Primary mucosa-associated lymphoid tissue lymphoma in the midbrain:A case report

BACKGROUND Primary non-dural central nervous system mucosa-associated lymphoid tissue(MALT)lymphoma is a rare indolent B-cell lymphoma,with only a few reported cases worldwide.CASE SUMMARY A 33-year-old man presented with a 5-mo history of left blepharoptosis and a 4-mo history of right limb numbness and weakness.Magnetic resonance imaging showed a significantly enhanced mass in the left midbrain.Subsequent positron emission tomography revealed that the lesion had increased glucose uptake.A stereotactic robotic biopsy supported a diagnosis of MALT lymphoma.Then he was treated with radiation therapy(30Gy/15F),which resulted in complete remission.We also review the literature on brain parenchymal-based MALT lymphoma,including the clinical presentation,treatment options,and outcomes.CONCLUSION Although there is no consensus on the optimal treatment for this rare disease,patients can respond well when treated with radiotherapy alone.

Annexin A1 Mimetic Peptide AC2-26 Inhibits Sepsis-induced Cardiomyocyte Apoptosis through LXA4/PI3K/AKT Signaling Pathway

The aim of the present study was to explore the effects of annexin A1(ANXA1) mimetic peptide AC2-26 on sepsis-induced cardiomyocyte apoptosis in vivo and in vitro and the underlying mechanisms.In the in vivo study,a rat septic model was established by the cecal ligation and puncture (CLP).The rats were divided into control group,sepsis group and AC2-26 group.The rats in the AC2-26 group were intraperitoneally injected with AC2-26(1mg/kg)2h before CLP,and those in the control group and sepsis group were injected with the same volume of normal saline.The myocardial tissue was examined by hematoxylin and eosin (HE)staining and transmission electron microscopy (TEM).Furthermore,myocardial apoptosis was measured by terminal dUTP nick end-labeling (TUNEL)assay.In the in vitro study,H9C2cells were cultured and divided into three groups:control group,in which cells were only given the basic culture medium;LPS group,in which cells were treated with 10μg/mL LPS;AC2-26 group,in which cells were treated with 0.5μmol/L AC2-262h before 10μg/mL LPS was given.The apoptosis of H9C2 cells was detected by flow cytometry.The levels of lipoxin A4 receptor (LXA4),phosphoinositide3-kinase (PI3K)and protein kinase B (PKB or AKT)protein were measured by Western blotting, the activity of NF-KB and the level of TNF-α by ELISA and the activities of caspase-3/8by using the caspase activity kits.The in vivo study showed that the myocardial pathological damage and myocardial ultrastructural damage were significantly alleviated and the myocardial apoptosis significantly decreased in the AC2-26 group as compared with the sepsis group (P<0.05 for all). The in vivo study revealed that the apoptosis of H9C2 cells was profoundly ameliorated in the AC2-26 group relative to the sepsis group (P<0.05).The protein expression levels of LXA4 were significantly up-regulated,and those of PI3K and AKT prominently down-regulated in the AC2-26 group when compared with those in the LPS group (P<0.05 for all).The activity of NF-κB was greatly inhibited and the le

