Clinicopathological features and prognosis of membranoproliferative-like Henoch-Schönlein purpura nephritis in children

Background: The aim of this retrospective study was to defi ne the clinical manifestations, pathological features and prognosis of children with membranoproliferative-like Henoch-Schönlein purpura nephritis (HSPN), representing International Study of Kidney Disease in Children (ISKDC) grade VI. Methods: Among 245 patients with HSPN treated in our hospital between 2008 and 2010, nine patients (3.7%) were diagnosed with HSPN of ISKDC grade VI (males=5, females=4, age: 9.5±2.03 years, mean±SD). The clinical features, laboratory and pathologicalfi ndings, treatment and outcome of the 9 patients were retrospectively analyzed. Results: Of the 9 patients, 7 (78%) presented with hematuria and nephrotic syndrome, and were treated with steroids (oral prednisone or intravenous methylprednisolone pulse therapy) and immunosuppressants (oral tripterygium glycosides or intravenous cyclophosphamide pulse therapy). One (11%) patient had hematuria and nephrotic range proteinuria (>50 mg/kg per 24 hours) and was treated with oral prednisone and tripterygium glycosides. Another (11%) patient presented with hematuria and moderate proteinuria (25-50 mg/kg per 24 hours) and was treated with oral tripterygium glycoside only. Histopathological examination showed diffuse glomerular mesangial and endocapillary proliferation, mesangial interposition, double-contour formation, podocyte hypertrophy, shedding, and cytoplasmic absorption droplets. The percentages of glomeruli with small cellular crescents varied from 4%-25% in 6 of 9 patients. Follow-up for 2 to 4 years showed excellent recovery in all patients. Conclusions: The main clinical feature of ISKDC grade VI HSPN in children is a nephrotic syndrome with hematuria. The excellent prognosis of the disease was probably related to early diagnosis and treatment with steroids and/or immunosuppressants, and mild degree of glomerulosclerosis and tubulointerstitial damage.

Large bone defects resulting from fractures and disease are a major clinical challenge,being often unable to heal spontaneously by the body’s repair *** of evidence have shown that hypoxia-induced overproduction of ROS in bone defect region has a major impact on delaying bone ***,replenishing excess oxygen in a short time cause high oxygen tension that affect the activity of osteoblast precursor ***,reasonably restoring the hypoxic condition of bone microenvironment is essential for facilitating bone ***,we designed ROS scavenging and responsive prolonged oxygen-generating hydrogels(CPP-L/GelMA)as a“bone microenvironment regulative hydrogel”to reverse the hypoxic microenvironment in bone defects ***-L/GelMA hydrogels comprises an antioxidant enzyme catalase(CAT)and ROS-responsive oxygen-releasing nanoparticles(PFC@PLGA/PPS)co-loaded liposome(CCP-L)and GelMA *** hypoxic condition,CPP-L/GelMA can release CAT for degrading hydrogen peroxide to generate oxygen and be triggered by superfluous ROS to continuously release the oxygen for more than 2 *** prolonged oxygen enriched microenvironment generated by CPP-L/GelMA hydrogel significantly enhanced angiogenesis and osteogenesis while inhibited ***,CPP-L/GelMA showed excellent bone regeneration effect in a mice skull defect model through the Nrf2-BMAL1-autophagy ***,CPP-L/GelMA,as a bone microenvironment regulative hydrogel for bone tissue respiration,can effectively scavenge ROS and provide prolonged oxygen supply according to the demand in bone defect region,possessing of great clinical therapeutic potential.

New Complexiton Solutions of （2＋1）-Dimensional Nizhnik-Novikov-Veselov Equations

In this paper, we extend the multiple Riccati equations rational expansion method by introducing a new ansatz. Using this method, many complexiton solutions of the （2＋ 1 ）-dimensional Nizhnik-Novikov-Veselov equations are obtained which include various combination of hyperbolic and trigonometric periodic function solutions, various combination of hyperbolic and rational function solutions, various combination of trigonometric periodic and rational function solutions, etc. The method can be also used to solve other nonlinear partial differential equations.

