. METHODS： Perindopril （an ACE-Ⅰ; 4 mg/d） and/or natural IFN （3 MU/L; 3 times a week） were administered for 12 mo to refractory CHC patients, and several indices of serum fibrosis markers were analyzed. RESULTS： ACE-Ⅰ decreased the serum fibrosis markers, whereas single treatment with IFN did not exert these inhibitory effects. However, IFN significantly augmented the effects of ACE-Ⅰ, and the combination treatment exerted the most potent inhibitory effects. The serum levels of alanine transaminase and HCV-RNA were not significantly different between the groups, whereas the plasma level of transforming growth factor-β was significantly attenuated almost in parallel with suppression of the serum fibrosis markers. CONCLUSION： The combination therapy of an ACE-Ⅰand IFN may have a diverse effect on disease progression in patients with CHC refractory to IFN therapy through its anti-fibrotic effect.

AIM To evaluate the diagnostic performance of angiotensinconverting enzyme(ACE)on significant liver fibrosis in patients with chronic hepatitis B(CHB). METHODS In total,100 patients with CHB who underwent liver biopsy in our hospital were enrolled,and 70 patients except for 30 patients with hypertension,fatty liver or habitual alcoholic consumption were *** compared histological liver fibrosis and serum ACE levels and evaluated the predictive potential to diagnose significant liver fibrosis by comparison with several biochemical marker-based indexes such as the aspartate aminotransferase(AST)-to-platelet ratio index(APRI),the fibrosis index based on four factors(FIB-4),the Mac-2 binding protein glycosylation isomer(M2BPGi)level and the number of platelets(Plt). RESULTS Serum ACE levels showed moderately positive correlation with liver fibrotic stages(R2=0.181).Patients with significant,advanced fibrosis and cirrhosis(F2-4)had significantly higher serum ACE levels than those with early-stage fibrosis and cirrhosis(F0-1).For significant fibrosis(≥F2),the 12.8 U/L cut-off value of ACE showed 91.7%sensitivity and 75.0%*** receiver-operating characteristic(ROC)curves analysis revealed that the area under the curve(AUC)value of ACE was 0.871,which was higher than that of APRI,FIB-4,M2BPGi and Plt. CONCLUSION The serum ACE level could be a novel noninvasive,easy,accurate,and inexpensive marker of significant fibrosis stage in patients with CHB.

X-Ray Diffraction Is a Promising Tool to Characterize Coral Skeletons

Objective Unilateral L5 spinal nerve ligature can induce neuropathic pain and NMDA receptor-independent synaptic potentiation in neurons in the latero-capsular part of the central nucleus of amygdale(CeLC).CeLC receives predominantly nociceptive information from the spinal dorsal horn via the lateral parabrachial nucleus(LPB)(spinoparabrachio -amygdaloid pathway) and is called the "nociceptive amygdala".However,it remains unknown about the morphological mechanisms underlying such synaptic plasticity at the ultrastructural *** To address this issue,one week after the left L5 spinal nerve ligation,neuropathic pain rats which received GFP-expressing lentivirus injection into LPB were used for electron microscopic and reconstruction *** operation rats with the lentivirus injection were used as a control *** LPB terminals in the right CeLC were shown as GFP-positive presynaptic buttons and made asymmetrical synapses mainly with dendritic *** spinal nerve ligature induced a significant difference in PSD length when compared to the control[(366.33±151.30) nm vs(284.43±105.12) nm,respectively;P<0.001, Kolmogrov-Smirnov test].And the results taken from serial ultrathin sections showed that PSD areas in right CeLC of neuropathic pain group had a significant increase compared to the sham group[(129725.9±4892.34) nm vs(89218.24±7147.75) nm,respectively;P<0.002).To approve this result,another marker of projecting fiber terminals in CeLC from LPB,CGRP immunoreactivity was used in both ligated and sham *** with the above results,spinal nerve ligature induced a significant enlargement of CGRP positive synaptic PSD lengths[(356.56±138.71) nm vs(269.18±92.61) nm, respectively,P<0.001]and areas[(115351.6±7302.79) nm vs(83801.82±7177.07) nm,respectively,P<0.01]in the right *** addition,the ratio of non-macular PSDs counted from either GFP-positive or CGRP-positive synapses had significant differences between operation and sham group

The skeletons of corals are made of calcium carbonate by biomineralization process, in the form of aragonite or calcite. To understand the characteristics of coral skeletons, especially mineralogy, crystal phases, organization and structure in individual species, X-ray powder diffraction techniques have gained increased interest in recent years as useful non-destructive tools. This review provides an overview on the recent progress in this field and briefly introduces the related experimental approach. The application of X-ray diffraction (XRD) to elucidating the structural and mechanical properties of mineral crystals in corals is reviewed in terms of characterization of CaCO3 crystal orientation. In addition, we discuss how this technique has increased our understanding of the function of the organic matrix proteins of calcified coral skeletons during mineral formation. Such information is helpful in deducing the mechanical and structural model of corals with respect to biomineralization system of skeletons.