To the Editor:A 66-year-old man complaining abdominal pain and bloating for 1 month was admitted to the Department of General Surgery of Xuanwu Hospital,Beijing,China.The patient had a normal body temperature and did not have symptoms such as nausea,diarrhea,or bone/joint pain.He had no history of allergies,no particular past medical or family history.He had taken no medications that could cause eosinophilia.No parasites were found in the stool.Abdominal computerized tomography(CT)scan showed thickening of the stomach wall and peristaltic stiffness with the swelling of the surrounding lymph nodes,indicating possibility of gastric cancer[Figure 1A].Biopsy from gastroscopy showed a massive eosinophil infiltration of the stomach wall[Figure 1B].Meanwhile,blood test showed white blood cell(WBC)count was elevated to 11.09×10^9/L,eosinophils were elevated to 2.53×10^9/L,hemoglobin 160 g/L,and platelet 269×10^9/L.A bone marrow biopsy showed normal cellularity with increased eosinophils.breakpoint cluster region/c-abl oncogene 1,non-receptor tyrosine kinase and Fip1-like1 and platelet-derived growth factor receptor alpha fusion genes were not detected.He was diagnosed as hypereosinophilic syndrome(HES)and treated with corticosteroids(prednisone 20 mg,oral,twice per day).The patient’s abdominal pain and bloating symptoms gradually reduced,and the total number of eosinophils was back to normal in 2 weeks.The follow-up gastroscopy performed at 3 and 6 months after treatment showed normal stomach wall[Figure 1C and Figure 1D].The corticosteroid was tapered and stopped after 8 months treatment.

By using the multi-taper method(MTM)of singular value decomposition(SVD),this study investigates the interdecadal evolution(10-to 30-year cycle)of precipitation over eastern China from 1951 to 2015 and its relationship with the North Pacific sea surface temperature(SST).Two significant interdecadal signals,one with an 11-year cycle and the other with a 23-year cycle,are identified in both the precipitation and SST fields.Results show that the North Pacific SST forcing modulates the precipitation distribution over China through the effects of the Pacific Decadal Oscillation(PDO)-related anomalous Aleutian low on the western Pacific subtropical high(WPSH)and Mongolia high(MH).During the development stage of the PDO cold phase associated with the 11-year cycle,a weakened WPSH and MH increased the precipitation over the Yangtze River Basin,whereas an intensified WPSH and MH caused the enhanced rain band to move northward to North China during the decay stage.During the development stage of the PDO cold phase associated with the 23-year cycle,a weakened WPSH and MH increased the precipitation over North China,whereas an intensified WPSH and the weakened MH increased the precipitation over South China during the decay stage.The 11-year and 23-year variabilities contribute differently to the precipitation variations in the different regions of China,as seen in the 1998flooding case.The 11-year cycle mainly accounts for precipitation increases over the Yangtze River Basin,while the 23-year cycle is responsible for the precipitation increase over Northeast China.These results have important implications for understanding how the PDO modulates the precipitation distribution over China,helping to improve interdecadal climate prediction.

A previously developed hybrid coupled model(HCM)is composed of an intermediate tropical Pacific Ocean model and a global atmospheric general circulation model(AGCM),denoted as HCMAGCM.In this study,different El Niño flavors,namely the Eastern-Pacific(EP)and Central-Pacific(CP)types,and the associated global atmospheric teleconnections are examined in a 1000-yr control simulation of the HCMAGCM.The HCMAGCM indicates profoundly different characteristics among EP and CP El Niño events in terms of related oceanic and atmospheric variables in the tropical Pacific,including the amplitude and spatial patterns of sea surface temperature(SST),zonal wind stress,and precipitation anomalies.An SST budget analysis indicates that the thermocline feedback and zonal advective feedback dominantly contribute to the growth of EP and CP El Niño events,respectively.Corresponding to the shifts in the tropical rainfall and deep convection during EP and CP El Niño events,the model also reproduces the differences in the extratropical atmospheric responses during the boreal winter.In particular,the EP El Niño tends to be dominant in exciting a poleward wave train pattern to the Northern Hemisphere,while the CP El Niño tends to preferably produce a wave train similar to the Pacific North American(PNA)pattern.As a result,different climatic impacts exist in North American regions,with a warm-north and cold-south pattern during an EP El Niño and a warm-northeast and cold-southwest pattern during a CP El Niño,respectively.This modeling result highlights the importance of internal natural processes within the tropical Pacific as they relate to the genesis of ENSO diversity because the active ocean–atmosphere coupling is allowed only in the tropical Pacific within the framework of the HCMAGCM.

