Advances in teleophthalmology and artificial intelligence for diabetic retinopathy screening:a narrative review

Background and Objective:Advances in teleophthalmology and artificial intelligence(AI)for diabetic retinal screening is of growing public health ***,only 30–40%of patients with diabetes adhere to recommended diabetes screening *** enhance early detection and reduce vision threatening complications,there has been a growing number of teleophthalmology programs and novel AI algorithms with the aim to improve eye care *** purpose of this review is to assess current literature on teleophthalmology and AI for use in diabetic retinopathy(DR)screening,and to discuss advances and barriers to these innovative ***:Literature review involving teleophthalmology and AI for DR screening,with focus on the past *** Content and Findings:Teleophthalmology has demonstrated the ability to increase DR screening rates,enable earlier eye care.access,and reduce healthcare *** AI-based DR screening programs appear accurate and effective,but detection of other ocular pathologies is still under development and not yet approved in the United ***,technological,financial,and legal barriers limit widespread adoption and long-term sustainability of teleophthalmology ***:The use of teleophthalmology and AI algorithms expands eye care.access and helps prevent vision loss from DR and potentially other sight threatening *** in the process utilized for arriving at a particular diagnosis or decision to refer,often referred to as the“black box”,remains a multifaceted issue within the field of telemedicine for developing trust and improving patient-centered outcomes.

Lactobacillus are considered promising therapeutic methods for nonalcoholic fatty liver disease(NAFLD).The effects of two strains of Ltmosilactobacillus mucosae on NAFLD were investigated in this ***-old male C57BL/6J mice were divided into 4 groups(n=8 per group,Control,Model,FZJTZ26M3,FGSYC17L3).*** FZJTZ26M3 reduced the mice 's body weight,liver weight,and adipose tissue weight after 12 weeks of *** to serum analysis,total cholesterol,triacylglycerol,and low-density lipoprotein cholesterol significantly decreased after *** FZJTZ26M3 *** pathology showed that *** FZJTZ26M3 was effective to ameliorate lipid deposition in NAFLD ***,the expression of the gene related to lipid metabolism in the liver and adipose tissue was analyzed,and the results indicated that *** FZJTZ26M3 could alleviate NAFLD by regulating lipid ***,the results of 16S rRNA gene sequencing revealed a drop in the relative abundance of Ruminococcaceae,which is linked to inflammation,but the relative abundance of a potential probiotic Akkermansia significantly increased after *** FZJTZ26M3 ***,*** FZJTZ26M3 could be a candidate to prevent NAFLD.

We demonstrate a novel,composite laser written 3D waveguide,fabricated in boro-aluminosilicate glass,with a refractive index contrast of 1.12×10−*** waveguide is fabricated using a multi-pass approach which leverages the respective refractive index modification mechanisms of both the thermal and athermal inscription *** present the study and optimisation of inscription parameters for maximising positive refractive index change and ultimately demonstrate a dramatic advancement on the state of the art of bend losses in laser-written *** 1.0 dB cm−1 bend loss cut-off radius is reduced from 10 mm to 4 mm,at a propagation wavelength of 1550 nm.

In sub-Saharan Africa(SSA),63%of new human immunodeficiency virus(HIV)infections in 2021 were among women,particularly adolescent girls,and young *** is a high incidence of HIV among pregnant and lactating women(PLW)in *** is estimated that the risk of HIV-acquisition during pregnancy and the postpartum period more than *** this article,we discuss the safety and effectiveness of drugs used for oral HIV pre-exposure prophylaxis(PrEP),considerations for initiating PrEP in PLW,the barriers to initiating and adhering to PrEP among them and suggest recommendations to address these ***/emtricitabine,the most widely used combination in SSA,is safe,clinically effective,and cost-effective among *** PLW who requests PrEP and has no medical contraindications should receive *** users who are pregnant or lactating may experience barriers to starting and adhering for a variety of reasons,including personal,pill-related,and healthcare facility-related *** address the barriers,we recommend an increased provision of information on PrEP to the women and the communities,increasing and/or facilitating access to PrEP among the PLW,and developing strategies to increase adherence.

