Objective: To investigate the clinical features of the multiple cutaneous and uterine leiomyomatosis (MCUL) syndrome, including the hereditary leiomyomatosis and renal cell cancer syndrome. Design: A case series of patients with multiple skin leiomyomas solicited via a circular letter to dermatologists. Setting: Research institute. Patients: A total of 108 affected individuals, including 46 probands and 62 affected relatives. Main Outcome Measures: The proportion of probands with underlying fumarate hydratase (FH) mutations, the penetrance of FH mutations, and clinicopathologic features of MCUL. Results: Forty-one (89% ) of 46 probands with multiple skin leiomyomas had evidence of germline FH mutations, which were highly penetrant. All 26 male mutation carriers had skin leiomyomas. Of 67 women with FH mutations, 46 (69% ) had both skin and uterine leiomyomas; 10 (15% ) had only skin leiomyomas;5(7% )had only uterine leiomyomas;and 6(9% )were clinically unaffected. Patients presented with skin leiomyomas at a mean age of 24 years and had a mean of 25 lesions. Forty-one individuals (89% ) reported painful lesions, particularly in response to cold or trauma. Fibroids were histologically unremarkable, highly symptomatic, and associated with a high risk of early hysterectomy. One individual had a very aggressive collecting duct renal cancer. The G354R FH mutation predisposed patients to uterine fibroids without skin leiomyomas (P=.03). Many patients with skin leiomyomas had not previously presented for medical attention. Fibroids were rarely recognized as cases of MCUL. Conclusions: Highly penetrant FH mutations underlie MCUL. Increased clinical awareness is important because of the associated risk of severe uterine fibroids and, in some cases, aggressive renal cancer.

Do tau-synaptic long-term depression interactions in the hippocampus play a pivotal role in the progression of Alzheimer’s disease?

Cognitive decline in Alzheimer’s disease correlates with the extent of tau pathology,in particular tau hyperphosphorylation that initially appears in the transentorhinal and related regions of the brain including the *** evidence indicates that tau hyperphosphorylation caused by either amyloid-βor long-term depression,a form of synaptic weakening involved in learning and memory,share similar *** from our group and others demonstrate that long-term depression-inducing low-frequency stimulation triggers tau phosphorylation at different residues in the hippocampus under different experimental conditions including ***,certain forms of long-term depression at hippocampal glutamatergic synapses require endogenous tau,in particular,phosphorylation at residue *** the exact mechanisms of interaction between tau and long-term depression may help our understanding of the physiological and pathological functions of tau/tau(hyper)*** first summarize experimental evidence regarding tau-long-term depression interactions,followed by a discussion of possible mechanisms by which this interplay may influence the pathogenesis of Alzheimer’s ***,we conclude with some thoughts and perspectives on future research about these interactions.

＜正＞The present study investigated the role of β-adrenoceptors and the downstream cAMP/PKA signaling pathway on the Aβinhibition of *** induction of LTP by a brief HFS in the dentate gyrus was prevented（101±2%） by preperfusion of Aβ（500 nM） for 40 min prior to *** inhibition can be prevented by β-adrenoceptors agonist,isoproterenol （5μM）,andβ2 agonists terbutaline（5μM）,but not the selective β1 agonist dobutamine in low concentration（2.5μM）.Dobutamine（20μM） also reversed Aβinhibition as dobutamine has someβ2 agonist action at high concentrations. This protection can be blocked byβ2 antagonist,IC1 ***,an agent that stimulate the cAMP/PKA pathway prevent Aβinhibition of *** PKA inhibitor,8-Br-Rp-cAMPs blocked the effect of forskolin and *** CaMKⅡinhibitor KN62 did not prevent the A β-mediated inhibition of LTP.Aβstill inhibited LTP in postnatal animals aged P10-P12 in which LTP is mediated solely by activation of PKA as CaMKII has not developed at this *** show that activation ofβ2,but not β1,and the PKA pathway prevents the Aβinhibition of *** findings identify that therapeutic treatment for AD may include agents that activateβ2 receptors and elevate cAMP.