Limitations are existed in current ensemble forecasting initial perturbation methods for describing the interactions among various spheres of the Earth system. In this study, a new method is proposed, namely, the coupled conditional nonlinear optimal perturbation(C-CNOP) method, which incorporates multisphere interactions much appropriately. The El Nino-Southern Oscillation(ENSO) is a typical ocean-atmosphere “coupling”(or “interaction”) phenomenon. The C-CNOP method is applied to ensemble forecasting of ENSO. It is demonstrated that the C-CNOP method can generate coupled initial perturbations(CPs) that appropriately consider initial ocean-atmosphere coupling uncertainty for ENSO ensemble forecasts. Results reveal that the CPs effectively improve the ability of ENSO ensemble-mean forecasts in both temporal variability of Nio3.4 sea surface temperature anomalies(SSTAs) and spatial variability of ENSO mature-phase SSTAs. Notably, despite the weakest ocean-atmosphere coupling strength in the tropical Pacific occurring during the boreal spring and summer, CPs still capture the uncertainties of this weak coupling when ENSO predictions are initialized at these seasons. This performance of CPs significantly suppresses the rapid increase of ENSO prediction errors due to the high ocean-atmosphere coupling instability during these seasons, and thus effectively extends the lead time of skillful ENSO forecasting. Hence, the C-CNOP method is a suitable initial perturbation approach for ENSO ensemble forecast that can describe initial ocean-atmosphere coupling uncertainty. It is expected that the CCNOP method plays a significant role in predictions of other high-impact climate phenomena, and even future Earth system predictions.

El Niño-Southern Oscillation(ENSO)is the strongest interannual climate mode influencing the coupled ocean-atmosphere system in the tropical Pacific,and numerous dynamical and statistical models have been developed to simulate and predict it.In some simplified coupled ocean-atmosphere models,the relationship between sea surface temperature(SST)anomalies and wind stress(τ)anomalies can be constructed by statistical methods,such as singular value decomposition(SVD).In recent years,the applications of artificial intelligence(AI)to climate modeling have shown promising prospects,and the integrations of AI-based models with dynamical models are active areas of research.This study constructs U-Net models for representing the relationship between SSTAs andτanomalies in the tropical Pacific;the UNet-derivedτmodel,denoted asτUNet,is then used to replace the original SVD-basedτmodel of an intermediate coupled model(ICM),forming a newly AI-integrated ICM,referred to as ICM-UNet.The simulation results obtained from ICM-UNet demonstrate their ability to represent the spatiotemporal variability of oceanic and atmospheric anomaly fields in the equatorial Pacific.In the ocean-only case study,theτUNet-derived wind stress anomaly fields are used to force the ocean component of the ICM,the results of which also indicate reasonable simulations of typical ENSO events.These results demonstrate the feasibility of integrating an AI-derived model with a physics-based dynamical model for ENSO modeling studies.Furthermore,the successful integration of the dynamical ocean models with the AI-based atmospheric wind model provides a novel approach to ocean-atmosphere interaction modeling studies.

Cell replacement therapy using neural progenitor cells(NPCs)has been shown to be an effective treatment for ischemic stroke.However,the therapeutic effect is unsatisfactory due to the imbalanced homeostasis of the local microenvironment after ischemia.Microenvironmental acidosis is a common imbalanced homeostasis in the penumbra and could activate acid-sensing ion channels 1a(ASIC1a),a subunit of proton-gated cation channels following ischemic stroke.However,the role of ASIC1a in NPCs post-ischemia remains elusive.Here,our results indicated that ASIC1a was expressed in NPCs with channel functionality,which could be activated by extracellular acidification.Further evidence revealed that ASIC1a activation inhibited NPC migration and neurogenesis through RhoA signaling-mediated reorganization of filopodia formation,which could be primarily reversed by pharmacological or genetic disruption of ASIC1a.In vivo data showed that the knockout of the ASIC1a gene facilitated NPC migration and neurogenesis in the penumbra to improve behavioral recovery after stroke.
