Nomograms for predicting pathological response to neoadjuvant treatments in patients with rectal cancer

BACKGROUND In recent decades, neoadjuvant therapy(NT) has been the standardized treatment for locally advanced rectal cancer(LARC). Approximately 8%-35% of patients with LARC who received NT were reported to have achieved a complete pathological response(pCR). If the pathological response(PR) can be accurately predicted, these patients may not need surgery. In addition, no response after NT implies that the tumor is destructive, resistant to both chemotherapy and radiotherapy, and prone to having a high metastatic potential. Therefore,developing accurate models to predict PR has great clinical significance and can help achieve individualized treatment in LARC patients.AIM To establish nomograms for predicting PR to different NT regimens based on pretreatment parameters for patients with LARC.METHODS Rectal cancer patients were identified from the database of The Sixth Affiliated Hospital, Sun Yat-sen University from January 2012 to December 2016. Logistic regression and nomograms were developed to predict the probability of pCR and good downstaging to ypT0-2N0M0(ypTNM 0-I), respectively, based on pretreatment parameters for all LARC patients. Nomograms were also developed for three NT regimens(capecitabine/deGramont-RT, mFOLFOX6, and m FOLFOX6-RT) to predict pCR probability.RESULTS Four hundred and three patients were included in this study; 72(17.9%) had pCR at the final pathology report, and 177(43.9%) achieved good downstaging to ypT0-2N0M0(ypTNM 0-I). The nomogram for predicting pCR probability showed that NT regimens, tumor differentiation, mesorectal fascia(MRF) status,and tumor length significantly influenced pCR probability. When predicting the probability of good downstaging, tumor differentiation, MRF status, and clinical T stage were the significant factors. Nomograms were developed based on NT regimens. For the capecitabine/de Gramont-RT group, the multivariate analysis showed that the neutrophil-lymphocyte ratio(NLR) was the only significant factor, thus we could not develop a nomogram for this regim

Clinical characteristics and survival outcomes of perianal Paget’s disease:A SEER population-based study

Objective The aim of the study was to analyze the clinical features of patients with perianal Paget’s disease(PPD)and investigate prognosis risk factors.Methods The SEER*Stat software was used to identify 116 PPD patients from 1975 to 2015 in the SEER research database.The Kaplan-Meier method was used to conduct a univariate analysis for PPD patients.The differences in survival rates were evaluated using the log-rank test.The differences in the clinicopathological features of PPD patients with or without anorectal carcinoma were compared using the chi-square test.Results The median survival time of PPD patients was 44 months.The median age of onset was 73 years old.The 43.10%of the patients were alive at the end of follow up,and only 12.93%of the patients died of PPD.Elderly(age>70 years;χ^2=9.453,P=0.002),poor differentiation(χ^2=46.557,P=0.000)and abdominal perineal resection(APR;χ^2=46.557,P=0.000)were unfavorable risk factors of prognosis.Nearly 50%of PPD had combined with other malignancies,and over 22.41%of those had multiple primary neoplasms(3 or more).PPDs predisposed concurrent malignancy,and 48.21%of PPD patients with other malignancies combined with anorectal carcinoma in the study.Stage(χ^2=10.127,P=0.018),and surgical method(χ^2=12.245,P=0.007)were statistically significant in the PPD patients with or without anorectal carcinoma.The 16.07%of patients had multiple lesions of Paget’s.Conclusion Patients with PPD have a favorable survival,while the disease-specific mortality is low.Diagnosed age,differentiation,and surgical methods were the influence factors of prognosis in PPD patients.PPDs with anorectal carcinoma is of most important in further investigation.

目的盆腔增生性毛根鞘瘤(PTT)经常被误诊为其他疾病,导致多次手术,增加病人痛苦。在此,我们致力于探究PTT的临床病理表现和放射学特征以帮助诊断。方法本研究仅纳入2011年9月至2018年12月间诊断为PTT的患者,回顾了其病变的临床病理特点和磁共振资料。结果4位患者为良性PTT,4位患者为恶性PTT(MPTT)。病变通常潜伏数年,然后突然扩速发展。MPTT患者年龄偏小(P=0.12)。从MRI上看,良性PTT表现为实性囊肿,而MPTT表现为广泛的瘘道。纳入患者广泛局部切除术后,切缘阴性,且无局部复发或转移。从病理学来看,PTT表现为分叶状,无边缘浸润,可伴有毛鞘角化特点,而MPTT存在延伸到周围真皮的明显的非典型鳞状细胞,并伴有软骨基质反应。结论临床医生可根据肿块明显且突然的进行性生长特点考虑为PTT,并应根据病变形态判断病变是否恶变。广泛的局部切除可得到一个良好的效果。

Using satellite-derived land surface temperatures to clarify the spatiotemporal warming trends of the Alborz Mountains in northern Iran

The Alborz Mountains are some of the highest in Iran,and they play an important role in controlling the climate of the country’s northern regions.The land surface temperature(LST)is an important variable that affects the ecosystem of this area.This study investigated the spatiotemporal changes and trends of the nighttime LST in the western region of the Central Alborz Mountains at elevations of 1500-4000 m above sea level.MODIS data were extracted for the period of 2000-2021,and the Mann-Kendall nonparametric test was applied to evaluating the changes in the LST.The results indicated a significant increasing trend for the monthly average LST in May-August along the southern aspect.Both the northern and southern aspects showed decreasing trends for the monthly average LST in October,November,and March and an increasing trend in other months.At all elevations,the average decadal change in the monthly average LST was more severe along the southern aspect(0.60°C)than along the northern aspect(0.37°C).The LST difference between the northern and southern aspects decreased in the cold months but increased in the hot months.At the same elevation,the difference in the lapse rate between the northern and southern aspects was greater in the hot months than in the cold months.With increasing elevation,the lapse rate between the northern and southern aspects disappeared.Climate change was concluded to greatly decrease the difference in LST at different elevations for April-July.

