Etiologies of tropical acute febrile illness in West Pahang, Malaysia: A prospective observational study

Objective: To determine the etiologies of tropical acute febrile illness(TAFI) in West Pahang, Malaysia and to investigate morbidity and mortality factors in relation to TAFI. Methods: A multicenter prospective cohort study was conducted between January and June 2016 in six district hospitals throughout the western part of Pahang State in Peninsular Malaysia. A total of 336 patients answered a standardized questionnaire and blood samples were collected for laboratory confirmation of infectious etiology. Descriptive analysis and logistic regression were performed to identify factors associated with TAFI. Results: A total of 336 patients were included. The patients were mainly Malays(70.2%), males(61.3%), aged(44.6±17.4) years, with more than half(58.9%) presenting with gastrointestinal symptoms. The majority were diagnosed with dengue(35.7%) while malaria(4.5%) was the least frequent. The in-hospital mortality due to TAFI was 9.2%. Patients with meliodosis had five times higher mortality [Adjusted OR: 5.002, 95% CI:(1.233, 20.286)]. Patients with comorbidities such as cardiovascular symptoms(P<0.001) and renal replacement therapy initiation(P<0.001) were significantly associated with in-hospital mortality in all TAFI. Conclusions: The etiology of TAFI in the western Pahang includes dengue, leptospirosis, malaria and melioidosis, which carry the highest risk of in-hospital mortality. The presence of cardiovascular symptoms may be used to assess the disease severity in TAFI, but more studies are needed in the future.

Plant-based milks are on the rise due to an increased awareness of their sustainability and health benefits. Currently， dairy milk is the most nutritionally complete beverage， but it suffers from the presence of indigestible lactose and allergenic proteins. Coconut milk has been around for a long time， but its application is limited due to a perceived lack of specific nutrients， high saturated fat levels， and low acceptability. Recent evidence indicates， however， that the saturated fat and other plant-based components found in coconut milk are good for metabolic outcomes and brain health. The conversion of coconut milk to yoghurt will further improve its functionality by boosting its existing nutritional qualities. In this article， the nutritional value of coconut milk， as well as its potential downsides， its application as yoghurt， and suggestions for enhancing its nutritional functionality will be examined.

In chemical enhanced oil recovery(CEOR),it is very important to utilize the excessive usage of chemicals.A great opportunity lies in adopting natural surfactants,since it is cheaper,ecosystem friendly,and less toxic than their counterpart synthetic *** the availability of natural surfactant sources,it is yet very early to decide on their ***,this research focuses on natural-saponin extracted from different raw materials available in the Middle East and their interaction with quartz-sand.A special focus was given to the adsorption isotherm models to describe the interaction with the reservoir *** raw materials were investigated are fenugreek,sugar beet leaves and *** main extraction method employed was the chemical extraction using the *** study used Uv–vis spectrometer to investigate the micellization behaviour and the consequent adsorption on *** presence of triterpenoid saponin is found dominant in all the sample,the intensity and purity differed according to the raw material *** critical micelle concentration(CMC)was at a close range of 4–5.5 wt%in all the *** highest adsorption was obtained by sugar beet leaves which is 192 g/*** is 25%and 37%higher than the Fenugreek and chickpeas,*** the salinity resulted in adsorption reduction between 2%and 56%.For the adsorption isotherms,it showed good agreement with the Langmuir model *** remarkable finding is that the sugar beet leaves heterogeneous model seems to be valid by Frendluich and Halsey isotherms.

The steady magnetohydrodynamic （MHD） mixed convection flow towards a vertical permeable surface with prescribed heat flux is investigated. The governing partial differential equations are transformed into a system of ordinary differential equations, which is then solved numerically by a finite-difference method. The features of the flow and heat transfer characteristics for different values of the governing parameters are analysed and discussed. Both assisting and opposing flows are considered. It is found that dual solutions exist for the assisting flow, besides the solutions usually reported in the literature for the opposing flow.

A viscous boundary layer flow of an electrically-conducting fluid over a moving flat plate in a parallel stream with a constant magnetic field applied outside the boundary layer parallel to the plate was investigated. The goveming system of partial differential equations was transformed to ordinary differential equations using a similarity transformation. The similarity equations were then solved numerically using a finite-difference scheme known as the Keller-box method. Numerical results of the skin friction coefficient, velocity profiles, and the induced magnetic field profiles were obtained for some values of the moving parameter, magnetic parameter, and reciprocal magnetic Prandtl number. The results indicate that dual solutions exist when the plate and the fluid move in the opposite directions up to a critical value of the moving parameter, whose value depends on the value of the magnetic parameter.

基于无线通信网的运行机制和实时性、非实时性混合业务对无线通信网络的不同服务质量(QoS)要求,设计了有限缓存下实时性业务优先接入控制策略,并建立了无线通信网络在有限缓存下混合业务带有优先接入控制的多服务台离散时间排队模型Geom_(1)±Geom_(2)/Geom_(1),Geom_(2)/s,s(PP)/n+s,s.精确划分了系统的状态组成,分析了系统性能和状态转移概率,获得了系统的稳态分布、平均队长、损失率、以及信道的平均利用率.最后,对所建模型进行数值模拟,得到各业务到达率、服务率、信道个数、缓冲器容量与各业务损失率的关系.针对网络拥塞控制,本文提出通过增加信道个数、提高优先级业务的服务率能有效地降低两类业务的损失率.
