Solitary rectal ulcer syndrome:Clinical features,pathophysiology,diagnosis and treatment strategies

Solitary rectal ulcer syndrome (SRUS) is an uncommon benign disease, characterized by a combination of symptoms, clinical findings and histological abnormalities. Ulcers are only found in 40% of the patients; 20% of the patients have a solitary ulcer, and the rest of the lesions vary in shape and size, from hyperemic mucosa to broad-based polypoid. Men and women are affected equally, with a small predominance in women. SRUS has also been described in children and in the geriatric population. Clinical features include rectal bleeding, copious mucus discharge, prolonged excessive straining, perineal and abdominal pain, feeling of incomplete defecation, constipation, and rarely, rectal prolapse. This disease has well-described histopathological features such as obliteration of the lamina propria by fibrosis and smooth muscle fibers extending from a thickened muscularis mucosa to the lumen. Diffuse collage deposition in the lamina propria and abnormal smooth muscle fiber extensions are sensitive markers for differ-entiating SRUS from other conditions. However, the etiology remains obscure, and the condition is frequently associated with pelvic floor disorders. SRUS is difficult to treat, and various treatment strategies have been advocated, ranging from conservative management to a variety of surgical procedures. The aim of the present review is to summarize the clinical features, pathophysiology, diagnostic methods and treatment strategies associated with SRUS. Combined trabeculectomy and vitrectomy for intractable glaucoma with severe ocular contusion involving the posterior segment

Background Intractable secondary glaucoma with severe ocular contusion involving the posterior segment is difficult to treat. In this study, we evaluated the safety and efficacy of combined trabeculectomy and vitrectomy for intractable glaucoma with severe ocular contusion involving the posterior segment. Methods This retrospective interventional study enrolled 18 eyes from 18 consecutive cases with uncontrolled intraocular pressure, angle recession exceeding 180°, dense vitreous hemorrhage, and/or limited retinal detachment after severe ocular contusion. Combined trabeculectomy and vitrectomy was performed after giving the patient sufficient anti-inflammatory and intraocular pressure （IOP）-Iowering medication. Follow-up averaged 26.2 （range, 6.0-48.0） months. Results The mean lOP decreased from （36.4±10.0） mmHg on maximum lOP-lowering medications to （14.6±4.4） mmHg on （0.7±1.2） topical medications at the final follow-up （P 〈0.0001）. During the first postoperative month, four eyes （22%） had a short hypertensive phase and six eyes （33%） had early hypotony, including one case （5.6%） of bleb leakage Seven eyes （39%） developed recurrent hypertension 3-6 months postoperatively, five of which were controlled by topical medications. Ten （55.6%） eyes were classified as a complete success, five （27.8%） as a qualified success, and three eyes （16.7%） as failures. Kaplan-Meier survival analysis for complete and cumulative success showed 53.5% and 80.0% survival at 48 months, respectively. Visual acuity was improved in 15 eyes （83.3%）. Three eyes （16.7%） had unchanged visual acuity, one （5.6%） of which developed atrophia bulbi despite a normal intraocular pressure. No recurrent retinal detachment or vitreous hemorrhage developed. Conclusion Combined trabeculectomy and vitrectomy is a viable surgical procedure for the management of intractable glaucoma with severe ocular contusion involving the posterior segment in selected cases.

Bilayer Compounded Polytetrafluoroethylene Membrane for Enhanced Oil-Water Emulsion Separation

In order to achieve efficient and durable oil-water emulsion separation,the membranes possessing high separation efficiency and mechanical strength attract extensive attention and are in great *** present study,a kind of polytetrafluoroethylene(PTFE)-based bilayer membrane was fabricated by electrospinning fibrous PTFE(fPTFE)on an expanded PTFE(ePTFE)*** morphological observation revealed that the fibrous structure of the fPTFE layer could be tailored by controlling the formulation of spinning *** addition of appropriate polyoxyethylene(PEO)would make the fibers in the fPTFE layer finer and more *** a result,the compounded membrane exhibited a small pore size of approximately 1.25µm and a substantial porosity nearing 80%.This led to super-hydrophobicity,characterized by a high water contact angle(WCA)of 149.8°,and facilitated rapid oil *** water-in-oil emulsion separation experiment further confirmed that the compounded membrane not only had a high separation efficiency closing 100%,but such an outstanding separation capacity could be largely retained,either through multiple cycles of use or through strong acid(pH=1),strong alkali(pH=12),or high-temperature(100°C)***,the mechanical behavior of the bilayer membrane was basically contributed by that of each layer in terms of their volume *** significantly,the poor creep resistance of fPTFE layer was suppressed by compounding with ePTFE ***,this study has laid the groundwork for a novel approach to create PTFE-based compounded membranes with exceptional overall characteristics,showing promise for applications in the realm of emulsion separation.

Modulation of starch digestibility using non-thermal processing techniques:A review

Obesity and type 2 diabetes are widespread throughout the world, especially in developed countries. Starch is an important part of human staple food, the modulating of starch digestibility is conducive to reducing postprandial blood glucose levels and alleviating the chronic disease caused by high caloric intake. The digestion properties of starch are correlated with its structural features, including crystallization, amylose/amylopectin ratio, non-starch components, etc. Among the modified methods applied to regulate starch digestibility, non-thermal processing techniques(NTPT) receive extensive attention due to the characteristics of safety, environmental friendliness and high efficiency. The influence and mechanism of NTPT on the digestion properties of starch are discussed in this review, including ultrasounds, high pressure, γ-irradiation, etc. NTPT induces the alternation of morphological and structural characteristics of native starch, changing their sensitivity to enzymes. The effects of NTPT on the digestibility of starch are highly related to the processing parameters and structure characteristics of native starch. The review shows that NTPT is an effective way to modulate the digestion properties of starch and prevent people from suffering from chronic diseases such as obesity and type 2 diabetes.
