Pyogenic liver abscess:An audit of 10 years’experience

AIM:To describe our own experience with pyogenic liver abscesses over the past 10 years and investigate the risk factors associated with failure of initial percutaneous ***:A retrospective study of records of 63 PLA patients presenting between 1998 and 2008 to Australian tertiary referral centre,were *** and hydatid abscesses were ***,clinical,radiological,and microbiological characteristics,as well as surgical/radiological interventions,were ***:Sixty-three patients(42 males,21 females) aged 65(±14) years[mean±(SD) ]had prodromal symptoms for a median(interquartile range;IQR) of 7(5-14) *** 59%of patients were febrile at presentation;however,the serum c-reactive protein was elevated in all 47 in whom it was *** function tests were non-specifically abnormal.67%of patients had a solitary abscess,while 32%had>3 abscesses with a median(IQR) diameter of 6.3(4-9) *** organisms were:Streptococcus milleri 25%,Klebsiella pneumoniae 21%,and Escherichia coli 16%.A presumptive cryptogenic cause was most common (34%).Four patients died in this series:one from sepsis,two from advanced cancer,and one from acute myocardial *** initial procedure was radiological aspiration±drainage in 54 and surgery in two patients.17%underwent surgical management during their *** hypoalbuminaemia[mean (95%cI) :32(29-35) g/L vs 28(25-31) g/L,P=0.045] on presentation was found to be the only factor related to failure of initial percutaneous therapy on univariate ***:PLA is a diagnostic challenge,because the presentation of this condition is *** antibiotics and radiological drainage in the first instance allows resolution of most PLAs;However,a small proportion of patients still require surgical drainage.

Background:During human locomotion,a sufficiently stiff foot allows the ankle plantar flexors to generate large propulsive *** foot stiffness(e.g.,via a carbon plate)increases the ankle’s external moment arm in relation to the internal moment arm(i.e.,increasing gear ratio),reduces plantar flexor muscles’shortening velocity,and enhances muscle force *** contrast,when activation of the foot’s intrinsic muscles is impaired,there is a reduction in foot and ankle work and metatarsophalangeal joint *** speculated that the reduced capacity to actively control metatarsophalangeal joint stiffness may impair the gearing function of the foot at the ***:We used a tibial nerve block to examine the direct effects of the intrinsic foot muscles on ankle joint kinetics,in vivo medial gastrocnemius’musculotendinous dynamics,and ankle gear ratio on 14 participants during maximal vertical ***:Under the nerve block,the internal ankle plantar flexion moment decreased(p=0.004)alongside a reduction in external moment arm length(p=0.021)and ankle joint gear ratio(p=0.049)when compared to the non-blocked *** medial gastrocnemius muscle-tendon unit and fascicle velocity were not different between conditions,the Achilles tendon was shorter during propulsion in the nerve block condition(pconclusion:In addition to their known role of regulating the energetic function of the foot,our data indicate that the intrinsic foot muscles also act to optimize ankle joint torque production and leverage during the propulsion phase of vertical jumping.

Seeing through a new lens: exploring the potential of city walking tour videos for urban analytics

city Walking Tour Videos(cWTVs)are a novel source of Volunteered Geographic Information providing street-level imagery through video sharing platforms such as *** demonstrate that these videos contain rich information for urban analytical applications,by conducting a mobility *** detect transport modes with a focus on active(pedestrians and cyclists)and motorised mobility(cars,motorcyclists and trucks).We chose the city of Paris as our area of interest given the rapid expansion of the bicycle network as a response to the covid-19 pandemic and compiled a video corpus encompassing more than 66 hours of video *** the detection of street names in the video and placename containing timestamps we extracted and georeferenced 1169 locations at which we summarise the detected transport *** results show high potential of cWTVs for studying urban mobility *** detected significant shifts in the mobility mix before and during the pandemic as well as weather effects on the volumes of pedestrians and *** with the observed increase in data availability over the years we suggest that cWTVs have considerable potential for other applications in the field of urban analytics.

This paper reviews the historical evolution of the language and organization surrounding the health of personnel in the British Army from‘hygiene’through to‘wellbeing’.It starts by considering the health of the army in the midnineteenth century and the emergence of military hygiene as a professional *** continues by looking at advances in military hygiene in the two world *** was replaced by the term‘health’in the 1950 s as the collective noun used by professionals working in this *** unity split when the professions of occupational medicine and public health established separate faculties and training ***,the health issues for the armed forces remain fundamentally *** forward,the term‘wellbeing’is helping to refresh the close relationships between executives,their medical advisers and those within the population of health professions charged with keeping the British Army *** core theme is the collaborations between civil society,executive leadership and medical services in maximizing the health of the military population from recruitment through to life as a veteran.

