BACKGROUND This study employs a descriptive phenomenological approach to investigate the challenges anesthesia nurses face in managing emergence delirium(ED),a common and complex postoperative complication in the post-anesthesia care unit.The role of nurses in managing ED is critical,yet research on their understanding and management strategies for ED is lacking.AIM To investigate anesthetic nurses’cognition and management experiences of ED in hopes of developing a standardized management protocol.METHODS This study employed a descriptive phenomenological approach from qualitative research methodologies.Purposeful sampling was utilized to select 12 anesthetic nurses from a tertiary hospital in Shanghai as research subjects.Semi-structured interviews were conducted,and the data were organized and analyzed using Colaizzi’s seven-step analysis method,from which the final themes were extracted.REsuLTS After analyzing the interview content,four main themes and eight subthemes were distilled:Inefficient cognition hinders the identification of ED(conceptual ambiguity,empirical identification),managing diversity and challenges(patientcentered safe care,low level of medical-nursing collaboration),work responsibilities and pressure coexist(heavy work responsibilities,occupational risks and stress),demand for high-quality management(expecting the construction of predictive assessment tools and prevention strategies,and pursuing standardized management processes to enhance management effectiveness).CONCLUSION Nursing managers should prioritize the needs and suggestions of nurses in order to enhance their nursing capabilities and provide guidance for standardized management processes.

Over the past two decades,(K_(0.5)Na_(0.5))NbO_(3)(KNN)-based lead-free piezoelectric ceramics have made significant progress.However,attaining a high electrostrain with remarkable temperature stability and minimal hysteresis under low electric fields has remained a significant challenge.To address this long-standing issue,we have employed a collaborative approach that combines defect engineering,phase engineering,and relaxation engineering.The LKNNS-6BZH ceramic,when sintered at T_(sint)=1170℃,demonstrates an impressive electrostrain with a d_(33) value of 0.276%and 1379 pm·V^(-1)under 20 kV·cm^(-1),which is comparable to or even surpasses that of other lead-free and Pb(Zr,Ti)O_(3)ceramics.Importantly,the electrostrain performance of this ceramic remains stable up to a temperature of 125℃,with the lowest hysteresis observed at 9.73%under 40 kV·cm^(-1).These excellent overall performances are attributed to the presence of defect dipoles involving V′_(A)-V∙∙_(O) and B′_(Nb)-V∙∙O,the coexistence of R-O-T multiphase,and the tuning of the trade-off between long-range ordering and local heterogeneity.This work provides a lead-free alternative for piezoelectric actuators and a paradigm for designing piezoelectric materials with outstanding comprehensive performance under low electric fields.

Multistage tungsten mineralization was recently discovered in the Mamupu copper-polymetallic deposit in the southern Yulong porphyry copper belt(YPCB),Tibet.This study reports the results of cathodoluminescence,trace element and Sr isotope analyses of Mamupu scheelite samples,undertaken in order to better constrain the mechanism of W mineralization and the sources of the ore-forming fluids.Three different types of scheelite are identified in the Mamupu deposit:scheelite A(Sch A)mainly occurs in breccias during the prograde stage,scheelite B(Sch B)forms in the chlorite-epidote alteration zone in the retrograde stage,while scheelite C(Sch C)occurs in distal quartz sulfide veins.The extremely high Mo content and negative Eu anomaly in Sch A represent high oxygen fugacity in the prograde stage.Compared with ore-related porphyries,Sch A has a similar REE pattern,but with higher ΣREE,more depleted HREE and slightly lower(^(87)Sr/^(86)Sr)i ratios.These features suggest that Sch A is genetically related to ore-related porphyries,but extensive interaction with carbonate surrounding rocks affects the final REE and Sr isotopic composition.Sch B shows dark(Sch B-I)and light(Sch B-II)domains under CL imaging.From Sch B-I to Sch B-II,LREEs are gradually depleted,with MREEs being gradually enriched.Sch C has the highest LREE/HREE ratio,which indicates that it inherited the geochemical characteristics of fluids after the precipitation of HREE-rich minerals,such as diopside and garnet,in the early prograde stage.The Mo content in Sch B and Sch C gradually decreased,indicating that the oxygen fugacity of the fluids changed from oxidative in the early stages to reductive in the later,the turbulent Eu anomaly in Sch B and Sch C indicating that the Eu anomaly in the Mamupu scheelite is not solely controlled by oxygen fugacity.The extensive interaction of magmatic-hydrothermal fluids and carbonate provides the necessary Ca^(2+)for the precipitation of scheelite in the Mamupu deposit.

