Genetic biomarkers for hepatocellular cancer risk in a caucasian population

AIM To uncover novel genetic markers that could contribute to predicting hepatocellular carcinoma(HCC)susceptibility in Caucasians. METHODS The present retrospective case-control study compared genotype frequencies between a cohort of HCC cases and two,independent,HCC-free,age/sex-matched control *** HCC cohort comprised 192 homogeneous patients that had undergone orthotopic liver *** first control group comprised167 patients that were matched to the HCC cohort for the percentage of hepatitis B(HBV)and/or hepatitis C(HCV)infections.A second control group included192 virus-free,healthy individuals that were used to evaluate the generalizability of the identified predictive *** cases and controls were *** three study populations were characterized with a panel of 31 markers derived from 21 genes that encoded key proteins involved in hepatocarcinogenesis-related *** study end-point was to assess the association between genetic variants and HCC onset. RESULTS Five genetic markers were identified as risk factors for HCC in high-risk patients infected with HBV/*** to a dominant model,reduced HCC risk was associated with three polymorphisms:ERCC1rs3212986(OR=0.46,95%CI:0.30-0.71,P=0.0005),GST-P1 rs1138272(OR=0.41,95%CI:0.21-0.81,P=0.0097),and CYP17A1 rs743572(OR=0.50,95%CI:0.31-0.79,P=0.0032).Conversely,according to a recessive model,increased HCC risk was associated with two polymorphisms:XRCC3 rs1799794(OR=3.70,95%CI:1.02-13.39,P=0.0461)and ABCB1 rs1128503(OR=2.06,95%CI:1.18-3.61,P=0.0111).These associations remained significant in a subgroup analysis,where patients were stratified according to viral status(HBV-or HCV-positive serology).Two variants exhibited a serology-specific effect:ABCB1 rs1128503(OR=4.18,95%CI:1.55-11.29,P=0.0048)showed an effect in the HBV-positive subgroup;and ERCC1 rs3212986(OR=0.33,95%CI:0.18-0.60,P=0.0003)showed an effect in the HCV-positive *** the five markers identified,E

在过去的十年里风险资本和私募基金经历了激剧的增长。近几年出现了新的发展趋势,以二次并购模式退出的并购投资有了全球范围的激增。尽管二次并购投资市场最近几年有了快速的增长,但是二次并购投资现象还没有完全的明朗化,并尚未得到认可和研究。本论文的目的是对二次并购投资的现行激增现象做一个深入的剖析。本人研究了二次并购投资增长的逻辑,并着重关注了其机制的特点、该现象的范围、近年来的演变及其背后的经济原理。本论文的第一部分以欧洲市场为重点,从结构和增长的角度定义了私募基金行为,杠杆并购投资和二次并购投资。同时回顾了与私募股权投资基金创造价值的文献资料。第二部分包括对从1998到2010年欧洲公司二次并购投资样本的实例分析,从而研究这些投资的回报和交易的价值创造过程。这部分也比较了第一轮交易和二次并购投资以便更好地理解二次并购投资是否有效地创造了价值。

For greening the building envelope several concepts can be used, for example green roofs, fa?ades greened with climbing plants or living wall systems (modular pre-vegetated panels), etc. Greening the building envelope allows to obtain a relevant improvement of the its effi- ciency, ecological and environmental benefits as well as an increase of the biodiversity. Since the interest restoring the environmental integ- rity of urban areas continues to increase, new developments in construction practices with beneficial environmental characteristics take place, as vertical greening systems. Applying green fa?ades is not a new concept and can offer multiple benefits as a component of cur- rent urban design;considering the relation be- tween the environmental benefits, energy sav- ing for the building and the vertical greening systems (material used, maintenance, nutrients and water needed) the integration of vegetation could be a sustainable approach for the enve- lope of new and existing buildings.

Are liver nested stromal epithelial tumors always low aggressive?

Nested stromal-epithelial tumor(NSET) is a nonhepatocytic and non-biliary tumor of the liver consisting of nests of epithelial and spindled cells with associated myofibroblastic stroma and variable intra-lesional calcification and ossification, which represents a very rare and challenging disease. Most of the reported cases have been treated with surgery, obtaining a long survival outcome. Here, we report the case of a 31-year-old Caucasian man who underwent surgery at our institution for a large, lobulated, multinodular mass of the right hemi-liver. The histological exam confirmed the diagnosis of NSET. After 6 mo from surgery, a liver recurrence was described and a chemoembolization was performed. After a further disease progression, based on the correlation between the histological features of the disease and those of the hepatoblastoma, a similar chemotherapy regimen(with cisplatin and ifosfamide/mesna chemotherapy, omitting doxorubicin due to liver impairment) was administered. However, infection of the biliary catheter required a dose modification of the treatment. No benefit was noted and a progression of disease was radiologically assessed after only four cycles. The worsening of the clinical status prevented further treatments, and the patient died a few months later. This case report documents how the NSET might have an aggressive and non-preventable behavior. No chemotherapy schedules with a proved efficacy are available, and new data are needed to shed light on this rare neoplasm.

