Stochastic weather generators create time series that reproduce key weather dynamics present in long-term *** dataset detailed herein is a large-scale gridded parameterization for CLImate GENerator(CLIGEN)that fills spatial gaps in the coverage of existing regional CLIGEN parameterizations,thereby obtaining near-global availability of combined *** dataset primarily covers countries north of 40°latitude with 0.25°spatial *** CLIGEN parameters were estimated based on 20-year records from four popular global climate *** parameters were statistically downscaled to estimate point-scale values,while point-scale temperature and solar radiation parameters were approximated by direct calculation from high-resolution *** parameter values were used in some cases,such as with wind ***-validation was done to assess the downscaling approach for six precipitation parameters using known point-scale values from ground-based CLIGEN *** parameter values were derived from daily accumulation records at 7,281 stations and high temporal resolution records at 609 *** sensitive parameters,monthly average storm accumulation and maximum 30-minute intensity,were shown have RMSE values of 1.48 mm and 4.67 mm hr^(−1),*** precipitation and the annual number of days with precipitation occurrence were both within 5%of ground-based parameterizations,effectively improving climate data availability.

Glioma is one of the most common malignant tumors of the central nervous system,leading high mortality rates in ***-induced emission(AIE)photosensitizers-based photodynamic therapy(PDt)has emerged as a promising therapeutic strategy for least-invasive treatment of glioma,which involves local irradiation of the tumor using an external near-infrared(NIR)***,most AIE photosensitizers suffered from poorly penetration of the visible light excitation,bad spatiotemporal resolution in deep tissues and low efficient blood-brain barrier(BBB)crossing ability,which greatly limited the clinical practice of AIE photosensitizers for especially deep-seated brain tumor *** this work,we developed a multifunctional NIR-driven theranostic agent through hybrid of AIE photosensitizers tIND with rare-earth doping nanoparticles(RENPs)NaGdF4:Nd/Yb/tm with up/down dual-mode conversion *** theranostic agent was further decorated with D-type neuropeptide DNPY for crossing BBB and targeting *** the 808-nm light irradiation,the down-conversion NIR-II luminescence could indicate the position glioma and the upconversion NIR-I luminescence could trigger the AIE photosensitizers producing reactive oxygen species to inhibit orthotopic glioma tumor growth in *** results demonstrate that the integration of Dtype neuropeptide,AIE photosensitizers and RENPs could be promising candidates for in vivo NIR-II fluorescence image-guided through-skull PDt treatments of brain tumors.

Objective:the objective of this study was to compare cardiac transplant operative and postoperative courses of patients with failed Fontan physiology who were initially palliated with a Norwood(FFN)to those without a prior Norwood(FF).Methods:A single-institution retrospective review of all patients with Fontan failure who under-went cardiac transplantation from 2003–2021 was completed-22 underwent prior Norwood(FFN)and 11 did not(FF).Descriptive and inferential statistics were calculated for operative course and patient ***:the operative course of the FFN cohort appeared to be more complex(not statistically significant,but clinically relevant)-this group exclusively experienced sternal re-entry events(3 of 22 patients)and concomitant neo-aor-tic reconstruction(6 patients),had a longer duration of surgery(median of 682 min vs.575.5 min),more time on circulatory arrest(median of 25.5 min vs.12.5 min),and more frequent use of open sternal management[50%of patients(11/22)vs.27.3%of patients(3/11)].Postoperatively,these patients underwent more mediastinal explora-tions[other than sternal closure;40.9%of patients(9/22)vs.18.2%of patients(2/11)],spent more time on mechanical ventilation(median of 5 days vs.2 days),had a longer length of stay(median of 30 days vs.19 days),and required more catheter-based re-interventions[22.7%of patients(5/22)vs.9.1%of patients(1/11)].Conclu-sion:Although underpowered,our results suggest that the operative course of FFN patients is more challenging,based mostly on neo-aortic arch *** turn,this likely leads to a more complex postoperative *** are currently collaborating with other institutions to increase the cohort size and power of the study.

After spinal cord injury,there is an extensive infiltration of immune cells,which exacerbates the injury and leads to further neural ***,a major aim of current research involves targeting the immune response as a treatment for spinal cord *** much research has been performed analyzing the complex inflammatory process following spinal cord injury,there remain major discrepancies within previous literature regarding the timeline of local cytokine *** objectives of this study were to establish an overview of the timeline of cytokine regulation for 2 weeks after spinal cord injury,identify sexual dimorphisms in terms of cytokine levels,and determine local cytokines that significantly change based on the severity of spinal cord *** were inflicted with either a mild contusion,moderate contusion,severe contusion,or complete transection,7 mm of spinal cord centered on the injury was harvested at varying times post-injury,and tissue homogenates were analyzed with a Cytokine/Chemokine 27-Plex *** demonstrated pro-inflammatory cytokines including tumor necrosis factorα,interleukin-1β,and interleukin-6 were all upregulated after spinal cord injury,but returned to uninjured levels within approximately 24 hours post-injury,while chemokines including monocyte chemoattractant protein-1 remained upregulated for days *** contrast,several anti-inflammatory cytokines and growth factors including interleukin-10 and vascular endothelial growth factor were downregulated by 7 days *** spinal cord injury,tissue inhibitor of metalloproteinase-1,which specifically affects astrocytes involved in glial scar development,increased more than all other cytokines tested,reaching 26.9-fold higher than uninjured *** a mild injury,11 cytokines demonstrated sexual dimorphisms;however,after a severe contusion only leptin levels were different between female and male *** conclusion,pro-inflammatory cytokines in