The Asian leaf litter toad genus Leptobrachella contains more than 100 species widely distributed in southwestern Asia.However,the systematic profiles of this group remain unresolved.Osteological morphology is important for taxonomic and phylogenetic studies.However,few studies have focused on the osteology of the genus.Herein,we comprehensively described the osteological features of a representative species L.bijie based on micro-CT scanning and double-staining methods.The results show that the skull of adult L.bijie is well-ossified,exhibiting sexual dimorphism and minimal intraspecific variation.The skull length is slightly greater than the width,with the maxilla slightly overlapping with the quadratojugal.The nasal connects with the sphenethmoid in males,but only part or not connects with the sphenethmoid in females.The transverse processes of the sacrum are robust and symmetrically butterfly-shaped,and the presacral vertebrae are procoelous.The pectoral girdle is arciferous.The phalangeal formula is 2-2-3-3 for the hand,and 2-2-3-4-3 for the foot.This study provides the first detailed and comprehensive osteological accounts of the genus Leptobrachella.

Prediction of mechanical ventilation outcome by early abdominal-visceral-blood-flow-and-function score in critically ill patients after cardiopulmonary bypass in the ICU: A prospective observational study

Background:Abdominal organs are important organs that sense and respond to ischemia and hypoxia,but there are few evaluation methods.We use ultrasonography to evaluate abdominal organ function and blood flow in patients with mechanical ventilation(MV)after cardiopulmonary bypass and to obtain a semiquantitative score for abdominal organ function and blood flow.Methods:Patients with cardiopulmonary bypass in the Critical Care Department of Peking Union Medical College Hospital in China from March to July 2021 were enrolled in this prospective observational study.The correlation of the abdominal-visceral-blood-flow-and-function score(AVBFS)with the duration of MV,number of days spent in the intensive care unit(ICU),acute physiology and chronic health evaluation II(APACHE-II),sequential organ failure assessment(SOFA),lactate,epinephrine,and norepinephrine use was analyzed,and the results were used to assess the predictive value of the receiver operating characteristic curve(ROC)regression analysis score for the duration of MV.Results:Of the 92 patients who underwent cardiopulmonary bypass,41 were finally included.The AVBFS were significantly correlated with the duration of MV,number of days spent in the ICU,APACHE-II score,SOFA score,and norepinephrine use time.The AVBFS in a group of patients using ventilators≥36 h were significantly higher than those obtained for a group of patients using ventilatorssults for the AVBFS at 0-12 h after ICU admission were as follows:area under the ROC curve(AUC)=0.876(95%confidence interval[CI]:0.767 to 0.984),cut-off value=2.5,specificity=0.842,and sensitivity=0.773.Conclusions:Abdominal visceral organ function and blood perfusion can be used to evaluate gastrointestinal function.It is related to early and late extubation after cardiac surgery.

Stretchable electronics that monitor joint activity and treat diseases based on liquid metal could be used in the development of healthcare applications.such devices can be seamlessly integrated with human skin.However,most high-precision microstructures and complex patterns are difficult to fabricate due to the limitations of conventional fabrication solutions,resulting in suboptimal performance under practical conditions.Here,a liquid-metal stretchable system utilizing natural leaf veins was reported as microstructures,which was based on a biomimetic concept and utilized an all-solution process for the preparation of complex microstructures.The systems are ultra-high tensile(800% tensile strain),environmentally stable(20 days) and mechanically durable(300-cycle).The system can accurately recognize the wearer's finger bending level as well as simple gesture signals.At the same time,the system acts as a wearable heater,which can realize the fast heating behavior of heating up to 50℃in 3 min under the human body-safe voltage(1.5 V).The tensile stability is demonstrated by the heterogeneous integration of lasers(405 nm) with the system interconnects for a stretchable and wearable light source.

