Intelligent Machine Learning Based Brain Tumor segmentation through Multi-Layer Hybrid U-Net with CNN Feature Integration

Brain tumors are a pressing public health concern, characterized by their high mortality and morbidity ***, the manual segmentation of brain tumors remains a laborious and error-prone task, necessitatingthe development of more precise and efficient methodologies. To address this formidable challenge, we proposean advanced approach for segmenting brain tumorMagnetic Resonance Imaging (MRI) images that harnesses theformidable capabilities of deep learning and convolutional neural networks (CNNs). While CNN-based methodshave displayed promise in the realm of brain tumor segmentation, the intricate nature of these tumors, markedby irregular shapes, varying sizes, uneven distribution, and limited available data, poses substantial obstacles toachieving accurate semantic segmentation. In our study, we introduce a pioneering Hybrid U-Net framework thatseamlessly integrates the U-Net and CNN architectures to surmount these challenges. Our proposed approachencompasses preprocessing steps that enhance image visualization, a customized layered U-Net model tailoredfor precise segmentation, and the inclusion of dropout layers to mitigate overfitting during the training ***, we leverage the CNN mechanism to exploit contextual information within brain tumorMRI images,resulting in a substantial enhancement in segmentation *** experimental results attest to the exceptionalperformance of our framework, with accuracy rates surpassing 97% across diverse datasets, showcasing therobustness and effectiveness of our approach. Furthermore, we conduct a comprehensive assessment of ourmethod’s capabilities by evaluating various performance measures, including the sensitivity, jaccard-index, andspecificity. Our proposed model achieved 99% accuracy. The implications of our findings are profound. Blood Pressure Profile and Glycemic Control of Type 2 Diabetics and Hypertensives at the Yalgado Ouedraogo University Hospital: A Review of 116 Cases

Objective: The association hypertension and diabetes is important. The two pathologies may influence each other. The aim was to study the correlation between glycemic control and blood pressure control and to determine the factors associated with blood pressure control. Methodology: This was a descriptive cross-sectional study with an analytical focus over 7 months. Patients were recruited as outpatients and all underwent ambulatory blood pressure measure, glycated hemoglobin and creatinine measurements, and assessment of compliance with treatment. Results: During this period 116 patients were collected. The predominance was female 69%. The mean age of the patients was 62 ± 7 years with a peak between 60 and 70 years. The average age of hypertension was 12 years and that of diabetes 6 1/2 years. The most frequently associated cardiovascular risk factor was a sedentary lifestyle (71.5%) after age. 57.8% of patients were not controlled at the office, with a predominance of systolic hypertension (58.2%). 61.6% of patients were controlled by ambulatory blood pressure measure, a rate of 47.8% of white coat hypertension. Glycemic control was observed in 42.2% of cases and 87% of patients had good renal function (glomerular filter rate ≥ 60 ml/mn). Therapeutic compliance was good in 53.4% of cases and dual therapy was the most used therapeutic modality 44.8% (52 patients) followed by triple therapy. The factors associated with poor blood pressure control were glycemic imbalance, non-compliance and monotherapy. Dual therapy had a protective effect. Conclusion: The association of hypertension and type 2 diabetes is frequent. The risk of occurrence increases with age. Ambulatory blood pressure measure is the best method to assess blood pressure control. Optimization of blood pressure control should also include optimization of glycemic control.

Polio eradication surveillance in sri Lanka,2019-2023

Objective:To evaluate the polio laboratory surveillance carried out from january,2019 to May,2023 by the Polio Regional Reference Laboratory,sri ***:This retrospective study analyzed all stool samples received under the acute flaccid paralysis(AFP)and immunodeficient vaccine-derived poliovirus(VDPV)surveillance at Polio Regional Reference Laboratory,sri Lanka from january,2019 to May,*** results of the testing methodologies were extracted from the laboratory data system,i.e.,poliovirus virus isolation,intra-typic differentiation/VDPV real time reverse transcriptase polymerase chain reaction(ITD/VDPV rRTPCR)and sequencing,along with the data on timing of reporting results,stool adequacy and *** was analyzed using descriptive ***:A total of 2141 stool samples from 1644 cases were received for AFP surveillance from sri Lanka(93.61%),Maldives(1.52%),and immunodeficient VDPV(4.86%)*** polioviruses(19/1644,1.15%)and non-polio enteroviruses(73/1644,4.44%)were isolated,while sabin-like 3 virus was detected in majority(12/19,63.15%)among the poliovirus *** polioviruses or circulating VDPVs were not detected among the *** all years of the study,the non-polio AFP detection rate was>1/100000 in children aged less than 15 years,whereas stool adequacy rate was>80%.All results were reported within 14 days of receipt,ensuring timely reporting as per global ***:The Polio Regional Reference Laboratory,sri Lanka plays a vital role in maintaining the polio-free status in the country through its robust laboratory surveillance,while adhering to the surveillance ***-detection of wild polioviruses and circulating VDPV during the study period reinforces the polio-free status in the country.