Several popular time-frequency techniques,including the Wigner-Ville distribution,smoothed pseudo-Wigner-Ville distribution,wavelet transform,synchrosqueezing transform,Hilbert-Huang transform,and Gabor-Wigner transform,are investigated to determine how well they can identify damage to structures.In this work,a synchroextracting transform(SET)based on the short-time Fourier transform is proposed for estimating post-earthquake structural damage.The performance of SET for artificially generated signals and actual earthquake signals is examined with existing methods.Amongst other tested techniques,SET improves frequency resolution to a great extent by lowering the influence of smearing along the time-frequency plane.Hence,interpretation and readability with the proposed method are improved,and small changes in the time-varying frequency characteristics of the damaged buildings are easily detected through the SET method.

The metal-organic framework(MOF)derived Ni–Co–C–N composite alloys(NiCCZ)were“embedded”inside the carbon cloth(CC)strands as opposed to the popular idea of growing them upward to realize ultrastable energy storage and conversion application.The NiCCZ was then oxygen functionalized,facilitating the next step of stoichiometric sulfur anion diffusion during hydrothermal sulfurization,generating a flower-like metal hydroxysulfide structure(NiCCZOS)with strong partial implantation inside CC.Thus obtained NiCCZOS shows an excellent capacity when tested as a supercapacitor electrode in a three-electrode configuration.Moreover,when paired with the biomass-derived nitrogen-rich activated carbon,the asymmetric supercapacitor device shows almost 100%capacity retention even after 45,000 charge–discharge cycles with remarkable energy density(59.4 Wh kg^(-1)/263.8μWh cm^(–2))owing to a uniquely designed cathode.Furthermore,the same electrode performed as an excellent bifunctional water-splitting electrocatalyst with an overpotential of 271 mV for oxygen evolution reaction(OER)and 168.4 mV for hydrogen evolution reaction(HER)at 10 mA cm−2 current density along with 30 h of unhinged chronopotentiometric stability performance for both HER and OER.Hence,a unique metal chalcogenide composite electrode/substrate configuration has been proposed as a highly stable electrode material for flexible energy storage and conversion applications.

Borehole deformation based in situ stress estimation using televiewer data

The knowledge of in situ stress is critical in safe and optimised extraction of minerals and energy resources.In situ stresses are either measured directly(e.***.overcoring)or estimated indirectly(e.***.borehole breakouts).Borehole breakout analysis for in situ stress estimation is considered a relatively simple and cost-efficient technique.This technique,however,poses certain limitations such as complexities with progressive formation of breakouts and it requires inputs such as rock failure parameters that are not often available.As a result,significant effort has been made to develop new indirect methods for in situ stress estimation.Borehole deformation analysis using four-arm caliper has been recently proposed for in situ stress estimation and has shown promising results.In this study,we demonstrate a new methodology that analyses the borehole televiewer data with the technique of borehole deformation analysis to estimate the in situ stresses through a field case study.The advantages and challenges of using borehole televiewer data for stress estimation based on borehole deformation analysis are discussed and the best practice to obtain the reliable results is explained.The limitations of using fourarm caliper and borehole deformation analysis for in situ stress estimation are also discussed and it is shown how televiewer data can overcome such limitations.Finally,the in situ stress results obtained from televiewer data and borehole deformation analysis are compared with independent in situ stress measurements to show the robustness and reliability of the proposed methodology.

Visualization-based prediction of dendritic copper growth in electrochemical cells using convolutional long short-term memory

Electrodeposition in electrochemical cells is one of the leading causes of its performance deterioration. The prediction of electrodeposition growth demands a good understanding of the complex physics involved, which can lead to the fabrication of a probabilistic mathematical model. As an alternative, a convolutional Long shortterm memory architecture-based image analysis approach is presented herein. This technique can predict the electrodeposition growth of the electrolytes, without prior detailed knowledge of the system. The captured images of the electrodeposition from the experiments are used to train and test the model. A comparison between the expected output image and predicted image on a pixel level, percentage mean squared error, absolute percentage error, and pattern density of the electrodeposit are investigated to assess the model accuracy. The randomness of the electrodeposition growth is outlined by investigating the fractal dimension and the interfacial length of the electrodeposits. The trained model predictions show a significant promise between all the experimentally obtained relevant parameters with the predicted one. It is expected that this deep learning-based approach for predicting random electrodeposition growth will be of immense help for designing and optimizing the relevant experimental scheme in near future without performing multiple experiments.

Climate Change and Its Impact on Brown Bear Distribution in Iran

Climate change is one of the threats in the recent century,affecting biodiversity directly and indirectly.Modeling the patterns of species distribution is one of useful tools for predicting the impacts of climate change on endangered species.Brown bear(Ursus arctos)plays an important role as a focal species in mountainous ecosystems.This study was aims to investigate the effects of future climate changes on the distribution of this species using an ensemble modeling method in R-software.For this purpose five algorithms including MAXENT,RF,MARS,GAM,GLM and BRT were used to predict the distribution of the species in the present climatic conditions as well as in the 2050s and 2070s.The results showed that temperature and precipitation were two main factors in the distribution of brown bears in Iran.Investigating the distribution of the brown bear in the future showed that suitability of its habitat will decrease in the western and central parts and increase in the northern parts.So a shift toward higher altitude will be expected for brown bear in the future.Therefore,in this condition it is imperative to upgrade the extent of protected areas for better conservation of brown bear.
