Complication rates in emergent endoscopy for foreign bodies under different sedation modalities:A large single-center retrospective review

BACKGROUND Foreign object ingestion(FOI)and food bolus impaction(FBI)are common causes of emergent endoscopic *** choice of sedation used is often dictated by physician *** endoscopists frequently prefer to use monitored anesthesia care(MAC)and general anesthesia(GA)as opposed to conscious sedation(CS)due to the concern for inadequate airway ***,there is insufficient data examining the safety of different sedation modalities in emergent endoscopic management of FOI and *** To investigate the complication rates of emergent endoscopic extraction performed under different sedation *** We conducted a retrospective chart review of patients presenting with acute FBI and FOI between 2010 and 2018 in two hospitals.A standardized questionnaire was utilized to collect data on demographics,endoscopic details,sedation practices,hospital stay and adverse *** recognized during and within 24 h of the procedure were considered early,whereas patients presenting with a procedure-related adverse event within two weeks of the index event were considered delayed *** rates of patients who underwent emergent endoscopic retrieval were compared based on sedation types,namely CS,MAC and ***-square analysis and multiple logistic regression were used to compare complication rate based on sedation *** Among the 929 procedures analyzed,353 procedures(38.0%)were performed under CS,278 procedures(29.9%)under MAC and the rest(32.1%)under *** median age of the subjects was 52 years old,with 57.4%being *** majority of the procedures(64.3%)were FBI with the rest being FOI(35.7%).A total of 132 subjects(14.2%)had chronic comorbidities while 29.0%had psychiatric *** most commonly observed early complications were mucosal laceration(3.8%)and bleeding(2.6%).The most common delayed complication was aspiration pneumonia(1.8%).A total of 20 patients(5.6%)could not adequately be sed

Rescue effects of aqueous seed extracts of Foeniculum vulgare and Carum carvi against cadmium-induced hepatic, renal and gonadal damage in female albino rats

Objective: To investigate the protective effects of aqueous seed extracts of fennel"Foeniculum vulgare"(FVE) and caraway "Carum carvi"(CCE) on liver, kidney and reproductive organs in female rats against cadmium chloride(CC) ***: A total of 36 adult female rats were divided into six groups, six in each *** group(fed normal diet), CC-treated group(50 mg CC/kg diet), CCE-treated group(150 mg CCE/kg diet), CCE + CC group, FVE(150 mg/kg diet) and FVE + CC. One month later, all rats were sacrificed and all samples were collected at proestrus ***: The toxic effects of CC were confirmed biochemically by significant increase of serum concentration of liver enzymes(P < 0.05), and creatinine(P < 0.001). Moreover,CC increased significantly the serum level of malondialdehyde(MDA) and decreased the total antioxidant capacity(TAC)(P < 0.001). In addition, serum concentrations of estrogen, progesterone, follicle-stimulating hormone(FSH) and luteinizing hormone(LH)were significantly decreased(P < 0.01). Histopathologically, CC-treated group revealed marked pathological changes in renal, hepatic, ovarian and uterine tissues. All toxic findings observed in liver and kidney with CC treatment were found to be ameliorated markedly after co-treatments with FVE or CCE. Furthermore, co-treatment of FVE with CC improved significantly all studied reproductive parameters(P < 0.01).Conclusions: Both FVE and CCE could be used as efficient treatments for liver and kidney against CC intoxication. Moreover, FVE could be utilized as a potent treatment to protect and improve female fertility from cadmium intoxication.

NANOPOROUS MATERIALS BASED ON SPIROKETAL AND SPIROTHIOKETAL POLYMERS FOR SEPARATING METHANOL-TOLUENE MIXTURE

Organic microporous materials based on spiroketal and spirothioketal polymers were synthesized through 1,3- dioxol-forming polymerization reaction between pentaerythritol or pentaerythritol tetrathiol and different types of cyclohexa- 1,4-dione derivatives. The structure of the prepared polymers was confirmed by NMR spectroscopy and molecular mass measurements. Nitrogen adsorption/desorption isotherms of the prepared polymers show a large amount of nitrogen adsorbed at low relative pressure indicating microporosity. These polymers have Brunauer Emmitt and Teller （BET） surface areas in the range from 492 （m^2g^-1） to 685 （m^2 g^-1）. The prepared polymers were found to be useful for pervaporation separation of methanol-toluene mixture with a separation factor up to 12.5 and fluxes, varying between 6.7×10-3 kg/（m^2 h） and 13.4 × 10^-3 kg/（m^2 h）.