Incidence of brain metastasis in patients with esophageal carcinoma

AIM:To determine the incidence of brain metastasis in a contemporary group of patients with carcinoma of the ***:Retrospective analysis of 53 patients with esophageal carcinoma who received radiotherapy as a component of treatment between 1998 and 2007,including patient and tumor characteristics,and subsequent diagnosis of brain *** association between the histological type of esophageal cancer and the incidence of brain metastasis was assessed using Fisher's exact ***:Forty-four of the fifty-three patients in this study had adenocarcinoma and nine had squamous cell carcinoma,ranging from stage ⅡA-Ⅳ*** treatment was surgery with neoadjuvant chemoradiotherapy (trimodality therapy) in 19% of patients;chemoradiotherapy in 42%;and surgery and adjuvant radiotherapy in 7%.Twenty-five percent of patients inthis study received palliative *** overall incidence of brain metastasis in this cohort was 13%.Adenocarcinoma was the primary tumor histology in all of the patients who developed brain metastasis,representing an incidence of 16% in this *** patients with squamous cell carcinoma received trimodality *** association between histology and brain metastasis was not statistically ***:The incidence of brain metastasis in this contemporary cohort of patients with esophageal carcinoma is higher than previously reported and was confined to those with adenocarcinoma.

Assessing the Impact of catastrophic Electricity Loss on the Food Supply chain

The food supply chain's heavy reliance on electricity poses significant vulnerabilities in the event of prolonged and widespread power disruptions. This study introduces a system-dynamics model that integrates five critical infrastructures-electric grid, liquid fossil fuels, Internet, transportation, and human workforce-to evaluate the resilience of food supply chains to major power outages. We validated the model using the 2019 Venezuelan blackouts as a case study, demonstrating its predictive validity. We then explored how more extreme electricity losses would disrupt the supply chain. More specifically, we modeled the impact of a large-scale cyberattack on the US electric grid and a high-altitude electromagnetic pulse(HEMP)event. A cyberattack severely damaging the US electric grid and allowing for recovery within a few weeks or months would lead to substantial drops in food consumption. However, it would likely still be possible to provide adequate calories to everyone, assuming that food is equitably distributed. In contrast, a year-long recovery from a HEMP event affecting most of the continental United States could precipitate a state of famine. Our analysis represents a first attempt at quantifying how food availability progressively worsens as power outages extend over time. Our open-source model is made publicly available, and we encourage its application to other catastrophic scenarios beyond those specifically considered in this work(for example, extreme solar storms, high-lethality pandemics).

convolutional neural network to learn building-shape representations for early-stage energy design

Machine learning(ML)supports energy-efficient design by quickly predicting the building performance at early ***,existing ML approaches cannot easily learn geometrical representations and their interactions with technical specifications for predicting *** approaches of artificial neural networks(ANN)use either numerical parameters to represent building geometry or component-based ML(cBML)to train trans-ferrable ML ***,both approaches have high generalisation errors for complex *** used a convolutional neural network(cNN)to learn geometrical representations of buildings and an autoencoder to optimise the training *** test existing approaches of ANN and cBML with a proposed approach of cNN for different sizes of training *** the biggest training dataset,ANN,cBML,and cNN have generalisation errors(root-mean-squared errors)of 1.35,1.60,and 0.77 kWh/a.m 2,*** cBML has lower prediction errors on small datasets,cNN is more suitable for large ***,cNN most often has better prediction accuracy than ANN and trains faster on encoded *** cNN captures the effects of building geometry on energy predictions better than ANN and cBML approaches,it is an attractive choice for early-stage energy design.

Use of non-timber forest products from invasive alien Prosopis species(mesquite) and native trees in South Africa: implications for management

Background: Prosopis species have been introduced to many areas outside their native range to provide benefits to local communities. Several Prosopis species and their hybrids(hereafter "mesquite") have, however, become naturalised and invasive and now generate substantial costs. Management options are limited because of the complex conflicts of interest regarding benefits and costs. Management policies and strategies must take account of such conflicts, but further insights are needed on the dimensions of uses and impacts before such information can be usefully applied. current policy in South Africa allows for the growth and use of mesquite in one province,but not in others where its control is mandatory. We report on a study to quantify the direct use and perceptions of non-timber forest products(NTFPs) from mesquite and native trees in South ***: Semi-structures household interviews were conducted with various stakeholder groups to identify what tree products are used, to ascertain amounts used as well as to gauge perceptions of natural resource use between different tree species and use over ***: The direct household use value of native trees was higher than that of mesquite, and local stakeholders attached greater value to products from native trees than from mesquite. Therefore, native trees are and will still be preferentially harvested, and mesquite is unlikely to offer protection to native species by providing an alternative source of products. Mesquite pods do, however, provide valuable additional resources(fodder and medicinal products). The use of both native trees and mesquite is decreasing as the incomes of poorer households rise and as alternative energy sources become available.
