AIM:To evaluate the potential use of colonoscopy and endoluminal ultrasonic biomicroscopy(eUBM)to track the progression of mouse colonic ***:Ten mice were treated with a single azoxy-methane intraperitoneal injection(week 1)followed by seven days of a dextran sulfate sodium treatment in their drinking water(week 2)to induce inflammationassociated colon *** was performed simultaneously with colonoscopy at weeks 13,17-20 and21.A 3.6-F diameter 40 MHz mini-probe catheter was used for eUBM *** ultrasound mini-probe catheter was inserted into the accessory channel of a pediatric flexible bronchofiberscope,allowing simultaneous acquisition of colonoscopic and eUBM *** image acquisition,the mice were anesthetized with isoflurane and kept in a supine position over a stainless steel heated surgical waterbed at 37℃.Both eUBM and colonoscopic images were captured and stored when a lesion was detected by colonoscopy or when the eUBM image revealed a modified colon wall *** the procedure,the colon was irrigated with water that was injected through a flush port on the mini-probe catheter and that acted as the ultrasound coupling medium between the transducer and the colon *** the acquisition of the last eUBM/colonoscopy section for each animal was completed,the colons were fixed,paraffin-embedded,and stained with hematoxylin and *** images acquired at the first time-point for each mouse were compared with subsequent eUBM/colonoscopic images of the same sites obtained in the following acquisitions to evaluate lesion ***:All 10 mice had eUBM and colonoscopic images acquired at week 13(the first time-point).Two animals died immediately after the first imaging acquisition and,consequently,only 8 mice were subjected to the second eUBM/colonoscopy imaging acquisition(at the second time-point).Due to the advanced stage of colonic tumorigenesis,5 animals died after the second time-point image acquisition,and t

Bio-mathematical models of viral dynamics to tailor antiviral therapy in chronic viral hepatitis

The simulation of the dynamics of viral infections by mathematical equations has been applied successfully to the study of viral infections during antiviral therapy. Standard models applied to viral hepatitis describe the viral load decline in the f irst 2-4 wk of antiviral therapy, but do not adequately simulate the dynamics of viral infection for the following period. The hypothesis of a constant clearance rate of the infected cells provides an unrealistic estimation of the time necessary to reach the control or the clearance of hepatitis B virus (HBV)/ hepatitis c virus (HcV) infection. To overcome the problem, we have developed a new multiphasic model in which the immune system activity is modulated by a negative feedback caused by the infected cells reduction, and alanine aminotransferase kinetics serve as a surrogate marker of infected-cell clearance. By this approach, we can compute the dynamics of infected cells during the whole treatment course, and find a good correlation between the number of infected cells at the end of therapy and the long-term virological response in patients with chronic hepatitis c. The new model successfully describes the HBV infection dynamics far beyond the third month of antiviral therapy under the assumption that the sum of infected and non-infected cells remains roughly constant during therapy, and both target and infected cells concur in the hepatocyte turnover. In clinical practice, these new models will allow the development of simulators of treatment response that will be used as an "automatic pilot" for tailoring antiviral therapy in chronic hepatitis B as well as chronic hepatitis c patients.

Surveillance for hepatocellular carcinoma in chronic viral hepatitis:Is it time to personalize it?

Surveillance with abdominal ultrasound with or without alpha-fetoprotein is recommended by clinical practice guidelines for patients who are considered to be at risk of developing hepatocellular carcinoma(Hcc),including those with cirrhosis,advanced fibrosis and special subgroups of chronic hepatitis B(cHB).Application of the standard surveillance strategy to all patients with chronic liver disease(cLD)with or without cirrhosis imposes major sustainability and economic burdens on healthcare ***,a number of Hcc risk scores were constructed,mainly from Asian cohorts,to stratify the Hcc prediction in patients with ***,even if less than for cHB,a few scoring systems were developed for chronic hepatitis c patients or cirrhotic patients with cLD of different ***,a few newsworthy Hcc-risk algorithms were developed for patients with cirrhosis using the combination of serologic Hcc markers and clinical ***,the Hcc risk stratification appears at hand by several validated multiple score systems,but their optimal performance is obtained only in populations who show highly homogenous clinic-pathologic,epidemiologic,etiologic and therapeutic characteristics and this limitation poses a major drawback to their sustainable use in clinical practice.A better understanding of the dynamic process driving the progression from cLD to Hcc derived from studies based on molecular approaches and genetics,epigenetics and liquid biopsy will enable the identification of new biomarkers to define the individual risk of Hcc in the near future,with the possibility to achieve a real and cost/effective personalization of surveillance.

Dectin- 1 isoforms contribute to distinct Th 1/Th 17 cell activation in mucosal candidiasis

The recognition of p-glucans by dectin-1 has been shown to mediate cell activation, cytokine production and a variety of antifungal responses. Here, we report that the functional activity of dectin-1 in mucosal immunity to candida albicans is influenced by the genetic background of the host. Dectin-1 was required for the proper control of gastrointestinal and vaginal candidiasis in c57BIJ6, but not BALB/c mice; in fact, the latter showed increased resistance in the absence of dectin-1. The susceptibility of dectin-l-deficient c57BL/6 mice to infection was associated with defects in IL-17A and aryl hydrocarbon receptor-dependent IL-22 production and in adaptive Th I responses. In contrast, the resistance of dectin- 1-deficient BALB/c mice was associated with increased IL-17A and IL-22 production and the skewing towards Thl/Treg immune responses that provide immunological memory. Disparate canonical/ noncanonical NF-KB signaling pathways downstream of dectin-1 were activated in the two different mouse strains. Thus, the net activity of dectin-1 in antifungal mucosal immunity is dependent on the host＇s genetic background, which affects both the innate cytokine production and the adaptive Th1/Th17 cell activation upon dectin-1 signaling.

