Stage Ⅳ breast cancer refers to breast cancer that has already metastasized to distant regions when initially diagnosed. Treatment for stage Ⅳ is intended to "prolong survival and palliate symptoms". Resection of a primary tumor is considered to be "effective only at alleviating chest symptoms and providing local control" in spite of the advances of imaging examination and medication for breast cancer. Molecular target and endocrine drugs are very effective and useful to tailor-make a treatment strategy according to breast cancer subtypes. Positron emission tomography-computed tomography can detect and diagnose the very small metastases and recurrences which can potentially be cured even if they are distant metastases. Recently, many retrospective studies have reported the survival benefit of surgery for breast cancer patients with metastases and some clinical trials which confirm the surgical prognostic benefit for them have started to enrol patients. The goal of treatment has to be clearly identified:increase the patient's survival time, provide local control or perform histology to determine the cancer's properties. The best evidence is absolutely essential to treat patients who need surgery at the right time. We need to evaluate the treatment strategy, including primary resection for stage Ⅳ breastcancer particularly, and find new evidence by prospective analysis.

Enriched endoplasmic reticulum-mitochondria interactions result in mitochondrial dysfunction and apoptosis in oocytes from obese mice

Background： Maternal obesity alters oocytes and subsequent fetal metabolism. An increasing number of studies have shown that the endoplasmic reticulums（ER） or mitochondria have important effects on oocyte quality, but there has been no study of the effect of mitochondria-associated ER membranes（MAMs） on oocyte quality. The present study was designed to assess whether the level of MAM and MAM-related proteins were different in oocytes from obese and control ***： First, oocytes from mice with high-fat-diet（HFD）-induced obesity had higher levels（either greater numbers or a higher proportion for the same numbers） of MAM than oocytes from control mice. The abundance of MAM-related proteins in oocytes from obese mice was significantly greater at both the messenger RNA and protein levels, including inositol 1,4,5-trisphosphate receptor, type 1（IP3R1）, inositol 1,4,5-trisphosphate receptor,type 2（IP3 R2） and phosphofurin acidic cluster sorting protein 2（PACS-2）. Further, there was an increase in mitochondrial Ca^（2＋）（[Ca^（2＋）]_m） which was associated with increased apoptosis and compromised cytoplasmic maturation in oocytes from obese mice. Down-regulation of MAM-related protein IP3R1 in oocytes from obese mice decreased [Ca^2＋]_mand apoptosis and improved cytoplasmic maturation but did not reduce the overal MAM level. However, down-regulating MAM-related protein PACS-2 in oocytes from obese mice did reduce the level of MAM and [Ca^（2＋）]_m, which decreased the rate of apoptosis and improved cytoplasmic maturation of oocytes from obese ***： It is possible that enriched MAM could increase [Ca^（2＋）]_m, and this increase has been found to be associated with increased apoptosis and compromised cytoplasmic maturation in oocytes from obese mice. This finding suggests a novel therapeutic target for obesity-induced oocyte defects.

Imitating the structure and function of the Pacinian corpuscle in the human body,a Bionic Cell(BC)with a liquid core was designed and made with polyvinylidene fluoride(PVDF),and an Electronic Skin(ES)sensor was successfully obtained by embedding the BCs into silicone *** passive sensing ability of the ES for surface vibration pressure was *** results showed that the ES could detect waveform,frequency,amplitude and other parameters of the surface,and it exhibited not only a high sensitivity of 0.46563 fc/Pa,but also a large pressure measurement range of 0.6 Pa–6014 Pa.A falling real object impact experiment was carried out,and the results indicated that the ES output charge was linearly related to the falling height of the object and logarithmically related to the mass of the ***,a crawling insect experiment and human pulse experiment were performed to study the perception ability of the ES to irregular *** to its special structure,simple fabrication process,and high sensitivity wide measurement range,the ES fabricated in this paper is expected to be used as robot skin with tactile perception ability in the near future.