SYNTHESIS, CHARACTERIZATION AND SPECTROSCOPIC INVESTIGATION OF PYRAZINOPORPHYRAZINE NETWORK POLYMER-SUPPORTED METAL (Ⅱ)-BASED CATALYSTS

Chloranil through condensation reaction with vicinal diamine such as diaminomaleonitrile produced heterocyclic monomer, p-benzoquinonebis[2,3-b; 2＇,3＇-b＇]pyrazine-5,6-dinitrile. The tetranitrile monomer was cyclo-tetramerised using lithium/pentanol and acetic acid affording the corresponding tetrap-benzoquinone bis[2,3-b; 2＇,3＇-b＇]pyrazinoporphyrazine）]- based network polymer （2H-Pz）. The tetranitril monomer was cyclo-tetramerised using metal salt and quinoline affording the corresponding porphyrazinato-metal II-based network polymers （M-Pz）, M = Co, Ni or Cu. Elemental analytical results, IR and NMR spectral data of the prepared molecules are consistent with their assigned formulations. Molecular masses and metal contents of the synthesized polymers proved to be of high molecular masses which confirm the efficiency of tetramerization polymerization and complexation reactions. The prepared pyrazinoporphyrazines were used as efficient catalysts for the oxidation of thiophenol and benzylthiol to their disulfides in the presence of air atmosphere. The results of oxidation of thiophenol and benzylthiol show that after 15 min the maximum yield of the corresponding disulfides reached 95%, 91%, respectively.

Is toxoplasmosis a potential risk factor for liver cirrhosis?

Objective:To document Toxoplasma gondii(***) antibody status in patients with liver disease,blood samples were taken from 180 hepatic patients and 180 healthy ***:Toxoplasma IgG antibody was detected using enzyme-linked immunosorbent assay and histopathological assessment of liver biopsy METAVIR score was ***:*** IgG antibodies were found in 32.8%of patients and in 22.2%of controls(P=0.02).Toxoplasma seropositivity was significantly associated with lymphadenopathy,history of blood transfusion and reflex impairment in *** hepatitis C virus(HCV)and chronic HCV-related cirrhosis groups compared to chronic HBV and chronic HBV-related cirrhosis groups expressed significantly higher prevalence of *** seropositivity(odds ratio(OR) =4;95%confidence interval(CI):1.3-12.6;P=0.013,OR=4.8;95%CI:1.5-14.9;P=0.006,respectively).Within the chronic HCV group,*** seropositivity significantly associated disease evolution as regards to METAVIR histopathological system for fibrosis and inflammation(OR=19.4;95%CI:2.3-165.2;P=0.0008,OR=0.29;95%CI:0.1-0.8;P=0.01,respectively).Albumin,international normalized ratio(INR) and platelets count were the laboratory parameters significantly altered in Toxoplasma-positrvc chronic HCV patients(P=0.00 l,0.03,0.04,respectively).Child-Pugh scoring for cirrhosis in chronic HCV group placed the majority of seropositive patient in class C with significant statistical difference compared to Child A reference group(OR= 0.08;95%67:0.01-0.5;P=0.003).Conclusions:Toxoplasma seropositivity was high in patients with cirrhosis and associated higher grades of inflammation and necrosis signifying disease evolution,suggesting that cirrhotic patients may thus form a risk group for toxoplasmosis.

AIM: To explore the approaches exerted by mesenchymal stem cells (MSCs) to improve Parkinson’s disease (PD) ***: MSCs were harvested from bone marrow of femoral bones of male rats, grown and propagated in culture. Twenty four ovariectomized animals were classified into 3 groups: Group (1) was control, Groups (2) and (3) were subcutaneously administered with rotenone for 14 d after one month of ovariectomy for induction of PD. Then, Group (2) was left untreated, while Group (3) was treated with single intravenous dose of bone marrow derived MSCs (BM-MSCs). SRY gene was assessed by PCR in brain tissue of the female rats. Serum transforming growth factor beta-1 (TGF-β1), monocyte chemoattractant protein-1 (MCP-1) and brain derived neurotrophic factor (BDNF) levels were assayed by ELISA. Brain dopamine DA level was assayed fluorometrically, while brain tyrosine hydroxylase (TH) and nestin gene expression were detected by semi-quantitative real time PCR. Brain survivin expression was determined by immunohistochemical procedure. Intravesical instillation of platelet-rich plasma for treatment of interstitial cystitis/bladder pain syndrome:A pilot study

