Assessment of multi-modality evaluations of obscure gastrointestinal bleeding

AIMTo determine the frequency of bleeding source detection in patients with obscure gastrointestinal bleeding (OGIB) who underwent double balloon enteroscopy (DBe) after pre-procedure imaging [multiphase computed tomography enterography (MPCTe), video capsule endoscopy (VCe), or both] and assess the impact of imaging on DBe diagnostic *** cohort study using a prospectively maintained database of all adult patients presenting with OGIB who underwent DBe from September 1st, 2002 to June 30th, 2013 at a single tertiary *** hundred and ninety five patients (52% females; median age 68 years) underwent DBe for OGIB. AVCe and/or MPCTe performed within 1 year prior to DBe (in 441 patients) increased the diagnostic yield of DBe (67.1% with preceding imaging vs 59.5% without). Using DBe as the gold standard, VCe and MPCTe had a diagnostic yield of 72.7% and 32.5% respectively. There were no increased odds of finding a bleeding site at DBe compared to VCe (OR = 1.3, P = 0.150). There were increased odds of finding a bleeding site at DBe compared to MPCTe (OR = 5.9, P ents with overt OGIB, diagnostic yield of DBe was not affected by preceding *** is a safe and well-tolerated procedure for the diagnosis and treatment of OGIB, with a diagnostic yield that may be increased after obtaining a preceding VCe or MPCTe. However, inpatients with active ongoing bleeding may benefit from proceeding directly to antegrade DBe.

Temperature and composition dependent screw dislocation mobility in austenitic stainless steels from large-scale molecular dynamics

extensive molecular dynamics simulations are performed to determine screw dislocation mobility in austenitic Fe_(0.7)NixCr_(0.3-x) stainless steels as a function of temperature ranging from 100 to 1300 K,resolved shear stress from 30 to 140 MPa,and Ni composition from 0.0 to 30.0 at%.These mobility data are fitted to a linear mobility law with a nonzero stress offset,referred to as the threshold *** find that both the linear drag coefficient and the threshold stress increase with Ni *** drag coefficient increases with temperature,whereas the threshold stress decreases with *** on these calculations,we determine fitting functions for the linear solute drag coefficient as a function of temperature and *** mobility laws determined in this study may serve to inform dislocation dynamics simulations pertinent to dislocation network evolution at elevated temperatures for a wide composition range of austenitic stainless steels.

Percutaneous transluminal angioplasty balloons for endoscopic ultrasound-guided pancreatic duct interventions

BACKGROUND endoscopic ultrasound(eUS)-guided main pancreatic duct(PD)access may be used when conventional endoscopic retrograde cholangiopancreatography(eRCP)techniques *** use of a percutaneous transluminal angioplasty balloon(PTAB),originally developed for vascular interventions,can be used to facilitate transmural(e.g.,transgastric)PD access and to dilate high-grade pancreatic *** To describe the technique,efficacy,and safety of PTABs for eUS-guided PD *** Patients who underwent eUS with use of a PTAB from March 2011 to August 2021 were retrospectively identified from a tertiary care medical center supply *** included 3-4 French angioplasty catheters with 3-4 mm balloons designed to use over a 0.018-inch *** primary outcome was technical *** outcomes included incidence of adverse events(Aes)and need for early *** A total of 23 patients were identified(48%female,mean age 55.8 years).Chronic pancreatitis was the underlying etiology in 13(56.5%)patients,surgically altered anatomy(SAA)with stricture in 7(30.4%),and SAA with post-operative leak in 3(13.0%).Technical success was achieved in 20(87%)*** Ae rate was 26%(n=6).All Aes were mild and included 1 pancreatic duct leak,2 cases of post-procedure pancreatitis,and 3 admissions for post-procedural *** patients required early *** eUS-guided use of PTABs for PD access and/or stricture management is feasible with an acceptable safety profile and can be considered in patients when conventional eRCP cannulation fails.

Assessment of the Neurological Activation in law enforcement under High Threat Situations: A Fuzzy Logic Approach

This paper discusses a law enforcement officer (LeO) study that involved expert and novice police deputies from a small-sized city located in the Southern U.S. A virtual reality range was utilized to simulate high threat scenarios that require split second decisions on the use of deadly force. A fuzzy-logic based controller was constructed to analyze electroencephalogram (eeG) data collected from the participants. The fuzzy controller made use of several functions associated with the different regions of the brain to correlate Brodmann areas to multiple outputs. electromagnetic Tomography (i.e. LOReTA) was used to identify where the signals from the surface electrodes originated within the brain through a process called source localization. Once the sources of the eeG signals were located, they were associated with corresponding Brodmann areas. The fuzzy controller then provided insights on the subjects’ exhibited neural activation behavior indicative of vision, memory, shape/distance, hearing/sound, and theory of mind. Comparing and contrasting experienced and novice officers allowed for a greater understanding of the neurological processes present in police deputies when dealing with high threat situations.