Outpatients with an acquired brain injury(ABI)experience physical,mental,and social *** can be classified into two subgroups based on mechanism of injury:mild traumatic brain injury(mtBI;e.g.,concussion)and other ABI(e.g.,stroke,brain aneurysm,encephalitis).Our understanding of habitual activity levels within ABI populations is limited because they are often collected using self-report *** purpose of this study was to,1)describe the habitual activity levels of outpatients with ABI using objective and self-report monitoring,and 2)compare the activity levels of outpatients with mtBI *** *** outpatients with other ABI(mean±standard deviation:[58±13]years,9 females)and 12 outpatients with mtBI([48±11]years,9 females)wore a thigh-worn activPAL 24 h/day(h/day)for *** with ABI averaged(6.0±2.3)h/day of upright time,(10.6±2.2)h/day of sedentary time,(5.6±2.7)h/day in prolonged sedentary bouts>1 h,(5960±3037)steps/day,and(11±13)minutes/day(min/day)of moderate-vigorous physical activity(MVPA).there were no differences between activPAL-derived upright,sedentary,prolonged sedentary time,and physical activity between the mtBI and other ABI groups(all,p>0.31).Outpatients with ABI overestimated their MVPA levels(t138 min/week)and underestimated sedentary time(-4.3 h/day)compared to self-report(all,pte self-reporting high activity levels,outpatients with ABI objectively exhibit highly inactive and sedentary *** habitual movement behaviours of our sample did not differ by mechanism of injury(i.e.,mtBI versus other ABI).targeting reductions in objectively measured sedentary time are needed to progressively improve the habitual movement behaviours of outpatients with ABI.

Objective:to identify possible stone-promoting microbes,we compared the profiles of microbes grown from stones of patients with and without metabolic syndrome(MetS).the association between MetS and urinary stone disease is well established,but the exact pathophysiologic relationship remains *** evidence suggests urinary tract dysbiosis may lead to increased nephrolithiasis ***:At the time of percutaneous nephrolithotomy,bladder urine and stone fragments were collected from patients with and without *** sample types were subjected to expanded quantitative urine culture(EQUC)and 16 S ribosomal RNA gene ***:Fifty-seven patients included 12 controls(21.1%)and 45 MetS patients(78.9%).Both cohorts were similar with respect to demographics and non-MetS *** controls had uric acid stone *** EQUC,bacteria were detected more frequently in MetS stones(42.2%)compared to controls(8.3%)(p=0.041).Bacteria also were more abundant in stones of MetS patients compared to *** validate our EQUC results,we performed 16 S ribosomal RNA gene *** 12/16(75.0%)sequence-positive stones,EQUC reliably isolated at least one species of the sequenced *** were detected in both“infectious”and“non-infectious”stone ***:Bacteria are more common and more abundant in MetS stones than control *** findings support a role for bacteria in urinary stone disease for patients with MetS regardless of stone composition.

BACKGROUND Improved adenoma detection rate(ADR)has been demonstrated with artificial intelligence(AI)-assisted ***,data on the real-world appli-cation of AI and its effect on colorectal cancer(CRC)screening outcomes is *** to analyze the long-term impact of AI on a diverse at-risk patient population undergoing diagnostic colonoscopy for positive CRC screening tests or *** AI software(GI Genius,Medtronic)was implemented into the standard proced-ure protocol in November *** was collected on patient demographics,procedure indication,polyp size,location,and *** screening outcomes were evaluated before and at different intervals after AI introduction with one year of *** We evaluated 1008 colonoscopies(278 pre-AI,255 early post-AI,285 established post-AI,and 190 late post-AI).the ADR was 38.1%pre-AI,42.0%early post-AI(P=0.77),40.0%established post-AI(P=0.44),and 39.5%late post-AI(P=0.77).there were no significant differences in polyp detection rate(PDR,baseline 59.7%),advanced ADR(baseline 16.2%),and non-neoplastic PDR(baseline 30.0%)before and after AI *** In patients with an increased pre-test probability of having an abnormal colonoscopy,the current generation of AI did not yield enhanced CRC screening metrics over high-quality *** the potential of AI in colonoscopy is undisputed,current AI technology may not universally elevate screening metrics across all situations and patient *** studies that analyze different AI systems across various patient populations are needed to determine the most effective role of AI in optimizing CRC screening in clinical practice.

In this editorial,we comment on the article by He et al,specifically in relation to the efficacy of bariatric surgery vs glucagon-like peptide-1 receptor agonist(GLP-1RA)therapy in the management of type 2 diabetes(t2D)associated with *** surgery has now also been shown to be safe and effective in pre-teens and teenagers with obesity and t2D,but information on newer GLP-1RAs in these groups is predictably *** older individuals(age>65 years),both bariatric surgery and GLP-1RA therapy improve cardiovascular ***-tric surgery is not infrequently associated with post-operative postprandial hypoglycemia,which is not the case with GLP-1RAs and,paradoxically,there is evidence that GLP-1RAs may reduce both the frequency and severity of postprandial *** trials of the long-term efficacy of bariatric surgery and GLP-1RAs are indicated.
