Ninety-day readmissions after inpatient cholecystectomy: A 5-year analysis

AIM To determine the incidence of readmission after cholecystectomy using 90 d as a time limit. METHODS We retrospectively reviewed all patients undergoing cholecystectomy at the General Surgery and Digestive System Service of the University Hospital of Guadalajara, Spain. We included all patients undergoing cholecystectomy for biliary pathology who were readmitted to hospital within 90 d. We considered readmission to any hospital service as cholecystectomyrelated complications. We excluded ambulatory cholecystectomy, cholecystectomy combined with other procedures, oncologic disease active at the time of cholecystectomy, finding of malignancy in the resection specimen, and scheduled re-admissions for other unrelated pathologies. RESULTS We analyzed 1423 patients. There were 71 readmissions in the 90 d after discharge, with a readmission rate of4.99%. Sixty-four point seven nine percent occurred after elective surgery(cholelithiasis or vesicular polyps) and 35.21% after emergency surgery(acute cholecystitis or acute pancreatitis). Surgical non-biliary causes were the most frequent reasons for readmission, representing 46.48%; among them, intra-abdominal abscesses were the most common. In second place were non-surgical reasons, at 29.58%, and finally, surgical biliary reasons, at 23.94%. Regarding time for readmission, almost 50% of patients were readmitted in the first week and most second readmissions occurred during the second month. Redefining the readmissions rate to 90 d resulted in an increase in re-hospitalization, from 3.51% at 30 d to 4.99% at 90 d. CONCLUSION The use of 30-d cutoff point may underestimate the incidence of complications. The current tendency is to use 90 d as a limit to measure complications associated with any surgical procedure.

Boosting hydrogen and chemicals production through ethanol electro-reforming on Pt-transition metal anodes

The aim of this work is to boost the combined hydrogen and added-values compounds generation(acetaldehyde, acetic acid and ethyl acetate) through ethanol electrochemical reforming using bimetallic anodes. In particular, the influence of the secondary metal on the electrochemical performance as well as on the product distribution was studied. For that purpose, Pt X/C electrocatalysts(where X corresponds to Cu, Co, Ni and Ru) were synthesized by the modified polyol method and tested in both half-cell and proton exchange membrane(PEM) cell configurations. Characterization results showed that incorporation of Ni and Co into the Pt matrix enhances the morphological properties of the material, providing smaller crystallite sizes, higher active surface areas and hence, better dispersion when comparing to Ru and Cu-based electrocatalysts. Ethanol oxidation reaction(EOR) was evaluated by cyclic, linear voltammetry and chronopotentiometry assays. Pt Co/C and Pt Ni/C exhibited the highest electrocatalytic activity at high polarization levels, which translate into an improvement of more than 30%(up to 1050 m A cm^(-2)) in the hydrogen production and chemical yields. On the other hand, Pt Ru/C results more advantageous for a lower potential interval(v) promoting the acetic acid production despite sacrificing ethanol conversion. Pt Cu/C presented the lowest results in both electrochemical performance and product distribution. Such differences in the electrochemical performance can be rationalized in terms of the synergistic effect between both metals(particle size distribution, grade of dispersion and hydrophilic behavior), which demonstrate that the incorporation of a different secondary metal plays an essential role in the EOR development.

Using a modern analogue to interpret depositional position in ancient fluvial-tidal channels:Example from the McMurray Formation,Canada

