BACKGROUND Achalasia is a rare benign esophageal motor disorder characterized by incomplete relaxation of the lower esophageal sphincter(LES). The treatment of achalasia is not curative, but rather is aimed at reducing LES pressure. In patients who have failed noninvasive therapy, surgery should be considered. Myotomy with partial fundoplication has been considered the first-line treatment for non-advanced achalasia. Recently, peroral endoscopic myotomy(POEM), a technique that employs the principles of submucosal endoscopy to perform the equivalent of a surgical myotomy,has emerged as a promising minimally invasive technique for the management of this *** To compare POEM and laparoscopic myotomy and partial fundoplication(LM-PF) regarding their efficacy and outcomes for the treatment of *** Forty treatment-naive adult patients who had been diagnosed with achalasia based on clinical and manometric criteria(dysphagia score ≥ II and Eckardt score > 3) were randomized to undergo either LM-PF or POEM. The outcome measures were anesthesia time, procedure time, symptom improvement, reflux esophagitis(as determined with the Gastroesophageal Reflux Disease Questionnaire), barium column height at 1 and 5 min(on a barium esophagogram), pressure at the LES, the occurrence of adverse events(AEs), length of stay(LOS), and quality of life(QoL).RESULTS There were no statistically significant differences between the LM-PF and POEM groups regarding symptom improvement at 1, 6, and 12 mo of follow-up(P = 0.192, P = 0.242, and P = 0.242, respectively). However, the rates of reflux esophagitis at 1, 6, and 12 mo of follow-up were significantly higher in the POEM group(P = 0.014, P < 0.001, and P = 0.002, respectively). There were also no statistical differences regarding the manometry values, the occurrence of AEs, or LOS. Three Different Doses of Intrathecal Morphine as Part of a Multimodal Regimen for Post-Cesarean Delivery Analgesia: A Randomized Double-Blinded Trial

Brazil is one of the countries that carry out cesarean delivery in the world. The pain after cesarean delivery (CD) is one of the main concerns of parturient. Intrathecal Morphine (ITM) is widely used for the treatment of pain after CD;however, the optimal dose with minimal side effects is to be established. Objectives: To compare the analgesia and side effects resulted by three different doses of ITM as part of a multimodal regimen for post-cesarean delivery analgesia. Methods: One hundred ninety-four patients were sampled in this randomized, double-blinded, three-arm study (group 0.075: ITM 0.75 mg;group 0.1: ITM 0.1 mg and group 0.15: ITM 0.15 mg). All patients received intravenous dexamethasone, dipyrone and ketoprofen. The primary endpoint was pain intensity measured at rest and on movement, while the secondary endpoint was overall satisfaction with pain relief, the incidence of nausea and vomiting, intensity of pruritus, and the use of additional tramadol, antipruritic and antiemetic use within 24 hours after postoperation. Results: The three groups showed similar pain intensity at rest and on movement. The pain intensity on movement was significantly higher than at rest in the groups (p 0.001). There were no significant differences among the groups regarding secondary endpoint. Conclusions: Our findings suggest that in cases of an elective CD, 0.075 mg of ITM produces postoperative analgesia of similar quality that provided by 0.1 or 0.15 mg ITM. The patients should have a multimodal analgesia approach that includes: ITM, anti-inflammatory agents and access to additional systemic analgesic if necessary.

The great major applications of thermal isolation are in the strip of drops and averages temperatures （up to 180 ℃）, which is used from aggressive materials to the nature, such as glass wool, rock wool, polystyrene, EPS among others. In spite of the effectiveness in the retention of the flow of heat, such materials possess considerable cost and they are discarded for long years to be decomposed. In that context, to adapt to the world politics about the preservation of the environment, a study began with intention of developing a material composite with thermal properties, originating from insulating industrial residues. In this research, the behavior of this composite was analyzed, when it submitted traction efforts. For this, a term-mechanic was used （DMA）, where the composite was differentiated under variation temperatures. It was also certain that the gradient temperature of the composite was the source of heat, the conductivity and thermal diffusion. Consequently, the function of the answers of the system was possible to be observed with the deformation of the composite with the tension and temperature variation, as well as the temperature gradient for each proportion of the composite.