Objective:The aim of this study was to evaluate the feasibility of intravesical instillation of platelet-rich plasma(PRP)to treat interstitial cystitis/bladder pain syndrome(IC/BPS).Materials and methods:Twenty PRP samples were tested in vitro for stability after exposure to urine samples with different pH.A pilot study involving 21 female patients with IC/BPS was conducted,and 6 weekly doses of autologous PRP(50 mL)were *** were followed up at the 2nd,4th,8th,and 12th weeks after terminating *** primary endpoint was the visual analog scale(VAS)for pain,and the secondary endpoints included the IC symptom index,IC problem index of the O'Leary-Sant questionnaire and global response assessment,urine culture,and *** was defined as a reduction in VAS by 30%or more compared with basal level,and adverse events were ***:The mean±SD of VAS was significantly reduced compared with basal level(4.4±2.6 vs.8.8±1.1,respectively,p=0.001).Meanwhile,80%of cases were considered successful,with a 50.1%reduction in the mean score compared with the basal *** mean±SD of IC symptom index and IC problem index significantly improved compared with the basal *** response assessment was markedly,moderately,and slightly improved in 2(10%),10(50%),and 5(25%)patients,respectively,and showed no change in 3(15%).Three patients had positive urine cultures at follow-up,but 1 withdrew after 2 sessions because of a lack of ***:Repeated intravesical instillation of PRP could be considered an effective and safe approach for treating IC/BPS.

The construction industry in the State of Kuwait has recently witnessed a rapid growth as the volume of construction projects increased significantly over the past decade. Because of the progress and growth of the industry and the size of the projects, the volume of disputes between the parties to the various contracts has increased accordingly. As projects become larger and more complex, their management and control become more difficult, especially in the absence of effective standard systems to for managing project progress and determining the delay, if any. Even the simple issues, as well as the large ones, if neglected and unaddressed in a timely manner, can cause huge problems that concerns to all project parties. Irrespective of addressing and managing construction delays legally, the resulting legal disputes always have other negative effects on the contracting parties. If the parties to the contract have an insight that enables them to forecast and identify the issues that may cause a conflict before it occurs, that eliminates a myriad of problems before they occur, and greatly reduces the chances of further conflict between contracting parties. This paper presents a framework for a novel progress reporting system that can help minimize contract disputes in the state of Kuwait.

Intermittent fasting(IF)is an intervention that involves not only dietary modific-ations but also behavioral changes with the main core being a period of fasting alternating with a period of controlled *** duration of fasting differs from one regimen to *** fasting(RF)is a religious fasting for Muslims,it lasts for only one month every one lunar *** this model of fasting,observers abstain from food and water for a period that extends from dawn to *** period of daily fasting is variable(12-18 hours)as Ramadan rotates in all seasons of the ***,longer duration of daily fasting is observed during the *** fact,RF is a peculiar type of *** is a dry IF as no water is allowed during the fasting hours,also there are no calorie restrictions during feeding hours,and the mealtime is exclusively *** three variables of the RF model are believed to have a variable impact on different liver *** was evaluated by different observational and interventional studies among patients with non-alcoholic fatty liver disease and it was associated with improve-ments in anthropometric measures,metabolic profile,and liver biochemistry regardless of the calorie restriction among lean and obese *** situation is rather different for patients with liver *** was associated with adverse events among patients with liver cirrhosis irrespective of the underlying etiology of *** patients developed new ascites,ascites were increased,had higher serum bilirubin levels after Ramadan,and frequently developed hepatic encephalopathy and acute upper gastrointestinal *** complications were higher among patients with Child class B and C cirrhosis,and some fatalities occurred due to *** transplant recipients as a special group of patients,are vulnerable to dehydration,fluctuation in blood immunosuppressive levels,likelihood of deterioration and hence observing RF without special precautions
