The efficacy of empiric arterial embolization for treating severe hemoptysis

Aim: Purpose of the study was to investigate the efficacy of empiric arterial embolization in order to achieve hemostasis in patients with massive hemoptysis. Materials and Methods: A retrospective review of histories and interventional studies of 56 patients (40 male, 16 female, median age 57 years;range, 16 - 83 years) referred for endovascular treatment of massive hemoptysis over a period of 17 years. Arteries supposed to supply the bleeding bronchoalveolar sections were embolized with particles in all cases. Digital subtraction angiographical (DSA) studies were analyzed with respect to the morphology of the embolized arteries. Arteries were termed pathologic when they were either hypertrophic or supplied hypervascular lung sections as well as actively bleeding branches. Empiric embolization was defined as endovascular occlusion of arteries without visible contrast-material extravasation on DSA studies. Results: Continuing hemoptysis was encountered in one (25%) of 4 patients with active contrast extravasation and in 11 (21%) of 52 empirically embolized patients: Six (19%) of 32 patients with pathologic arteries visible on aortography, 3 (18%) of 17 with pathologic arteries visible by selective arteriography and 2 (67%) of 3 with no visible pathologic arteries. From 6 patients (11%, 5 male, 1 female) who died within 30 days after embolization, 3 suffered from tuberculosis while 3 had malignant tumors. Three had ongoing hemoptysis. One patient died of multiple organ failure caused by post-interventional paraplegia and consecutive pneumonia. Conclusion: In patients with hemoptysis, empirical embolization is effective when pathologic bronchial arteries can be identified by DSA.

Short-term elevation of glucocorticoids(GCs)is one of the major physiological mechanisms by which vertebrates cope with challenging environmental or social factors(stressors).however,when exposure to stressors occurs repeatedly or over a prolonged period of time,animals may experience chronic elevation of GCs,which reduces the immune response efficiency and can lead to higher intensity of parasitic ***,we used invasive gray squirrels Sciurus carolinensis in troduced in Norther n Italy and their 2 most prevale nt gastrointesti nal parasites,the n ematode Strongyloides robustus and coccidia of the genus Eimeria,as a model to investigate relati on ships among macroparasite infection and concentrations of fecal glucocorticoid metabolites(FGMs),an integrated measure of circulating *** results revealed an association of FGMs with infection by ***,but not with *** with higher FGMs appear to be responsible for the greatest *** egg shedding within gray squirrel populations,thus possibly acting as ***,FGMs were negatively associated with adult ***,suggesting that the abundance of adults of this nematode species does not induce elevation in FGMs,but is only affected by it through immun e-mediated effects on its fecun *** ally,the relati on ship betwee n ***(both eggs and adult parasites)and FGMs was not linear,suggesting that only high levels of physiological stress in fluence parasite inf *** fin dings highlight that the direction and magnitude of the stress-infection relationship may depend not only on the specific hostparasite system,but also on the different life stages of the same parasite.

Urban Soil Pollution with heavy Metals in hama Floodplain,Syria

The Orontes river basin can be considered one of the most polluted areas in the eastern Mediterranean due to the intense urban occupation, intensive agriculture irrigation, and large numbers of different industries activities. The objective of the study was to assess the extent and severity of heavy metal pollution of arable soils of hama floodplain, in order to provide a general insight vision of pollution status in this intensive agricultural production area. The present and spatial distribution of four heavy metals (Cd, Cu, Pb, Zn) concentration have been examined in 5 monitoring sites of river’s water along the part of the river passing through the plain of hama;and also from 93 samples of topsoil from the area surrounding the city of hama have been examined. The concentrations of heavy metals in both river water and soil were within the international standards. Water analyses indicated ph of moderately alkaline, no irrigation problem related to ammonium nitrogen and phosphate phosphorus, while the Electrical Conductivity (EC) has referred to an increasing problem. Geo-accumulation Index (Igeo) refers to strong building up for Cd, Pb, and very strongly for Cu. Soil analyses refer to clayey, calcareous, and alkaline soil with ph ranges from 7.5 to 8.6, with significantly higher organic matter content especially near sewage plants, attributed to irrigation with untreated sewage sludge water. Geostatistical analysis of data showed up normal spatial distribution related to the high variation between the values of the studied elements;whereas the Cu and Cd concentrations were higher than allowable limit near the steel, rubber wheel factories and reach 127 for Cu and 9.8 μg·g-1 for Cd. Additionally, a high concentration of Cu was significantly associated with organic matter content. The concentration of Pb was within the limits, with the exception of riverbanks where the values of Pb exceed 95 μg·g-1, with (Igeo) of 4.22 refers strongly to very strong accumulation. To

Glucocorticoids(GCs)are involved in the regulation of an animal's energetic *** stressful situations,they are part of the neuroendocrine response to cope with environmental *** react to aversive stimuli also through behavioral responses,defined as coping *** in captive and wild populations,individuals differ in their behavior along a proactive-reactive *** animals exhibit a bold,active explorative and social personality,whereas reactive ones areshy,less active-explorative and less ***,we test the hypothesis that personality traits and physiological responses to stressors covary,with more proactive individuals having a less pronounced GC stress *** wild populations of invasive gray squirrels Sciurus carolinensis,we measuredfecal glucocorticoid metabolites(FGMs),an integrated measure of circulating GCs,and 3 personality traits(activity,sociability,and exploration)derived from open field test(OFT)and mirror image stimulation(MIS)*** squirrels had higher FGMs in Autumn than in Winter and males with scrotal testes had higher FGMs than nonbreeding *** varied with body mass and population *** expressed more activity exploration at higher than at lower density and heavier squirrels had higher scores for activity exploration than animals that weighed *** in FGM concentrations was not correlated with the expression of the 3 personality ***,our results do not support a strong association between the behavioral and physiological stress responses but show that in wild populations,where animals experience varying environmental conditions,the GC endocrine response and the expression of personality are uncorrelated traits among individuals.