Objective:This study aims to provide a theoretical foundation for the development and practical application of human immunodeficiency virus(HIV)/acquired immunodeficiency syndrome(AIDS)prevention and control policy systems in China by scrutinizing,analyzing,and synthesizing the evolution of Chinese policies in this domain.Methods:Utilizing grounded theory,we employed NVivo12 software to perform text content analysis,mining,and coding classification,along with visualization techniques,on policy texts sourced from Chinese government platforms,including the official website of China’s State Council.We considered four analytical dimensions—time,subject,type,and object.Results:The National Health Commission and the State Council emerged as the primary entities engaged in policymaking for HIV/AIDS in China.We identified four distinct stages in the evolution of these policies,culminating in a novel‘sun-shaped’HIV/AIDS prevention and control policy network model with an emphasis on‘knowledge,attitude,and practice’at its nucleus,which aligns with national conditions and societal progress.Furthermore,the focal groups of these policies have been dynamically refined and updated over time.Conclusions:Our findings introduce a‘sunshaped’HIV/AIDS prevention and control policy network model specific to China.We observed a conceptual policy shift towards prioritizing overall human health rather than confining the focus to disease treatment.Additionally,in light of China’s growing elderly population,the imperative to address HIV/AIDS prevention and control among older adults is an issue that warrants increased attention.

AIM: The present study was designed to examine the effect of agmatine, the decarboxylated product of L - arginine byL -- arginine decarboxylase, on convulsion in the mouse maximal electroshock (MES) test and mouse glutamate -- inducedconvulsion test. Background:Acute gouty arthritis(AGA)is an inflammatory joint disease with a high prevalence.Typical medical interventions,including nonsteroidal anti-inflammatory drugs,colchicine and glucocorticoids,can have serious adverse reactions.Huzhang Granule(HZG),a compound Chinese herbal medicine,has been used to treat AGA for more than 30 years with satisfactory effects and no significant adverse reactions.However,the efficacy and safety of HZG in AGA patients remains unknown.Objective:The present investigation was designed to examine the efficacy and safety profile of HZG in managing AGA patients.Design,setting,participants and interventions:The current study was conducted as a noninferiority,randomized controlled clinical trial on 180 eligible enrolled participants.Participants were randomly assigned into the HZG and etoricoxib groups.Treatments were administered for 5 d,during which the HZG group received HZG and placebo etoricoxib,while the etoricoxib group received etoricoxib and placebo HZG in the same ratio(1:1).Main outcome measures:The primary outcome was pain experienced by the patient in the gout-afflicted joint from days 2 to 5 of the treatment window.The pain level was measured via a visual analogue scale,ranging from 0 mm to 100 mm.The secondary outcomes comprised joint tenderness and swelling,reduction of inflammatory biomarkers,and the patient’s and investigator’s global evaluations of therapeutic response.Results:The mean reduction in pain was-51.22 mm(95%confidence interval[CI],[-53.42,-49.03]mm)for the HZG and-52.00 mm(95%CI,[-54.06,-49.94]mm)for the etoricoxib groups.The mean difference between the two groups was 0.78 mm(95%CI,[-2.25,3.81]mm).All additional efficacy endpoints,covering decreased inflammation and pain relief,yielded compelling proof of noninferiority.Patients in the HZG group exhibited a comparatively lower rate of adverse events compared to those in the etoricoxib group(4.44%vs 13.33%;P≤0.05).Conclusion:HZG and etoricoxib groups demonstrated similar levels of analgesic effectiveness