In Japan, the shortage of personnel is a problem in long-term care nursing and rehabilitative care prevention. Nevertheless, Japan has taken measures to compensate for these shortages by promoting medical and nursing care activities using robotic technologies, and employing human resources from overseas. The purpose of this study was to determine potential legal issues and subsequent implications for care during prevention gymnastic exercises for the elderly using Pepper in long-term health facilities. The application program of Care-Prevention Gymnastics Exercises for Pepper (Pepper with CPGE) was made by the Xing Company Japan. Currently, care workers become intermediaries for the safe use of Pepper with CPGE. However, it was realized that some legal issues may arise if Pepper with CPGE alone will carry out these preventive care programs for the elderly without the presence of care workers as intermediaries. In this situation, using Pepper with CPGE alone to conduct care prevention gymnastics will require safety measures to prevent these possible practice issues and anticipate implications for care. In this regard, determining detailed target levels of rehabilitation exercise demands and environmental setting safety become essential factors. The use of humanoid robots in healthcare is expected to influence more practice protocols in contemporary and futurist rehabilitative human care. The identification of possible safety issues in performance and environmental situations, and implications for care are critical to ensure safe and valuable rehabilitative health care practices for the elderly population.

Superior mesenteric artery(SMA) syndrome is defined as a compression of the third portion of the duodenum by the abdominal aorta and the overlying SMA. SMA syndrome associated with anorexia nervosa has been recognized, mainly among young female patients. The excessive weight loss owing to the eating disorder sometimes results in a reduced aorto-mesenteric angle and causes duodenal obstruction. Conservative treatment, including psychiatric and nutritional management, is recommended as initial therapy. If conservative treatment fails, surgery is often required. Currently, traditional open bypass surgery has been replaced by laparoscopic duodenojejunostomy as a curative surgical approach. However, single incision laparoscopic approach is rarely performed. A 20-yearold female patient with a diagnosis of anorexia nervosa and SMA syndrome was prepared for surgery after failed conservative management. As the patient had body image concerns, a single incision laparoscopic duodenojejunostomy was performed to achieve minimal scarring. As a result, good perioperative outcomes and cosmetic results were achieved. We show the first case of a young patient with SMA syndrome who was successfully treated by single incision laparoscopic duodenojejunostomy. This minimal invasive surgery would be beneficial for other patients with SMA syndrome associated with anorexia nervosa, in terms of both surgical and cosmetic outcomes.

High-Order FEM Formulation for 3-D Slope Instability

High-order finite element method (FEM) formulation also referred to as spectral element method (SEM) formulation is currently implemented in this paper for 3-dimensional (3-D) elasto-plastic problems in stability assessment of large- scale slopes (vegetated and barren slopes) in different instability conditions such as seismic and saturation. We have reviewed the SEM formulation, and have sought its applicability for vegetated slopes. Utilizing p (high-order polynomial degree or spectral degrees) and h (mesh operation for quality meshing in required elemental budgets) refining techniques in the existing FEM, the complexity of problem domain can be well addressed in greater numerical stability. Unlike the existing FEM formulation, this high-order FEM employs the same integration and interpolation points to achieve a progressive response of the instability, which drastically reduces the computational costs (formation of diagonalized mass matrix) and offers significant benefits to slope instability computations for serial and parallel implementations. With this formulation, we have achieved the following three qualities in slope instability modeling: 1) geometric flexibility of the finite elements, 2) high computational efficiency, and 3) reliable spectral accuracy. A sample problem has also been presented in this paper, which has accommodated all aforesaid numerical qualities.

Subtropical “Yambaru” forest, situated in the northern part of Okinawa Island of Japan, has a precious ecosystem inhabited by many endemic species. However, this region is also the center for forestry on Okinawa. Therefore, sustainable forestry activities should take into consideration the natural environment. To contribute to sustainable forest management in the region, we conducted prediction of the site index at fine-scale resolution by using multiple regression analysis with easily calculated topographic factors. For the multiple regression analysis with site index as a dependent variable, three topographic factors (the effective relief, openness, and elevation) were adopted as independent variables. Approximately 68% of the variance was explained, and the effective relief was the variable with the greatest influence. This means that it is possible to predict forest productivity at a finer scale of resolution than ever before. For sustainable forest management of sites where environmental conservation and forestry are conflicting, it is useful to estimate the site index at the finest scale of resolution practically available in the field. It might be possible to improve estimation accuracy by examining further environmental factors in the future.

A pilot randomized control study to evaluate endoscopic resection using a ligation device for rectal carcinoid tumors

AIM： Rectal carcinoid tumors smaller than 10 mm can be resected with local excision using endoscopy. In order to remove rectal carcinoid tumors completely, we evaluated endoscopic mucosal resection with a ligation device in this pilot control randomized study. METHODS： Fifteen patients were diagnosed with rectal carcinoid tumor （less than 10 mm） in our hospital from 1993 to 2002. There were 9 males and 6 females, with a mean age 61.5 years （range, 34-77 years）. The patientshad no complaints of carcinoid syndrome symptoms. Fifteen patients were randomly divided into 2 groups： 7 carcinoid tumors were treated by conventional endoscopic resection, and 8 carcinoid tumors were treated by endoscopic resection using a ligation device. RESULTS： All rectal carcinoid tumors were located at the middle to distal rectum. The size of the tumors varied from 3 mm to 10 mm and background characteristics of the patients were not different in the two groups. The rate of complete removal of carcinoid tumors using a ligation device （100%, 8/8） was significantly higher than that of conventional endoscopic resection （57.1%, 4/7）. The three patients had tumor involvement of deep margin, for which additional treatment was performed. No complications occurred during or after endoscopic resection using a ligation device. All patients in the both groups were alive during the 3-year observation period. CONCLUSION： Endoscopic resection using a ligation device is a useful and safe method for resection of small rectal carcinoid tumors.
