Diversity of Flowering Plants in Nubri Valley, Manaslu Conservation Area, Central Nepal

Central Nepal has long history in the field of botanical investigation in contrast with different parts of the nation. Nevertheless, the present study area (Manaslu Conservation Area) is still under-explored. The present floristic study was carried out with the aim of fulfilling this gap by giving particular attention on flowering plants and their documentation. Two field visits were directed in May-June 2012 and September-October 2012 in two unique seasons with a specific end goal to gather plant samples of both seasons. On the contrary, herbarium specimens were prepared and later identified at Tribhuvan University Central Herbarium (TUCH) and National Herbarium and Plant Laboratory (KATH). The present study uncovers the presence of 276 species of flowering plants belonging to 78 families and 200 genera of which 63 families belong to dicotyledons and 15 families belong to monocotyledons. Among these, 17 species (6.15%) of climbers, 178 species (64.49%) of herbs, 58 species (21.01%) of shrubs and 23 species (8.33%) of trees are found. Economically important flowering plants comprise about 156 species where 75 species are used as medicine, 29 species as fuel wood, 23 species as fodder, 29 species are edible, 6 species are for religious purpose, 5 specifically for construction purpose, 10 for ornamental and 13 species reported to have miscellaneous uses. MCA accounts to 9 flowering plant species for research and development and agro-technology development and 12 species are listed under IUCN and CAMP threatened categories. MCA and its affinity are likewise house to 10 endemic flowering plants. As from the study it can be inferred that Manaslu Conservation Area (MCA) is a treasure hub of flowering plants that not only represent its economic importance rather it is also a house to threaten and endemic flowering plants. Thus, proper management plans must be implemented for the conservation of resources in MCA.

Transaminitis is an indicator of mortality in patients with COVID-19:A retrospective cohort study

BACKGROUND Since its discovery in Wuhan,China in December of 2019,the novel coronavirus has progressed to become one of the worst pandemics seen in the last 100 ***,there has been an increased interest in the hepatic manifestations of coronavirus disease 19(COVID-19).AIM To describe the demographic and clinical characteristics of COVID-19 positive patients and study the association between transaminitis and all-cause *** This is a descriptive retrospective cohort study of 130 consecutive patients with a positive COVID PCR test admitted between March 16,2020 to May 14,2020 at a tertiary care University-based medical *** Wilcoxon-rank sum test and paired t-test were used for comparing non-parametric and parametric continuous variables respectively and a multivariable logistic regression models to study the association between transaminitis and mortality using SAS version 9.4(SAS Institute,Cary,NC,United States).RESULTS Out of the 130 patients,73(56%)patients were found to have transaminitis and 57(44%)did *** compared to patients without transaminitis,the transaminitis group was found to have a higher median body mass index(30.2 kg/m2 vs 27.3 kg/m2,P=0.04).In the multivariate analysis those with transaminitis were found to have 3.4 times higher odds of dying as compared to those without transaminitis adjusting for gender,the Age-adjusted Charlson Comorbidity Index and admission to the intensive care unit(P=0.03).CONCLUSION Our study showed that transaminitis on admission was associated with severe clinical outcomes such as admission to the intensive care unit,need for mechanical ventilation,and mortality.

We have proposed a new model for the calculation of excitonic electroabsorption based on modified previously reported models for bulk structure. The excitonic absorption spectra in high purity GaAs have been theoretically studied in the presence of electric field at room temperature (RT). The Stark shift, linewidth broadening of exciton and extinction ratio have been calculated as a function of electric field. For the validity of our model we have compared with experimental result.