Brain areas activated by uncertain reward-based decision-making in healthy volunteers

Reward-based decision-making has been found to activate several brain areas, including the ven- trolateral prefronta~ lobe, orbitofrontal cortex, anterior cingulate cortex, ventral striatum, and mesolimbic dopaminergic system. In this study, we observed brain areas activated under three de- grees of uncertainty in a reward-based decision-making task （certain, risky, and ambiguous）. The tasks were presented using a brain function audiovisual stimulation system. We conducted brain scans of 15 healthy volunteers using a 3.0T magnetic resonance scanner. We used SPM8 to ana- lyze the location and intensity of activation during the reward-based decision-making task, with re- spect to the three conditions. We found that the orbitofrontal cortex was activated in the certain reward condition, while the prefrontal cortex, precentral gyrus, occipital visual cortex, inferior parietal lobe, cerebellar posterior lobe, middle temporal gyrus, inferior temporal gyrus, limbic lobe, and midbrain were activated during the ＇risk＇ condition. The prefrontal cortex, temporal pole, inferior temporal gyrus, occipital visual cortex, and cerebellar posterior lobe were activated during am- biguous decision-making. The ventrolateral prefrontal lobe, frontal pole of the prefrontal lobe, orbi- tofrontal cortex, precentral gyrus, inferior temporal gyrus, fusiform gyrus, supramarginal gyrus, infe- rior parietal Iobule, and cerebellar posterior lobe exhibited greater activation in the ＇risk＇ than in the ＇certain＇ condition （P 〈 0.05）. The frontal pole and dorsolateral region of the prefrontal lobe, as well as the cerebellar posterior lobe, showed significantly greater activation in the ＇ambiguous＇ condition compared to the ＇risk＇ condition （P 〈 0.05）. The prefrontal lobe, occipital lobe, parietal lobe, temporal lobe, limbic lobe, midbrain, and posterior lobe of the cerebellum were activated during deci- sion-making about uncertain rewards. Numerical simulation is used to investigate the flow field in a model centrifugal fan for steam power stations in order to improve the performance. During testing the model fan, it is found that the efficiency is only 62.5% with inlet box, without it the efficiency is 83%. In addition, the strong vibration of test rig is observed with inlet box. It would be highly desirable if the aerodynamics of the fan could be studied. Therefore, numerical simulation is carried out to investigate the internal flow characteristics of a model fan with inlet box. The results from CFD analysis show that the whole region of the inlet box is occupied by a spiral vortex rotating inversely as the rotor＇s direction, which significantly affect the most flow＇region inside the fan. For this reason, a dummy plate is arranged in the inlet box to impede the generation of the spiral vortex, the results from CFD after the reform demonstrate that the modification is quiet effective, the former large spiral vertex has been destroyed effectively, the large one is superseded in favor of two small vortexes. However, two small vortexes have little effect on the inner flow of the rotor and the following parts. Finally, the efficiency of the model fan is improved by the test and the strong vibration of the test rig disappears. This type of modification has been used in steam power stations, the fan efficiency raises to 84% successfully.

Fenton or photocatalytic degradations of organic contaminants are recognized as promising approaches to address the increasing environmental pollution ***,we develop the effective synergistic catalysis reaction of Fenton and photocatalysis based on a loofah sponge-like Fe_(2)O_(x)/C nanocomposite,which exhibits excellent nitrobenzene photocatalytic degradation *** is noted that Fe2O3 nanoparticles with surface Fe^(2+) species were encapsulated with an ultrathin carbon layer(denoted as Fe_(2)O_(x)/C)via a supramolecular self-sacrificing template and following thermal treatment *** experimental results indicated that the thin layer carbon coating not only inhibited the Fe iron leaching from the Fe_(2)O_(x)but also prompted the separation and transferring of electrons–hole *** introduction of Fe_(2)O_(x)/C enables the Fenton reaction to induce a rapid Fe^(2+)/Fe^(3+)cycle,and meanwhile,together with the photocatalytic reaction to produce continuous active substances for the subsequent degradation catalytic reaction without successive H2O2,resulting in the inexpensive and the effective photocatalytic *** a result,100%nitrobenzene(100 mg/L)was degraded and 97%of the organic carbon was mineralized in 90 min using the Fe_(2)O_(x)/C(0.1 g/L)at a low H_(2)O_(2) dosage(0.50 mM),under air mass(AM)1.5 *** calculations confirmed that the Fe_(2)O_(x)/C-600 with thin carbon layer promoted the dissociation of H2O2 and the·OH *** synergistic catalysis of this work may provide new ideas for low-cost and more efficient treatment of pollutants.