Medical treatment for gastro-entero-pancreatic neuroendocrine tumours

Gastro-entero-pancreatic neuroendocrine neoplasms(GEPNENs) represents a various family of rare tumours. Surgery is the first choice in GEP-NENs patients with localized disease whilst in the metastatic setting many other treatment options are available. Somatostatin analogues are indicated for symptoms control in functioning tumours. Furthermore they may be effective to inhibit tumour progression. GEP-NENs pathogenesis has been extensively studied in the last years therefore several driver mutations pathway genes have been identified as crucial factors in their tumourigenesis. GEP-NENs can over-express vascular endothelial growth factor(VEGF), basic-fibroblastic growth factor, transforming growth factor(TGF-α and-β), platelet derived growth factor(PDGF), insulin-like growth factor-1(IGF-1) and their receptors PDGF receptor, IGF-1 receptor, epidermal growth factor receptor, VEGF receptor, and c-kit(stem cell factor receptor) that can be considered as potential targets. The availability of new targeted agents, such as everolimus and sunitinib that are effective in advanced and metastatic pancreatic neuroendocrine tumours, has provided new treatment opportunities. Many trials combing new drugs are ongoing.

AIM To uncover novel genetic markers that could contribute to predicting hepatocellular carcinoma(Hcc)susceptibility in caucasians. METHODS The present retrospective case-control study compared genotype frequencies between a cohort of Hcc cases and two,independent,Hcc-free,age/sex-matched control *** Hcc cohort comprised 192 homogeneous patients that had undergone orthotopic liver *** first control group comprised167 patients that were matched to the Hcc cohort for the percentage of hepatitis B(HBV)and/or hepatitis c(HcV)infections.A second control group included192 virus-free,healthy individuals that were used to evaluate the generalizability of the identified predictive *** cases and controls were *** three study populations were characterized with a panel of 31 markers derived from 21 genes that encoded key proteins involved in hepatocarcinogenesis-related *** study end-point was to assess the association between genetic variants and Hcc onset. Although some authors suggest that there is mitotic division in the heart,most cardiomyocytes do not have the capacity to regenerate after myocardial infarction and when this occurs there is a deterioration of contractile function,and if the area of infarction is extensive ventricular remodeling may occur,leading to the development of heart *** transplantation into the myocardium with the goal of recovery of cardiac function has been extensively studied in recent years. The effects of cell therapy are based directly on the cell type used and the type of cardiac *** myocardial ischemia in the hibernating myocardium, bone marrow cells have functional benefits,however these results in transmural fibrosis are not evident. In these cases there is a benefit of implantation with skeletal myoblasts,for treating the underlying cause of disease,the loss of cell contractility.

Late Regularization of the Alveolar Ridge in a Patient with a Prosthesis and Special Needs: A case Report

Residual ridge shape is important for the seating of a prosthesis. The objective of this case report was to illustrate the importance of reduction maneuvers with regard to alveolar bone plates after multiple tooth extractions in patients with prostheses and special needs. In the current patient, there were difficulties during the adaptation and fitting of the upper denture. Bilateral bone resection was indicated in the molar buccal region. Late regularization of the alveolar ridge, which was accomplished in hospital, was necessary for the preparation, adaptation, and use of the denture. The general and systemic health care of the patient should be monitored throughout surgical intervention. Though tooth extractions are common procedures, particular attention is required to ensure adequate alveolar ridge remodeling that is conducive to oral rehabilitation.

comparison of clonidine and Metoprolol Effectiveness on the control of Post-Operative Blood Pressure in Otorhinolaryngology-Head and Neck Surgeries: A clinical Trial

Objectives: Adequate control of hemodynamic parameters in patients undergoing otolaryngology-head and neck (OLHN) surgeries is one of the objectives of peri-operative anesthesia management. In this context, we seek to evaluate the effectiveness of metoprolol compared to clonidine in the post-operative control of systolic (SBP) and diastolic (DBP) blood pressures, and heart rate (HR), when these medications are used in the immediate pre-operative period. Method: this is a randomized double-blind clinical trial. Patients over 18 years old, according to the American Society of Anesthesiologists Physical Status Classification (ASA) I or II, who would undergo OLHN surgery, presenting with greater than 140 mmHg (SBP) or 90 mmHg (DBP), upon arrival at the surgical ward, were included in the study. 46 patients were randomized into two groups (C or M) who received clonidine (75 or 150 mcg) or metoprolol (5 or 10 mg), respectively. The averages of SBP, DBP and HR were compared between groups upon arrival at the post anesthesia care unit (PACU) and four hours post-surgery. Results: In group C, SBP (127.7 ± 18.8 vs 137.3 ± 14.1 mmHg, p = 0.03) and DBP (73.1 ± 15.46 vs 82.6 ± 7.9 mmHg, p 0.01) were lower, when compared with group M. No difference was observed in HR immediately upon PACU arrival. In addition, for four hours post-surgery, no change was noted in the SBP, DBP or HR. Conclusion: metoprolol was less effective than clonidine in reducing systolic and diastolic blood pressures in the immediate post-operative period of OLHN surgeries.