Comparison of adenosine stress and dobutamine stress by real-time myocardial contrast echocardiography in detecting myocardium ischemia in dogs

Background Real-time perfusion imaging （RTPI） using ultrasound contrast agents has shown good ＂accuracy＂ in detecting myocardial infarction, however its accuracy in the assessment of peri-infarct ischemia and stress echocardiography are not known. The aim of this study was to determine the accuracy of RTPI in assessment of peri-infarct ischemia during dobutamine and adenosine stress. Methods We employed the RTPI modality （Agilent and ATL Philips） in a canine model （18 dogs） of distal coronary occlusion and proximal coronary stenosis. Using coronary flow probe recordings, the physiologic significance of proximal coronary stenosis was established by confirming abolition of the coronary reserve. The contrast agent Optison was given as a slow bolus injection at baseline, during prolonged distal coronary occlusion, during adenosine bolus stress and during dobutamine stress. Triphenyltetrazolium chloride （TTC） staining was used to verity a distal infarction. RTPI recordings at baseline, the distal coronary occlusion and stress protocols were randomly mixed and reviewed blindly. Results In all but one dog, RTPI detected a distal infarct as small as 9% of the left ventricle. The sensitivity, specificity and overall diagnostic accuracy of RTPI in the detection of distal infarcts were： 94%, 89% and 92%, respectively. The sensitivity, specificity, and overall diagnostic accuracy of RTPI in the assessment of peri-infarction ischemia were 83%, 92% and 88% for adenosine stress and 95%, 86% and 91% for dobutamine stress, respectively. Conclusions Even small distal infarcts can be detected by RTPI; peri-infarct ischemia can be accurately recognized by RTPt during stress; adenosine and dobutamine stress appear equally reliable in the RTPI evaluation of peri-infarct ischemia.

Cancer Incidence and Mortality in China,2007

Objective： Cancer incidence and mortality data collected from population-based cancer registries were analyzed to present the overall cancer statistics in Chinese registration areas by age, sex and geographic area in 2007. Methods： In 2010, 48 cancer registries reported cancer incidence and mortality data of 2007 to National Central Cancer Registry of China. Of them, 38 registries＇ data met the national criteria. Incidence and mortality were calculated by cancer sites, age, gender, and area. Age-standardized rates were described by China and World population. Results： The crude incidence rate for all cancers was 276.16/100,000 （305.22/100,000 for male and 246.46/100,000 for female; 284.71/100,000 in urban and 251.07/100,000 in rural）. Age-standardized incidence rates by China and World population were 145.39/100,000 and 189.46/100,000 respectively. The crude mortality rate for all cancers was 177.09/100,000 （219.15/100,000 for male and 134.10/100,000 for female; 173.55/100,000 in urban and 187.49/100,000 in rural）. Age-standardized mortality rates by China and World population were 86.06/100,000 and 116.46/100,000, respectively. The top 10 most frequently common cancer sites were the lung, stomach, colon and rectum, liver, breast, esophagus, pancreas, bladder, brain and lymphoma, accounting for 76.12% of the total cancer cases. The top 10 causes of cancer death were cancers of the lung, liver, stomach, esophagus, colon and rectum, pancreas, breast, leukemia, brain and lymphoma, accounting for 84.37% of the total cancer deaths. Conclusion： Cancer remains a major disease threatening people＇s health in China. Prevention and control should be enhanced, especially for the main cancers.

Morphine attenuates frustration-like behavior induced by sucrose reward deprivation in rats

In this study, a T-maze-based frustration model in rats was established using sucrose-reward deprivation, The results revealed that rats maintained a 75% preference for the sucrose-reward arm in the reward phase. During the sucrose-deprivation frustration phase, both the preference for the sucrose-deprivation arm （62.5%） and time spent waiting in the sucrose-deprivation arm decreased. Acute injection of morphine increased the preference in a dose-dependent fashion, and prolonged the waiting duration in the sucrose-deprivation arm. These findings indicate that morphine specifically inhibited the frustration response induced by sucrose reward deprivation. To further elucidate the pharmacological mechanisms involved, the opioid receptor antagonist naloxone was given to model rats prior to the injection of morphine. The results revealed that naloxone administration markedly attenuated the anti-frustration-like effects of 3 mg/kg morphine treatment. These findings suggest that morphine attenuates the frustration-like response to reward deprivation in rats through the opioid receptor.