Estimation of Global Solar Radiation Using Clearness Index and Cloud Transmittance Factor at Trans-Himalayan Region in Nepal

This paper presents the global solar radiation (GSR) and cloud transmittance factor (cf) measured at the horizontal surface over a period of one year from 2009 to 2010 using CMP6 Pyranometer and NILUUV Irradiance Meter at Lukla (Latitude 26.69oN,Longitude 86.73?E and Altitude 2850 m) in the foothills of the Mt Everest (8850 m high). Monthly and seasonal variations of global solar radiation as well as correlation between clearness index and cloud transmittance factor at Lukla are presented. The annual average daily global solar radiation is about 3.83 kWh/sq·m/day which is sufficient to promote solar active and passive energy technology at high mountain terrain where there is no other viable alternative sources of energy. The maximum and minimum global solar radiation of 5.33 and 2.08 kWh/sq·m/day is recorded in April and September 2010 respectively. The seasonal variation of solar energy is about 2.87 kWh/sq·m/day and 4.83 kWh/sq·m/day in summer and spring respectively which is not in line with the general trend. The coefficient of determination (R2) between cloud transmittance factor (cf) and clearness factor (K) is found to be 0.97. This novel result can be utilized to estimate the global solar radiation at the horizontal surface where K and cf are available.

Retrospective analysis of complications related to endoscopic retrograde cholangio-pancreatography in patients with cirrhosis vs patients without cirrhosis

BACKGROUND There is minimal objective data regarding adverse events related to endoscopic retrograde cholangio-pancreatography(ERCP)in patients with cirrhosis compared to those without cirrhosis and even fewer data comparing complications among cirrhosis patients based on severity of *** To determine if patients with cirrhosis are at increased risk of adverse events related to ERCP:mainly pancreatitis,bleeding,perforation,cholangitis,and mortality;And to see if higher Child-Pugh(CP)score and Model for End-Stage Liver Disease(MELD)score are associated with higher post-ERCP *** We performed a retrospective analysis of 692 patients who underwent ERCP and analyzed the impact of cirrhosis etiology,gender,type of sedation used during procedure,interventions performed,and co-morbidities on the rate of complications in cirrhosis patients as compared to non-cirrhosis *** Overall complications were higher in those with cirrhosis as compared to those without cirrhosis(P=0.015 at significance level of 0.05).CP class,especially CP class C,was shown to be associated with a significantly higher rate of ERCP complications as compared to CP class A and CP class B(P=0.010 at significance level of 0.05).CONCLUSION The results of our study reaffirm that liver cirrhosis has an impact on the occurrence of complications during *** study shows that CP class seems to be more reliable as compared to MELD score in predicting complications of ERCP in cirrhosis patients.

Aim: To identify, analyse and control the outbreak of Carbapenamase producing Klebsiella pneumoniae. Objectives: 1) To detect multidrug resistant K. pneumoniae at the earliest and isolate patients. 2) To find out the predisposing causes for the occurrence of this outbreak. 3) To break the chain of infection transmission. 4) To reduce the risk of Hospital acquired infections. Methods: This retrospective study along with the surveillance was conducted from January 2017 to March 2017 at HCG multispecialty cancer hospital, Bangalore, India. Results: Total 15 patients were diagnosed with KPC infection during the first month of the study period. Those affected were mostly Male patients (73%), admitted in ICU (73%) for further treatment. In our study, the incidence of KPC infection was mostly found with bloodstream infections (60%), mostly seen in those with central lines (80%) followed by patients on ventilatory support (66%). Before the outbreak of KPC infection, all the patients (100%) had already been treated with higher antibiotics including Carbapenems. In our study, only nine out of fifteen patients (60%) could be salvaged with treatment and were discharged. Conclusions: Hospital Infection Control Committee’s regular screening and the training of healthcare professionals are vital for the control of the outbreak.

A peat core of 10 m length was collected from the Hani peatland,Jilin Province,northeast China（42°43’N,126°31’E）which is close to the Northern Peatlands *** peat land consisted of scattered humps with a sphagnum composition,which is anindicator that the peat land is continuously evolving from mineratrophic（groundwaterfed）to ombrotrophic nature（rain water fed）.The average dry bulk density and the ashcontent were 0.1g/cm3 and 20%with both closely following trends in the ***14C age dating of the top 550 cm showed that the peat accumulation for the last sevenmillennia with peat accumulation rate of approx.12 cm/*** Titanium（Ti）as anindicator of mineral input,the average concentration was about 0.2%with two peaksat ca.550 cm and 50 *** increment of Ti and density at ca.550 cm indicates thechanges in either source area of the minerals or the changes in depositional flux *** ca.50 cm,the absence of Ti peak coinciding with the increment of densitymay reflect the changes in plant composition and thus the humification of the *** mercury（Hg）the average concentration was 150 ng/g,and showed few ***,for Pb,the concentration remained fairly stable between 50 and *** 50 cm,the concentration increased by more than six *** the Ti profile,two things could be interpreted:1)Pb is predominantly ofatmospheric origin,and 2)above 50 cm,anthropogenic input may be responsible forincrease in *** is also supported by another lithogenic element such as ***,for the precise timing of increment for the last ca.100 years,age datingother than 14C is required.

