A new algorithm for the diagnosis of celiac disease

Celiac disease(CD)affects at least 1%of the Western population but remains largely *** our laboratory,we utilize a novel algorithm to diagnose pediatric CD that offers both high sensitivity and high specificity for diagnosis in an outpatient *** aim of the present study was to challenge this algorithm and to test its performance in children and adults suspected of having *** a three-assay algorithm,screening with the most sensitive tissue transglutaminase(tTG)complexed with deamidated gliadin peptide neoepitope immunoglobulin A(IgA)1IgG assay and confirming with the two specific tTG IgA and tTG IgA1IgG assays,we examined the serological results from 112 children aged 0–17 years old and 60 adults in comparison to their respective biopsy *** algorithm performance was calculated by statistical *** use of the new algorithm enabled us to diagnose CD with 98%sensitivity,93%specificity and 95%accuracy in the pediatric group and 94%sensitivity,92%specificity and 93%accuracy in the total population *** false-negative cases in the adult group were attributed to previous adherence to a gluten-free diet,and the single false-negative result in a young child became a true positive after 6 *** have also monitored three celiac patients before and after diagnosis and found that the algorithm may be suitable for disease *** newly proposed three-assay algorithm for celiac detection is very reliable in both children and *** to the high performance of this assay,the further need for confirmatory intestinal biopsies will be reassessed.

Dear Editor On July 29,2021,the Israeli Ministry of Health approved the third anti-Coronavirus disease 2019(COVID-19)vaccination(3rd-BNT162b2-booster-dose),leading to a sharp daily drop in diagnosed positive COVID-19 cases and mortality rates in Israel[1].The booster dose increased severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)neutralization efficiency approximately a hundred-fold compared to individuals receiving only the second dose,providing significant protection against infection[2].

Seed wasp invasions promoted by unregulated seed trade affect vegetal and animal biodiversity

Cone and seed insects are considered the most important predators of tree seeds during the pre-dispersal phase of *** them,exotic seed chalcids in the genus Megastigmus invaded Europe as a result of the rapidly-increasing and mostly unregulated seed trade for afforestation and ornamental *** their economic impact in seed orchards,until recently,little attention was paid to the ecological impact of these *** the present study,selected case studies of alien Megastigmus *** considered to assess their specific impact on the potential of natural regeneration of native woody plants and on the native entomofauna competing for seed *** re-analyzed data from former studies that did not focus on these ecological interactions and,here,present previously unpublished *** of Douglas-fir,true cedars,true firs and wild roses were sampled all over Europe,and the relative importance of the native and invasive chalcid species was assessed as well as their specific impact on seed *** most cases,the recent arrival of alien chalcids resulted in a significant decrease in the regeneration potential of the host *** the absence of competitors,alien chalcids occupied the entire seed niche in Douglas-fir,but their impact tended to decrease after the arrival of invasive seed *** firs,alien chalcids tended to displace the native chalcids,but not in wild roses and cedars,where their damage was *** biological traits that might explain invasive success of alien chalcids are ***,no general invasive patterns seem to exist.

Charge disproportionation and site-selective local magnetic moments in the post-perovskite-type Fe_(2)O_(3) under ultra-high pressures

The archetypal 3d Mott insulator hematite,Fe_(2)O_(3),is one of the basic oxide components playing an important role in mineralogy of Earth’s lower *** high pressure-temperature behavior,such as the electronic properties,equation of state,and phase stability is of fundamental importance for understanding the properties and evolution of the Earth’s ***,we study the electronic structure,magnetic state,and lattice stability of Fe_(2)O_(3)at ultra-high pressures using the density functional plus dynamical mean-field theory(DFT+DMFT)*** the vicinity of a Mott transition,Fe_(2)O_(3) is found to exhibit a series of complex electronic,magnetic,and structural *** particular,it makes a phase transition to a metal with a post-perovskite crystal structure and site-selective local moments upon compression above 75 *** show that the site-selective phase transition is accompanied by a charge disproportionation of Fe ions,with Fe^(3±δ)and δ~0.05–0.09,implying a complex interplay between electronic correlations and the *** results suggest that site-selective local moments in Fe_(2)O_(3) persist up to ultra-high pressures of~200–250 GPa,i.e.,sufficiently above the core-mantle *** latter can have important consequences for understanding of the velocity and density anomalies in the Earth’s lower mantle.

