Effectiveness of Ledipasvir/Sofosbuvir with / without Ribavarin in Liver Transplant Recipients with Hepatitis C

Background and Aims:Recurrent infection of hepatitis C virus (HCV) in liver transplant (LT) recipients is universal and associated with significant morbidity and ***:we retrospectively evaluated the safety and efficacy of ledipasvir/sofosbuvir with and without ribavirin in LT recipients with recurrent genotype 1 hepatitis ***:Eighty-five LT recipients were treated for recurrent HCV with ledipasvir/sofosbuvirwith and without ribavirin for 12 or 24 *** mean (± standard deviation [SD]) time from LT to treatment initiation was 68 (±71) *** mean (± SD) age of the cohort was 63 (±8.6) years *** recipients were male (70%).Baseline alanine transaminase,total bilirubin,and HCV ribonucleic acid (RNA) values (± SD) were 76.8 (±126)mg/dL,0.8 (±1.3) U/L,and 8,010,421.9 (±12,420,985)IU/mL,*** of 43 recipients who were treated with ribavirin required drug cessation due to side effects,with 4 of those being anemia *** recipient discontinued the ledipasvir/***-one percent of recipients had undetectable viral levels at 4 weeks after starting therapy,and all recipients had complete viral suppression at the end of *** sustained viral response at 12 weeks after completion of therapy was 94%.Conclusion:Ledipasvir and sofosbuvir with and without ribavirin therapy is an effective and well-tolerated interferon-free treatment for recurrent HCV infection after *** is not uncommon in LT recipients receiving ribavirin.

Background and Aims:All-oral interlferon-free antivirals are highly effective in treating recurrent hepatitis C (HCV) infection in liver transplant (LT) *** aim of the study was to assess immunosuppression needs after achieving a sustained viral response (SVR).Methods:we compared immunosuppression needs before and after achieving a SVR in adult LT recipients treated for recurrent HCV infection with alloral direct acting ***:we identified 52 liver LT treated recipients who achieved a *** median (25th and 75th percentile interquartile range [IQR]) age was 62 years (57.75,65).Most recipients received tacrolimus (TAC) for their immunosuppressant *** achieving SVR,there was no statistically significant difference in daily dose of TAC unadjusted per weight (p > 0.05).However,there was a statistically significant decrease in daily dose of TAC adjusted per weight,serum levels of TAC,and the product of glomerular filtration rate and *** statistically significant differences in cyclosporine unadjusted/adjusted per weight daily dose or serum levels were ***:Immunosuppression needs were increased for those patients treated with TAC but not *** recipients prescribed TAC require close monitoring after treatment completion to avoid potential risk of acute rejection.

Lithium-ion batteries are used in a wide range of ***,their cycle life suffers from the problem of capacity fade,which includes calendar and cycle *** effects of storage time,temperature and partial charge-discharge cycling on the capacity fade of Li-ion batteries are investigated in this *** calendar aging and cycle aging are presented based on the storage and cycling experiment on LiCoO_(2)/graphite cells under different storage temperature and different ranges of state of charge(SOC).Based on the measurement data,a one-component and a double-component aging model are presented to respectively describe the capacity fade caused by calendar and cycle *** calendar aging of LiCoO_(2)/graphite batteries is mainly affected by temperature and SOC during the *** SOC and change in SOC(ΔSOC)are the main factors affecting battery degradation during cycling operation.

Objective: Erectile dysfunction (ED) is a condition of insufficient penile erection, consistently or recurrently, for sexual activity. Tumor necrosis factor-alpha (TNF-α) induces transforming growth factor-beta (TGF-β), which causes the transition of epithelial cells into mesenchymal cells that affect ED. This study aimed to evaluate the roles of TNF-α, TGF-β, degree of lower urinary tract symptoms, and prostatic volume for the presence of ED in benign prostatic hyperplasia (BPH) ***: Our study performed an analytic observational retrospective cohort study using secondary data from four hospitals in Bali, Indonesia, including medical records and other administrative data. The sample was BPH patients with several history ***: Our sample was 83 respondents, ranging from 50 years to 80 years, 61 respondents with ED and 22 with non-ED. The International Prostate Symptom Score showed a significant result, which indicates that ED is more common in patients with higher International Prostate Symptom Score (p=0.002). Moreover, the TNF-α of ≥43.9 pg/mg and TGF-β of ≥175.8 pg/mL were significantly associated with the presence of ED in BPH patients (pwith ED (p=0.947).Conclusion: TNF-α, TGF-β, and lower urinary tract symptoms severity can predict the occurrence of ED in BPH, while prostatic volume was not significant.

while much progress has been made in capturing high-quality facial performances using motion capture markers and shape-from-shading,high-end systems typically also rely on rotoscope curves hand-drawn on the *** curves are subjective and difficult to draw consistently;moreover,ad-hoc procedural methods are required for generating matching rotoscope curves on synthetic renders embedded in the optimization used to determine three-dimensional(3D)facial pose and *** propose an alternative approach whereby these curves and other keypoints are detected automatically on both the image and the synthetic renders using trained neural networks,eliminating artist subjectivity,and the ad-hoc procedures meant to mimic *** generally,we propose using machine learning networks to implicitly define deep energies which when minimized using classical optimization techniques lead to 3D facial pose and expression estimation.

Liver transplantation for hepatocellular carcinoma:an update

BACKGROUND: Hepatocellular carcinoma (HCC) is a heterogeneous malignancy with multiple etiologies, high incidence, and high mortality. The standard surgical management for patients with HCC consists of locoregional ablation, surgical resection, or liver transplantation, depending on the background state of the liver. Eighty percent of patients initially presenting with HCC are unresectable, either due to the extent of tumor or the level of underlying hepatic dysfunction. while in patients with no evidence of cirrhosis and good hepatic function resection has been the surgical treatment of choice, it is contraindicated in patients with moderate to severe cirrhosis. Liver transplantation is the optimal surgical treatment. DATA SOURCES: PubMed search of recent articles (from January 2000 to March 2011) was performed looking for relevant articles about hepatocellular carcinoma and its treatment. Additional articles were identified by evaluating references from selected articles. RESULTS: Here we review criteria for transplantation, the types, indications, and role of locoregional therapy in treating the cancer and in downstaging for possible later transplantation. we also summarize the contribution of immunosuppression and adjuvant chemotherapy in the management and prevention of HCC recurrence. Finally we discuss recent advances in imaging, tumor biology, and genomics as we delineate the remaining challenges for the diagnosis and treatment of this disease. CONCLUSIONS: Much can be improved in the diagnosis and treatment of HCC. A great challenge will be to improve patient selection to criteria based on tumor biology. Another will be to incorporate systemic agents post-operatively in patients at high risk for recurrence, paying close attention to efficacy and safety. The future direction of the effort in treating HCC will be to stimulate prospective trials, develop molecular imaging of lymphovascular invasion, to improve recipient selection, and to investigate biomarkers