The fluvial-tidal transition(FTT)is a complex depositional zone,where fluvial flow is modified by tides as rivers approach a receiving marine *** in the relative importance of tidal versus fluvial processes lead to a distinctive distribution of sediments that accumulate on channel *** FTT generally consists of three broad zones:(1)a freshwater-tidal zone;(2)a tidally influenced freshwater to brackish-water transition:and(3)a zone of relatively sustained brackish-water conditions with stronger tides.A very common type of deposit through the fluvial-tidal transition,especially on the margins of migrating channels,is inclined heterolithic stratification(IHS).At present,a detailed account of changes in the character of IHS across the FIT of a paleo-channel system has not been reported,although a number of modern examples have been *** fill this gap,we quantitatively assess the sedimentology and ichnology of IHS from seven cored intervals in three geographic areas situated within the youngest paleovalley("A"valley)in the Lower Cretaceous McMurray Formation of *** compare the data to trends defined along the FTT in the present-day Fraser River in British *** to interpret paleo-depositional position in the ancient fluvial-tidal *** determined that the mean mudstone thickness is 8.2 cm in the southern study area(SA).Mean thickness increases to 11 cm in the central study area(CA),and decreases again to 4.4 cm in the northern study area(NA).The proportion of mudstone is 31%in SA,44%in CA,and 27%in *** mean bioturbation intensity in sands varied from 0.29 in SA and *** 0.28 in *** the other hand,thickness-weighted mean bioturbation intensity(Bl)in mudstone increases from 1.46 in *** 1.77 in *** is 1.94 in *** ichnological diversity also increased from south to *** results show sinilar trends to those of the Fraser River,enabling the identification of a freshwater

Derivation of the Spiral Motion of an Eruptive Prominence and Its Explanation

A 2D velocity field of the eruptive prominence （EP） of 1991 March 5 is obtained from its spectral data observed at the Yunnan Observatory and the velocity distributions along the entrance slit are derived for different observing frames. Under the assumption that matter in the EP undergoes axial, radial and possible rotational motions, we construct a theoretical velocity distribution of the EP along the entrance slit, to derive, by fitting, the angular velocity of rotation ω and the other three parameters （axial velocity v0, radial velocity vr and the angle between the EP plane and the line of sight Ф）. We found： an averaged angular velocity ω of 3.0 × 10^-3 arc s^-1 and the variation of ω with the height above the solar limb. As the EP rises, the matter within it in fact moves along a spiral path around its axis. The spiral motion may be explained by the theory of plasma ‘double pole diffusion＇（DPD） caused by a sharp density gradient between the eruptive prominence and the surrounding corona. A theoretical angular velocity ω′ is estimated based on the DPD and basically coincides with ω obtained from the optimal velocity fitting.

Experimental study of friction dissipators for seismic protection of building structures

This paper presents the results from unidirectional shaking table tests of two reduced scale steel models of a building frame, with one and two floors, respectively. These frames incorporate friction dissipators at every floor. The inputs are sine-dwells and artificial and registered earthquakes. This study is part of a larger research project aiming to assess the seismic efficiency of friction dissipators by means of an integrated numerical and experimental approach. Inside this framework, the main objectives of these experiments are to：（i） collect a wide range of results to calibrate a numerical model derived within the project, （ii） clarify some of the most controversial issues about friction dissipators （including behavior for inputs containing pulses, capacity to reduce resonance peaks, introduction of high frequencies in the response, and self- generated eccentricities）, （iii） better understand their dynamic behavior, （iv） provide insight on the feasibility and reliability of using simple friction dissipators for seismic protection of building structures and （v） characterize the hysteretic behavior of these devices. Most of these objectives are satisfactorily reached and relevant conclusions are stated.

Survival of some food-borne bacteria in kefir produced by microbial levan and pullulan

Kefir is a fermented milk product produced by kefir grains traditionally or lyophilized starter *** quality characteristics of kefir produced at 22℃ and 30℃ using kefir grains after addition of microbial levan and pullulan were investigated for 14 ***,the survival of different Gram-positive and Gram-negative bacteria strains which were inoculated before and after fermentation were investigated during *** pH and titratable acidity were in the ranges of 4.67–4.39 and 1.10–0.87%,*** highest exopolysaccharide values(458.3–465.3 mg/kg)were found in the samples produced with the addition of 1%(w/v)of microbial levan at 30℃.Yeast counts were in the ranges of 6.73–5.11 log CFU/mL while the lactobacilli and lactic streptococci counts were in the ranges of 9.64–7.91 and 9.64–8.69 log CFU/mL,*** microbial polysaccharide addition did not show any significant differences in the enumeration of lactic acid bacteria in kefir(p>0.05).All Gram-positive and Gram-negative bacteria grew/survived during fermentation however Listeria monocytogenes was the most susceptible test bacterium to metabolites during *** highest reduction in the counts of *** was determined as 2.39 log units after 24 h of fermentation at 30℃ in the samples which were inoculated before fermentation and produced with the addition of *** samples produced at 30℃ without polysaccharide addition were considered as the best with respect to appearance,taste,consistency and overall acceptability.