Poly-and perfluoroalkyl substances(PFASs)are important environmental contaminants globally and in the early 2000s they were shown to be ubiquitous contaminants in Arctic *** reviews by Butt et *** Letcher et *** covered studies on levels and trends of PFASs in the Arctic that were available to *** purpose of this review is to focus on more recent work,generally published between 2009 and 2018,with emphasis on PFASs of emerging concern such as perfluoroalkyl carboxylates(PFCAs)and short-chain perfluoroalkyl sulfonates(PFSAs)and their *** measurements over the period 2006e2014 have shown that fluorotelomer alcohols(FTOHs)as well as perfluorobutanoic acid(PFBA)and perfluoroctanoic acid(PFOA)are the most prominent PFASs in the arctic atmosphere,all with increasing concentrations at Alert although PFOA concentrations declined at the Zeppelin Station(Svalbard).Results from ice cores show generally increasing deposition of PFCAs on the Devon Ice cap in the Canadian arctic while declining fluxes were found in a glacier on *** extensive dataset exists for long-term trends of long-chain PFCAs that have been reported in Arctic biota with some datasets including archived samples from the 1970s and *** in PFCAs over time vary among the same species across the North American Arctic,East and West Greenland,and *** long term time series show a decline from higher concentrations in the early *** there have been recent(post 2010)increasing trends of PFCAs in ringed seals in the Canadian Arctic,East Greenland polar bears and in arctic foxes in *** biological sampling is helping to determine these relatively short term *** levels of some PFCAs have been explained by continued emissions of long-chain PFCAs and/or their precursors and inflows to the Arctic Ocean,especially from the North *** the effectiveness of biological sampling for temporal trends in long-chain PFCAs an

Mesh Infection and Migration after Umbilical Hernia Repair

Mesh infection and migration are dangerous and common complications after hernia repair. Many factors cause mesh migration, which may or may not be associated to infection. Before performing hernia repair using a mesh, it is important to choose the right device to avoid this kind of complication, above all in the presence of contaminated wounds or fields. We describe two cases of mesh infection and migration after umbilical hernia repair which were treated, in accordance with recommendations in the literature, by removing the infected mesh and replacing it with a biological mesh. Our experience confirms the feasibility of using biological mesh to perform umbilical hernia repair after infection, with consequent migration, of the previously placed mesh. Our cases are the first to be reported in which mesh migration is associated with infection after umbilical hernia repair, and they demonstrate the advantages of biologic implants in abdominal wall reconstruction.

Comparison of Clonidine and Metoprolol Effectiveness on the Control of Post-Operative Blood Pressure in Otorhinolaryngology-Head and Neck Surgeries: A Clinical Trial

Objectives: Adequate control of hemodynamic parameters in patients undergoing otolaryngology-head and neck (OLHN) surgeries is one of the objectives of peri-operative anesthesia management. In this context, we seek to evaluate the effectiveness of metoprolol compared to clonidine in the post-operative control of systolic (SBP) and diastolic (DBP) blood pressures, and heart rate (HR), when these medications are used in the immediate pre-operative period. Method: this is a randomized double-blind clinical trial. Patients over 18 years old, according to the American Society of Anesthesiologists Physical Status Classification (ASA) I or II, who would undergo OLHN surgery, presenting with greater than 140 mmHg (SBP) or 90 mmHg (DBP), upon arrival at the surgical ward, were included in the study. 46 patients were randomized into two groups (C or M) who received clonidine (75 or 150 mcg) or metoprolol (5 or 10 mg), respectively. The averages of SBP, DBP and HR were compared between groups upon arrival at the post anesthesia care unit (PACU) and four hours post-surgery. Results: In group C, SBP (127.7 ± 18.8 vs 137.3 ± 14.1 mmHg, p = 0.03) and DBP (73.1 ± 15.46 vs 82.6 ± 7.9 mmHg, p 0.01) were lower, when compared with group M. No difference was observed in HR immediately upon PACU arrival. In addition, for four hours post-surgery, no change was noted in the SBP, DBP or HR. Conclusion: metoprolol was less effective than clonidine in reducing systolic and diastolic blood pressures in the immediate post-operative period of OLHN surgeries.

Obstructive Sleep Apnea Syndrome Implications on Health and Adherence to CPAP Treatment

Respiratory disorders during sleep have as a general characteristic the alteration of the respiratory cycle while sleeping. The most outstanding characteristic of Obstructive Sleep Apnea Syndrome (OSA) is the partial (hypopnea) or total (apnea) obstruction of the upper airway that occurs repeatedly during sleep. The OSA is global public health issue. When it is not treated, OSA represents a cost two or three times higher of the institutional resources for health. Studies conducted in different countries indicate that the prevalence of the OSA goes from 2% to 10% in general population. The OSA is a serious sleep disorder that has negative implications on multiple systems of the organism. It is associated with hypertension, diabetes and the metabolic syndrome. When OSA coexists with a heart disease or ischemic heart disease, it significantly raises the probability of a heart failure. The use of Continuous Positive Airway Pressure (CPAP) is so far the most effective method for OSA treatment. Intervention at different levels (physiological, educational and psychological intervention) appears to be important in adherence to CPAP treatment.