＜正＞Inappropriate processing of the proteolytic fragment of amyloid precursor protein,amyloidβ-protein（Aβ）,in early stages of Alzheimer’s disease（AD） may lead to the release of stable small oligomers that are highly mobile with the potential to be more toxic than larger fibrillar ***,the importance of such soluble species of Aβin triggering synaptic dysfunction,cognitive impairment long before significant neurodegeneration,has become apparent. In accordance with that,support for an involvement of NMDARs in the cognitive deficits of AD is provided by the current clinical use of ***,the ability of the NMDA receptor（NMDAR） antagonist memantine to prevent synthetic Aβ-mediated rapid functional deficits in synaptic plasticity was assessed in the rat in *** with a clinically relevant,NMDAR blocking concentration of memantine partially inhibited the induction of long-term potentiation（LTP） in the dentate gyrus and prevented further inhibition caused by exposure to Aβ*** acute incubating the slice for 2 h with memantine and chronic oral administration rescue the inhibition of LTP by Aβ.The acute protection afforded by memantine,albeit in a narrowdose range,against the rapid disruptive effects of soluble Aβ1-42 on synaptic plasticity strongly implicate NMDAR-dependent reversible dysfunction of synaptic mechanisms in Aβ-mediated cognitive impairment.

Highly efficient nonlinear optical（NLO） materials with well-defined architectures in the wavelength and subwavelength length scales are of particular importance for next generation of integrated photonic circuits. Fluorenone analogues have been demonstrated to be promising candidates as building blocks for assembly of organic NLO materials thanks to their synergistic supramolecular interactions and brilliant optical properties. Here we have studied the polymorphs of a phenylethynyl functionalized fluorenone derivative, and their controlled self-assembly for microstructures with different morphologies. These polymorphic microcrystals exhibit very distinctive NLO properties, highly related to their supramolecular and electronic structures.

Intensity-specific considerations for exercise for patients with inflammatory bowel disease

The rising prevalence of inflammatory bowel disease(IBD)necessitates that patients be given increased access to costeffective interventions to manage the *** is a non-pharmacologic intervention that advantageously affects clinical aspects of IBD,including disease activity,immune competency,inflammation,quality of life,fatigue,and psychological *** is well established that exercise performed at low-to-moderate intensity across different modalities manifests many of these diseased-related benefits while also ensuring patient *** less is known about higher-intensity *** aim of this review is to summarize findings on the relationship between strenuous exercise and IBD-related *** healthy adults,prolonged strenuous exercise may unfavorably alter a variety of gastrointestinal(GI)parameters including permeability,blood flow,motility,and neuro-endocrine *** intensity-and gut-specific changes are hypothesized to worsen IBD-related clinical presentations such as diarrhea,GI bleeding,and colonic *** this,there also exists the evidence that higher-intensity exercise may positively influence microbiome as well as alter the inflammatory and immunomodulatory changes seen with *** findings recognize that safety for IBD patients doing prolonged strenuous exercise is no more compromised than those doing lower-intensity *** with prolonged,strenuous exercise may be achieved with a.j.stments including adequate hydration,nutrition,drug avoidance,and careful attention to patient history and *** work is needed to better understand this intensity-dependent relationship so that guidelines can be created for IBD patients wishing to participate in high-intensity exercise or sport.

Controlling the gelation temperature of biomimetic polyisocyanides

Thermosensitive polymers show an entropy-driven transition from a well-solvated to a poorly solvated polymer chain, resulting in a more compact globular conformation. The transition at the lower critical solution temperature（LCST） is often sharp, which allows for a wide range of smart material *** the LCST, oligo（ethylene glycol）-substituted polyisocyanides（PICs） form soft hydrogels, composed of polymer bundles similar to biological gels, such as actin, fibrin and intermediate filaments. Here, we show that the LCST of PICs strongly depends linearly on the length of the ethylene glycol（EG） tails; every EG group increases the LCSTand thus the gelation temperature by nearly 30 ℃. Using a copolymerisation approach, we demonstrate that we can precisely tailor the gelation temperature between 10 ℃ and 60 ℃and, consequently, tune the mechanical properties of the PIC gels.
