Three Different Doses of Intrathecal Morphine as Part of a Multimodal Regimen for Post-Cesarean Delivery Analgesia: A Randomized Double-Blinded Trial

Brazil is one of the countries that carry out cesarean delivery in the world. The pain after cesarean delivery (CD) is one of the main concerns of parturient. Intrathecal Morphine (ITM) is widely used for the treatment of pain after CD;however, the optimal dose with minimal side effects is to be established. Objectives: To compare the analgesia and side effects resulted by three different doses of ITM as part of a multimodal regimen for post-cesarean delivery analgesia. Methods: One hundred ninety-four patients were sampled in this randomized, double-blinded, three-arm study (group 0.075: ITM 0.75 mg;group 0.1: ITM 0.1 mg and group 0.15: ITM 0.15 mg). All patients received intravenous dexamethasone, dipyrone and ketoprofen. The primary endpoint was pain intensity measured at rest and on movement, while the secondary endpoint was overall satisfaction with pain relief, the incidence of nausea and vomiting, intensity of pruritus, and the use of additional tramadol, antipruritic and antiemetic use within 24 hours after postoperation. Results: The three groups showed similar pain intensity at rest and on movement. The pain intensity on movement was significantly higher than at rest in the groups (p 0.001). There were no significant differences among the groups regarding secondary endpoint. Conclusions: Our findings suggest that in cases of an elective CD, 0.075 mg of ITM produces postoperative analgesia of similar quality that provided by 0.1 or 0.15 mg ITM. The patients should have a multimodal analgesia approach that includes: ITM, anti-inflammatory agents and access to additional systemic analgesic if necessary.

Freshwater biota are more comprehensive and direct indicators of biological impacts,and more meaningful to the public than water quality or physical habitat *** biotic data and the multiple biological indicators developed from them offer a much richer array of data for assessing the impacts of pollution controls than a limited set of physical or chemical *** recent decades,assemblage-based assessments by ecologists,environmental scientists,and water quality agencies have been employed globally for determining the condition of,and threats to,freshwater ecosystems.A key step in this advance has been the development of multimetric indices(MMIs)or indices of biotic integrity(IBIs)based on quantitative assessments of algae,macrophyte,macroinvertebrate,fish or riparian bird *** Europe,where biological assemblages are mandated for assessing freshwater ecosystem health,many indices are ***,the proliferation of MMIs globally has not always occurred through the application of rigorous study designs and monitoring protocols,nor have they always effectively incorporated functional metrics,stressor assessments,and statistical ***,in this review,we discuss eleven major concerns with the development and application(including logistical limitations)of multimetric indicators based on freshwater biota to encourage more rigorous and widely applicable(transferable)MMI use and ***,our concerns focus on reference conditions;sampling effort,methods,and season;trophic guild definition;metric comprehensiveness,options,screening and scoring;and MMI *** could also benefit from increased attention to ecological mechanisms and metric development,to further improve our understanding of anthropogenic impacts as well as rehabilitation effects on freshwater ecosystems *** closer attention to study designs,ecological mechanisms and metric development should further improve our understa

Objective: To evaluate tubal patency using hysterosalpingography after clinical treatment of tubal pregnancy. Method: Of 80 patients who underwent hysterosalpingography after clinical treatment of tubal pregnancy from April 1994 toFebruary 2002, 30 were treated with a single 50 mg/m2 dose of methotrexate intramuscularly (n=30) and 50 were followed up expectantly. Results: Patency of the ipsilateral tube was 84% after methotrexate treatment and 78% after expectant management. In addition, contralateral tubal patency was 97% after methotrexate treatment and 92% after expectant management. There were no statistically significant differences between the groups. Conclusion: Findings from this study suggest similar tubal patency rates after methotrexate treatment and expectant management.

Basidiomata of a Phallus species were found near bamboo trees in the Rio de Janeiro Botanical *** species was described as new to science and was named as Phallus fluminensis based on morphological characteristics.A detailed description,color images of microstructures and basidiomata of ***,a comparison of *** with its morphologically closely related taxa and a key to the genus Phallus in Brazil are given.A comparative analysis of *** collections from the type locality was also made.
