AIM: To investigate the presence or absence of high amplitude propagating contractions (HAPC), as well as the other measures of colonic motility, in persons with spinal cord injury (SCI). METHODS: Prolonged colonic ambulatory manometric studies were performed on 14 male volunteers: 8 with SCI (mean age, 59 ± 13 years; mean duration of injury, 13 ± 4 years) and 6 healthy able-bodied controls (mean age, 57 ± 10 years). A solid-state manometry catheter was endoscopically clipped to the splenic flexure. Recording was performed for > 24 h after manometric catheter placement. RESULTS: HAPC were absent in individuals with SCI during pre-sleep, sleep, and post-sleep phases. HAPC were significantly increased after awakening in non-SCI controls (0.8 ± 0.2 HAPC/h vs 10.5 ± 2.0 HAPC/h, P < 0.005). The motility index was lower in those with SCI than in controls preand post-sleep (SCI vs non-SCI: Pre-sleep, 2.4 ± 0.4 vs 8.8 ± 1.9, P < 0.01; Post-sleep, 4.3 ± 0.8 vs 16.5 ± 4.5, P < 0.05). However, a sleepinduced depression of colonic motility was observed in both the SCI and non-SCI groups (Pre-sleep vs Sleep, non-SCI: 8.8 ± 1.9 vs 2.1 ± 0.9, P < 0.002; SCI: 2.4 ± 0.4 vs 0.2 ± 0.03, P < 0.001), with the motility index of those with SCI during sleep not significantly different than that of the controls. CONCLUSION: HAPC were not observed in individuals with SCI preor post-sleep. A sleep-induced depression in general colonic motility was evident in SCI and control subjects.

This is a review education paper on the current ophthalmology simulators utilized worldwide for undergraduate and postgraduate training. At present, various simulators such as the EYE Exam Simulator （Kyoto Kagaku Co. Ltd., Kyoto, Japan）, Eyesi direct ophthalmoscope simulator （VRmagic, GmbH, Mannheim, Germany）, Eyesi indirect ophthalmoscope simulator （VRmagic, GmbH, Mannheim, Germany） and Eyesi cataract simulators （VRmagic, GmbH, Mannheim, Germany）. These simulators are thought to be able to reduce the initial learning curve for the ophthalmology training but further research will need to be conducted to assess the effectiveness of the simulation-assisted ophthalmology training. Future research will be of great value to assess the medical students and residents responses and performance regarding the usefulness of the individual eye simulator.

Global temperature is predicted to increase in the end of the century and one of the primary consequences of this warming is the sea level rise. Considering the vulnerabilities on coastal systems and water resources, it is important to evaluate the potential effects of this rising in coastal areas, since the saline intrusion on rivers would be intensified, leading to problems related to water quality. In this context, the present work aimed to verify saline intrusion changes along an important river, São Francisco Canal, located in Rio de Janeiro State, Brazil. For this purpose, a hydrodynamic modeling was performed using SisBaHiA, considering different sea levels and tide conditions. According to the results, it was verified the intensification on saline intrusion and higher salinity values due to a sea level rise of 0.5 m. These results show that new licenses for water withdrawals must be carefully analyzed as the fluvial flow plays an important role to contain the saltwater intrusion on the studied river. Accordingly, it is recommended the evaluation of climate change effects in order to choose best strategies to reduce coastal vulnerability, and the use of this theme on environmental licensing and territorial planning, integrating water planning with coastal management.

BACKGROUND In-hospital cardiac arrest(IHCA) portends a poor prognosis and survival to discharge rate. Prognostic markers such as interleukin-6, S-100 protein and high sensitivity C reactive protein have been studied as predictors of adverse outcomes after return of spontaneous circulation(ROSC); however; these variables are not routine laboratory tests and incur additional cost making them difficult to incorporate and less attractive in assessing patient's prognosis. The neutrophil-lymphocyte ratio(NLR) is a marker of adverse prognosis for many cardiovascular conditions and certain types of cancers and sepsis. We hypothesize that an elevated NLR is associated with poor outcomes including mortality at discharge in patients with *** To determine the prognostic significance of NLR in patients suffering IHCA who achieve *** A retrospective study was performed on all patients who had IHCA with the advanced cardiac life support protocol administered in a large urban community United States hospital over a one-year period.