目前从大量的随机对照临床试验(RCT)中已得出结论性证据:对于经历着令人痛苦的更年期症状的围绝经期和绝经后妇女,任何形式的全身HRT均可以缓解这些症状,没有其它治疗手段能产生相似效果。尽管这种证据与日俱增,但如何提供最好的临床医疗以减轻绝经症状和预防较远期的绝经后退化性疾病,仍在争论之中。当提供更年期医药时,HRT剂量和方案的个体化是基于与症状的严重程度和绝经时间相关的最低有效剂量的原则。然而,在有症状的妇女中,尚没有多少关于长期应用不同HRT方案的资料,赖以说明心血管疾病,乳腺癌和骨质疏松症基线风险的影响。在大多数病例中,个体化的处方联同生活方式的调整就可能对更年期症状、生活质量(QoL)、性问题和骨质疏松症产生最终有益的作用,罕见严重副反应。欧洲男女更年期协会(EMAS)基于最近的流行病学研究,其中不少是来自WHI中单用雌激素组的结果,支持在有症状的妇女中进行新HRT方案的研究,以提高临床中受益和风险之间的平衡,包括生活质量方面特有的信息: 并解释美国和欧洲之间在治疗模式上的传统差异,以及在BMI、生活方式和饮食习惯方面的不同所产生的影响。对于早绝经(小于45岁) 妇女,现有资料支持HRT所特有的总体益处。目前,具有骨质疏松性骨折高危险性的妇女可考虑应用较长期的全身HRT,尤其是当其它治疗措施既不恰当又疗效不佳时,因为此时收益大于风险。而对于泌尿生殖道症状,已认为长期的局部雌激素治疗可能安全和有效。

Aim: To describe the diagnostic and therapeutic management of cytomegalovirus (CMV) anterior uveitis unassociated with retinal necrosis in immunocompetent patients. Methods: Patients referred between 2001 and 2003 for management of unilateral, chronic, recurrent uveitis associated with secondary glaucoma underwent extensive investigation including laboratory tests for herpes virus infections. Specific antiviral treatment was initiated in all cases and the level of ocular inflammation was evaluated during the follow up. Results: Five patients, three men and two women, were included. Median age was 50 years old (range 30-80 years). Anterior unilateral uveitis without iris atrophy was observed in all cases. Uveitis was chronic in three cases and recurrent in two cases. Glaucoma was observed in all patients with amedian intraocular pressure of 30 mmHg (range 22-43 mmHg). Five patients responded initially to specific anti-CMV therapy. Even though glaucoma surgery was necessary in two cases, both ocular inflammation and glaucoma were controlled in all cases. Relapses occurred in three cases after cessation of therapy, requiring prolonged maintenance therapy with valganciclovir. Conclusions: CMV infection and specific antiviral therapy should be considered in all cases of relapsing or chronic iridocyclitis and secondary glaucoma. Maintenance regimens of valganciclovir may be necessary to prevent further relapses.

Physical rehabilitation of lung transplant candidates and recipients plays an important in optimizing physical function prior to transplant and facilitating recovery of function post-transplant. As medical and surgical interventions in lung transplantation have evolved over time, there has been a demographic shift of individuals undergoing lung transplantation including older individuals, those with multiple co-morbidites, and candidates with respiratory failure requiring bridging to transplantation. These changes have an impact on the rehabilitation needs of lung transplant candidates and recipients. This review provides a practical approach to rehabilitation based on research and clinical practice at our transplant centre. It focuses on functional assessment and exercise prescription during an uncomplicated and complicated clinical course in the pre-transplant, early and late post-transplant periods. The target audience includes clinicians involved in pre- and posttransplant patient care and rehabilitation researchers.
