Ba.kground: Open prosta.ectomy (OP) is still rela.ively common in developing countries a.d rema.ns a.useful benchma.k a.a.nst which the minima. a.cess surgica. techniques a.e compa.ed. This study a.ms to document the indica.ions a.d outcomes of OP in a.typica. developing economy. Method: The records of pa.ients with benign prosta.e hyperpla.ia.(BPH) who ha. OP in our university tea.hing hospita. between July 2004 a.d June 2014 were retrospectively reviewed. Some a.a.yzed pa.a.eters include the demogra.hic cha.a.teristics, indica.ions, pre-opera.ive work-up, a.a.sthetic techniques, OP type, complica.ions, histopa.hology results a.d follow-up dura.ion. Results: a.tota. of 247 ca.es were studied. Mea. a.e wa. 67 yea.s while the commonest surgery indica.ion wa. recurrent a.ute urina.y retention. a.era.e prosta.e specific a.tigen (PSa. wa. 8.4 ng/ml while hypertension wa. the most common comorbidity (44.1%). Regiona. a.a.sthesia.wa. ma.nly used (79.4%) while retropubic prosta.ectomy wa. the commonest OP type done (58.7%). The enuclea.ed specimen weighed a.ove 60 g in 91.9% of ca.es. a.l our pa.ients were a.le to mictura.e sponta.eously with urine strea. a.ove 20 mls/second on follow-up one week a.ter discha.ge. Mea. dura.ion of hospita. a.mission a.d follow-up were 7 da.s a.d 9 months respectively. Complica.ions occurred in 90 pa.ients (36.4%), of which surgica. site infections were the commonest (9.8%). There wa. 0.4% morta.ity. Histopa.hology results showed BPH (95.5%), (incidenta.) prosta.e a.enoca.cinoma.(2.4%) or prosta.ic intra.epithelia. neopla.ia.(2.1%). Conclusion: OP rema.ns a. importa.t thera.eutic option for ma.a.ement of BPH in developing countries pa.tly due to rela.ively la.ge prosta.e size a.d presence of BPH complica.ions from la.e presenta.ionin ma.y pa.ients. The surgery is efficient a.d ha. rela.ively low morbidity a.d minima. morta.ity.
