Pancreatic Castleman disease treated with laparoscopic distal pancreatectomy

BACKGROUND: Castleman disease is an uncommon lympho-proliferative disorder most frequently occurring in the mediastinum. Abdominal forms are less frequent, with pancreatic localization of the disease in particular being extremely rare. Only seventeen cases have been described in the world literature. METHOD: This report describes an interesting and unusual case of pancreatic Castleman disease treated with laparoscopic resection. RESULTS: A 48-year-old woman presented with epigastric pain. CT scan showed a well-encapsulated mass on the ventral border of the pancreas. Endosonography with fine needle aspiration biopsy was performed. Biopsy showed lymphoid elements and structures of a normal lymph node. The patient was treated with laparoscopic distal pancreatectomy. The pancreas was transected with a Ligasure device and the pancreatic stump was secured with a manual suture. One year after surgery the patient was complaint-free and showed no signs of recurrence of the disease. CONCLUSIONS: Laparoscopic distal pancreatectomy is a feasible and safe method for the treatment of lesions in the body and tail of the pancreas. Transection of the pancreas with a Ligasure device offers the advantages of low bleeding and low risk of pancreatic fistula.

The treatment of rectal cancer has been improved a great deal within the last 20 years. Major progress has been made in the preoperative evaluation by introducing MRI- imaging as a basis for the further management. Neoadjuvant radiochemotherapy has been shown to be effective in downstaging of advanced tumours. The surgical technique has been improved in many respects.- Total mesorectal excision has reduced local recurrences, sphincter saving techniques such as low anterior resection and intersphincteric resection reduced the need for a permanent stoma to 10%-20%. Recently the introduction of minimal invasive techniques and the application of robotic systems have reduced the surgical trauma.

Traditional Chinese medicines (TCMs), a complex system of natural resources with many diverse components, are widely used as approved medicinal agents in China. Quality control of TCMs is a huge challenge for the government and for testing institutes and is associated with numerous scientific issues. Among these considerations include the following questions: How many components are in TCMs? How can the multiple components in TCMs be comprehensively delineated and subsequently characterized? What is the level and range of these (active) metabolites within these multiple-component TCMs, in order to recommend standards? and What are the qualities required for a marker constituent to be selected, and from a practical perspective, how can these components be assessed with low cost and in a short time? All of these factors require significant and deep thinking in order to understand the individualistic chemistry of TCM in order to develop enhanced TCM quality standards for improved and consistent patient care. In this review, the latest exploratory research in TCM chemistry analytical techniques and methods is summarized in order to begin to develop responses to these scientific issues. Advances in these methods have included multidimensional separation for liquid chromatography–high-resolution mass spectrometry (LC–HRMS), smart triggering data-dependent acquisition of LC–HRMS, target analysis with liquid chromatography–mass spectrometry (LC–MS), supercritical fluid chromatography, and data mining of large mass spectrometry (MS) datasets. In addition, two quality strategies have been introduced in order to save reference standards and the analysis time for a TCM quality standard, including the application of the single standard to determine multi-components (SSDMC) and monomethodheterotrait matrix methods. Finally, a series of future improvements for analytical methods for TCMs are proposed.

Most ground filtering algorithms are primarily designed for airborne LiDAR point cloud processing and their successful use in identifying ground points from photogrammetric point clouds remains *** compared six ground filtering algorithms implemented in Metashape,ArcGIS,CloudCompare,LAStools,and *** used UAV photogrammetry-based(acquired under leaf-off conditions)and airborne LiDAR(leaf-on)point clouds of the same area to:(i)compare accuracy of generated DTMs;(ii)evaluate the effect of vegetation density and terrain slope on filtering accuracy;and(iii)assess which algorithm parameters have the greatest effect on the filtering *** results show that the performance of filtering algorithms was affected by the point cloud type,terrain slope and vegetation *** results were generally better for LiDAR(RMSE 0.13–0.19 m)than for photogrammetric(RMSE 0.19–0.23 m)point *** behavior in varying vegetation and terrain conditions was consistent for LiDAR point ***,when applied on photogrammetric point clouds,the algorithms’behavior was inconsistent,especially in areas of steep slope(except for the Progressive Triangulated Irregular Network in LAStools).Parameters related to the selection of the initial minimum elevation ground points were the most influential in all algorithms and point clouds.

Epitaxial Ni–Mn–Ga thin films have promising application potential in micro-electro-mechanical sensing and actuation systems. To date, large abrupt magnetization changes have been observed in some epitaxial Ni–Mn–Ga thin films, but their origin-either from magnetically induced martensite variant reorientation(MIR) or magnetic domain evolution-has been discussed controversially. In the present work, we investigated the evolutions of the magnetic domain and microstructure of a typical epitaxial Ni–Mn–Ga thin film through wide-field magneto-optical Kerr-microscopy. It is demonstrated that the abrupt magnetization changes in the hysteresis loops should be attributed to the magnetic domain evolution instead of the MIR.

Towards Real-World Applications of Online Learning Spiral Recurrent Neural Networks

Distributed intelligent systems like self-organizing wireless sensor and actuator networks are supposed to work mostly autonomous even under changing environmental conditions. This requires robust and efficient self-learning capabilities implementable on embedded systems with limited memory and computational power. We present a new solution called Spiral Recurrent Neural Networks (SpiralRNN) with an online learning based on an extended Kalman filter and gradients as in Real Time Recurrent Learning. We illustrate its stability and performance using artificial and real-life time series and compare its prediction performance to other approaches. SpiralRNNs perform very stable and show an ac-curacy which is superior or similar to other state-of-the-art approaches. In a memory capacity evaluation the number of simultaneously memorized and accurately retrievable trajectories of fixed length was counted. This capacity turned out to be a linear function of the size of the recurrent hidden layer, with a memory-to-size ratio of 0.64 for shorter trajectories and 0.31 for longer trajectories. Finally, we describe two potential applications in building automation and logistics and report on an implementation of online learning SpiralRNN on a wireless sensor platform under the TinyOS embedded operating system.

Bryophytes are broadly used as bioindicators. However, the internal distribution of accumulated elements in the moss tissue is little known. Sampling was carried out in The West Carpathians, Slovakia, in autumn 2012. Seven replicates have been used. The samples were analyzed by XRF Spectrometer Delta Classic. S, Pb, K, Ca, Cr, Mn, Fe, Cu, Rb, Sr, Mo, Ba and Zn were determined. For ordination analysis we used principal component analysis, statistical graphics system STATISTICA have been used for the correlation analysis and for analysis of variance. Results show that sulphur, zinc, chromium, manganese, molybdenum, kalcium and copper are preferentially accumulated in the capsula. While lead favors gametophyte, potassium and strontium prefer accumulation in sporophyte. Iron significantly accumulates in the more-year segments, while zinc in the stems. Copper, chromium and sulphur are accumulated preferentially in The Fatra Mts.