Thoracoscopic esophagectomy is related to better outcomes in early adenocarcinoma of esophagogastric junction tumors

Effect of Ultrasound and Xylanase Treatment on the Physical-Mechanical Properties of Bleached Eucalyptus Kraft Pulp

The modification on the fiber structure of bleached eucalyptus kraft pulp is a very attractive alternative for improve- ments in the properties of paper production. The enzymatic treatment by xylanases and ultrassonic treatments modify the characteristics of the fibers, has been reported. Therefore, the purpose of this study was to evaluate the influence of ultrasonic waves as a facilitator of the action of enzymes (hemicellulase) by modifying the physicochemical nature of fiber eucalyptus Kraft pulp, in order to improve the physical and mechanical properties of the paper. But it was observed the that idea of ultrasound acts as facilitator for action of enzymes can’t be affirmed, since in most properties XA-1 and XA-2 were equal statistically. It may be noted that the junction of ultrasound and xylanase provided improves on tensile index, specific elastic modulus and tensile energy absorption and a decrease of tear index on the mechanical properties of handsheet and it increased the opacity when the ultrasound was applied before xylanase.

Suiting Dynamic Models of Fixed-Bed Catalytic Reactors for Computer-Based Applications

This work investigated the applicability of heterogeneous and pseudo-homogeneous models to predict the dynamic behavior of a fixed-bed catalytic reactor. Some issues concerning the dynamic behavior of the system were discussed, such as the prediction of the inverse response phenomenon. The proposed models (Het- erogeneous I and II and Pseudo-homogeneous) were able to predict with qualitative similarity the main characteristics of the dynamic behavior of a fixed-bed catalytic reactor, including the inverse response. The computational time demanded for the solution of the heterogeneous models was 10 to 50% longer than in the case of the pseudo-homogeneous model, making the use of the former suitable for applications where computational time is not the major restriction (off-line applications). On the other hand, when on-line applications are required, the simplified model (Pseudo-homogeneous model) showed to be a good alternative because this model was able to predict (qualitatively) the dynamics of the reactor using a faster and easier numerical solution.

Application of Molecular Distillation Process to Extend the True Boiling Point Curve of Petroleum Residues 400 ℃ ＋

True Boiling Point （TBP） distillation is one of the most common experimental techniques for determination of petroleum properties. The methods for performing TBP distillation experiments are described by ASTM D2892 and by ASTM D5236. However, these methods are established for petroleum fractions that reach temperatures up to 565 ~C. In this work, two petroleum residues were distilled in a falling film molecular distillation prototype and the data were used to obtain the extension of the TBP curve above a temperature of 565 ~C. It was possible to extend the TBP curve of both petroleum up to temperatures close to 700 ~C with consistency and continuity in comparison to the standard curve. In addition, an amount of raw material that was been treated as residue could be reused.

We present numerical studies on the switching characteristics of a fiber Bragg grating (FBG) with modulation in the third order nonlinear index of refraction along it’s length. The FBG is operating in a continuous wave regime (CW). This study was done taking into account the possible asymmetry brought by the non harmonic modulation of the nonlinearity, leading to different reflection and transmission characteristics, that depend on the direction of propagation along the modulated nonlinear FBG. This phenomenon may be useful for applications like an optical isolator. It was found that for a set of values of the modulation parameter, the FBG can exhibit multistable states. The numerical studies were obtained starting from the coupled-mode equations solved from the coupled-mode theory and simulated using the fourth-order Runge-Kutta method.

“Specialty rices” is a common name for varieties that present uncommon grains with distinctive features as pericarp color, grain shape and size or aroma. Most of these rices are related to cultural traditions and ceremonies, but some have higher nutritionally value than white rice. Considering that today people are searching for foods that could help to improve and maintain health, specialty rices would also be considered in some way as functional food. As a specific market, the economic values of these grains are also higher than those of white rice. So, the new red rice variety SCS119 Rubi and the new black rice variety SCS120 Onix were developed in order to attend the specific and economically important specialty rices market, and should be consumed as a whole rice grain. Experimental tests performed with these varieties showed that they could be recommended to all rice-producing regions of Santa Catarina State, Brazil.