Neonicotinoid insecticides(NNIs)have been intensively used and exploited,resulting in their presence and accumulation in multiple environmental *** herein investigated the current levels of eight major NNIs in the harbin section of the Songhua River in northeast China,providing the first systematic report on NNIs in this *** least four NNIs in water and three in sediment were detected,with total concentrations ranging from 30.8 to 135 ng L^(-1) and from 0.61 to 14.7 ng g^(-1) dw,*** spatial variations in surface water NNIs concentrations were observed in tributary than mainstream(phe intensive human activities(e.g.,horticulture,urban landscaping,and household pet flea control)and the discharge of wastewater from many treatment *** was a significant positive correlation(phe concentrations of residual imidacloprid(IMI),clothianidin(CLO),and S4NNIs in the sediment and total organic carbon(TOC).Due to its high solubility and low octanol-water partition coefficient(Kow),the sediment-water exchange behavior shows that NNIs in sediments can re-enter into the water *** exposure risk was assessed using the relative potency factor(RPF),which showed that infants have the highest exposure risk(estimated daily intake(SIMIeq EDI):31.9 ng kg^(-1) bw$d^(-1)).The concentration thresholds of NNIs for aquatic organisms in the harbin section of the Songhua River were determined using the species sensitivity distribution(SSD)approach,resulting in a value of 355 ng L^(-1) for acute hazardous concentration for 5%of species(hC5)and 165 ng L^(-1) for chronic *** organisms at low trophic levels were more vulnerable to potential harm from NNIs.

Evaluation of the string test for the detection of helicobacter pylori

AIM: helicobacter pylori can be diagnosed by invasive ornon-invasive tests but to obtain bacteria for culture andantibiotic susceptibility testing, an upper GI endoscopy isoften required. The string test may be a minimally-invasivealternative method of obtaining h. pylori samples. This studyevaluates the sensitivity and specificity of the string test inthe diagnosis of h. pylori in comparison with endoscopicmeans of ***: This was a prospective open comparative studyof patients with dyspepsia with endoscopy-based tests asgold standard (defined as a positive CLO test and antralhistology). Fasting patients swallowed the encapsulated-string(Entero-test hp), which was withdrawn after 1 hour. Thegastric juice from the string was plated onto h. pylori-selectivemedia for culture. helicobacter pylori was identified by typicalcolony morphology, gram stain and biochemical test ***: Thirty dyspeptic patients were recruited of whom21 (70 %) were positive for h. pylori according to the goldstandard. The sensitivity, specificity, positive predictive value,and negative predictive value for the string test were 38 %,100 %, 100 % and 41% respectively, and for endoscopicbiopsies 81%, 100 %, 100 %, 69 % respectively (P=0.004).Logistic regression showed that only abundant growthdensity from endoscopic biopsy cultures to be a predictor ofa positive string test (P=0.018).CONCLUSION: The string test is an alternative method toendoscopy in obtaining h. pylori but has a low sensitivitycompared to endoscopic biopsies.

Dear Editor,Pancreatic ductal adenocarcinoma(PDAC)remains a significant health problem with an increase in the incidence and a five-year survival rate of only 10%[1].The Phosphoinositide 3-kinase-protein kinase-B-mechanistic target of rapamycin(PI3K-AKT-mTOR)pathway is a driver pathway in PDAC and an important therapeutic target[2].We[3]and others[4–6]have demonstrated that the mTOR kinase is a therapeutic target in PDAC,and rationally designed mTOR inhibitor(mTORi)-based combination therapies are emerging[2].

[目的]减少植物光呼吸的消耗能有效提高光合效率,有助于提高植物生物量。将细菌乙醇酸代谢途径导入植物建立光呼吸支路,可以把乙醇酸代谢产生的CO_2直接释放到叶绿体中,促进Rubisco羧化反应,从而提高光合效率。[方法]本研究运用农杆菌介导法,将乙醇酸脱氢酶基因GDh导入常规陆地棉受体材料R15建立光呼吸支路,通过体细胞愈伤诱导法在含抗生素的筛选培养基中获得再生植株。[结果]分子检测结果显示外源基因被成功导入棉花受体,并获得36株转基因材料。通过对转基因植株及对照的光合参数测定及田间表型观察,发现转基因植株比对照净光合速率提高26.99%～37.29%,株高增加10.07%～16.18%,叶片鲜重增加21.59%～44.25%,干重增加11.06%～17.79%,表明GDh降低了棉花光呼吸消耗,增加了植株生物量。[结论]本研究为筛选高光合效率及高产棉花种质资源提供了新的思路。