Differential expression of the S100 gene family members in oral squamous cell carcinomas

Purpose: To examine the expression profile of 16 members of the S 100 gene family at the mRNA level by semiquantitative reverse transcription-polymerase chain reaction (sRT-PCR) in 27 cases of oral squamous cell carcinomas (OSCCs)/their pair-wised normal controls obtained from Sudanese patients, and to confirm the sRT-PCR results by performing quantitative real time-polymerase chain reaction (qRT-PCR) for 6 of the 16 genes examined。Methods: Total RNA was extracted from all the OSCCs/normal controls, followed by cDNA synthesis and RT-PCR。For the sRT-PCR, gel electrophoretic images were captured and analyzed by Quantity one software。For the 6 genes examined by qRT-PCR, relative standard curve method was used to quantify the mRNA level。Wilcoxon matched-pair signed rank test was thereafter used for the statistical analysis。Results: With sRT-PCR, 4 (25%; S100A4, S100A6, S100A8, S100A14) out of the 16 S100 gene members examined were found to be significantly down regulated (P< 0.05) in the tumors compared to the controls。None of the S 100 gene members examined were found to be significantly up regulated, qRT-PCR results confirmed the significant down regulation of the S 100A4, SI00A6 and SI00AI4 in the tumors examined。No significant association was found between the status of gene deregulation and patients' clinicopathological variables。Conclusions: S 100 gene family members might play an important role in the pathogenesis of the OSCCs examined。Findings of the present work warrant in-depth studies of the S 100 gene family members, in particular, the S100A4, S100A6, S100A8 and S100A14 to further understand their possible role(s) in OSCC tumorigenesis。

髓内钉治疗胫骨近端骨折:解剖学、生物力学及钉的设计原理

背景:因为胫骨近端解剖结构的特殊性,使用髓内钉作为内固定器械治疗时存在一定困难,早期报道并发症发生率较高。随着胫骨髓内钉结构设计的不断进步,以及髓内钉固定技术的不断发展,临床报道应用该技术治疗的优良率也在逐渐增高,使用髓内钉内固定治疗胫骨近端骨折有逐渐增多的趋势。目的:对髓内钉内固定治疗胫骨近端骨折的最新进展进行综述。方法:以"proximal tibial fracture","intramedullary nail","胫骨近端骨折","髓内钉内固定"为关键词。在Pub Med、Web of science、万方、中国知网等数据库进行检索,共查阅文献742篇,通过阅读文献题目及摘要,判断是否与胫骨近端骨折髓内钉内固定有关,排除文献685篇,最终纳入57篇文献进行分析。结果与结论:(1)胫骨近端解剖结构不同于胫骨干,受传统髓内钉固定技术及髓内钉结构的限制,当其用于治疗胫骨近端骨折时并发症发生率较高;(2)髓内钉治疗技术不断发展,新型髓内钉出现,增加了近端锁钉,并改进锁钉的位置和方向,对近端骨折块的把持力更强;(3)联合钢板固定、Poller钉的应用、入路的改进等髓内钉固定技术的发展,也增加了该技术对胫骨近端骨折固定的强度;(4)经多项生物力学测试,表现优于其他内固定方式,且临床应用中体现出了髓内钉固定技术的创伤小、血运破坏少、固定牢固、术后早期活动、骨折愈合时间短等优势,尤其对于胫骨近端骨折合并胫骨干骨折,近端软组织损伤重的患者,更加适合;(5)但在临床应用该技术时,术者必须对胫骨近端解剖特点有足够的了解,并能熟练掌握髓内钉技术及各种辅助技术,才能够取得满意的临床疗效。
