Lennert’ s lymphoma (LL) is a T-cell lymphoma characterized by the presence of.atypical T lymphocytes, admixed with histiocytes and epithelioid granulomas. Patients present with superf.cial lymph node involvement, mainly in the cervical areas; thoracic adenopathies and involvement of.deep abdominal lymph nodes are rare. Cutaneous involvement is inf.equent, reported to occur in only 4-11% of.patients, and even rarer is the onset of.cutaneous lesions as f.rst sign of.a recurrence. We report a f.male patient who presented with papules and nodules on the trunk and upper limbs as the f.rst manif.station of.recurrent LL.

Pancreatic metastasis f.om mycosis f.ngoides mimicking primary pancreatic tumor

Mycosis f.ngoides(Mf. is a cutaneous T-cell lymphoma that can undergo local progression with possible systemic dissemination. We report a case of.a patient af.ected by Mf.with a pancreatic mass that was a diagnostic challenge between primitive tumor and pancreatic metastasis f.om Mf. Clinical setting f.ndings and imaging studies raised the suspicion of.a pancreatic primary neoplasm. A diagnostic clue was provided by the combined histomorphologic/immunohistochemical study of.pancreatic and cutaneous biopsies, which revealed a pancreatic localization of.Mf. Considering the rarity of.metastatic localization of.Mf.to the pancreas, we next investigated whether chemokinechemokine receptor interactions could be involved in the phenomenon to provide new insight into the possible mechanisms underlying metastatic localization of.Mf.to the pancreas. Histological analyses of.archival pancreatic tissue demonstrated that glucagon-secreting cells of.the pancreatic islets expressed the CCL27 chemokine, which may have attracted in our case metastatic Mf.cells expressing the complementary receptor CCR10.

Background. Papular mucinosis is a rare dermatological disorder characterized by papules, nodules or plaques resulting f.om mucin deposits in the dermis, and to a certain degree, f.om f.brosis, without thyroid dysf.nction. Case report. A 42-year old man consulted f.r symmetrical papular lesions on the f.ce, neck and shoulders. The lesions had gradually spread over some 20 years. Laboratory f.ndings were normal. Histopathologic examination showed dif.use mucin deposits in the superf.cial and middle layers of.the dermis, thick bands of.collagen, prolif.ration of.f.broblasts and mild perivascular inf.ltration by mononuclear cells. All of.these f.ndings pointed to a diagnosis of.papular mucinosis. Discussion. The specif.c interest of.this case is the predominant involvement of.the f.ce and the spread of.the lesions. According to the updated rongioletti classif.cation, this patient is presenting an atypical and novel f.rm of.papular mucinosis of.a “ mild papular” type with f.cial involvement. The 20-year history without systemic involvement or paraprotein suggests to us that this localized f.rm will not develop into the systemic f.rm or scleromyxoedema.