Effect of Chaiqinchengqi decoction on inositol requiring enzyme 1α in alveolar macrophages of dogs with acute necrotising pancreatitis induced by sodium taurocholate

OBJECTIVE: To investigate the effect of Chaiqinchengqi decoction(CQCQD) on inositol requiring enzyme 1α(IRE1α) in alveolar macrophages(AMs)of the dog model of acute necrotising pancreatitis(ANP) induced by sodium ***: Fifteen beagle dogs were randomised into a control group,ANP group and CQCQD group(n = 5 per group). ANP was induced by a retrograde duct injection of 50 mg/kg of 5% sodium taurocholate. The dogs in the control group received injections of the same volume of saline as the sodium taurocholate. After the models were induced,the dogs in the CQCQD group were administered 10 m L/kg CQCQD every 2 h for 6 h. Two hours after the last administration of either CQCQD or saline,they were sacrificed by anaesthesia. AMs were collected to determine the IRE1α and Interleukin-1β(IL-1β)m RNA and protein expression,and pancreatic tissues were collected for histopathology ***: Compared with the ANP group,the m RNA and protein expression of IRE1α and the protein expression of IL-1β of AMs in the CQCQD group were significantly down-regulated,and the pancreatic histopathology score of the CQCQD group also was lower. There was no significant difference in the m RNA expression of IL-1β of AMs between the two ***: The CQCQD-induced down-regulation of the IL-1β protein expression may involve the down-regulation of the m RNA and protein expression of IRE1α in AMs.

Combination of Quercetin, Cinnamaldehyde and Hirudin Protects Rat Dorsal Root Ganglion Neurons against High Glucose-Induced Injury through Nrf-2/HO-1 Activation and NF-κB Inhibition

Objective： TO examine the effects of the combination of quercetin （Q）, cinnamaldehyde （C） and hirudin （H）, a Chinese medicine formula on high glucose （HG）-induced apoptosis of cultured dorsal root ganglion （DRG） neurons. Methods： DRG neurons exposed to HG （45 mmol/L） for 24 h were employed as an in vitro model of diabetic neuropathy. Cell viability, reactive oxygen species （ROS） level and apoptosis were determined. The expression of nuclear factor of Kappa B （NF- κB）, inhibitory kappa B α （IκBα ）, phosphorylated IκBα and Nf-E2 related factor 2 （Nrf2） were examined using reverse transcription-polymerase chain reaction （RT-PCR） and Western blot assay. The expression of hemeoxygenase-1 （HO-1）, interleukin-6 （IL-6）, tumor necrosis factor （TNF-α） and caspase-3 were also examined by RT-PCR and Western blot assay. Results： HG treatment markedly increased DRG neuron apoptosis via increasing intracellular ROS level and activating the NF- κB signaling pathway （P〈0.05）. Co-treatment with Q, C, H and their combination decreased HG-induced caspase-3 activation and apoptosis （P〈0.05 or P〈0.01）. The expressions of NF- κB, IL-6 and TNF-α were down- regulated, and Nrf2/HO-1 expression was up-regulated （P〈0.05 or P〈0.01）. QCH has better effect in scavenging ROS, activating Nrf-2/HO-1, and down-regulating the NF- κB pathway than other treatment group. Conclusions： DRG neurons＇ apoptosis was increased in diabetic conditions, which was reduced by QCH formula treatment. The possible reason could be activating Nrf-2/HO-1 pathway, scavenging ROS, and inhibition of NF- κB activation. The effect of QCH combination was better than each monomer or the combination of the two monomers.

Timing and structural controls on skarn-type and vein-type mineralization at the Xitian tin-polymetallic deposit, Hunan Province, SE China

Xitian tin-polymetallic deposit, located in the eastern Hunan Province, SE China, hosts quartz vein and skarn in the contact zone between carbonate and two stages granites. Critical geodynamic questions for South China are whether different types of mineralization form in the same time and how the magmatism–tectonic system controls the ore-forming process. Based on the distribution of the orebodies, six cassiterite samples from different types of mineralization are collected for dating. In-situ LA-MCICP-MS U–Pb isotopic data yielded concordia low intercept ages between 154 and 157 Ma, indicating that different types of mineralization belong to the same magmatism–mineralization system. Coupled with the study of the kinematic indicators, it suggests that the structural control of the wall rocks constrain the types of mineralization. These results provide further evidence of a close temporal link between the structure and the tin-polymetallic mineralization in Xitian deposit. Considering the structure in the district, granite dome plays an important role in the ore-forming process. The age and structural signatures in Xitian deposit are the response to the subduction of Pacific Plate.