We fit various color–magnitude diagrams(CMDs) of the high-latitude Galactic globular clusters NGC 5024(M53),NGC 5053,NGC 5272(M3),NGC 5466,and NGC 7099(M30) by isochrones from the Dartmouth stellar evolution Database and Bag of stellar Tracks and Isochrones for α–enrichment [α/Fe] = +*** the CMDs,we use data sets from Hubble space Telescope,Gaia,and other sources utilizing,at least,25 photometric filters for each *** obtain the following characteristics with their statistical uncertainties for NGC 5024,NGC 5053,NGC 5272,NGC 5466,and NGC 7099,respectively:metallicities [Fe/H] =-1.93 ± 0.02,-2.08 ± 0.03,-1.60 ± 0.02,-1.95 ± 0.02,and-2.07 ± 0.04 dex with their systematic uncertainty 0.1 dex;ages 13.00 ± 0.11,12.70 ± 0.11,11.63 ± 0.07,12.15 ± 0.11,and 12.80 ± 0.17 Gyr with their systematic uncertainty 0.8 Gyr;distances(systematic uncertainty added) 18.22 ± 0.06 ± 0.60,16.99 ± 0.06 ± 0.56,10.08 ± 0.04 ± 0.33,15.59 ±0.03 ± 0.51,and 8.29 ± 0.03 ± 0.27 kpc;reddenings e(B-V) = 0.023 ± 0.004,0.017 ± 0.004,0.023 ± 0.004,0.023 ± 0.003,and 0.045 ± 0.002 mag with their systematic uncertainty 0.01 mag;extinctions AV= 0.08 ± 0.01,0.06 ± 0.01,0.08 ± 0.01,0.08 ± 0.01,and 0.16 ± 0.01 mag with their systematic uncertainty 0.03 mag,which suggest the total Galactic extinction AV= 0.08 across the whole Galactic dust to extragalactic objects at the North Galactic *** horizontal branch morphology difference of these clusters is explained by their different metallicity,age,mass-loss efficiency,and loss of low-mass members in the evolution of the core-collapse cluster NGC 7099 and loose clusters NGC 5053 and NGC 5466.

We have recently proposed a new technique of plasma tailoring by laser-driven hydrodynamic shockwaves generated on both sides of a gas jet[Marquès et al.,*** 28,023103(2021)].In a continuation of this numerical work,we study experimentally the influence of the tailoring on proton acceleration driven by a high-intensity picosecond laser in three cases:without tailoring,by tailoring only the entrance side of the picosecond laser,and by tailoring both sides of the gas *** tailoring,the acceleration is transverse to the laser axis,with a low-energy exponential spectrum,produced by Coulomb *** the front side of the gas jet is tailored,a forward acceleration appears,which is significantly enhanced when both the front and back sides of the plasma are *** forward acceleration produces higher-energy protons,with a peaked spectrum,and is in good agreement with the mechanism of collisionless shock acceleration(CsA).The spatiotemporal evolution of the plasma profile is characterized by optical shadowgraphy of a probe *** refraction and absorption of this beam are simulated by post-processing 3D hydrodynamic simulations of the plasma *** with the experimental results allows estimation of the thickness and near-critical density of the plasma slab produced by tailoring both sides of the gas *** parameters are in good agreement with those required for CsA.

The secondary injury cascade after spinal cord injury:an analysis of local cytokine/chemokine regulation

After spinal cord injury,there is an extensive infiltration of immune cells,which exacerbates the injury and leads to further neural ***,a major aim of current research involves targeting the immune response as a treatment for spinal cord *** much research has been performed analyzing the complex inflammatory process following spinal cord injury,there remain major discrepancies within previous literature regarding the timeline of local cytokine *** objectives of this study were to establish an overview of the timeline of cytokine regulation for 2 weeks after spinal cord injury,identify sexual dimorphisms in terms of cytokine levels,and determine local cytokines that significantly change based on the severity of spinal cord *** were inflicted with either a mild contusion,moderate contusion,severe contusion,or complete transection,7 mm of spinal cord centered on the injury was harvested at varying times post-injury,and tissue homogenates were analyzed with a Cytokine/Chemokine 27-Plex *** demonstrated pro-inflammatory cytokines including tumor necrosis factorα,interleukin-1β,and interleukin-6 were all upregulated after spinal cord injury,but returned to uninjured levels within approximately 24 hours post-injury,while chemokines including monocyte chemoattractant protein-1 remained upregulated for days *** contrast,several anti-inflammatory cytokines and growth factors including interleukin-10 and vascular endothelial growth factor were downregulated by 7 days *** spinal cord injury,tissue inhibitor of metalloproteinase-1,which specifically affects astrocytes involved in glial scar development,increased more than all other cytokines tested,reaching 26.9-fold higher than uninjured *** a mild injury,11 cytokines demonstrated sexual dimorphisms;however,after a severe contusion only leptin levels were different between female and male *** conclusion,pro-inflammatory cytokines in