Background: Vaginal discharge syndrome is a common condition across the world with the main causes being infectious. Rare causes include obstructed hemivagina with ipsilateral renal agenesis (OHVIRA syndrome). Case Presentation: This case involves an 18-year-old woman with a 9-year history of vaginal discharge treated unsuccessfully by multiple physicians and different health facilities. After she presented to our hospital gynecology clinic with the same complaint she investigated with blood, urine, discharge wet-mount, pelvic ultrasound, and abdominopelvic computed tomography at different times. Preoperatively she was suspected to have OHVIRA syndrome which was successfully surgically treated with a full return of function. Conclusion: This case emphasizes the need to consider non-infectious causes commonly congenital mullerian anomaly in young women presented with long-time vaginal discharge symptoms soon after menarche and which are nonresponding to different multiple medical treatments. In peripheral health institutions like ours, it is best to consider computed tomography if available but better not to forget at least abdominopelvic ultrasound to look for congenital Mullerian anomalies.

Background:Return to play(RTP)in elite rugby is managed using a 6-stage graduated RTP protocol,which can result in clearance to play within 1 week of *** aimed to explore how symptom,cognitive,and balance presentation and evolution during concussion screens 2 h(head injury assessment(HIA2)and 48 h(HIA3)after injury were associated with time to RTP)to identify whether a more conservative graduated RTP may be ***:A retrospective cohort study was conducted in 380 concussed rugby players from elite men’s rugby over 3 consecutive *** were classified as shorter or longer returns,depending on whether RTP occurred within 7 days(allowing them to be considered to play the match 1 week after injury)or longer than 8 days,***,cognitive,and balance performance during screens was assessed relative to baseline(normal or abnormal)and to the preceding screen(improving or worsening).Associations between sub-test abnormalities and RTP time were explored using odds ratios(OR,longer ***).Median day absence was compared between players with abnormal or worsening results and those whose results were normal or ***:Abnormal symptom results during screens 2 h and 48 h after concussion were associated with longer return time(HIA2:OR=2.21,95%confidence interval(95%CI):1.39-3.50;HIA3:OR=3.30,95%CI:1.89-5.75).Worsening symptom number or severity from the time of injury to 2 h and 48 h post-injury was associated with longer return(HIA2:OR=2.49,95%CI:1.36-4.58;HIA3:OR=3.34,95%CI:1.10-10.15).Median days absence was greater in players with abnormal symptom results at both HIA2 and *** and balance performance were not associated with longer return and did not affect median days ***:Symptom presentation and evolution within 48 h of concussion were associated with longer RTP *** may guide a more conservative approach to RTP,while still adhering to individualized concussion management principles.

Vitamin A and its biologically active derivative,retinoic acid(RA),are important for many immune ***,in particular,is essential for the development of immune cells,including neutrophils,which serve as a front-line defense against *** vitamin A deficiency has been linked to higher susceptibility to infections,the precise role of vitamin A/RA in host-pathogen interactions remains poorly ***,we provided evidence that RA boosts neutrophil killing of methicillin-resistant Staphylococcus aureus(MRSA).RA treatment stimulated primary human neutrophils to produce reactive oxygen species,neutrophil extracellular traps and the antimicrobial peptide cathelicidin(LL-37).Because RA treatment was insufficient to reduce MRSA burden in an in vivo murine model of skin infection,we expanded our analysis to other infectious *** did not affect the growth of a number of common bacterial pathogens,including MRSA,Escherichia coli K1 and Pseudomonas aeruginosa;however,RA directly inhibited the growth of group A Streptococcus(GAS).This antimicrobial effect,likely in combination with RA-mediated neutrophil boosting,resulted in substantial GAS killing in neutrophil killing assays conducted in the presence of ***,in a murine model of GAS skin infection,topical RA treatment showed therapeutic potential by reducing both skin lesion size and bacterial *** findings suggest that RA may hold promise as a therapeutic agent against GAS and perhaps other clinically significant human pathogens.