The use of flours or phenolic extracts obtained from non-traditional sources or agri-food industry by-products has been a strategy used to formulate new bakery products with characteristics of a functional ***,phenolic compounds present great structural diversity,as well as the ability to interact in a complex way with the macromolecules that constitute the bread ***,the addition of these flours,extracts or pure compounds produces various effects on the microstructure of bread,and several of its sensory *** is mainly due to interactions between phenolic compounds and gluten *** objective of this review is to analyze some of the most recent published works on the addition of phenolic compounds in wheat bread to identify the type of positive and negative effects that have been observed and how they can be related to the physicochemical interactions between phenolic compounds and the macromolecules that constitute the food matrix,mainly *** effect of monomeric and polymeric phenolic compounds on the strength of these interactions and on the properties of dough and bread are discussed.

Synthesis and photoluminescence studies of Ba(Gd,Ln)B_9O_(16):Eu^(3+)(Ln=la,Y) phosphors for n-Uv LED lighting and display devices

BaGdB9O16：Eu^3＋ and Ba（Gd,Ln）B9O16：Eu^3＋（Ln=la, Y） phosphors were synthesized by solid state diffusion method. The X-ray diffraction, scanning electron microscopy（SEM） and photoluminescence properties were investigated. The as-synthesized BaGdB9O16：Eu^3＋ and Ba（Gd,Ln）B9O16：Eu^3＋（Ln=la, Y） phosphors showed strong red emission under ultraviolet light excitation. By partially substituting Gd^3＋ by la^3＋ and Y^3＋ ions in the host BaGdB9O16：Eu^3＋ materials, the maximum emission intensity was observed for the optimum composition of BaGd0.91la0.8B9O16Eu0.01 phosphor. The experimental results indicated that the Eu doped BaGdB9O16 and Ba（Gd,Ln）B9O16（Ln=la, Y） phosphors were promising red phosphors, which might find potential applications in near-Uv excited LED lighting as well as display devices. Comparison of as-synthesized phosphors with standard phosphor used in CFL（compact fluorescent lamp） was also done. Energy transfer mechanism of Gd^3＋ to Eu^3＋ was also discussed in this paper.

Interest in renewable energy sources has increased in recent years due to environmental concerns about global warming and air pollution,reduced costs and improved efficiency of *** the European Union（EU）energy directive,biomass is a suitable renewable *** aim of this study was to experimentally quantify and characterize the emission of particulate matter（PM（2.5））resulting from the combustion of two biomass fuels（chipped residual biomass from pine and eucalypt）,in a pilot-scale bubbling fluidized bed（BFB）combustor under distinct operating *** variables evaluated were the stoichiometry and,in the case of eucalypt,the leaching of the *** CO and PM（2.5）emission factors were lower when the stoichiometry used in the experiments was higher（0.33±0.1 g CO/kg and 16.8±1.0 mg PM（2.5）/kg,dry gases）.The treatment of the fuel by leaching before its combustion has shown to promote higher PM（2.5）emissions（55.2±2.5 mg/kg,as burned）.Organic and elemental carbon represented 3.1 to 30 wt.% of the particle mass,while carbonate（CO3^（2-））accounted for between 2.3 and 8.5 wt.%.The particulate mass was mainly composed of inorganic matter（71% to 86% of the PM（2.5）mass）.Compared to residential stoves,BFB combustion generated very high mass fractions of inorganic *** was the water soluble ion in higher concentration in the PM（2.5）emitted by the combustion of eucalypt,while calcium was the dominant water soluble ion in the case of pine.
